1Extensively drug-resistant bacteria are an independent predictive factor of mortality in 130 patients with spontaneous bacterial peritonitis or spontaneous bacteremia显示文摘AIM: To evaluate the epidemiology and outcomes of culture-positive spontaneous bacterial peritonitis(SBP) and spontaneous bacteremia(SB) in decompensated cirrhosis.METHODS: We prospectively collected clinical, laboratory characteristics, type of administered antibiotic, susceptibility and resistance of bacteria to antibiotics in one hundred thirty cases(68.5% males) with positive ascitic fluid and/or blood cultures during the period from January 1, 2012 to May 30, 2014. All patients with SBP had polymorphonuclear cell count in ascitic fluid > 250/mm3. In patients with SB a thorough study did not reveal any other cause of bacteremia. The patients were followed-up for a 30-d periodfollowing diagnosis of the infection. The final outcome of the patients was recorded in the end of follow-up and comparison among 3 groups of patients according to the pattern of drug resistance was performed.RESULTS: Gram-positive-cocci(GPC) were found in half of the cases. The most prevalent organisms in a descending order were Escherichia coli(33), Enterococcus spp(30), Streptococcus spp(25), Klebsiella pneumonia(16), S. aureus(8), Pseudomanas aeruginosa(5), other Gram-negative-bacteria(GNB)(11) and anaerobes(2). Overall, 20.8% of isolates were multidrug-resistant(MDR) and 10% extensively drugresistant(XDR). Health-care-associated(HCA) and/or nosocomial infections were present in 100% of MDR/XDR and in 65.5% of non-DR cases. Meropenem was the empirically prescribed antibiotic in HCA/nosocomial infections showing a drug-resistance rate of 30.7% while third generation cephalosporins of 43.8%. Meropenem was ineffective on both XDR bacteria and Enterococcus faecium(E. faecium). All but one XDR were susceptible to colistin while all GPC(including E. faecium) and the 86% of GNB to tigecycline. Overall 30-d mortality was 37.7%(69.2% for XDR and 34.2% for the rest of the patients)(log rank, P = 0.015). In multivariate analysis, factors adversely affecting outcome included XDR infection(HR = 2.263, 95%CI: 1.005-5.095, P = 0.049), creatinine(HR = 1.125, 95%CI: 1.024-1.236, P = 0.015) and INR(HR =1.553, 95%CI: 1.106-2.180, P = 0.011).CONCLUSION: XDR bacteria are an independent lifethreatening factor in SBP/SB. 2Genetic Diversity and Structure of Tunisian Local Pear Germplasm as Revealed by SSR Markers显示文摘Growing pear has a long tradition in Tunisia, and numerous local cultivars possessing an excellent adaptability and resilience potential to climatic variation are present. This large adaptability is associated with an important genetic diversity, which is threatened to erosion.Appropriate measures have to be taken in order to properly evaluate and conserve this local material. Microsatellite markers were used to assess the level of genetic diversity among Tunisian pear germplasm, and compare it with some European varieties and wild pear species. 61 pear accessions representing eight groups(six groups from Tunisia, one from Northern Europe and another group composed of wild pear) have been genotyped using SSR markers derived from apple and pear. The pear accessions showed a significant polymorphism and 95 polymorphic alleles were found. The number of alleles per locus varied from 5 for CH04e03 locus to 14 for CH01d09 locus with an average of 9.4 alleles per locus.Moreover, the mean gene diversity(H_e) per locus ranged from 0.192 to 0.752. Genetic distance values and cluster analyses revealed high genetic similarities among the Tunisian groups. Factorial correspondence analysis(FCA) categorized the accessions into three independent groups where Tunisian local accessions agglomerated together distantly from European and wild pear accessions. Additionally, UPGMA dendrogram grouped accessions into two clusters, confirmed thereafter by the Bayesian model-based Structure analysis. The results showed 16 putative triploid accessions found in the local germplasm. 