| 4 | Survival analysis of cholangiocarcinoma:A 10-year experience in Malaysia显示文摘AIM:To investigate the clinical features and survival of patients treated for cholangiocarcinoma in our institution and to analyze the factors affecting their survival.METHODS:This retrospective cohort study assessed patients diagnosed with cholangiocarcinoma between January 1997 and December 2007 at the University Malaya Medical Centre in Malaysia.The clinical data and associated outcomes were collected using a structured proforma.RESULTS:Of the 69 patients diagnosed with cholangiocarcinoma,38 (55%) were male;mean patient age was 61 years.Twelve patients (17%) had intrahepatic,38 (55%) had perihilar and 19 (28%) had distal tumors.Only 12 patients underwent curative surgery,including seven R0 resections.Only one patient died within 30 d after surgery.The overall median survival was 4 mo,whereas the median survival of R0 resected patients was 16 mo.The overall 1-,2-and 3-year cumulative survival rates were 67%,17% and 17%,respectively.Survival rates were significantly associated with curative resection (P=0.002),intrahepatic tumor (P=0.003),negative margin status (P=0.013),early tumor stage (P=0.016),higher tumor differentiation (P=0.032) and absence of jaundice (P=0.038).Multivariate analysis showed that tumor location was a significant independent predictor of patient survival.CONCLUSION:Curative,margin-negative resection of early stage,well-differentiated intrahepatic tumors is associated with improved patient survival. | Ahmad Ramzi Yusoff Mohd Muzammil Abdul Razak Yoong Boon Koon R Vijeyasingam Siti Zuraidah Mahmud | 2012 | World Journal of Gastroenterology2012，18，5： | 7 |

| 6 | Mechanisms and molecular approaches for heat tolerance in rice (Oryza sativa L.) under climate change scenario显示文摘Rice, a staple cereal crop in many parts of the world, has been confronted with multiple environmental stresses including high temperature, negatively impacts the booting as well as anthesis growth stages. The situation is further complicated by the changing climatic conditions, resulting in gradual escalation of temperature as well as changing the rainfall pattern and frequency, thus raising a concern of food security worldwide. The situation can be combat by developing rice varieties with excellent genetics with improved morpho-physiological, biochemical, and molecular mechanisms, together can minimize the adverse effects of heat stress. Here, several strategies(encompassing genetic and genomic, and mechanisms involved) for mitigating the impact of high temperature on rice have been discussed. | 9 | Tween 80 containing lipid nanoemulsions for delivery of indinavir to brain显示文摘Indinavir is a protease inhibitor used in the treatment of HIV infection.However,it has limited efficacy in eradicating the virus in the brain due to efflux by P-glycoprotein(P-gp)expressed at the blood–brain barrier(BBB).The objective of this work was to develop an o/w lipid nanoemulsion(LNE)of indinavir using Tween 80 as co-emulsifier to improve its brain specific delivery.LNEs were prepared with different compositions and were characterized for globule size,polydispersity index,zeta potential and in vitro drug release.Five formulations were then evaluated for drug content,entrapment efficiency and stability after which brain uptake studies were carried out using fluorescent labeled LNEs and pharmacokinetic(PK)and tissue distribution studies were conducted after intravenous administration in mice.Brain uptake of indinavir was shown to be improved for a 1%Tween 80 containing formulation(F5)compared to a formulation containing 0.3%cholesterol(F2).In PK studies,the brain level of indinavir subsequent to administration of F5 was significantly(Po0.05)higher than produced by administration of a drug solution(2.44-fold)or a control nanoemulsion(F1)(1.48-fold)or formulation F2(1.6-fold).The increased brain specific accumulation of indinavir from F5 is probably due to enhanced low density lipoprotein-mediated endocytosis and P-gp inhibition by Tween 80 at the BBB.These results suggest Tween 80 containing LNEs could provide a simple but effective means of delivering indinavir to brain. | Kandadi Prabhakar Syed Muzammil Afzal Goparaboina Surender Veerabrahma Kishan | 2013 | Acta Pharmaceutica Sinica B2013，3，5： | 2 |
