1腹壁疝补片修补相关感染:病理学、治疗和结局显示文摘目的分析应用合成补片行疝修补后出现需要去除补片的手术部位感染并发症病人的临床及人口统计学特点,补片植入手术的细节以及感染发生后的治疗和结局。方法从病人资料表中获得病人的感染相关因素(人口统计学参数、疝修补术特点和使用的网片)。收集32例连续病例的标本(补片和组织),行细菌培养和显微镜检查。前瞻性评估补片取出后的结局。结果 22例接受了切口疝修补术,10例接受了腹股沟疝修补术,大部分病人手术时间较长,28例出现过早期伤口并发症(浆液肿或血肿)。在'补片植入-取出'间期,部分病人进行了保守治疗,例如局麻下引流或者窦道切除。22例的网片被完全取出(其中9例是补片部分取出后),其余10例部分取出补片治疗有效。大部分补片(24例)是多纤丝聚丙烯网片,补片周围组织的显微镜观察发现有白细胞浸润、巨细胞反应、胶原纤维无序排列以及脓肿形成。32例共进行了51例次手术治疗,补片取出后,有6例复发和2例肠瘘。平均随访时间为40(30~97)个月。结论大多数需要去除补片的感染病例前次疝修补术时间较长,并且有未经治疗的术后早期伤口并发症。补片部分取出通常不会成功,而且会带来并发症。取出全部补片的外科探查手术应该在全麻下进行。Tolino MJ Tripoloni DE Ratto R Garcia MI 2010中国实用外科杂志2010,30,12:20
2Current view of the immunopathogenesis in inflammatory bowel disease and its implications for therapy显示文摘Although the aetiology of inflammatory bowel disease (IBD) remains unknown, the pathogenesis is gradually being unravelled, seeming to be the result of a combination of environmental, genetic, and immunological factors in which an uncontrolled immune response within the intestinal lumen leads to inflammation in genetically predisposed individuals. Multifactorial evidence suggests that a defect of innate immune response to microbial agents is involved in IBD. This editorial outlines the immunopathogenesis of IBD and their current and future therapy. We present IBD as a result of dysregulated mucosal response in the intestinal wall facilitated by defects in epithelial barrier function and the mucosal immune system with excessive production of cytokines growth factors, adhesion molecules, and reactive oxygen metabolites, resulting in tissue injury. 3New aspects in celiac disease显示文摘Celiac disease (CD) is a common autoimmune disorder characterized by an immune response to ingested gluten and has a strong HLA association with HLA- DQ2 and HLA-DQ8 molecules, but human HLA-DQ risk factors do not explain the entire genetic susceptibility to gluten intolerance. CD is caused by the lack of immune tolerance (oral tolerance) to wheat gluten. In this sense, the expression of soluble HLA-G in CD is of special interest because the molecule plays an important role in the induction of immune tolerance. The enhanced expression of soluble HLA-G found in CD may be part of a mechanism to restore the gluten intolerance. 5Accelerating phase-field-based microstructure evolution predictions via surrogate models trained by machine learning methods显示文摘The phase-field method is a powerful and versatile computational approach for modeling the evolution of microstructures and associated properties for a wide variety of physical,chemical,and biological systems.However,existing high-fidelity phase-field models are inherently computa fonally expensive,requiring high-performance computing resources and sophisticated numefical integration schemes to achieve a useful degree of accuracy.In this paper,we present a computationally inexpensive,accurate,data-driven surmogate model that directly learns the microstructural evolution of targeted systems by combining phase-field and history-dependent machine-learning techniques.We integrate a statisti cally representative,low-dimensianal description of the micrastructure,obtained direcdy from phase-field simulations,with either a time-series multivariate adaptive regression splines autoregressive al garithm or a lang short-term memory neural network.The neural-network-trained surrogate model shows the bestperformance and accurately predicts the nanlinear microstructure evolutian of a two-phase mixture during spinodal decomposition in seconds,without the need for'on-the-fly'solutions of the phase-field equations of motion.We also show that the predictions from our machine learned surrogate model can be fed directly as an input into a classical high-fideli ty phase-field model in order to accelerate the high-fidelity phase field simulations by leaping in time.Such machine-learned phase-field framework opens a promising path forward to use accelerated phase field simulations for discovering,understanding,and predicting processing-microstructure-performance relationships.David Montes de Oca Zapiain James A.Stewart Rémi Dingreville 2021npj Computational Materials2021,,1:4
