1带血管肋间神经移位与骶神经根选择性束间吻接行截瘫患者尿便功能重建(英文)显示文摘背景:T12以上脊髓节段损伤截瘫,由于失去了与高级中枢的联系而无自立排尿排便功能。目的:采用带血管肋间神经移位与S2~4神经根行选择性束间吻接重建截瘫患者的尿便功能。设计:自身前后对照观察。对象:选择1990-01/2000-12解放军第二军医大学长海医院骨科T9~L2平面外伤性截瘫患者30例。截瘫平面T9~T11者17例,T12~L2者13例。全部患者均已施行过椎板减压内固定等手术,其中24例已再次手术取出内固定。单位:解放军第二军医大学长海医院骨科。方法:切取患者截瘫平面以上的两组正常的肋间神经及伴行肋间动、静脉(一般为7,8或9,10肋间神经)。T11以上者于锁骨中线将其切断,结扎远端后将近端游离至肋提肌外缘处,经肌下遂道转移至椎管内。取相应长度的腓肠神经剪成2段,一端与肋间神经端端束外膜联合缝合,另一端分开束组后与部分切断的S2~4神经根行选择性束间缝合;脊髓损伤平面为T11以下者,则于腹壁切口切取第10,11肋间神经或肋下神经,经侧腹壁隧道转移至腰部,用移植的腓肠神经与S1,2平面后路显露并选择性切断部分神经束的S2~4神经根相桥接。术后1,3,6,12,24个月及随访时评定患者的尿便功能;术前、术后进行尿流动力学检查。主要观察指标:①患者尿便功能评定结果。②患者尿流动力学检查。结果:30例患者,平均随访5年,均进入结果分析。①患者尿便功能评定结果:26例(86.6%)排尿、排便感觉恢复,23例(76.7%)恢复了排尿反射,19(63%)逼尿肌和尿道括约肌主动收缩功能恢复。②患者尿流动力学检查:术后最大尿流率、剩余尿量和逼尿肌最大收缩压均较术前有明显改善[(12.0±3.0)比(2.0±0.3)mL/s,(80±12)比(150±30)mL,(11.76±3.43)比(5.88±1.47)kPa,P<0.05];术后低顺应性9例,逼尿肌无反射7例;均较术前人数显著减少(术前依次为26,27例)。结论:采用带血管肋间神经移位与S2,3,4神经根行选择性束间吻接可重建部分截瘫患者的排尿排便功能和臀、会阴和外阴部感觉。张少成 马玉海 Johnson Laurance 王志伟 瞿创予 张振伟 胡玉华 张传森 党瑞山 张秋林 2006中国临床康复2006,10,17:4
