1An attending physician float shift for the improvement of physician productivity in a crowded emergency department显示文摘BACKGROUND:Patients backlogged in the emergency department(ED) waiting for an inpatient bed(boarders) continue to require the attention of ED physicians,exacerbating crowding in the ED.To address this problem,we added a 'float shift' to our winter schedule solely to care for boarders.We sought to quantify the effect of this float shift,hypothesizing greater physician productivity.METHODS:We performed a retrospective observational study in our community hospital ED,measuring the number of new patients seen in each 10-hour shift in the presence or absence of a float shift physician.We calculated the number of new patients seen per shift for each of the 7 daily shifts,during February(float shift scheduled) and May(float shift unscheduled) of 2008.We then compared the mean number of patients seen per shift in February with May.RESULTS:Total monthly patient volume was 6 656 for February and 6 775 for May,with the mean daily census being 230 and 219 patients,respectively.The number of new patients seen during each shift was greater in February than in May,with a mean increase of 1.1 patients per shift(with the float shift).Surveying participants about intervention effectiveness showed 92%of residents,but only 65%of attending physicians,in favor of maintaining the float shift.CONCLUSION:The presence of a 'float shift' physician caring only for boarding patients allows other physicians to maintain and even increase their productivity in our ED,despite the presence of longer throughput times and increased time on diversion.Muhammad Umer Nasim Chintan Mistry Robert Harwood Erik Kulstad Laura Tommaso 2013World Journal of Emergency Medicine2013,4,1:6

3Interactions Between Temperature and Sugars in the Regulation of Leaf Senescence in the Perennial Herb Arabis alpina L.显示文摘Annual plants usually flower and set seed once before senescence results in the death of the whole plant (monocarpic senescence). Leaf senescence also occurs in polycarpic perennials; even in 'evergreen' species individual leaves senesce. In the annual model Arabidopsis thaliana sugars accumulate in the senescent leaves and senescence is accelerated by high sugar availability. Similar to A. thaliana, sugar contents increased with leaf age in the perennial Arabis alpina grown under warm conditions (22 C day/18 night). At 5 C, sugar contents in non-senescent leaves were higher than at a warm temperature, but dependent on the accession, either sugars did not accumulate or their contents decreased in old leaves. In A. alpina plants grown in their natural habitat in the Alps, sugar contents declined with leaf age. Growth at a cold temperature slightly delayed senescence in A. alpina. In both warm and cold conditions, an external glucose supply accelerated senescence, but natural variation was found in this response. 4Experimental investigation of a high aspect ratio,low speed contra-rotating fan stage with complex inflow distortion显示文摘This paper focuses on the response of a high aspect ratio,low speed contra-rotating fan with complex inflow distortion.The total pressure at the inlet is artificially distorted by means of two different sets of screens with different porosities to generate a hub-strong complex distortion and a tip-strong complex distortion.Detailed flow analyses were conducted for the design speed of rotor-1 in combination with off design speeds of rotor-2 both for the design and the peak pressure mass flow rates.In order to understand the extent of inlet distortion,the distortion sector was rotated circumferentially at intervals of 151 to cover the entire annulus.Detailed measurements of total pressure,static pressure,velocity components and flow angles were carried out at the inlet of the first rotor,between the two rotors and at the exit of the second rotor using three seven hole probes.The study reveals a few interesting aspects on the effect of complex inflow distortion on the flow behavior of a contra-rotating stage.The presence of a low porosity screen reduces the magnitude of axial velocity and generates higher spread of distortion near the localized region of placement of the screen.The hub-strong complex distortion has a greater effect of presence of this low porosity screen in both circumferential and radial directions.This leads to higher stagnation pressure variation at the inlet.On the other hand,for the tip-strong complex distortion case,the extent of distortion is observed to be higher in the circumferential direction towards the casing rather than radial.The localized improvement in the flow(in tip-strong inflow distortion)near the tip region improves the performance both in terms of pressure rise and efficiency of stage compared to hub–strong complex distortion.Chetan Mistry A.M.Pradeepn 2014Propulsion and Power Research2014,3,2:3
5Investigation of anticancer potencies of newly generated Schiff base imidazolylphenylheterocyclic-2-ylmethylenethiazole-2-amines显示文摘A new series of multi-heterocyclic Schiff base was constructed starting from 4'-(imidazol-1-yl[56_TD$IF])-acetophenone which was converted to its 2-bromoethanone precursor which on cyclic condensation with thiourea yielded final thiazol-2-amine intermediate(3) to be reacted with substituted aldehydes to generate final imidazolylphenylheterocyclic-2-ylmethylenethiazole-2-amines(4a–4i). New Schiff base was investigated for their in vitro cytotoxic efficacies against a panel of three human cancer cell lines namely, MCF7(human breast cancer), HCT116(human colon cancer), and DU145(human prostate cancer) and one normal skin fibroblast(SF). Most of these synthetic derivatives shown important cytotoxic actions against individual carcinoma cell line collections, but weak actions against SF, which is as anticipated. Observations of SAR suggested that the difference in the characteristics of substituents attached to the Schiff base function leads to the interesting variations within pharmacological effects of resultant molecular systems. 6Protective effect of Sida cordata leaf extract against CCl_4 induced acute liver toxicity in rats显示文摘Objective:To investigate the hepatoprotective potential of Sida cordata(Malvaceae)(S.cordata) in experimental rats to validate its traditional claim.Methods:Wister albino rats were divided into 6 groups:Croup 1 served as control;CroupⅡserved as hepatotoxic(CCl4 treated) group; CroupⅢ,Ⅳand V served as(100,200 and 400 mg/kg b.w.) S.cordata leaf extract(SCLE) treated groups;CroupⅥserved as positive control(Silymarin) treated group.Liver marker enzymes serum glutamate oxyloacetic transaminase,serum glutamic pyruvic transaminase,pancreatic enzymatic antioxidants superoxide dismutase(SOD),lipid peroxidation,catalase(CAT),reduced glutathione(CSH) were measured and compared along with histopathological studies.Results: Obtained results show that the treatment with SCI.E significantly(P<0.05-<0.001) and dosedependently reduced CCl4 induced elevated serum level of hepatic enzymes.Furthermore, SCLE significantly(up to P<0.001) reduced the lipid peroxidation in the liver tissue and restored activities of defence antioxidant enzymes CSH,SOD and CAT towards normal levels,which was confirmed by the histopathological studies.Conclusions:The results of this study strongly indicate the protective effect of SCLE against CCl4 induced acute liver toxicity in rats and thereby scientifically support its traditional use.Sunil Mistry KR Dutt J Jena 2013Asian Pacific Journal of Tropical Medicine2013,6,4:2
7普萘洛尔治疗多发性气道血管瘤显示文摘该文旨在报道普萘洛尔治疗婴幼儿气道血管瘤的经验。5周龄女性婴儿发生上呼吸道阻塞症状,支气管镜检查诊断为血管瘤,给以口服泼尼松治疗,并同时给予普萘洛尔。普萘洛尔给药过程中监测患儿血压、脉搏及血糖水平。普萘洛尔剂量为每天2mg/kg,泼尼松剂量递减。治疗10天后,肿瘤体积明显缩小。服用8个月后,肿瘤完全消失,仅可见黏膜毛细血管轻度扩张。婴幼儿呼吸道血管瘤可引起窒息,危及患者生命,普萘洛尔是治疗此部位血管瘤的有效方法。Mistry N Tzifa K 2010中国口腔颌面外科杂志2010,8,3:2