6Frequency and prognostic role of mucosal healing in patients with Crohn's disease and ulcerative colitis after one-year of biological therapy显示文摘AIM:To assess the endoscopic activity before and after a one-year period of biological therapy and to evaluate the frequency of relapses and need for retreatment after stopping the biologicals in patients with Crohn’s disease(CD)and ulcerative colitis(UC).METHODS:The data from 41 patients with CD and 22 patients with UC were assessed.Twenty-four CD patients received infliximab,and 17 received adalimumab.The endoscopic severity of CD was quantified with the simplified endoscopic activity score for Crohn’s disease in CD and with the Mayo endoscopic subscore in UC.RESULTS:Mucosal healing was achieved in 23 CD and7 UC patients.Biological therapy had to be restarted in78%of patients achieving complete mucosal healing with CD and in 100%of patients with UC.Neither clinical remission nor mucosal healing was associated with the time to restarting the biological therapy in either CD or UC.CONCLUSION:Mucosal healing did not predict sustained clinical remission in patients in whom the biological therapies had been stopped.Klaudia Farkas Péter László Lakatos Mónika Szcs va Pallagi-Kunstár Anita Bálint Ferenc Nagy Zoltán Szepes Noémi Vass Lajos S Kiss Tibor Wittmann Tamás Molnár 2014World Journal of Gastroenterology2014,20,11:2

8Learning two-phase microstructure evolution using neural operators and autoencoder architectures显示文摘Phase-field modeling is an effective but computationally expensive method for capturing the mesoscale morphological and microstructure evolution in materials.Hence,fast and generalizable surrogate models are needed to alleviate the cost of computationally taxing processes such as in optimization and design of materials.The intrinsic discontinuous nature of the physical phenomena incurred by the presence of sharp phase boundaries makes the training of the surrogate model cumbersome.We develop a framework that integrates a convolutional autoencoder architecture with a deep neural operator(DeepONet)to learn the dynamic evolution of a two-phase mixture and accelerate time-to-solution in predicting the microstructure evolution.We utilize the convolutional autoencoder to provide a compact representation of the microstructure data in a low-dimensional latent space.After DeepONet is trained in the latent space,it can be used to replace the high-fidelity phase-field numerical solver in interpolation tasks or to accelerate the numerical solver in extrapolation tasks.Vivek Oommen Khemraj Shukla Somdatta Goswami Rémi Dingreville George Em Karniadakis 2022npj Computational Materials2022,,1:2
9Interaction of the major inflammatory bowel disease susceptibility alleles in Crohn’s disease patients显示文摘AIM:To investigate the interaction of interleukin-23 receptor(IL23R)(rs1004819 and rs2201841),autophagy-related 16-like 1(ATG16L1)(rs2241880), caspase recruitment domain-containing protein 15 (CARD15)genes,and IBD5 locus in Crohn's disease(CD) patients. METHODS:A total of 315 unrelated subjects with CD and 314 healthy controls were genotyped.Interactions and specific genotype combinations of a total of eight variants were tested.The variants of IBD5locus(IGR2198a_1 rs11739135 and IGR2096a_1 rs12521868),CARD15(R702W rs2066845 and L1007fs rs2066847),ATG16L1(rs2241880)and IL23R (rs1004819,rs2201841)genes were genotyped by PCR-RFLP,the G908R(rs2066844)in CARD15 was determined by direct sequencing. RESULTS:The association of ATG16L1 T300A with CD was confirmed[P=0.004,odds ratio(OR)=1.69, 95%CI:1.19-2.41],and both IL23R variants were found to represent significant risk for the disease(P= 0.008,OR=2.05,95%CI:1.20-3.50 for rs1004819 AA;P<0.001,OR=2.97,95%CI:1.65-5.33 for rs2201841 CC).Logistic regression analysis of pairwise interaction of the inflammatory bowel disease (IBD)loci indicated that IL23R,ATG16L1,CARD15 and IBD5(IGR2198a_1)contribute independently to disease risk.We also analysed the specific combina- tions by pair of individual ATG16L1,IL23R rs1004819, rs2201841,IGR2198a_1,IGR2096a_1 and CARD15 genotypes for disease risk influence.In almost all cases,the combined risk of susceptibility pairs was higher in patients carrying two different risk-associated gene variants together than individuals with just one polymorphism.The highest OR was found for IL23R rs2201841 homozygous genotype with combination of positive CARD15 status(P<0.001,OR=9.15,95% CI:2.05-40.74).
