1Endoscopic ultrasound-guided fine needle aspiration in the differentiation of type 1 and type 2 autoimmune pancreatitis显示文摘AIM:To investigate the usefulness of endoscopic ultrasound-guided fine needle aspiration(EUS-FNA) in the differentiation of autoimmune pancreatitis(AIP).METHODS:We retrospectively reviewed 47 of 56 AIP patients who underwent EUS-FNA and met the Asian diagnostic criteria.On 47 EUS-FNA specimens,we evaluated the presence of adequate material and characteristic features of lymphoplasmacytic sclerosing pancreatitis(LPSP) and idiopathic duct-centric pancreatitis(IDCP) mentioned in the International Consensus Diagnostic Criteria and examined if these findings make a contribution to the differential diagnosis of type 1 and type 2 AIP.A disposable 22-gauge needle was used for EUS-FNA.RESULTS:Adequate specimens including pancreatic tissue for differentiating AIP from cancer were obtained from 43 of 47 patients who underwent EUSFNA.EUS-FNA was performed from the pancreatic head in 21 cases,which is known to be technically difficult when performed by core biopsy;there was no significant difference in the results compared with pancreatic body-tail.Nine of 47 patients met level 1 findings of LPSP and 5 patients met level 2 findings of LPSP.No one met level 1 findings of IDCP,but 3 patients met level 2 findings of IDCP.Of 10 seronegative cases,2 cases were diagnosed with 'definitive type 1 AIP',and 3 cases were diagnosed with 'probable type 2 AIP' when considering both the level 2 histological findings and response to steroids.CONCLUSION:EUS-FNA is useful in the differentiation of type 1 and type 2 AIP,particularly in seronegative cases.Takuya Ishikawa Akihiro Itoh Hiroki Kawashima Eizaburo Ohno Hirosh Matsubara Yuya Itoh Yosuke Nakamura Takeshi Hiramatsu Masanao Nakamura Ryoji Miyahara Naoki Ohmiya Hidemi Goto Yoshiki Hirooka 2012World Journal of Gastroenterology2012,18,29:8

5Empirical correlations for predicting the self-leveling behavior of debris bed显示文摘Studies on the self-leveling behavior of debris bed are crucial for the assessment of core-disruptive accident (CDA) occurred in sodium-cooled fast reactors (SFR). To clarify this behavior over a comparatively wider range of gas velocities, a series of experiments were performed by injecting nitrogen gas uniformly from a pool bottom. Current experiments were conducted in a cylindrical tank, in which water, nitrogen gas and different kinds of solid particles, simulate the coolant, vapor (generated by coolant boiling) and fuel debris, respectively. Based on the quantitative data obtained (mainly the time variation of bed inclination angle), with the help of dimensional analysis technique, a set of empirical correlations to predict the self-leveling development depending on particle size, particle density and gas injection velocity was proposed and discussed. It was seen that good agreement could be obtained between the calculated and experimental values. Rationality of the correlations was further confirmed through detailed analyses of the effects of experimental parameters such as particle size, particle density, gas flow rate and boiling mode. 9Clinical oncology for pancreatic and biliary cancers:Advances and current limitations显示文摘In the early 2000s,the main stream of endoscopic ultrasonography(EUS)changed from a mechanical scanning method to electronic radial or linear scanning methods.Subsequently,useful applications in trans-abdominal ultrasonography came within reach of EUS.In particular,contrast-enhanced EUS(CE-EUS)and EUS-elastography became cutting-edge diagnostic modalities for pancreatic disorders.Each type of pancreatic disorder has characteristic hemodynamics.CE-EUS uses color Doppler flow imaging and harmonic imaging to classify pancreatic lesions.EUS-elastography can assess tissue hardness by measuring its elasticity.This parameter appears to correlate with the malignant potential of the lesions.Tissue elasticity studies can provide information on both its pattern and distribution.The former is the conventional method of morphological diagnosis,but it is restricted to observations made in a region of interest(ROI).The latter is an unbiased analysis that can be performed by image analysis software and is theoretically constant,regardless of the ROI.Though EUSfine needle aspiration(FNA)is also a very useful diagnostic tool,there are several limitations.Diagnostic EUS-FNA of pancreatic cystic lesions has marginal utility mainly due to low sensitivity.Therefore,in particular,endoscopists should keep this limitation in mind.Yoshiki Hirooka Akihiro Itoh Hiroki Kawashima Eizaburo Ohno Takuya Ishikawa Yuya Itoh Yosuke Nakamura Takeshi Hiramatsu Masanao Nakamura Ryoji Miyahara Naoki Ohmiya Masatoshi Ishigami Yoshiaki Katano Hidemi Goto 2011World Journal of Clinical Oncology2011,2,5:1
10A preliminary study on the effects of line and selective thinning on bird communities in Hokkaido, northern Japan显示文摘In Japan, selective thinning is a common thinning method, though line thinning receives much attention because of its economic merits. In this study, we examined effects of the two thinning methods on bird communities in Todo fir (Abies sachalinensis) plantations in Hokkaido, Japan. We surveyed bird species in forests under four different management types - unthinned, selectively thinned, line-thinned plantation, and naturally regenerated forest (here after referred to as natural forest) stands - using a line-transect method. We also investigated vegetation structure (canopy tree and understory) of these stands. Bird species richness did not differ between natural forests and plantations, while bird total abundance was greater in plantations than in natural forests. Bird species richness and total abundance were comparable among the three management types for plantations. Abundances of 10 bird species were different among the four management types, and five species were more abundant in line-thinned plantations. However, two species were more abundant in selectively thinned stands than in line-thinned stands, and they frequently appeared in natural forests. There were no distinct differences in vegetation structure among the management types for plantations. 15Diabetes therapies in hemodialysis patients: Dipeptidase-4 inhibitors显示文摘Although several previous studies have been published on the effects of dipeptidase-4(DPP-4) inhibitors in diabetic hemodialysis(HD) patients, the findings have yet to be reviewed comprehensively. Eyesight failure caused by diabetic retinopathy and aging-related dementia make multiple daily insulin injections difficult for HD patients. Therefore, we reviewed the effects of DPP-4 inhibitors with a focus on oral antidiabetic drugs as a new treatment strategy in HD patients with diabetes. The following 7 DPP-4 inhibitors are available worldwide: sitagliptin, vildagliptin, alogliptin, linagliptin, teneligliptin, anagliptin, and saxagliptin. All of these are administered once daily with dose adjustments in HD patients. Four types of oral antidiabetic drugs can be administered for combination oral therapy with DPP-4 inhibitors, including sulfonylureas, meglitinide, thiazolidinediones, and alpha-glucosidase inhibitor. Nine studies examined the antidiabetic effects in HD patients. Treatments decreased hemoglobin A1 c and glycated albumin levels by 0.3% to 1.3% and 1.7% to 4.9%, respectively. The efficacy of DPP-4 inhibitor treatment is high among HD patients, and no patients exhibited significant severe adverse effects such as hypoglycemia and liver dysfunction.
