| 3 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 4 | Gastric cancer: Epidemiology, risk factors and prevention strategies显示文摘Gastric cancer(GC) is a global health problem, with more than 1 million people newly diagnosed with GC worldwide each year. GC is more prevalent in less developed countries than in more developed countries. About half of all GC cases worldwide occur in East Asia, notably China. Globally, overall incidence rates of GC are declining, which is potentially attributed to a decrease in Helicobacter pylori(H. pylori) infection and the use of refrigeration to preserve foods rather than salt. GC is a multifactorial disease, and its occurrence and development were impacted by environmental and genetic factors. H. pylori infection is the primary risk factor for GC, especially for non-cardia. The prognosis of GC is poor due to stages at the first diagnosis. The 5-year survival rate is less than 10% when patients are diagnosed at an advanced stage, but the rate is as high as 85% if patients are detected at an earlier stage. Endoscopic screening can potentially prevent GC by early diagnosis and early treatment and has been widely adopted in screening programs in East Asian countries, such as Japan and Korea. | 7 | Identification of a Stable Quantitative Trait Locus for Percentage Grains with White Chalkiness in Rice (Oryza sativa)显示文摘High chalkiness is a major problem in many rice-producing areas of the world, especially in hybrid rice (Oryza sativa L.) in China. We previously showed a major quantitative trait locus for the percentage of grains with white chalkiness (QTLqPGWC-8) in the interval G1149-R727 on chromosome 8 using a chromosome segment substitution line (CSSL). Here, we selected the line-CSSL50 harboring the QTLqPGWC-8 allele from the CSSLs derived from a cross between Asominori (as a recurrent parent) and IR24 (as a donor parent), which had higher percentage chalkiness, markedly different from that of Asominori. There were also significant differences in starch granules, appearance of amylose content (AAC) and milling qualities between Asominori and CSSL50, but not in grain size or thousand grain weight (TGW). The BC4F2 and BC4F3 populations from a cross between CSSL50 and Asominori were used for fine mapping of qPGWC-8. We narrowed down the location of this QTL to a 142 kb region between Indel markers 8G-7 and 8G-9. QTLqPGWC-8 accounted for 50.9% of the difference in PGWC between the parents. | 10 | Opening of an early Paleozoic limited oceanic basin in the northern Altyn area: Constraints from plagiogranites in the Hongliugou-Lapeiquan ophiolitic mélange显示文摘This paper reports petrological and geochemical features and zircon U-Pb age of plagiogranite from the Hongliugou-Lapeiquan ophiolitic mélange belt in the northern Altyn Tagh. The zircon U-Pb dating results yield a mean 238U/206Pb age of 512.1 - 1.5 Ma, representing an emplacement time of the plagiogranites in the Middle Cambrian. The plagiogranites are interpreted to have derived from anatexis of hydrated amphibolites by ductile shearing during transports of the oceanic crust. Thus it is believed that the formation age of such type of plagiogranite was coeval to or slightly younger than the spreading of the Hongliugou-Lapeiquan limited oceanic basin. | 12 | Silicon intensity Mach–Zehnder modulator for single lane 100 Gb/s applications显示文摘In this paper, a substrate removing technique in a silicon Mach–Zehnder modulator(MZM) is proposed and demonstrated to improve modulation bandwidth. Based on the novel and optimized traveling wave electrodes,the electrode transmission loss is reduced, and the electro-optical group index and 50 Ω impedance matching are improved, simultaneously. A 2 mm long substrate removed silicon MZM with the measured and extrapolated 3 dB electro-optical bandwidth of >50 GHz and 60 GHz at the-8 V bias voltage is designed and fabricated.Open optical eye diagrams of up to 90 GBaud∕s NRZ and 56 GBaud∕s four-level pulse amplitude modulation(PAM-4) are experimentally obtained without additional optical or digital compensations. Based on this silicon MZM, the performance in a short-reach transmission system is further investigated. | 13 | Effects of Xinfeng capsule on pulmonary function based on treg-mediated notch pathway in a rat model of adjuvant arthritis显示文摘OBJECTIVE:To observe the impact of xinfeng