22018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). 4Assessing the health risk of heavy metals in vegetables to the general population in Beijing,China显示文摘A systematic survey of As, Cd, Cr, Cu, Ni, Pb and Zn concentrations in vegetables from 416 samples (involving 100 varieties) in Beijing was carried out for assessing the potential health risk to local inhabitants. The results indicated that the metal concentrations in vegetables ranged from < 0.001 to 0.479 μg/g fresh weight (fw) (As), < 0.001 to 0.101 μg/g fw (Cd), < 0.001 to 1.04 μg/g fw (Cr), 0.024 to 8.25 μg/g fw (Cu), 0.001 to 1.689 μg/g fw (Ni), < 0.001 to 0.655 μg/g fw (Pb) and 0.01 to 25.6 μg/g fw (Zn), with average concentrations of 0.013, 0.010, 0.023, 0.51, 0.053, 0.046 and 2.55 μg/g fw, respectively. The results showed that the concentrations of As, Cr, Cu, Cd, Pb and Ni in vegetables from open-fields were all significantly higher than those grown in greenhouses. In addition, in local-produced vegetables, all HMs except Zn were significantly higher than those in provincial vegetables. The estimated daily intake (EDI) of As, Cd, Cr, Cu, Ni, Pb and Zn from vegetables was 0.080, 0.062, 0.142, 3.14, 0.327, 0.283 and 15.7 μg/(kg body weight (bw)·d) for adults, respectively. Arsenic was the major risk contributor for inhabitants since the target hazard quotient based on the weighted average concentration (THQw) of arsenic amounted to 44.3% of the total THQ (TTHQ) value according to average vegetable consumption. 5Mesozoic contraction deformation in the Yanshan and northern Taihang mountains and its implications to the destruction of the North China Craton显示文摘Mesozoic contraction deformation in the Yanshan and Taihang mountains is characterized by basement-involved thrust tectonics,basement-cored buckling anticlines and ductile thrust and nappe tectonics.Most of these deformations are orientated west-east,west-northwest and northeast to north-northeast.The contraction deformations began in the Permian,continued through the Triassic and Jurassic and terminated in the Early Cretaceous,and constitute an important part of the destruction of the North China Craton.It is estimated,from balanced cross-section reconstructions,that the north-south shortening of the central part of the Yanshan belt before 135 Ma was around 38%.The initial crust thickness,pre-dating the major contraction deformation in late Paleozoic and early Mesozoic,was estimated to be around 35 km based on paleogeographic characteristics.Assuming that the inferred depth of ductile thrusting deformation,20-25 km,was the crust thickness involved in the contraction deformation,and also assuming that the N-S contraction deformation was accommodated by vertical crust thickening,the thickness of the crust after the contraction deformation was expected to be around 47-50 km.This was the approximate crust thickness required for the eclogitization of the lower crust for delamination.The gravity potential accumulated by the isostatic uplift of the thickened crust,together with the decrease in crustal strength caused by the coeval magmatisms associated with the contraction deformation,led to the subsequent extensional collapse of the middle and upper crust although the regional stress regime associated with the plate interactions remained constant.It is inferred that the Mesozoic contraction deformations in the Yanshan and Taihang mountains were not only a significant tectonic process contributing to the destruction of the craton in middle and upper crust but also stimulated delamination at a deep level and the extension of the shallow crust.In other words,both the suspected delamination of the lower crust and upper mantle and the well constrained extension deformations of the shallow crust in the eastern North China Craton during the late Mesozoic are a consequence of crust thickening due to previous contractions.Extensional deformations could be expected to occur independently in the shallow crust,and are not necessarily associated with or responding to delamination at a deep level.ZHANG ChangHou LI ChengMing DENG HongLing LIU Yang LIU Lei WEI Bo LI HanBin LIU Zi 2011Science China Earth Sciences2011,54,6:40