3Human immunodeficiency virus and hepatotropic viruses comorbidities as the inducers of liver injury progression显示文摘Hepatotropic viruses induced hepatitis progresses much faster and causes more liver-related health problems in people co-infected with human immunodeficiency virus(HIV). Although treatment with antiretroviral therapy has extended the life expectancy of people with HIV, liver disease induced by hepatitis B virus(HBV) and hepatitis C virus(HCV) causes significant numbers of non-acquired immune deficiency syndrome(AIDS)-related deaths in coinfected patients. In recent years, new insights into the mechanisms of accelerated fibrosis and liver disease progression in HIV/HCV and HIV/HBV co-infections have been reported. In this paper, we review recent studies examining the natural history and pathogenesis of liver disease in HIV-HCV/HBV co-infection in the era of direct acting antivirals(DAA) and antiretroviral therapy(ART). 4Contrast-enhanced ultrasonography parameters in neural network diagnosis of liver tumors显示文摘AIM:To study the role of time-intensity curve(TIC) analysis parameters in a complex system of neural networks designed to classify liver tumors.METHODS:We prospectively included 112 patients with hepatocellular carcinoma(HCC)(n = 41),hypervascular(n = 20) and hypovascular(n = 12) liver metastases,hepatic hemangiomas(n = 16) or focal fatty changes(n = 23) who underwent contrast-enhanced ultrasonography in the Research Center of Gastroenterology and Hepatology,Craiova,Romania.We recorded full length movies of all contrast uptake phases and post-processed them offline by selecting two areas of interest(one for the tumor and one for the healthy surrounding parenchyma) and consecutive TIC analysis.The difference in maximum intensities,the time to reaching them and the aspect of the late/portal phase,as quantified by the neural network and a ratio between median intensities of the central and peripheral areas were analyzed by a feed forward back propagation multi-layer neural network which was trained to classify data into five distinct classes,corresponding to each type of liver lesion.RESULTS:The neural network had 94.45% training accuracy(95% CI:89.31%-97.21%) and 87.12% testing accuracy(95% CI:86.83%-93.17%).The automatic classification process registered 93.2% sensitivity,89.7% specificity,94.42% positive predictive value and 87.57% negative predictive value.The artificial neural networks(ANN) incorrectly classified as hemangyomas three HCC cases and two hypervascular metastases,while in turn misclassifying four liver hemangyomas as HCC(one case) and hypervascular metastases(three cases).Comparatively,human interpretation of TICs showed 94.1% sensitivity,90.7% specificity,95.11% positive predictive value and 88.89% negative predictive value.The accuracy and specificity of the ANN diagnosis system was similar to that of human interpretation of the TICs(P = 0.225 and P = 0.451,respectively).Hepatocellular carcinoma cases showed contrast uptake during the arterial phase followed by wash-out in the portal and first seconds of the late phases.For the hypovascular metastases did not show significant contrast uptake during the arterial phase,which resulted in negative differences between the maximum intensities.We registered wash-out in the late phase for most of the hypervascular metastases.Liver hemangiomas had contrast uptake in the arterial phase without agent wash-out in the portallate phases.The focal fatty changes did not show any differences from surrounding liver parenchyma,resulting in similar TIC patterns and extracted parameters.CONCLUSION:Neural network analysis of contrastenhanced ultrasonography-obtained TICs seems a promising field of development for future techniques,providing fast and reliable diagnostic aid for the clinician.Costin Teodor Streba Mihaela Ionescu Dan Ionut Gheonea Larisa Sandulescu Tudorel Ciurea Adrian Saftoiu Cristin Constantin Vere Ion Rogoveanu 2012World Journal of Gastroenterology2012,18,32:11
