1不稳定性心绞痛、急性非Q波心肌梗死不同抗栓疗法的对比研究显示文摘目的 观察不同抗栓方案对急性冠状动脉综合征心脏事件、出血风险和预后的影响。方法 本研究为前瞻性、多中心、随机、开放试验 ,入选患者随机分为静脉滴注普通肝素组和皮下注射低分子量肝素组。入选对象为不稳定性心绞痛或非Q波心肌梗死 ,入选前 4 8小时以内至少有一次心绞痛发作 ,ST段无抬高。肝素 10 0IU/kg静注 ,续 10 0 0IU/h ,维持活化的部分凝血活酶时间 (APTT)或活化的全血凝固时间 (ACT)于正常的 1 5~ 2 0倍 ,连续 7日。低分子量肝素 0 4~ 0 6ml,每日两次皮下注射 ,连续 7日。主要观察终点 :随访治疗 3 0日内发生急性心肌梗死、心脏性或非心脏性死亡和药物治疗无法控制心绞痛 ,需行急性血运重建术。住院至少 7日 ,随访至治疗后 3 0日。结果 本研究共入选符合条件的患者 4 0 2例 ,两组在性别、年龄、心血管危险因素和心绞痛发作方面差异无显著性。治疗后 7日 ,两组用药期间平均胸痛发作次数差异无显著性 ,但肝素组有更多的患者需口服硝酸甘油缓解胸痛 ;病死率在低分子量肝素组较普通肝素组低 ,两组比较P值为 0 0 62 ,复合终点事件 (死亡、心肌梗死和紧急血管重建 )在低分子量肝素组明显下降。低分子量肝素组出血事件明显少于肝素组。治疗开始后 3 0日 ,低分子量肝素组死亡和复合终点事?Clinical Collaborative Study Group of Low Molecular Weight Heparin (Correspondent: HU Dayi, XU Juntang. 2Mo SILTCIDE SYNTHISIS BY DUAL ION BEAM DEPOSITION显示文摘Mo suicides Mo5 Si3 with high quality were prepared using ion beam deposition equipment with two Filter Metal Vacuum Arc Deposition (FMEVAD). When the number of alternant deposition times was 198, total thickness of the coating is 40nm. The coatings with droplet free can be readily obtained, so the surface is smooth. TEM observation shows that Mo and Si alternant deposition coating is compact structure. The fine Mo suicide grains densely distributed in the coating. 5Review of renal cell carcinoma and its common subtypes in radiology显示文摘Representing 2%-3% of adult cancers, renal cell carcinoma(RCC) accounts for 90% of renal malignancies and is the most lethal neoplasm of the urologic system. Over the last 65 years, the incidence of RCC has increased at a rate of 2% per year. The increased incidence is at least partly due to improved tumor detection secondary to greater availability of high-resolution cross-sectional imaging modalities over the last few decades. Most RCCs are asymptomatic at discovery and are detected as unexpected findings on imaging performed for unrelated clinical indications. The 2004 World Health Organization Classification of adult renal tumors stratifies RCC into several distinct histologic subtypes of which clear cell, papillary and chromophobe tumors account for 70%, 10%-15%, and 5%, respectively. Knowledge of the RCC subtype is important because the various subtypes are associated with different biologic behavior, prognosis and treatment options. Furthermore, the common RCC subtypes can often be discriminated non-invasively based on gross morphologic imaging appearances, signal intensity on T2-weighted magnetic resonance images, and the degree of tumor enhancement on dynamic contrast-enhanced computed tomography or magnetic resonance imaging examinations. 6Contrast-enhanced 3D ultrasound in the radiofrequency ablation of liver tumors显示文摘Liver metastases and hepatocellular carcinomas are two of the most common causes of cancer deaths in the world.Radiofrequency ablation(RFA) is a well recognized,effective and minimally invasive means of treating malignant hepatic tumors.This article describes the use of contrast-enhanced 3D ultrasound(CE-3DUS) in the staging,targeting and follow-up of patients with liver tumors undergoing RFA.In particular,its value in the management of large hepatic lesions will be illustrated.Current limitations of CE-3DUS and future developments in the technique will also be discussed.In summary,CE-3DUS is useful in the RFA of liver tumors with improved detection and display of occult lesions and recurrence,in the assessment of lesional geometry and orientation for a more accurate planning and guidance of multiple RFA needle electrodes in large tumors and in the evaluation of residual or recurrent disease within the immediate and/or subsequent follow-up periods.Edward Leen Senthil Kumar Shahid A Khan Gavin Low Keh Oon Ong Paul Tait Mike Averkiou 2009World Journal of Gastroenterology2009,15,3:19

