| 1 | Hepatocellular carcinoma surveillance:An evidence-based approach显示文摘Hepatocellular carcinoma(HCC) makes up 75%-85% of all primary liver cancers and is the fourth most common cause of cancer related death worldwide. Chronic liver disease is the most significant risk factor for HCC with 80%-90% of new cases occurring in the background of cirrhosis. Studies have shown that early diagnosis of HCC through surveillance programs improve prognosis and availability of curative therapies. All patients with cirrhosis and high-risk hepatitis B patients are at risk for HCC and should undergo surveillance. The recommended surveillance modality is abdominal ultrasound(US) given that it is cost effective and noninvasive with good sensitivity. However, US is limited in obese patients and those with non-alcoholic fatty liver disease(NAFLD). With the current obesity epidemic and rise in the prevalence of NAFLD, abdominal computed tomography or magnetic resonance imaging may be indicated as the primary screening modality in these patients. The addition of alpha-fetoprotein to a surveillance regimen is thought to improve the sensitivity of HCC detection.Further investigation of serum biomarkers is needed. Semiannual screening is the suggested surveillance interval. | 3 | Effect of pipe characteristics in umbrella arch method on controlling tunneling-induced settlements in soft grounds显示文摘Recent developments in tunneling have stimulated design practitioners to more effectively utilize the underground spaces.However,tunneling at shallow depth in soft grounds gives rise to concerns associated with tunnel instability.Umbrella arch method(UAM),as a pre-reinforcement approach of tunnels in complex geological conditions,is widely used to maintain the tunnel stability.Quantitative assessment of the impacts of the entire approach and forepoling pipe features on tunnel stability remains challenging due to the complex nature of the UAM application.This study aimed to assess the effect of pipe design parameters on reinforcing the tunnels excavated in soft grounds.This practical investigation considered the actual field conditions attributed to the tunneling procedure and UAM deployment.Then,the tunneling process was modeled and the tunnel excavation-induced settlements were calculated.The post-processed results confirmed that deploying the UAM substantially reduced the tunnel crown and ground surface settlements by 76%and 42%,respectively.Investigation of various design parameters of pipes underscored the significance of incorporating the optimum value for each individual parameter into design schemes to more effectively control the settlements.Additionally,contrasting the settlement reduction rates(SRRs)for pipe design variables showed that the tunnel stability is more sensitive to the changes in the values of diameter and length,compared to values of the installation angle and center-tocenter distance of the pipes. | Ali Morovatdar Massoud Palassi Reza S.Ashtiani | 2020 | Journal of Rock Mechanics and Geotechnical Engineering2020,12,5: | 6 |
| 4 | Clinical outcomes of tricuspid valve repair accompanying left-sided heart disease显示文摘AIM To determine whether the need for additional tricuspid valve repair is an independent risk factor when surgery is required for a left-sided heart disease.METHODS One hundred and eighty patients(68 ± 12 years,79 males) underwent tricuspid annuoplasty.Cox proportionalhazards regression model for multivariate analysis was performed for variables found significant in univariate analyses.RESULTS Tricuspid regurgitation etiology was functional in 154 cases(86%),organic in 16 cases(9%),and mixed in10 cases(6%),respectively.Postoperative mortality at 30 days was 11.7%.Mean follow-up was 51.7 mo with survival at 5 years of 73.5%.Risk factors for mortality were acute endocarditis [hazard ratio(HR) = 9.22(95%CI:2.87-29.62),P < 0.001],ischemic heart disease requiring myocardial revascularization [HR = 2.79(1.26-6.20),P = 0.012],and aortic valve stenosis [HR = 2.6(1.15-5.85),P = 0.021].Significant predictive factors from univariate analyses were double-valve replacement combined with tricuspid annuloplasty [HR = 2.21(1.11-4.39),P = 0.003] and preoperatively impaired ejection fraction [HR = 1.98(1.04-3.92),P = 0.044].However,successful mitral valve repair showed a protective effect [HR = 