5引进海岛棉种质的SSR遗传多样性分析显示文摘本文利用95对SSR分子标记多对来自于前俄罗斯的33分海岛棉,以及5份来自埃及,1份来自美国,1份来自阿尔巴尼亚,9份来自新疆,7份来自国内云南,江苏,等地共56份海岛棉进行遗传多样性分析,结果如下:95对SSR引物在56个海岛棉品种中共测出了384个等位基因,其中多态性等位基因296个,占77.1%。每对引物等位基因变幅是2~9个,平均为4.1个。基因型多样性指数(H)在0.035~2.931之间,平均为0.879。Shannon信息指数(I)在0.09~4.417之间,平均为1.363 。各指数的趋势一致。95对SSR引物的多态性信息含量PIC变幅为0.035~ 0. 926,平均为0.698。56份海岛棉品种两两间的相似系数,分布在0.585~0.952之间,主要集中在0.6~ 0.8之间,占整个数据的97%,平均遗传相似系数为0.7。从整体上来看,56份海岛棉的遗传相似系数较高.从聚类图上可以看出,以遗传距离为0.69为标准,56份海岛棉品种可分为4类。第1类主要是前苏联海岛棉为主的27份品种,其中来自埃及的吉扎77和来自 美国的派字棉以及中239都归到了这一类。第二类稍微复杂,但主要是以来自新疆的6份海岛棉和来自埃及的吉扎系列的4份为主。第3类包括8个品种,主要是来自于前苏联的6个品种和来自于新疆的巴3116和河北的冀B91-45.第4类只有一个来自于前苏联的L-8007,独自成一类。研究结果再次说明SSR 作为一种共显性标记,尤为适用于海岛棉及其亲本的系谱分析及鉴定。因而, SSR标记技术可以为海岛棉育种实践工作中的亲本选配,提供可靠的分子水平上的依据。潘兆娥 何守朴 贾银华 Larisa P.Podolnaya 孙君灵 王立如 杜雄明 2014植物遗传资源学报2014,15,2:10

7Real-time sono-elastography in the diagnosis of diffuse liver diseases显示文摘AIM: To analyze whether computer-enhanced dynamic analysis of elastography movies is able to better characterize and differentiate between different degrees of liver fibrosis. METHODS: The study design was prospective. A total of 132 consecutive patients with chronic liver diseases and healthy volunteers were examined by transabdominal ultrasound elastography. All examinations were done by two doctors.RESULTS: Due to the limitations of the method, we obtained high-quality elastography information in only 73.48% of the patients. The κ-means clustering method was applied to assess the inter-observer diagnosis varia- bility, which showed good variability values in accordance with the experience of ultrasound examination of every observer. Cohen’sκ test indicated a moderate agreement between the study observers (κ = 0.4728). Furthermore, we compared the way the two observers clustered the patients, using the test for comparing two proportions (t value, two-sided test). There was no statistically significant difference between the two physicians, regardless of the patients’ real status. 8基于随机森林特征变量优化的湿地植物分类与密度反演显示文摘以长江口滨海湿地为研究区域,采用随机森林算法对滨海湿地植被进行分类。在提取Landsat-8 OLI影像植被指数和水体指数的基础上,提出利用植被指数季节差值对模型进行特征变量优化,分析了长江口滨海湿地植物群落分布的空间特征。以所占面积最大的互花米草(入侵物种)为例,采用多元线性回归模型结合实地测量数据,估算了秋季的互花米草植物密度的空间特征。提出的多时相遥感数据结合随机森林特征变量优化方法,可以较为便捷地提取长江口湿地3种优势物种的空间分布特征,与最大似然法相比,分类精度有较大提高,总体分类精度由78.35%提高至87.55%,Kappa系数由0.72提高至0.84。该方法适用于存在“异物同谱”问题的湿地植物群落研究。刘曙光 董行 娄厦 DORZHIEVNA Radnaeva Larisa NIKITINA Elena 2021同济大学学报(自然科学版)2021,49,5:8
9长江口近岸地区抗生素抗性基因与微生物群落分布特征显示文摘由于抗生素的大量使用,环境中微生物对抗生素的抗性不断增加,抗生素抗性基因(ARGs)问题越来越严重,严重威胁生态安全和人类健康.为研究长江口近岸地区水体和底泥沉积物中的ARGs和微生物群落的分布特征,通过野外调查采集了8个站点的水样和沉积物样本,对2种磺胺类抗性基因(sul1、sul2)、6种四环素类抗性基因(tetM、tetC、tetX、tetA、tetO、tetQ)、1种整合子基因intI1、16S rRNA基因和微生物群落进行检测分析.结果表明,长江口近岸地区10种抗性基因的检出率为100%.其中,整合子基因intI1和水样中多种ARGs呈显著正相关关系.变形菌门(Proteobacteria)和拟杆菌门(Bacteroidota)是长江口近岸地区水环境中的优势菌门;Chloroplast为水体中的主要菌属,Chloroplast和Nitrospira为沉积物中的主要菌属.在水体中,硝化螺旋菌门(Nitrospirota)是4种四环素类抗性基因(tetX、tetA、tetO和tetQ)共同的潜在宿主;在沉积物中,Sva0485是sul1和intI1的共同潜在宿主.微生物群落的分布是长江口近岸地区ARGs迁移转化的重要影响因素.徐秋鸿 刘曙光 娄厦 Larisa Dorzhievna Radnaeva Elena Nikitina Makhinov Aleksei Nikolavich Araruna José Tavares 金宇辰 李鑫 2023环境科学2023,44,1:8

12Liver as a target of human immunodeficiency virus infection显示文摘Liver injury is a characteristic feature of human immunodeficiency virus(HIV) infection, which is the second most common cause of mortality in HIV-infected patients. Now it is recognized that liver plays a key role in HIV infection pathogenesis. Antiretroviral therapy(ART), which suppresses HIV infection in permissive immune cells, is less effective in hepatocytes, thereby making these cells a silent reservoir of HIV infection. In addition to direct hepatotoxic effects of HIV, certain ART treatment modalities provide hepatotoxic effects. The exact mechanisms of HIV-triggered chronic hepatitis progression are not elucidated, but the liver is adversely affected by HIV-infection and liver cells are prominently involved in HIV-elicited injury. These effects are potentiated by second hits like alcohol. 