6Reduced expression of E-cadherin/catenin complex in hepatocellular carcinomas显示文摘AIM: To examine the immunoreactivity of E-cadherin and four subtypes of catenin family in human hepatocellular carcinomas (HCCs) and to investigate the correlation between expression of E-cadherin/ catenin complex and clinicopathologic parameters of HCC patients. METHODS: An immunohistochemical study for E-cadherin and catenins was performed on 97 formalin-fixed, paraffin-embedded specimens of HCC. RESULTS: Reduced expression of E-cadherin, α-, β-, γ-catenin and p120 was observed in 69%, 76%, 63%, 71% and 73%, respectively. Both expressions of E-cadherin and catenin components were significantly correlated with tumor grade (P = 0.000). It showed significant difference between expression of catenin members and tumor stage (P = 0.003, P = 0.017, P = 0.007 and P = 0.000, respectively). The reduced expression of E-cadherin in HCCs was significantly correlated with intrahepatic metastasis (IM) and capsular invasion (P = 0.008, P = 0.03, respectively). A close correlation was also observed between the expression of catenins and the tumor size (P = 0.002, P = 0.034, P = 0.016 and P = 0.000, respectively). In addition, the expression of each catenin was found correlated with IM (P = 0.012, P = 0.049, P =0.026 and P = 0.014, respectively). No statistically significant difference was observed between the expression level of E-cadherin/catenin complex and lymph node permission, vascular invasion and satellite nodules. Interestingly, only expression of p120 showed correlation with AFP value (P = 0.035). The expression of E-cadherin was consistent with α-, β-, γ-catenin and p120 expression (P = 0.000). Finally, the abnormal expression of E-cadherin/catenin complex was significantly associated with patients' survival (P = 0.0253, P = 0.0052, P = 0.003, P = 0.0105 and P = 0.0016, respectively). Nevertheless, no component of E-cadherin/catenin complex was the independent prognostic factor of HCC patients. CONCLUSION: Down-regulated expressions of E-cadherin, catenins and p120 occur frequently in HCCs and contribute to the progression and development of tumor. 9Early screening and diagnosis strategies of pancreatic cancer:a comprehensive review显示文摘Pancreatic cancer is a highly malignant digestive system tumor with a poor prognosis.Most pancreatic cancer patients are diagnosed at an advanced stage or even metastasis due to its highly aggressive characteristics and lack of typical early symptoms.Thus,an early diagnosis of pancreatic cancer is crucial for improving its prognosis.Currently,screening is often applied in high-risk individuals to achieve the early diagnosis of pancreatic cancer.Fully understanding the risk factors of pancreatic cancer and pathogenesis could help us identify the high-risk population and achieve early diagnosis and timely treatment of pancreatic cancer.Notably,accumulating studies have been undertaken to improve the detection rate of different imaging methods and the diagnostic accuracy of endoscopic ultrasound-guided fine-needle aspiration(EUS-FNA)which is the golden standard for pancreatic cancer diagnosis.In addition,there are currently no biomarkers with sufficient sensitivity and specificity for the diagnosis ofpancreatic cancer to be applied in the clinic. As the only serum biomarkerapproved by the United States Food and Drug Administration, carbohydrateantigen 19-9 (CA19-9) is not recommended to be used in the early screeningof pancreatic cancer because of its limited specificity. Recently, increasingnumbers of studies focused on the discovering of novel serum biomarkersand exploring their combination with CA19-9 in the detection of pancreaticcancer. Besides, the application of liquid biopsy involving circulating tumor cells(CTCs), circulating tumor DNA (ctDNA), microRNAs (miRNAs), and exosomesin blood and biomarkers in urine, and saliva in pancreatic cancer diagnosis aredrawing more and more attention. Furthermore, many innovative technologiessuch as artificial intelligence, computer-aided diagnosis system, metabolomicstechnology, ion mobility spectrometry (IMS) associated technologies, and novelnanomaterials have been tested for the early diagnosis of pancreatic cancer andhave shown promising prospects. 10Pathways of China's PM;air quality 2015–2060 in the context of carbon neutrality显示文摘Clean air policies in China have substantially reduced particulate matter(PM;) air pollution in recent years,primarily by curbing end-of-pipe emissions.However,reaching the level of the World Health Organization(WHO) guidelines may instead depend upon the air quality co-benefits of ambitious climate action.Here,we assess pathways of Chinese PM;air quality from 2015 to 2060 under a combination of scenarios that link global and Chinese climate mitigation pathways(i.e.global 2℃-and 1.5℃-pathways,National Determined Contributions(NDC) pledges and carbon neutrality goals) to local clean air policies.We find that China can achieve