0.32(0.10-0.98),P = 0.046].Additionally,in instances where tricuspid regurgitation required the need for concomitant tricuspid valve repair,mortality predictor scores such as Euroscore 2 could be shortened to a simple Euroscore-tricuspid comprised of only 7 inputs.The explanation may lie in the fact that significant tricuspid regurgitation following leftsided heart disease represents an independent risk factor encompassing several other factors such as pulmonary arterial hypertension and dyspnea.CONCLUSION Tricuspid annuloplasty should be used more often as a concomitant procedure in the presence of relevant tricuspid regurgitation,although it usually reveals an overly delayed correction of a left-sided heart disease. | Kasra Azarnoush Ahmad S Nadeemy Bruno Pereira Massoud A Leesar Céline Lambert Alaa Azhari Vedat Eljezi Nicolas Dauphin Etienne Geoffroy Lionel Camilleri | 2017 | World Journal of Cardiology2017,9,10: | 4 |

| 7 | Tumor shrinkage by cyclopamine tartrate through inhibiting hedgehog signaling显示文摘The link of hedgehog (Hh) signaling activation to human cancer and synthesis of a variety of Hh signaling inhibitors raise great expectation that inhibiting Hh signaling may be effective in human cancer treatment. Cyclopamine (Cyc), an alkaloid from the Veratrum plant, is a specific natural product inhibitor of the Hh pathway that acts by targeting smoothened (SMO) protein. However, its poor solubility, acid sensitivity, and weak potency relative to other Hh antagonists prevent the clinical development of Cyc as a therapeutic agent. Here, we report properties of cyclopamine tartrate salt (CycT) and its activities in Hh signaling-mediated cancer in vitro and in vivo. Unlike Cyc, CycT is water soluble (5-10 mg/mL). The median lethal dose (LD50) of CycT was 62.5 mg/kg body weight compared to 43.5 mg/kg for Cyc, and the plasma half-life (T1/2) of CycT was not significantly different from that of Cyc. We showed that CycT had a higher inhibitory activity for Hh signaling-dependent motor neuron differentiation than did Cyc (IC50 = 50 nmol/L for CycT vs. 300 nmol/L for Cyc). We also tested the antitumor effectiveness of these Hh inhibitors using two mouse models of basal cell carcinomas (K14cre:Ptch1neo/neo and K14cre:SmoM2YFP). After topical application of CycT or Cyc daily for 21 days, we found that all CycT-treated mice had tumor shrinkage and decreased expression of Hh target genes. First, we discuss the algebraic expressions and the corresponding properties of clocked power signals. Then the design procedure is summed up for converting complementary CMOS logic gates employing DC power to the power-clocked CMOS gates employing AC power. On this basis, the design of differential cascode voltage switch logic (DCVSL) circuits employing AC power clocks is proposed. The PSPICE simulations using a sinusoidal power-clock demonstrate that the designed power-clocked DCVSL circuit has a correct logic function and low power characteristics. | 10 | Competing Risks Data Analysis with High-dimensional Covariates:An Application in Bladder Cancer显示文摘Analysis of microarray data is associated with the methodological problems of high dimension and small sample size. Various methods have been used for variable selection in highdimension and small sample size cases with a single survival endpoint. However, little effort has been directed toward addressing competing risks where there is more than one failure risks. This study compared three typical variable selection techniques including Lasso, elastic net, and likelihood-based boosting for high-dimensional time-to-event data with competing risks. The performance of these methods was evaluated via a simulation study by analyzing a real dataset related to bladder cancer patients using time-dependent receiver operator characteristic(ROC) curve and bootstrap.632+ prediction error curves. The elastic net penalization method was shown to outperform Lasso and boosting. Based on the elastic net, 33 genes out of 1381 genes related to bladder cancer were selected. By fitting to the Fine and Gray model, eight genes were highly significant(P < 0.001). Among them, expression of RTN4, SON, IGF1 R, SNRPE, PTGR1, PLEK, and ETFDH was associated with a decrease in survival time, whereas SMARCAD1 expression was associated with an increase in survival time. | 11 | A