1砷代谢与癌症、心血管疾病以及糖尿病的关系:流行病学证据的系统回顾显示文摘[背景]有关砷代谢对个体癌症、心血管疾病和糖尿病易感性的证据尚未得到正式和全面的回顾。[目的]本研究旨在系统研究流行病学调查中砷代谢与癌症、心血管疾病以及糖尿病结局的关系。其次,描述全球范围内不同人群间砷代谢的变异。[方法]以2016年1月前为限,检索Medline/PubMed和EMBASE,并应用预先确定的排除标准。采用成分数据分析来描述砷代谢生物标志的分布并评估砷暴露与代谢之间的关联。[结果]共有28项研究符合纳入标准,其中12项与癌症相关,9项与心血管疾病相关,7项与糖尿病结局相关。无机砷百分比(i As%)、单甲基砷百分比(MMA%)和二甲基砷百分比(DMA%)的中位数(四分位间距)分别为11.2%(7.8%~14.9%)、13.0%(10.4%~13.6%)和74.9%(69.8%~80.0%)。各项研究结果表明,砷暴露水平越高,则iAs%越高,DMA%越低,但与MMA%无关。就癌症而言,大多数研究发现MMA%越高、DMA%越低,则全位点癌症、泌尿道上皮肿瘤、肺癌和皮肤癌的发生风险越高。就心血管疾病而言,通常MMA%越高,则颈动脉粥样硬化和临床心血管疾病的发生风险越高,但与高血压无关。就糖尿病结局而言,MMA%越低且DMA%越高,则代谢综合征和糖尿病的发生风险越高。[结论]人群中i As%和DMA%的水平与砷暴露水平相关,但MMA%与砷暴露水平无关。研究结果提示,MMA%越高,癌症和心血管疾病发病风险增加;而MMA%越低,糖尿病和代谢综合征发病风险增加。需要更多的人群研究和实验室研究来进一步评价和了解砷暴露对砷代谢的影响,以及砷代谢对发病的影响。殷园园(译) 金泰廙(校) Kuo CC Moon KA Wang SL 2018环境与职业医学2018,35,12:2