both its near-term climate goals(peak emissions) and PM;air quality annual standard(35 μg/m;) by 2030 by fulfilling its NDC pledges and continuing air pollution control policies.However,the benefits of end-of-pipe control reductions are mostly exhausted by 2030,and reducing PM;exposure of the majority of the Chinese population to below 10 μg/m;by 2060 will likely require more ambitious climate mitigation efforts such as China’ s carbon neutrality goals and global1.5℃-pathways.Our results thus highlight that China’s carbon neutrality goals will play a critical role in reducing air pollution exposure to the level of the WHO guidelines and protecting public health.Jing Cheng Dan Tong Qiang Zhang Yang Liu Yu Lei Gang Yan Liu Yan Sha Yu Ryna Yiyun Cui Leon Clarke Guannan Geng Bo Zheng Xiaoye Zhang Steven JDavis Kebin He 2021National Science Review2021,8,12:23
11EW-trending uplifts along the southern side of the central segment of the Altyn Tagh Fault, NW China: Insight into the rising mechanism of the Altyn Mountain during the Cenozoic显示文摘The Altyn Tagh Fault and the Altyn Mountain define respectively the tectonic and geographical northern edges of the Tibetan Plateau, and figure prominently in the growth and rising mechanism of the plateau. The rhombus-shaped Altyn Mountain has long been thought to have an intimate relation with the Altyn Tagh Fault; however, its formation mechanism remains unclear and debatable. In this paper, we focus on the EW-trending uplifts in the Altyn Mountain, and investigated three Cenozoic sedimentary sections in the vicinity of the EW-trending uplifts located along the southern side of the central segment of the Altyn Tagh Fault. Magnetostratigraphy and pollen analysis were used to constrain ages of the sediments. Clast composition of conglomerate and paleocurrents obtained from clast imbrications were applied to determine the provenance. We also established a geological section parallel to the Altyn Tagh Fault on sedimentary facies across the northwestern Qaidam Basin. The results indicate that these en-echelon EW-trending uplifts formed as early as ca. 36 Ma and were preferred to be under the control of basal shear of the Altyn Tagh Fault in the middle-lower crust, symbolizing the early uplift of the Altyn Mountain during the Cenozoic. 12Gender disparity in hepatocellular carcinoma(HCC):multiple underlying mechanisms显示文摘On the global scale, hepatitis B virus(HBV) infection is the main cause of hepatocellular carcinoma(HCC) especially in regions of Asia where HBV infection is endemic. Epidemiological studies show that the incidence of inflammation-driven HCC in males is three times as high as in females. Recent studies suggest that sex hormones have a crucial role in the pathogenesis and development of HBV-induced HCC. We found that the estrogen/androgen signaling pathway is associated with decreased/increased transcription and replication of HBV genes and can promote the development of HBV infections by up/downregulating HBV RNA transcription and inflammatory cytokines levels, which in turn slow down the progression of HBV-induced HCC. Additionally, sex hormones can also affect HBV-related HCC by inducing epigenetic changes. The evidence that both morphology and function of the human liver are affected by sex hormones was found over 60 years ago. However,the underlying molecular mechanism largely remains to be elucidated. This review focuses mainly on the molecular mechanisms behind the sex difference in HCC associated with HBV and other factors. 13Management of cytokine release syndrome related to CAR-T cell therapy显示文摘Chimeric antigen receptor T (CAR-T) cell therapy is a novel cellular immunotherapy that is widely used to treat hematological malignancies, including acute leukemia, lymphoma, and multiple myeloma. Despite its remarkable clinical effects, this therapy has side effects that cannot be underestimated. Cytokine release syndrome (CRS) is one of the most clinically important and potentially life-threatening toxicities. This syndrome is a systemic immune storm that involves the mass cytokines releasing by activated immune cells. This phenomenon causes multisystem damages and sometimes even death. In this study, we reported the management of a patient with recurrent and refractory multiple myeloma and three patients with acute lymphocytic leukemia who suffered CRS during CAR-T treatment. The early application of tocilizumab, an anti-IL-6 receptor antibody, according to toxicity grading and clinical manifestation is recommended especially for patients who suffer continuous hyperpyrexia, hypotensive shock, acute respiratory failure, and whose CRS toxicities deteriorated rapidly. Moreover, low doses of dexamethasone (5-10 mg/day) were used for refractory CRS not responding to tocilizumab. 