Controller-Based Architecture for Information Centric Network Construction and Topology management显示文摘Information-Centric Networking(ICN) has recently emerged as a result of the increased demand to access contents regardless of their location in the network services. This new approach facilitates content distribution as a service of the network with lower delay and higher security in comparison with the current IP network. Applying ICN in current IP infrastructure leads to major complexities. One approach to deploy ICN with less complexity is to integrate ICN with Software Defined Networking(SDN). The SDN controller manages the content distribution, caching, and routing based on the users' requests. In this paper, we extend these context by addressing the ICN topology management problem over the SDN network to achieve an improved user experience as well as network performance. In particular, a centralized controller is designed to construct and manage the ICN overlay. | 14 | Methanolic extract of African mistletoe(Viscum album) improves carbohydrate metabolism and hyperlipidemia in streptozotocin-induced diabetic rats显示文摘Objective:To justify the use of African mistletoe(AM) Viscum album {V.album) in folkoric medicine to treat diabetes.Methods:In one experiment,the fasting blood glucose(FBG) leveb of diabetic rats were monitored for 4 h.Diabetic rats were treated with AM at doses of 50 mg/kg(AM1) and 100 mg/kg(AM2),glibenclamide(GB)(positive control) and saline solution(SS).In another experiment,diabetic rats were treated with AM2,GB and SS daily for 3 weeks.Results:AM1 and AM2 elicited significant(P<0.05) hypoglycaemic effects within 4 h of extract administration. AM1 and AM2 decreased the FBG by 41%and 49%,respectively,at 2 h.AM2 was found to lower FBC by 51%,relative to baseline,which was comparable to GB at 3 h.In the second experiment, AM2 and GB significantly(P<0.05) decreased the FBG by 34%and 51%,respectively.This was followed by marked decrease in levels of HbAlC in AM2- and GB- treated diabetic rats.AM2 significantly(P<0.05) decreased the STZ-induced increase in levels of serum triglyceride,urea, lactate dehydrogenase,α-amylase and low-density lipoprotein-cholesterol.Furthermore, diabetic rats treated with AM2 had significanlly(P<0.05) elevated high-density lipoprotein-cholesterol. | 15 | New Insights into Intravascular Imaging of Coronary Bifurcation Lesions and Left Main Stenosis:What Have We Accomplished?显示文摘Coronary bifurcation lesions(CBL)are one of the most challenging lesions to treat in interventional cardiology.Intravascular imaging including intravascular ultrasound(IVUS)and optical coherence tomography(OCT)is paramount for the assessment of lesion characteristics and suboptimal stent results and to improve the outcome of percutaneous coronary intervention(PCI).After upgrading the use of IVUS and OCT by the American College of Cardiology/American Heart Association guidelines,their use has increased.Likewise,there has been significant improvement in image quality and the profile of system(ie,IVUS catheter size).The main indication for the use of IVUS includes:assessment of lesion characteristics,optimization of stent deployment,optimal stent expansion,exclusion of plaque burden>50%,and edge dissection.Intravascular imaging plays a significant role in the assessment and treatment of left main(LM)coronary artery stenosis and CBL.In particular,the use of intravascular imaging for alleviating mechanical issues during stenting and managing stent thrombosis has significantly increased.Overall,a compelling body of evidence has shown that the use of IVUS or OCT during PCI leads to optimal stenting results and mitigates the risk of adverse cardiac events.However,to date,the American College of Cardiology/American Heart Association Guidelines have assigned a Class IIb recommendation for IVUS or OCT,while the European Society of Cardiology Guidelines upgraded it to the Class IIa recommendation.This review summarized the role of IVUS and OCT for the assessment of stenosis and stent optimization in patients with CBL and LM stenosis.In addition,we discuss new insights into the role of IVUS-and OCT-guided stenting in patients with CBL and LM stenosis provided by the ongoing randomized trials. | Massoud A.Leesar Gregory O.Von Mering Hani Jneid | 2022 | Cardiology Discovery2022,2,3: | 2 |
