| 2 | The impact of endoscopic sinus surgery and aspirin desensitization on psychological burden in aspirin-exacerbated respiratory disease显示文摘Background:Aspirin-exacerbated respiratory disease(AERD)is an aggressive inflammatory disorder of the upper and lower respiratory tract.Corticosteroids,leukotriene modifiers,endoscopic sinus surgery(ESS),aspirin(ASA)desensitization,and biological immu-nomodulators are currently used to treat the disorder.Objective:The objective of this study was to determine the psychosocial impact of ESS and ASA desensitization on AERD patients.Methods:All AERD patients who underwent complete ESS were divided into two cohorts based on ASA desensitization status.The psychosocial metrics of the SNOT-22 were collected and analyzed at the following time points:pre-operative,1-month,3-month,6-month,and 12-month after ESS.Results:One hundred and eighty-four AERD patients underwent ESS from November 2009 to November 2018.From this group,130 patients underwent ASA desensitization(AD cohort)and 54 patients remained non-desensitized(ND cohort).AD patients showed a significantly greater reduction in total SNOT-22 scores over the study period compared to ND patients(p=0.0446).Analysis of SNOT-22 psychosocial metrics showed a significantly greater improvement in patient productivity in the AD cohort when compared to the ND cohort(p=0.0214).Further,a sub-group analysis accounting for subject attrition showed a significantly greater improvement in both productivity and concentration in AD patients when compared to the ND cohort(productivity:p=0.0068;concentration:p=0.0428).Conclusions:ESS followed by ASA desensitization decreases the overall psychosocial burden in AERD patients with a significant improvement in perceived productivity and concentration.This has significant implications given the psychosocial impact of chronic diseases. | Tran Locke Auddie Sweis Theodore Lin Brian Sweis Patrick Gleeson Michael Kohanski Nithin Adappa James Palmer John Bosso | 2020 | World Journal of Otorhinolaryngology-Head and Neck Surgery2020,6,4: | 1 |

| 14 | A critical review of the fatigue life prediction of asphalt mixtures and pavements显示文摘New pavement construction techniques and the increased use of recycled materials have led to unexpected and premature pavement failure in recent years.The pavement’s exposure to daily and seasonal extreme temperature and repeated vehicular loads accumulate damage.Pavement cracking occurs once the cumulative damage surpasses the material’s cracking threshold.Fatigue crack is the most common pavement cracking type.Over the past four decades,researchers have carried out numerous experiments and analyses to understand pavement cracking.This paper aims to provide an overview of fatigue cracking and discuss various fatigue test methods for characterizing asphalt concrete mixtures.The article also discusses the most common phenomenological and mechanistic models for predicting the fatigue life of asphalt concrete pavements based on different fatigue test results.The paper details the implementation of the commonly used numerical models found in numerical simulation software and their prediction ability for the fatigue life of a pavement structure.Two major flaws in current evaluation methods are the sensitivity of experimental results and the lack of reliability of some predictive models.Multiscale asphalt material characterization is the ongoing practice for determining the most appropriate performance evaluation tool.However,proceeding with future research objectives is unrealistic until the accuracy of the tests and reliability of the predictions can be verified against actual field results.This critical review of the fatigue life predictions of asphalt mixtures and pavements should help to refine or redefine the right course of action for future research. | Nithin Sudarsanan Youngsoo Richard Kim | 2022 | Journal of Traffic and Transportation Engineering(English Edition)2022,9,5: | 1 |
| 15 | Carcinosarcoma of the nasal cavity and paranasal sinuses:Review of the national cancer database显示文摘Background:Carcinosarcomas are rare,aggressive malignancies that can arise in the nasal cavity and paranasal sinuses.There are limited outcome data available.Accordingly,we sought to use the National Cancer Database(NCDB)to characterize patient demographics and outcomes.Method:A retrospective analysis of the NCDB from 2004 to 2016 for patients with sinonasal carcinosarcoma was conducted.Results:Thirty patients were included.The patients were predominantly male(n=20),white(n=23),and privately insured(n=15),with an average age of 62.4 years.The nasal cavity was the most common subsite(n=14),followed by the maxillary sinus(n=8).Most patient were treated with surgery followed by radiation(n=23),with the remaining undergoing surgery alone(n=4),radiation alone(n=2),or no treatment(n=1).One-third(n=10)received adjuvant chemotherapy.The 1-and 5-year overall survival(OS)in the cohort were 79.2%and 43.3%,respectively.Univariate log-rank testing showed OS varied based on intervention(P<0.029),sex(P<0.042),and age(P<0.025),while on multivariate analysis none of these factors independently predicted OS.Conclusions:We describe the demographics and presenting features of a national cohort of sinonasal carcinosarcoma patients.Future research is needed to identify predictors of overall survival,and to assess the optimal roles for radiation and systemic chemotherapy. | Jacob C.Harris Jacob G.Eide Rijul S.Kshirsagar Jason A.Brant James N.Palmer Nithin D.Adappa | 2023 | World Journal of Otorhinolaryngology-Head and Neck Surgery2023,9,2: | 0 |
| 16 | Safety and efficacy of balloon angioplasty compared to stent-basedstrategies with pulmonary vein stenosis:A systematic review and meta-analysis显示文摘BACKGROUND Pulmonary vein stenosis(PVS)is an uncommon but known cause of morbidity and mortality in adults and children and can be managed with percutaneous revascularization strategies of pulmonary vein balloon angioplasty(PBA)or pulmonary vein stent implantation(PSI).AIM To study the safety and efficacy outcomes of PBA vs PSI in all patient categories with PVS.METHODS We performed a literature search of all studies comparing outcomes of patients evaluated by PBA vs PSI for PVS.We selected all published studies comparing PBA vs PSI for PVS with reported outcomes of restenosis and procedure-related complications in all patient categories.In adults,PVS following atrial fibrillation ablation and in children PVS related to congenital etiology or post-procedural PVS following total or partial anomalous pulmonary venous return repair were included.The patient-centered outcomes were risk of restenosis requiring re-intervention and procedural-related complications.The metaanalysis was performed by computing odds ratios(ORs)using the random effects model based on underlying statistical heterogeneity.RESULTS Eight observational studies treating 768 severe PVS in 487 patients met our inclusion criteria.The age range of patients was 6 months to 70 years and 67%were males.The primary outcome of the re-stenosis requiring re-intervention occurred in 196 of 325 veins in the PBA group and 111 of 443 veins in the PSI group.Compared to PSI,PBA was associated with a significantly increased risk of re-stenosis(OR 2.91,95%CI:1.15-7.37,P=0.025,I2=79.2%).Secondary outcomes of the procedurerelated complications occurred in 7 of 122 patients in the PBA group and 6 of 69 in the PSI group.There were no statistically significant differences in the safety outcomes between the two groups(OR:0.94,95%CI:0.23-3.76,P=0.929,I^(2)=0.0%).CONCLUSION Across all patient categories with PVS,PSI is associated with reduced risk of re-intervention and is as safe as PBA and should be considered first-line therapy for PVS. | Pradyumna Agasthi Srilekha Sridhara Pattara Rattanawong Nithin Venepally Chieh-Ju Chao Hasan Ashraf Sai Harika Pujari Mohamed Allam Diana Almader-Douglas Yamini Alla Amit Kumar Farouk Mookadam Douglas L Packer David R Holmes Jr Donald J Hagler Floyd David Fortuin Reza Arsanjani | 2023 | World Journal of Cardiology2023,15,2: | 0 |