16Meridional movement of northern and southern equatorial ionization anomaly crests in the East-Asian sector during 2002-2003SSW显示文摘The present paper investigates the asymmetrical variability of the location of the north and the south equatorial ionization anomaly(EIA) crests in the East-Asian sector,along with their association with simultaneous observations of equatorial electrojet(EEJ) strength,geomagnetic activity index,and solar flux index during the 2002-2003 sudden stratospheric wanning(SSW) event.Analysis of these observations indicates the existence of a large-scale quasi 16-day periodic meridional movement in both EIA crests,and also reveals a strong correlation between the quasi 16-day oscillation in geomagnetic latitudes of the EIA crest and EEJ strength.The latitude of the northern/southern EIA crest and the EEJ strength indicate that obvious synchronous periodic oscillations were in-phase in the northern and southern hemisphere when the SSW occurred.In addition,it is also found that both the EIA crest location and amplitude of the periodic movement of the EIA locations exhibit hemispheric asymmetry.The amplitude of the periodic movement of the EIA location in the southern hemisphere is larger than that of the northern hemisphere,and the southern EIA crest is further off from the equator than the north one.Understanding these asymmetries requires a combination of mechanisms that involve at least trans-equator meridional winds and the position of a sub-solar point;however,potential disturbances in neutral winds associated with the SSW may additionally complicate the equatorial ionospheric dynamics.MO XiaoHua ZHANG DongHe Larisa GONCHARENKO ZHANG ShunRong HAO YongQiang XIAO Zuo PEI JiaZheng Akimasa YOSHIKAWA CHAU HaDuyen 2017Science China Earth Sciences2017,60,4:3

18Yin and Yang of mesenchymal stem cells and aplastic anemia显示文摘Acquired aplastic anemia(AA) is a bone marrow failure syndrome characterized by peripheral cytopenias and bone marrow hypoplasia. It is ultimately fatal without treatment, most commonly from infection or hemorrhage. Current treatments focus on suppressing immune-mediated destruction of bone marrow stem cells or replacing hematopoietic stem cells(HSCs) by transplantation. Our incomplete understanding of the pathogenesis of AA has limited development of targeted treatment options. Mesenchymal stem cells(MSCs) play a vital role in HSC proliferation; they also modulate immune responses and maintain an environment supportive of hematopoiesis. Some of the observed clinical manifestations of AA can be explained by mesenchymal dysfunction. MSC infusions have been shown to be safe and may offer new approaches for the treatment of this disorder. Indeed, infusions of MSCs may help suppress auto-reactive, T-cell mediated HSC destruction and help restore an environment that supports hematopoiesis. Small pilot studies using MSCs as monotherapy or as adjuncts to HSC transplantation have been attempted as treatments for AA. 20Microstructures,Strength Characteristics and Wear Behavior of the Fe-based P/M Composites after Sintering or Infiltration with Cu–Sn Alloy显示文摘Some properties of the Fe-based P/M composites sintered and reinforced by infiltration with Cu-Sn alloy were described.It is shown that the hardness of the sintered material is 2.5 times lower,tensile strength is 1.7 times lower and the wear resistance is 2.5-3.3 times lower in comparison with those of the infiltrated material.The presence of pores on the friction surface of the sintered material affects the features of the wear process.Due to the specific morphology of copper in the infiltrated material,the phenomenon of selective mass transfer is observed and worn surfaces have a spongy-capillary texture.Larisa N.Dyachkova Eugene E.Feldshtein 2015Journal of Materials Science & Technology2015,31,12:2
