1Esophageal cancer: A Review of epidemiology, pathogenesis, staging workup and treatment modalities显示文摘Esophageal cancer is a serious malignancy with regards to mortality and prognosis. It is a growing health concern that is expected to increase in incidence over the next 10 years. Squamous cell carcinoma is the most common histological type of esophageal cancer worldwide, with a higher incidence in developing nations. With the increased prevalence of gastroesophageal reflux disease and obesity in developed nations, the incidence of esophageal adenocarcinoma has dramatically increased in the past 40 years. Esophageal cancer is staged according to the widely accepted TNM system. Staging plays an integral part in guiding stage specific treatment protocols and has a great impact on overall survival. Common imaging modalities used in staging include computed tomography, endoscopic ultrasound and positron emission tomography scans. Current treatment options include multimodality therapy mainstaysof current treatment include surgery, radiation and chemotherapy. 2岩石节理直剪试验颗粒流宏细观分析显示文摘基于颗粒流理论和PFC程序,在解决建模过程中悬浮颗粒的消除、恒定法向荷载伺服机制的施加、拟静力加载状态的选取等问题后,较为完善地实现岩石节理PFC数值直剪试验,并分别从宏观和细观角度深入探讨节理在直剪试验过程中的力学演化特征和破坏机制。结合已有的节理直剪试验成果,进行室内试验和计算结果的对比分析,验证计算方法的可靠性。研究成果如下:(1)随恒定法向荷载的增大,剪切应力及其峰值时刻的剪切位移增大,节理面上黏结破坏颗粒增多,而剪切阻抗和节理剪胀效应却降低;(2)随剪切位移的增加,节理面上粒间法向接触数不断减少,接触矢量方向逐渐向剪切荷载施加方向偏转,而粒间接触压力不断增大,裂纹不断沿节理面附近产生,破裂频数在剪切应力达到峰值时最为强烈;(3)数值试验得到的剪切阻抗值普遍高于试验值,但减小模型颗粒半径可有效降低计算剪切阻抗值。室内试验和计算结果对比分析表明,新提出的颗粒流计算方法非常适用于岩石节理直剪试验的数值模拟,可为室内节理直剪试验和PFC节理模型细观力学参数选取的深入研究提供有益的参考。周喻 MISRA A 吴顺川 张晓平 2012岩石力学与工程学报2012,31,6:56
3等效岩体随机节理三维网络模型构建方法研究显示文摘由于目前仍难以将建立的结构面网络模型直接应用于岩体力学行为的分析,构建反映节理真实空间状态的等效岩体模型是岩体工程稳定性分析与计算的研究基础。以内蒙古白云鄂博铁矿东矿为研究背景,通过现场节理测线法调查,运用概率统计理论对节理倾向、倾角、间距和迹长进行统计分析、偏差校正并建立概率分布模型。在此基础上,根据Monte Carlo随机模拟理论并采用Matlab软件,获取节理模拟数据,并通过OPEN GL三维可视化技术构建能充分反映工程岩体节理分布特征的等效岩体随机节理三维网络模型,模拟结果与实测数据具有良好的统计相似性。该研究成果可为等效岩体技术所利用,并为后续开展节理岩体变形特性、强度特性、尺寸效应、各向异性、表征单元体积(REV)、关键块体识别、开挖效应、破裂机理等量化分析研究奠定基础。吴顺川 周喻 高永涛 MISRA A 2012岩石力学与工程学报2012,31,A01:23

5Ileoscopy in patients with ileocolonic tuberculosis显示文摘AIM: To study the role of retrograde terminal ileoscopy in patients suspected to have ileocolonic tuberculosis. METHODS: A retrospective analysis was performed for patients undergoing colonoscopy for suspected ileocolonic tuberculosis between January 2000 and June 2004, in whom retrograde ileoscopy had been performed. Only patients diagnosed with tuberculosis on the basis of histological findings of either a caseating granuloma or those having non-caseating granuloma or a collection of epithelioid cells at a minimum of one endoscopic lesion (either in the colon or the terminal ileum) on histology with good responses to conventional anti-tuberculous drugs were enrolled for the study. RESULTS: Fifty-three patients were included. The terminal ileum was involved in only 11 patients. Eight of these patients had involvement of the cecum too. Two patients had ileal lesions without cecal involvement; however, lesions were noted in the ascending colon. In one patient the whole colon was normal and only the terminal ileum showed nodularity and ulceration. Histological examination of the ileal biopsies obtained from the ileal lesions showed noncaseating granulomas in five, collection of epithelioid cells in four and nonspecific histology in two patients. Mucosal biopsies obtained from the lesion in the terminal ileum, in the patient in whom mucosal abnormality was observed to be confined to the ileum, showed noncaseating granulomas. In two other patients the ileal biopsies alone showed histological evidence of tuberculosis with biopsies from the colonic lesions showing non-specific inflammatory changes only. CONCLUSION: Retrograde ileoscopy should be performed in all patients undergoing colonoscopy for suspected ileocolonic tuberculosis. In some patients only the terminal ileum may be involved and histological examination may reveal evidence of tuberculosis only from the lesions in the terminal ileum. 