| 1 | Multivariate statistical analysis of clinicopathologic factors influencing survival of patients with bile duct carcinoma显示文摘AIM: To evaluate the influence of various clinicopathologicfactors on survival of patients with bile duct carcinoma aftercurative resection.METHODS: A retrospective analysis was made for 86 casesof bile duct carcinoma treated from January 1981 toSeptember 1995. Fifteen clinicopathologic factors possiblyinfluencing survival were selected. Independent variableswere first analyzed by univariate methods. Survival forvariable was estimated by the method of Kaplan and Meier.The variables that were statistically significant by univarlateanalysis were included in a multivariate analysis, which wereconfirmed using the Cox stepwise proportion hazard modelwith the help of SPSS 10.0 for Windows software.RESULTS: The overall cumulative survival rate was 72.6 %at 1 year, 32.4 % at 3 years, and 18.7 % at 5 years. Theresults of univariate analysis showed that the majorsignificant prognostic factors influencing survival of thesepatients were histological type of lesion, lymph nodemetastasis, pancreatic invasion, duodenal invasion, pedneuralinvasion, macroscopic vessel involvement, resected surgicalmargin and depth of cancer invasion (P=0.02, 0.02, 0.004,0.005, 0.01, 0.43, 0.03 and 0.04). Age, sex, location of tumor,size of tumor, macroscopic type of lesions, hepaticmetastasis, and hepatic invasion were not significantlyassociated with prognosis (P>0.05). Pancreatic invasion,perineural invasion and lymph node metastases were thethree most important prognostic factors by multivariateanalysis using the Cox proportional hazards model.CONCLUSION: Pancreatic invasion, pedneural invasion andlymph node metastases are the most important prognosticfactors for bile duct carcinoma after curative resection. | He, P Shi, JS Chen, WK Wang, ZR Ren, H Li, H | 2002 | World Journal of Gastroenterology2002，8，5： | 10 |
| 2 | Improved technique of vascular anastomosis for small intestinal transplantation in rats显示文摘AIM To establish a new improved vascularanastomotic technique to simplify the surgicaltechnique and increase the survival rate of smallintestinal transplantation in rats.METHODS The graft removed en blocconsisted of entire small intestine,portal veinand aortic segment with superior mesentericartery.The graft was perfused in situ and thegut lumen was irrigated during the operation.Heterotopic small bowel transplantation wasperformed by microvascular end-to-sideanastomosis between the donor aortic segmentwith superior mesenteric artery and the recipientabdominal aorta,and by the formation of a'Cuff'anastomosis between the donor portalvein and the recipient left renal vein.Both endsof the grafts were exteriorized as stomas.RESULTS A total of 189 intestinaltransplantations were performed in rats,33 ofwhich were involved in the formal experimentalgroup,with a survival rate of 84.8%.Theaverage time for the donor surgery was 80min±10min;for graft repair 10min±3min;and forrecipient surgery 95min±15min.The averagetime for the arterial anastomosis and the veinanastomosis was 18min±5min and 1min,respectively.The warm ischemic time and coldischemic time were 22min±5min and less than60min,respectively.The whole operation wascompleted by a single surgeon,the operativetime being about 3 hours.CONCLUSION The vascular anastomosis used in this study could simplify surgical technique,reduce the operative time and elevate thesurvival rate of small intestinal transplantationin rats. | Li YX Li JS Li N | 2000 | World Journal of Gastroenterology2000，6，2： | 8 |
| 3 | Efficacies of β-L-D4A against Hepatitis B virus in 2.2.15 cells显示文摘AIM:To investigate the antiviral effect of beta-L-enantiomer of 2',3'-didehydro-2',3'-dideoxyadenosine (β-L-D4A) on 2.2.15 cells transfected with the hepatitis B virus (HBV) genome. METHODS:Lamivudine (3TC) as a positive control. Then, HBV DNA in treated 2.2.15 cells and the Hepatitis B surface antigen (HBsAg) in the culture supernatants were detected to determine the inhibitory effect of β-L-D4A. At the same time, 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) was used to detect the survival ratio of 2.2.15 cells. RESULTS:β-L-D4A has a dose-dependent inhibitory effect on HBV DNA replication;this effect was apparent when the concentration was above 1 mol/L. When β-L-D4A was at the highest concentration, 100 mol/L, the HBsAg inhibition ratio was above 50%. The Therapeutic index (TI) of β-L-D4A was above 2.1. | 4 | Lipopolysaccharide decreasing albumin expression in rat hepatocytes显示文摘The relations of the severity of hypoalbu-minemia to morbidity and mortality of patients with critical illness illustrate the need for better understanding of molecular mechanism of hypoalbuminemia. This study was undertaken to investigate the response of albumin synthesis to lipopolysaccharide(LPS)in rat hepatocytes in vitro in early acute phase of sepsis. METHODS:Hepatocytes were cultured at an initial cell density of 1.5×106 cells/well in 3 ml culture medium. There were two groups of samples which received either normal saline or 1 μg/L LPS randomly. Albumin mRNA in hepatocytes was assessed by reverse transcription-poly-merase chain reaction(RT-PCR) and albumin level in the supernatant was measured by ELISA at 0,2,8,12,24 hours after exposure. Meanwhile, the albumin precursor was evaluated at the same time points by flow cytometry. RESULTS:The quantitative changes of mRNA, albumin precursor and its protein were analogous. All of them tended to decline at 12 hours post-treatment and did not decrease significantly until 24 hours after LPS exposure. Meanwhile, albumin mRNA decreased about 30% and the levels of albumin precursor and albumin reduced approximately 50%. CONCLUSIONS: LPS can inhibit albumin synthesis in rat hepatocytes by prevention of albumin transcription. Moreover, the response of hepatic albumin synthesis to LPS changes with the stage of sepsis process.