| 18 | Microdebrider complications in sinus surgery:Analysis of the openFDA database显示文摘Objective:Functional endoscopic sinus surgery is a commonly performed otolaryngologic procedure that often uses the microdebrider device for tissue removal.Given the ubiquitous nature of the instrument,we sought to better define the patterns of device failure using the postmarket surveillance openFDA database.Methods:The openFDA database was queried for all microdebrider‐related adverse events from January 1,2000 to November 1,2020.Descriptive information on the nature of device failure and any associated patient injury was compiled.Reports not directly related to device failure were excluded from the analysis.Results:A total of 641 events were included in the analysis.The most common device failure was overheating(n=348,54.3%),followed by material separation(n=173,27%),and inconsistent device activation(n=52,8.1%).Of the reported events,the vast majority did not result in patient harm(n=579,90.3%).On review of the remaining cases,only 24 events(3.7%)resulted in true harm to the patient,defined as a temporary or permanent injury or>30 min of additional anesthesia time.Of these cases,the need to reschedule surgical cases(n=5,0.8%),retained foreign body(n=5,0.8%),and thermal tissue injury(n=3,0.5%)were the most common.Five patients suffered an injury due to surgeon error unrelated to device malfunction(n=5,0.8%).Conclusions:Microdebrider device failures are extremely rare.When they do occur,less than 10%result in patient harm.In cases of patient harm related to microdebrider failure,preoperative testing of the device before use could prevent many of the reported malfunctions. | Elias S.Saba Jacob Hoerter Jeremy Chang David W.Chou Chris Xiao Jacob G.Eide Rijul S.Kshirsagar James N.Palmer Nithin D.Adappa | 2023 | World Journal of Otorhinolaryngology-Head and Neck Surgery2023,9,4: | 0 |

| 20 | Prediction of permanent pacemaker implantation after transcatheter aortic valve replacement:The role of machine learning显示文摘BACKGROUND Atrioventricular block requiring permanent pacemaker(PPM)implantation is an important complication of transcatheter aortic valve replacement(TAVR).Application of machine learning could potentially be used to predict preprocedural risk for PPM.AIM To apply machine learning to be used to predict pre-procedural risk for PPM.METHODS A retrospective study of 1200 patients who underwent TAVR(January 2014-December 2017)was performed.964 patients without prior PPM were included for a 30-d analysis and 657 patients without PPM requirement through 30 d were included for a 1-year analysis.After the exclusion of variables with near-zero variance or≥50%missing data,167 variables were included in the random forest gradient boosting algorithm(GBM)optimized using 5-fold cross-validations repeated 10 times.The receiver operator curve(ROC)for the GBM model and PPM risk score models were calculated to predict the risk of PPM at 30 d and 1 year.RESULTS Of 964 patients included in the 30-d analysis without prior PPM,19.6%required PPM post-TAVR.The mean age of patients was 80.9±8.7 years.42.1%were female.Of 657 patients included in the 1-year analysis,the mean age of the patients was 80.7±8.2.Of those,42.6%of patients were female and 26.7%required PPM at 1-year post-TAVR.The area under ROC to predict 30-d and 1-year risk of PPM for the GBM model(0.66 and 0.72)was superior to that of the PPM risk score(0.55 and 0.54)with a P value<0.001.CONCLUSION The GBM model has good discrimination and calibration in identifying patients at high risk of PPM post-TAVR. | Pradyumna Agasthi Hasan Ashraf Sai Harika Pujari Marlene Girardo Andrew Tseng Farouk Mookadam Nithin Venepally Matthew R Buras Bishoy Abraham Banveet K Khetarpal Mohamed Allam Siva K Mulpuru MD Mackram F Eleid Kevin L Greason Nirat Beohar John Sweeney David Fortuin David R Jr Holmes Reza Arsanjani | 2023 | World Journal of Cardiology2023,15,3: | 0 |