7Folic acid supplementation inhibits recurrence of colorectal adenomas:A randomized chemoprevention trial显示文摘AIM: To determine whether folic acid supplementation will reduce the recurrence of colorectal adenomas, the precursors of colorectal cancer, we performed a double-blind placebo-controlled trial in patients with adenomatous polyps. METHODS: In the current double-blind, placebo-controlled trial at this VA Medical Center, patients with colorectal adenomas were randomly assigned to receive either a daily 5 mg dose of folic acid or a matched identical placebo for 3 years. All polyps were removed at baseline colonoscopy and each patient had a follow up colonoscopy at 3 years. The primary endpoint was a reduction in the number of recurrent adenomas at 3 years. RESULTS: Of 137 subjects, who were eligible after confirmation of polyp histology and run-in period to conform compliance, 94 completed the study; 49 in folic acid group and 45 in placebo group. Recurrence of adenomas at 3-year was compared between the two groups. The mean number of recurrent polyps at 3-year was 0.36 (SD, 0.69) for folic acid treated patients compared to 0.82 (SD, 1.17) for placebo treated subjects, resulting in a 3-fold increase in polyp recurrence in the placebo group. Patients below 70 years of age and those with left-sided colonicadenomas or advanced adenomas responded better to folic acid supplementation. 8Value of screening endoscopy in evaluation of esophageal,gastric and colon cancers显示文摘Esophageal,gastric,and colorectal cancers are deadly diseases that continue to plague our world today.The value of screening endoscopy in evaluating these types of cancers is a critical area of discussion due to a potential reduction in morbidity and mortality.This article describes how to identify a good screening test and explains what are important criteria in the field of screening endoscopy.Furthermore,the current status and progress of screening endoscopy for esophageal,gastric,and colorectal cancer will be evaluated and discussed.Mass screening programs have not been implemented for esophageal and gastric carcinomas in those with average or low risk populations.However,studies of high-risk populations have found value and a cost-benefit in conducting screening endoscopy.Colorectal cancer,on the other hand,has had mass screening programs in place for many years due to the clear evidence of improved outcomes.As the role of endoscopy as a screening tool has continued to develop,newer technology and techniques have emerged to improve its utility.Many new image enhancement techniques and computer processing programs have shown promise and may have a significant role in the future of endoscopic screening.These developments are paving the way for improving the diagnostic and therapeutic capability of endoscopy in the field of gastroenterology.Tae H Ro Michelle A Mathew Subhasis Misra 2015World Journal of Gastroenterology2015,21,33:9

10Parenteral nanosuspensions:a brief review from solubility enhancement to more novel and specific applications显示文摘Advancements in in silico techniques of lead molecule selection have resulted in the failure of around 70% of new chemical entities(NCEs). Some of these molecules are getting rejected at final developmental stage resulting in wastage of money and resources. Unfavourable physicochemical properties affect ADME profile of any efficacious and potent molecule, which may ultimately lead to killing of NCE at final stage. Numerous techniques are being explored including nanocrystals for solubility enhancement purposes. Nanocrystals are the most successful and the ones which had a shorter gap between invention and subsequent commercialization of the first marketed product. Several nanocrystal-based products are commercially available and there is a paradigm shift in using approach from simply being solubility enhancement technique to more novel and specific applications. Some other aspects in relation to parenteral nanosuspensions are concentrations of surfactant to be used, scalability and in vivo fate. At present, there exists a wide gap due to poor understanding of these critical factors,which we have tried to address in this review. 11Helicobacter pylori and gastric cancer: Indian enigma显示文摘Helicobacter pylori(H. pylori) is a gram negative microaerophilic bacterium which resides in the mucous linings of the stomach. It has been implicated in the causation of various gastric disorders including gastric cancer. The geographical distribution and etiology of gastric cancer differ widely in different geographical regions and H. pylori, despite being labeled as a grade Ⅰ carcinogen, has not been found to be associated with gastric cancer in many areas. Studies in Asian countries such as Thailand, India, Bangladesh, Pakistan, Iran, Saudi Arabian countries, Israel and Malaysia, have reported a high frequency of H. pylori infection co-existing with a low incidence of gastric cancer. In India, a difference in the prevalence of H. pylori infection and gastric cancer has been noted even in different regions of the country leading to a puzzle when attempting to find the causes of these variations. This puzzle of H. pylori distribution and gastric cancer epidemiology is known as the Indian enigma. In this review we have attempted to explain the Indian enigma using evidence from various Indian studies and from around the globe. 12碳纤维增强固体浮力材料性能研究显示文摘使用浓H_2SO_4/浓HNO_3的混酸体系对碳纤维(CF)的表面进行处理得到氧化碳纤维(OCF)。采用超声分散和模具浇注成型法制备氧化碳纤维/空心玻璃微珠(HGMs)/环氧树脂固体浮力材料,研究了材料的密度、抗压强度、吸水率和断面形貌。使用ABAQUS数值模拟软件,建立了碳纤维增强固体浮力材料的模型,研究了碳纤维对材料应力分布的影响。结果表明,碳纤维经过氧化后,表面形成了羟基和羧基等基团,提高了自身与环氧树脂的界面结合状态。随着氧化碳纤维含量的增加,复合材料的占空比逐渐提高,但密度变化不大,而其抗压强度呈现先升高后降低的趋势。当碳纤维含量为5%(质量分数)时,抗压强度达41MPa,提高约13.8%。固体浮力材料吸水率随碳纤维含量增加而提高,但所有试样的吸水率均小于2%。复合材料模型受压后的应力云图表明,碳纤维能够有效代替固体浮力材料基体承载很大一部分载荷,空心玻璃微珠球壳所受应力降低,减少了空心玻璃微珠破碎和裂纹源的产生,从而对固体浮力材料起到增强作用。王耀声 亚斌 周秉文 孟令刚 Devesh Misra 张兴国 2018功能材料2018,49,8:5