xapsule(XFC) on pulmonary function in a rat model of adjuvant arthritis(AA) and to investigate the mechanism of action.METHODS:Forty rats were randomly divided into four groups of ten:normal control(NC);model control(MC);tripterygium glycosides tablet(TPT);and xinfeng capsule(XFC).Except for the NC group,AA was induced in all rats by intracutaneous injection of 0.1 mL Freund's complete adjuvant in the right paw on the 19th day.NC and MC groups were given(0.9%) physiological saline.The TPT and XFC groups were given TPT(10 mg/kg) and XFC(1.2 g/kg),respectively.Thirty days after administration,changes in paw edema(E),the arthritis index(AI),pulmonary function,levels of regulatory T-cells(Treg),ultrastructure of lung tissue,and expression of Notch receptors and ligands in lung tissue were observed.RESULTS:In the MC group,E and the AI were increased and pulmonary function significantly decreased;the structure of alveolar type-II cells was damaged;ratios of Treg in peripheral blood were reduced;and expression of Notch receptors such as Notch3 and Notch4 and ligands such as Delta1 in lung tissue were significantly increased whereas expression of Notch1,Jagged1 and Jagged2 were significantly decreased.After intervention with XFC,E and the AI were decreased;pulmonary function was enhanced;the structure of alveolar type-II cells was improved;and expression of Treg,Notch1,Jagged1,Jagged2 was elevated,whereas that of Notch3,Notch4 and Delta1 was reduced.CONCLUSION:XFC can not only inhibit E and the AI and improve joint symptoms,it can also improve pulmonary function and reduce inflammation in lung tissue.These actions could be carried out through increases in the expression of Treg,Notch receptors(Notch1) and ligands(Jagged1,Jagged2),and reductions in the expression of Notch3,Notch4 and Delta1.These phenomena would reduce the deposition of immune complexes and the inflammatory response in lung tissue,thereby improving joint symptoms and pulmonary function. | Lei Wan Jian Liu Chuanbing Huang Yuan Wang Xi Shen Wandong Zhang Guizheng Wang Haixia Fan Yao Ge Ruilian Chen Yunxiang Cao Ruikai Zong Li Lei | 2012 | Journal of Traditional Chinese Medicine2012,32,3: | 17 |
| 14 | Evaluation of liver functional reserve by combining D-sorbitol clearance rate and CT measured liver volume显示文摘AIM: Our research attempted to evaluate the overall functional reserve of cirrhotic liver by combination of hepatic functional blood flow, liver volume, and ChildPugh′s classification, and to discuss its value of clinical application.METHODS: Ninety two patients with portal hypertension due to hepatic cirrhosis were investigated. All had a historyof haematemesis and hematochezia, esophageal and gastric fundus varices, splenomegaly and hypersplenia.A 2-year follow-up was routinely performed and no one was lost. Twenty two healthy volunteers were used as control group. Blood and urine samples were collected 4times before and after intravenous D-sorbitol infusion.The hepatic clearance (CLH) of D-sorbitol was then calculated according to enzymatic spectrophotometric method while the total blood flow (QToTAL) and intrahepatic shunt (RINs) were detected by multicolor Doppler ultrasound, and the liver volume was measured by spiral CT. Data were estimated by t-test, variance calculation and chi-squared test. The relationships between all these parameters and different groups were investigated according to Child-Pugh classification and postoperative complications respectively.RESULTS: Steady blood concentration was achieved 120 mins after D-sorbitol intravenous infusion, which was (0.358±0.064) mmoⅠ@L-1 in cirrhotic group and (0.189±0.05)mmol@L-1 in control group (P<0.01). CLH=(812.7±112.4) ml@min-1,QTOTAL=(1280.6±131.4) ml@min-1, and RINS=(36.54±10.65)%in cirrhotic group and CLH=(1248.3±210.5) ml.min-1, QTOTAL=(1362.4-±126.9) ml@min-1, and RINS=(8.37±3.32) % in control group (P<0.01). The liver volume of cirrhotic group was 1057±249 cm3, 851±148 cm3 and 663±77 cm3 in Child A, B and C group respectively with significant difference (P<0.001).The average volume of cirrhotic liver in Child B, C group was significantly reduced in comparison with that in control group (P<0.001). The patient, whose liver volume decreased by 40 % with the CLH below 600 ml@min-1, would have a higher incidence of postoperative complications. There was no strict correspondent