| 2 | Femtojoule electro-optic modulation using a silicon– organic hybrid device显示文摘Energy-efficient electro-optic modulators are at the heart of short-reach optical interconnects,and silicon photonics is considered the leading technology for realizing such devices.However,the performance of all-silicon devices is limited by intrinsic material properties.In particular,the absence of linear electro-optic effects in silicon renders the integration of energy-efficient photonic–electronic interfaces challenging.Silicon–organic hybrid(SOH)integration can overcome these limitations by combining nanophotonic silicon waveguides with organic cladding materials,thereby offering the prospect of designing optical properties by molecular engineering.In this paper,we demonstrate an SOH Mach–Zehnder modulator with unprecedented efficiency:the 1-mm-long device consumes only 0.7 fJ bit^(-1) to generate a 12.5 Gbit s^(-1) data stream with a bit-error ratio below the threshold for hard-decision forward-error correction.This power consumption represents the lowest value demonstrated for a non-resonant Mach–Zehnder modulator in any material system.It is enabled by a novel class of organic electro-optic materials that are designed for high chromophore density and enhanced molecular orientation.The device features an electro-optic coefficient of r33<180 pm V^(-1) and can be operated at data rates of up to 40 Gbit s^(-1). | Sebastian Koeber Robert Palmer Matthias Lauermann Wolfgang Heni Delwin L Elder Dietmar Korn Markus Woessner Luca Alloatti Swen Koenig Philipp C Schindler Hui Yu Wim Bogaerts Larry R Dalton Wolfgang Freude Juerg Leuthold Christian Koos | 2015 | Light(Science & Applications)2015，4，1： | 4 |

| 4 | Nephropathy in dietary hyperoxaluria:A potentially preventable acute or chronic kidney disease显示文摘Hyperoxaluria can cause not only nephrolithiasis and nephrocalcinosis,but also renal parenchymal disease histologically characterized by deposition of calcium oxalate crystals throughout the renal parenchyma,profound tubular damage and interstitial inflammation and fibrosis.Hyperoxaluric nephropathy presents clinically as acute or chronic renal failure that may progress to endstage renal disease(ESRD).This sequence of events,well recognized in the past in primary and enteric hyperoxalurias,has also been documented in a few cases of dietary hyperoxaluria.Estimates of oxalate intake in patients with chronic dietary hyperoxaluria who developed chronic kidney disease or ESRD were comparable to the reported average oxalate content of the diets of certain populations worldwide,thus raising the question whether dietary hyperoxaluria is a primary cause of ESRD in these regions.Studies addressing this question have the potential of improving population health and should be undertaken,alongside ongoing studies which are yielding fresh insights into the mechanisms of intestinal absorption and renal excretion of oxalate,and into the mechanisms of development of oxalate-induced renal parenchymal disease.Novel preventive and therapeutic strategies for treating all types of hyperoxaluria are expected to develop from these studies. | Robert H Glew Yijuan Sun Bruce L Horowitz Konstantin N Konstantinov Marc Barry Joanna R Fair Larry Massie Antonios H Tzamaloukas | 2014 | World Journal of Nephrology2014，3，4： | 3 |

| 7 | Lung cancer screening: Should we be excluding people with previous malignancy?显示文摘The National Lung Screening Trial(NLST) was a large,randomized, controlled study showing a 20% reduction of lung cancer mortality and 7% reduction of all cause mortality using annual low dose computed tomography(LDCT) in a high risk population. NLST excluded people with a previous history of cancer treatment within the past 5 years and all people with a history lung cancer. The aim of this work is to review how lung cancer screening trials addressed the confounding effect of previous malignancy. We also review the subsequent recommendations by the United States Preventative Task Force Services, multiple professional societies and the Center for Medicaid and Medicare Services which defer either to NLST criteria or, clinician judgment or refrain from asserting any recommendation on the topic, respectively. Implications of lung cancer screening in the setting of previous malignancies, specifically lung, head and neck, esophageal, gastric, breast, colorectal cancer and lymphoma are also discussed. With lung cancer screening, an antecedent malignancy introduces the possibility of discovering metastasis as well as lung cancer. In some circumstances diagnosis and treatment of oligometastatic disease may confer a survival benefit. The survival benefit of treating either lung cancer or oligometastatic disease as result of lung cancer screening has yet to be determined.
