| 2 | Pre-recorded instructional audio vs. dispatchers' conversational assistance in telephone cardiopulmonary resuscitation: A randomized controlled simulation study显示文摘BACKGROUND:To assess the effectiveness of the telephone chest-compression-only cardiopulmonary resuscitation(CPR)guided by a pre-recorded instructional audio when compared with dispatcher-assisted resuscitation.METHODS:It was a prospective,blind,randomised controlled study involving 109 medical students without previous CPR training.In a standardized mannequin scenario,after the step of dispatcher-assisted cardiac arrest recognition,the participants performed compression-only resuscitation guided over the telephone by either:(1)the pre-recorded instructional audio(n=57);or(2)verbal dispatcher assistance(n=52).The simulation video records were reviewed to assess the CPR performance using a 13-item checklist.The interval from call reception to the first compression,total number and rate of compressions,total number and duration of pauses after the first compression were also recorded.RESULTS:There were no significant differences between the recording-assisted and dispatcher-assisted groups based on the overall performance score(5.6±2.2 vs.5.1±1.9,P>0.05)or individual criteria of the CPR performance checklist.The recording-assisted group demonstrated provided(170.2±48.0 vs.156.2±60.7).CONCLUSION:When provided by untrained persons in the simulated settings,the compression-only resuscitation guided by the pre-recorded instructional audio is no less efficient than dispatcher-assisted CPR.Future studies are warranted to further assess feasibility of using instructional audio aid as a potential alternative to dispatcher assistance. | Alexei Birkun Maksim Glotov Herman Franklin Ndjamen Esther Alaiyex Esther Alaiye Temidara Adeleke Sergey Samarin | 2018 | World Journal of Emergency Medicine2018,9,3: | 6 |
| 3 | Mountain Pastures and Grasslands in the SW Tien Shan,Kyrgyzstan-Floristic Patterns,Environmental Gradients,Phytogeography,and Grazing Impact显示文摘Vast grasslands are found in the walnut-fruit forest region of southern Kyrgyzstan,Middle Asia.Located above the worldwide unique walnutfruit forests and used for grazing,they play a pivotal role in the mixed mountain agriculture of local farmers.Accordingly,these pastures are subject to an increasing utilization pressure reflecting the changing political and social conditions in the transformation process from a Soviet republic to an independent state.A first detailed analysis of mountain pasture vegetation in the Ferghana Range answers the following questions:What are the main plant community types among Kyrgyzstan's mountain pastures? What are the main environmental gradients that shape their species composition? Which phytogeographical distribution types are predominant? How does grazing affect community composition and species richness in these grasslands? Species composition was classified by cluster analysis;underlying environmental gradients were explored using DCA.A dataset of 395 relevés was used for classification,and a subset of 79 relevés was used in a DCA to analyze the correlation between vegetation,environment,and grazing impact.The investigated pastures were classified into four distinctive plant communities.The site factors altitude,heat load,inclination and grazing impact were found to be the major determinants of the vegetation pattern.A significant overlap between floristic composition and structural and spatial properties was shown.The majority of the species pool consisted of Middle Asian endemics and Eurosiberian species.However,disturbance-tolerant species played a significant role with respect to species composition and coverage of the herbaceous layer in vast areas of southern Kyrgyzstan's mountain pastures.In general,an intense grazing impact is clearly reflected by both species composition and structural variables of plant communities.The highly diverse and unique ecosystem is modified by an increasing utilization pressure.In order to maintain vital processes and functioning of this valuable ecosystem-in both economical and ecological terms-,it is indispensable to adopt appropriate pasture management strategies. | BORCHARDT Peter SCHICKHOFF Udo SCHEITWEILER Sabrina KULIKOV Maksim | 2011 | Journal of Mountain Science2011,8,3: | 4 |

| 5 | Education in cardiopulmonary resuscitation in Russia:A systematic review of the available evidence显示文摘BACKGROUND: To summarise and appraise cumulative published scientific evidence relevant to cardiopulmonary resuscitation(CPR) education in Russia.DATA RESOURCES: We searched Medline, Scopus, Science Direct and Russian Science Citation Index databases from December 1991 to December 2016 to identify studies pertaining to the field of CPR education that were carried out by Russian researchers and/or investigated the topic of interest for Russia/Russian population. Reference lists of eligible publications, contents pages of relevant Russian journals and Google Scholar were also searched. There was no limitation based on publication language or study design.RESULTS: Of 7 964 unique citations identified, 22 studies were included. All studies were published from 2009 to 2016, mainly in Russian. Only three studies were reported to be randomized controlled. Non-medical individuals constituted 17% of studied populations. Most of the studies aimed to assess effects of CPR educational interventions, generally suggesting positive influence of the training conducted. The studies were highly heterogeneous as for methodological approaches, structure and duration of educational interventions, evaluation methods and criteria being used. Methodological quality was generally poor, with >40% publications not passing quality screening and only 2 studies meeting the criteria of moderate high quality.CONCLUSION: The results suggest paucity, low population coverage, high thematic and methodological heterogeneity and low quality of the studies addressing CPR education, which were carried out in the Russian Federation. | 9 | Empagliflozin alleviates podocytopathy and enhances glomerular nephrin expression in db/db diabetic mice显示文摘BACKGROUND Modern guidelines recommend sodium-glucose cotransporter-2(SGLT2)inhibitors as the preferred antihyperglycemic agents for patients with type 2 diabetes and chronic kidney disease.However,the mechanisms underlying the renal protective effect of SGLT2 inhibitors are not fully understood.structure of podocytes and nephrin expression in glomeruli in db/db diabetic mice.METHODS We treated 8-wk-old male db/db mice with EMPA(10 mg/kg/d)or vehicle for 8 wk.Age-matched male db/+mice were included as non-diabetic controls.Parameters of body composition,glycemic and lipid control,and plasma concentrations of leptin,insulin and glucagon were assessed.We evaluated renal hypertrophy as kidney weight adjusted to lean mass,renal function as plasma levels of creatinine,and albuminuria as the urinary albumin-to-creatinine ratio(UACR).Renal structures were studied by light and transmission electron microscopy with a focus on mesangial volume and podocyte structure,respectively.Glomerular nephrin and transforming growth factor beta(TGF-β)were assessed by immunohistochemistry.RESULTS Severe obesity and hyperglycemia developed in db/db mice prior to the start of the experiment;increased plasma concentrations of fructosamine,glycated albumin,cholesterol,leptin,and insulin,and elevated UACR were detected.Mesangial expansion,glomerular basement membrane thickening,and increased area of TGF-βstaining in glomeruli were revealed in vehicle-treated mice.Podocytopathy was manifested by effacement of foot processes;nephrin-positive areas in glomeruli were reduced.EMPA decreased the levels of glucose,fructosamine and glycated albumin,UACR,kidney hypertrophy,mesangial expansion,glomerular basement membrane thickening,and glomerular TGF-βstaining,alleviated podocytopathy and restored glomerular staining of nephrin.CONCLUSION These data indicate that EMPA attenuates podocytopathy in experimental diabetic kidney disease.The anti-albuminuric effect of EMPA could be attributed to mitigation of podocyte injury and enhancement of nephrin expression. | Vadim V Klimontov Anton I Korbut Iuliia S Taskaeva Nataliya P Bgatova Maksim V Dashkin Nikolai BOrlov Anna S Khotskina Evgenii L Zavyalov Thomas Klein | 2020 | World Journal of Diabetes2020,11,12: | 2 |