relationship between CLH, liver volume and Child-Pugh′s classification.CONCLUSION: The hepatic clearance of D-sorbitol, CT measured liver volume can be reliably used for the evaluation of hepatic functional blood flow and liver metabolic volume. | 17 | Effects of viral infection and microbial diversity on patients with sepsis:A retrospective study based on metagenomic next-generation sequencing显示文摘BACKGROUND: The study aims to investigate the performance of a metagenomic next-generationsequencing (NGS)-based diagnostic technique for the identifi cation of potential bacterial and viral infectionsand eff ects of concomitant viral infection on the survival rate of intensive care unit (ICU) sepsis patients.METHODS: A total of 74 ICU patients with sepsis who were admitted to our institution from February1, 2018 to June 30, 2019 were enrolled. Separate blood samples were collected from patients for bloodcultures and metagenomic NGS when the patients’ body temperature was higher than 38 °C. Patients’demographic data, including gender, age, ICU duration, ICU scores, and laboratory results, were recorded.The correlations between pathogen types and sepsis severity and survival rate were evaluated.RESULTS: NGS produced higher positive results (105 of 118;88.98%) than blood cultures(18 of 118;15.25%) over the whole study period. Concomitant viral infection correlated closelywith sepsis severity and had the negative effect on the survival of patients with sepsis. However,correlation analysis indicated that the bacterial variety did not correlate with the severity of sepsis.CONCLUSIONS: Concurrent viral load correlates closely with the severity of sepsis and thesurvival rate of the ICU sepsis patients. | 19 | Impact of Traffic Emissions on Local Air Quality and the Potential Toxicity of Traffic-related Particulates in Beijing, China显示文摘Objective Air-borne particulates from different sources could have different physicochemical properties and inflammatory potentials. This study aims to characterize the chemical compositions and the toxicity of ambient particulate matter (PM) associated with traffic emissions. Methods The concentrations of trace elements, organic carbon (OC), elemental carbon (EC) and polycyclic aromatic hydrocarbons (PAHs) in PM 2.5 and PM 10 were measured in samples collected at sites in Beijing, China. Their toxic effects on the pulmonary system of rats were investigated. Biochemical parameters (LDH, T-AOC, TP) and inflammatory cytokine(IL-6, IL-1, TNF-α) levels were measured in the lungs of rats exposed to traffic-related PM. Oxidative damage was observed. PM samples were taken from a near road site and an off road site in summer time in 2006. Results The concentrations of the USEPA priority pollutant PAHs in both PM 10 and PM 2.5 were higher (299.658 and 348.412) at the near road site than those (237.728 and 268.472) at the off road site. The similar trend was observed for the concentrations of trace elements in PM. Compared to coarse particles (PM 10 ), fine particles (PM 2.5 ) have a greater adsorption capacity to enrich toxic elements than inhalable particles. Decrease in antioxidant capacity and an increase in the amount of lipid peroxidation products in rat lung tissues was observed. | 20 | FGF21作为运动因子在有氧运动抑制心梗心肌细胞凋亡中的作用及其机制探讨显示文摘目的:探讨成纤维细胞生长因子21(fibroblast growth factor 21,FGF21)在有氧运动抑制心肌梗死(myocardialinfarction,MI)大鼠心肌细胞凋亡中的作用及其机制。方法:雄性Sprague-Dawley大鼠40只,体重180~220g,心梗模型采用左冠状动脉前降支永久性结扎的方法,术后随机分为假手术组(S组)、心梗安静组(MI组)和心梗+有氧运动组(ME组),S组只穿线不结扎。ME组在术后1周进行4周跑台训练。训练后次日腹腔麻醉,测定LVSP、LVEDP和±dp/dtmax评价心功能。培养H9C2大鼠心肌细胞并采用浓度为400μmol/L的H2O2刺激4h构建心肌细胞凋亡模型,采用AMPK激动剂AICAR和FGF21受体FGFR1抑制剂PD166866干预H9C2大鼠心肌细胞。Masson染色观察分析心肌胶原容积分数(collagenvolumefraction,CVF%)。Western blotting检测心肌FGF21、FGFR1、Bax、Bcl-2的蛋白表达及PI3K-Akt通路蛋白的表达,RTqPCR测定心肌FGF21和FGFR1的表达,TUNEL染色观察心肌细胞凋亡情况。结果:与S组比较,MI组心肌FGF21、FGFR1、Bax/Bcl-2以及PI3K、Akt蛋白表达显著升高,FGF21和FGFR1表达显著升高,TUNEL阳性颗粒显著增加,胶原纤维百分比升高,心功能下降;与MI组比较,ME组心肌FGF21、FGFR1以及PI3K、Akt蛋白表达进一步升高,Bax/Bcl-2显著下降,TUNEL阳性颗粒显著减少,胶原纤维百分比下降,心功能改善;且心肌FGF21表达与抑制心肌细胞凋亡和心功能改善呈显著正相关。PD166866干预H9C2大鼠心肌细胞Bax/Bcl-2表达显著升高,TUNEL阳性颗粒显著增加,炎性因子TNF-α/IL-10显著升高,PI3K-Akt蛋白表达显著下降;AMPK激动剂AICAR单一干预或与FGF21受体抑制剂PD166866二者联合干预均可显著逆转此不利影响。结论:有氧运动可显著上调心梗大鼠心肌FGF21/FGFR1的表达,其机制是通过激活下游PI3K-Akt信号通路,降低心梗心脏炎症反应,进而抑制心肌细胞凋亡。表明,FGF21/FGFR1/PI3K-Akt信号通路在有氧运动抑制心肌细胞凋亡、降低心肌胶原纤维化面积扩增和改善心功能方面发挥重要作用。 | 田振军 李明晓 XI Lei | 2018 | 体育科学2018,38,12: | 12 |