8Computational Research on Modular Undulating Fin for Biorobotic Underwater Propulsor显示文摘Biomimetic design employs the principles of nature to solve engineering problems. Such designs which are hoped to be quick,efficient,robust,and versatile,have taken advantage of optimization via natural selection. In the present research,an environment-friendly propulsion system mimicking undulating fins of stingray was built. A non-conventional method was considered to model the flexibility of the fins of stingray. A two-degree-of-freedom mechanism comprised of several linkages was designed and constructed to mimic the actual flexible fin. The driving linkages were used to form a mechanical fin con-sisting of several fin segments,which are able to produce undulations,similar to those produced by the actual fins. Owing to the modularity of the design of the mechanical fin,various undulating patterns can be realized. Some qualitative observations,obtained by experiments,predicted that the thrusts produced by the mechanical fin are different among various undulating patterns. To fully understand this experimental phenomenon is very important for better performance and energy saving for our biorobotic underwater propulsion system. Here,four basic undulating patterns of the mechanical fin were performed using two-dimensional unsteady computational fluid dynamics (CFD) method. An unstructured,grid-based,unsteady Navier-Stokes solver with automatic adaptive re-meshing was used to compute the unsteady flow around the fin through twenty complete cycles. The pressure distribution on fin surface was computed and integrated to provide fin forces which were decomposed into lift and thrust. The pressure force and friction force were also computed throughout the swimming cycle. 14Acute-on-chronic liver failure in a multi-ethnic Asian city:A comparison of patients identified by Asia-Pacific Association for the Study of the Liver and European Association for the Study of the Liver definitions显示文摘AIM To explore the applicability of the Asia-Pacific Association for the Study of the Liver(APASL) and European Association for the Study of the Liver(EASL) guidelines for acute-on-chronic liver failure(ACLF) in profiling patients and determining the outcome.METHODS Patients admitted to a tertiary hospital in Singapore with acute decompensation of liver disease from January 2004to July 2014 are screened for ACLF according to the APASL and EASL criteria. The patients' data(including basic demographics, information about existing chronic liver disease, information about the acute decompensation, relevant laboratory values during admission, treatment, and outcome) are retrospectively analyzed to determine the background, precipitating factors and outcome.RESULTS A total of 458 liver patients is analyzed, and 78 patients with ACLF are identified. Sixty-three patients(80.8%) meet the APASL criteria, 64 patients(82.1%) meet the EASL criteria, and 49 patients(62.8%) fulfilled both criteria. The most common causes of acute liver injury are bacterial infections(59.0%), hepatitis B flare(29.5%), and variceal bleeding(24.4%). The common aetiologies of the underlying chronic disease included hepatitis B(43.6%), alcoholic(20.5%) and cryptogenic(11.5%) liver disease. The overall mortality rate is 61.5%. Increased age, the number of organ failures(as per CLIF-SOFA score), peak creatinine, INR, and amylase levels are associated with increased mortality or the need for liver transplantation. 14.3% of patients undergo liver transplantation with a 100% 1-year survival rate. 16Sequential organ failure assessment score is superior to other prognostic indices in acute pancreatitis显示文摘BACKGROUND Acute pancreatitis(AP)is a common surgical condition,with severe AP(SAP)potentially lethal.Many prognostic indices,including;acute physiology and chronic health evaluation II score(APACHE II),bedside index of severity in acute pancreatitis(BISAP),Glasgow score,harmless acute pancreatitis score(HAPS),Ranson’s score,and sequential organ failure assessment(SOFA)evaluate AP severity and predict mortality.AIM To evaluate these indices'utility in predicting severity,intensive care unit(ICU)admission,and mortality.METHODS A retrospective analysis of 653 patients with AP from July 2009 to September 2016 was performed.The demographic,clinical profile,and patient outcomes were collected.SAP was defined as per the revised Atlanta classification.Values for APACHE II score,BISAP,HAPS,and SOFA within 24 h of admission were retrospectively obtained based on laboratory results and patient evaluation recorded on a secure hospital-based