13Treatment of wheat straw using tannase and white-rot fungus to improve feed utilization by ruminants显示文摘Background: Current research to enrich cattle feed has primarily focused on treatment using white rot fungi, while there are scarce reports using the enzyme tannase, which is discussed only in reviews or in the form of a hypothesis. In this context, the aim of the present study was to evaluate the effect of tannase on wheat straw(WS)and also the effect of lyophilized tannase at concentrations of 0.1%, 0.2%, and 0.3%(w/w) on WS followed by fermentation with Ganoderma sp. for 10 d and compared in relation to biochemical parameters, crude protein(CP)content, and nutritional value by calculating the C/N ratio in order to improve the nutritional value of cattle feed.Results: Penicillium chorlesii, a tannase-producing microorganism, produced 61.4 lU/mL of tannase in 54 h when 2%(w/v) tannic acid(TA) was initially used as a substrate in medium containing(% w/v) sucrose(1.0), NaN03(1.0), and MgS〇4(0.08 pH, 5.0) in a 300-L fermentor(working volume 220 L), and concomitantly fed with 1.0%(w/v) TA after24 h. The yield of partially purified and lyophilized tannase was 5.8 lU/mg. The tannin-free myco-straw at 0.1%(w/w) tannase showed 37.8%(w/w) lignin degradation with only a 20.4%(w/w) decrease in cellulose content and the in vitro feed digestibility was 32.2%. 16Carcinogenic Helicobacter pylori in gastric pre-cancer and cancer lesions: Association with tobacco-chewing显示文摘AIM:To investigate the low gastric cancer incidence rate relative to the highly prevalent Helicobacter pylori(H.pylori)infection;data relevant to H.pylori infection during gastric carcinogenesis in Indian patients is currently lacking.METHODS:The present study examines the prevalence of H.pylori infection in DNA derived from 156endoscopic gastric biopsies of different disease groups that represent gastric pre-cancer[intestinal metaplasia(n=15),dysplasia(n=15)],cancer[diffuse adenocarcinoma(n=44),intestinal adenocarcinoma(n=21)],and symptomatic but histopathologically-normal controls(n=61).This was done by generic ureC polymerase chain reaction(PCR)and cagA-specific PCR that could specifically identify the carcinogenic H.pylori strain.RESULTS:Our analysis showed the presence of H.pylori infection in 61%of symptomatic histopathologically-normal individuals,however only 34%of control tissues were harboring the cagA+H.pylori strain.A similar proportion of H.pylori infection(52%)and cagA(26%)positivity was observed in the tumor tissue of the gastric cancer group.In comparison,H.pylori infection(90%)and cagA positivity(73%)were the highest in gastric pre-cancer lesions.In relation to tobacco and alcohol abuse,H.pylori infection showed an association with tobacco chewing,whereas we did not observe any association between tobacco smoking or alcohol abuse with prevalence of H.pylori infection in the tissue of any of the patient groups studied.CONCLUSION:High incidence of H.pylori infection and carcinogenic cagA positive strain in pre-cancer lesions during gastric carcinogenesis may beArvind Pandey Satyendra Chandra Tripathi Sutapa Mahata Kanchan Vishnoi Shirish Shukla Sri Prakash Misra Vatsala Misra Suresh Hedau Ravi Mehrotra Manisha DwivediZ Alok C Bharti 2014World Journal of Gastroenterology2014,20,22:4
17Low temperature friction stir welding of P91 steel显示文摘Bead-on-plate friction stir welds were made on P91 alloy with low and high rotational speeds(100 and 1000 RPM) to study their effects on weld microstructural changes and impression creep behavior. Temperatures experienced by the stir zone were recorded at the weld tool tip. Different zones of welds were characterized for their microstructural changes, hardness and creep behavior(by impression creep tests). The results were compared with submerged arc fusion weld. Studies revealed that the stir zone temperature with 100 RPM was well below A_(c1) temperature of P91 steel while it was above A_(c3) with 1000 RPM.