online electronic platform.Data with<10%missing data was imputed via mean substitution.Other patient information such as demographics,disease etiology,and patient outcomes were also derived from electronic medical records.RESULTS The mean age was 58.7±17.5 years,with 58.7%males.Gallstones(n=404,61.9%),alcohol(n=38,5.8%),and hypertriglyceridemia(n=19,2.9%)were more common aetiologies.81(12.4%)patients developed SAP,20(3.1%)required ICU admission,and 12(1.8%)deaths were attributed to SAP.Ranson’s score and APACHE-II demonstrated the highest sensitivity in predicting SAP(92.6%,80.2%respectively),ICU admission(100%),and mortality(100%).While SOFA and BISAP demonstrated lowest sensitivity in predicting SAP(13.6%,24.7%respectively),ICU admission(40.0%,25.0%respectively)and mortality(50.0%,25.5%respectively).However,SOFA demonstrated the highest specificity in predicting SAP(99.7%),ICU admission(99.2%),and mortality(98.9%).SOFA demonstrated the highest positive predictive value,positive likelihood ratio,diagnostic odds ratio,and overall accuracy in predicting SAP,ICU admission,and mortality.SOFA and Ranson’s score demonstrated the highest area under receiver-operator curves at 48 h in predicting SAP(0.966,0.857 respectively),ICU admission(0.943,0.946 respectively),and mortality(0.968,0.917 respectively).CONCLUSION The SOFA and 48-h Ranson’s scores accurately predict severity,ICU admission,and mortality in AP,with more favorable statistics for the SOFA score.Thomas Zheng Jie Teng Jun Kiat Thaddaeus Tan Samantha Baey Sivaraj K Gunasekaran Sameer P Junnarkar Jee Keem Low Cheong Wei Terence Huey Vishal G Shelat 2021World Journal of Critical Care Medicine2021,10,6:9
17Characterization of microarc oxidation coatings on pure titanium显示文摘The morphology, composition, and phase structure of the oxide coatings produced on the surface of pure titaniumby alternating-current microarc discharge in aluminate solution were investigated by X-ray diffraction and scanning electronmicroscopy. The profiles of the hardness H and the elastic modulus E in the coatings were determined using a nanoindenta-tion method. The concentration distributions of Ti, Al, and O in the coating show that this coating over 30 μm thick containstwo layers: an outer layer and an inner layer. The oxide coating is mainly composed of TiO2 rutile and Al2TiO5 compounds.During oxidation, the temperature in the microarc discharge channel was very high to make the local coating molten. Fromthe surface to the interior of the coating, H and E increase gradually, and then reach maximum values of 9.78 GPa and 176GPa respectively at a distance of 7 μm from the coating/titanium interface. 18HIGH-ENERGY PROTON IRRADIATION EFFECTS ON GaAs/Ge SPACE SOLAR CELLS显示文摘This paper reports the high-energy proton irradiation effects on GaAs/Ge space solar cells. The solar cells were irradiated by protons with energy of 5-20MeV at fluence ranging from 1×109cm2 to 7×1013cm-2, and then their electric parameters were measured at AMO. It was shown that the Iac, Voc,. and Pmax degrade as the fluence increasing, respectively, but the degradation rates of L,, V0 and Pmax decrease as the proton energy increasing, and the degradation is relative to proton irradiation-induced defect with a level of E -0.41eV in irradiated GaAs/Ge cells.R. 19Numerical and Experimental Research on Modular Oscillating Fin显示文摘Fishes are famous for their ability to position themselves accurately even in turbulent flows. This ability is the result of the coordinated movement of fins which extend from the body. We have embarked on a research program designed to develop an agile and high efficient biologically inspired robotic fish based on the performance of hybrid mechanical fins. To accomplish this goal, a mechanical ray-like fin actuated by Shape Memory Alloy (SMA) is developed, which can realize both oscillatory locomotion and undulatory locomotion. We first give a brief introduction on the mechanical structure of our fin and then carry out theoretic analysis on force generation. Detailed information of these theoretical results is later revealed by Computational Fluid Dynamic (CFD), and is final validated by experiments.