14Sevoflurane postconditioning protects against myocardial ischemia/reperfusion injury by restoring autophagic flux via an NO-dependent mechanism显示文摘Volatile anesthetics improve postischemic cardiac function and reduce infarction even when administered for only a brief-time at the onset of reperfusion.A recent study showed that sevoflurane postconditioning (SPC)attenuated myocardial reperfusion injury, but the underlying mechanisms remain unclear,in this study,we examined the effects of sevoflurane on nitric oxide (NO)release and autophagic flux during the myocardial ischemia/reperfusion (I/R)injury in rats in vivo and ex vivo.Male rats were subjected to 30 min ischemia and 2 h reperfusion in the presence or absence of sevoflurane (1.0 minimum alveolar concentration)during the first 15 min of reperfusion.We found that SPC significantly improved hemodynamic performance after reperfusion,alleviated postischemic myocardial infarction,reduced nicotinamide adenine dinucleotide content loss,and cytochrome c.release in heart tissues.Furthermore,SPC significantly increased the phosphorylation of endothelial nitric oxide synthase (NOS)and neuronal nitric oxide synthase,and elevated myocardial NOS activity and NO production.All these effects were abolished by treatment with an NOS inhibitor NG-nitro-L-arginine methyl ester (L-NAME,10 mg/kg,i.v.).We also observed myocardial I/R-induced accumulation of autophagosomes in heart tissues,as evidenced by increased ratios of microtubule:associated protein 1 light chain 3 Ⅱ/Ⅰ,upregulation of Beclin 1 and P62,and reduced lysosome-assciated membrane protein-2 expression.SPC significantly attenuated I/R- impaired autophagic flux,which were blocked by L-NAME.Moreover,pretreatment with the autophagic flUX blocker chloroquine (10 mg/kg,i.p.)increased autophagosome accumulation in SPC-treated heart following I/R and blocked SPC-induced cardioprotection.The same results were also observed in a rat model of myocardial I/R injury ex vivo,suggesting that SPC protects rat hearts against myocardial reperfusion injury by restoring I/R-impaired autophagic flux yia an NO-dependent mechanism.Shi-gang Qiao Ying Sun Bo Sun An Wang Jia Qiu Lei Hong Jian-zhong An Chen Wang Hui-ling Zhang 2019Acta Pharmacologica Sinica2019,40,1:21
15Anatomy of composition and nature of plate convergence: Insights for alternative thoughts for terminal India-Eurasia collision显示文摘The pattern and timing of collision between India and Eurasia have long been a major concern of the international community. However, no consensus has been reached hitherto. To explore and resolve the disagreements in the Himalayan study,in this paper we begin with the methodology and basic principles for the anatomy of composition and nature of convergent margins,then followed by an effort to conduct a similar anatomy for the India-Eurasia collision. One of the most common patterns of plate convergence involves a passive continental margin, an active continental margin and intra-oceanic basins together with accreted terranes in between. The ultimate configuration and location of the terminal suture zone are controlled by the basal surface of the accretionary wedge, which may show fairly complex morphology with Z-shape and fluctuant geometry. One plausible method to determine the terminal suture zone is to dissect the compositions and structures of active continental margins. It requires a focus on various tectonic elements belonging to the upper plate, such as accretionary wedges, high-pressure(HP)-ultra-high-pressure(UHP) metamorphic rocks, Barrovian-type metamorphic rocks and basement nappes, together with superimposed forearc basins.Such geological records can define the extreme limits and the intervening surface separating active margin from the passive one,thus offering a general sketch for the surface trace of the terminal suture zone often with a cryptic feature. Furthermore, the occurrence of the cryptic suture zone in depth may be constrained by geophysical data, which, in combination with outcrop studies of HP-UHP metamorphic rocks, enables us to outline the terminal suture zone. The southern part of the Himalayan orogen records complicated temporal and spatial features, which are hard to be fully explained by the classic 'two-plate-one-ocean' template,therefore re-anatomy of the compositions and nature for this region is necessitated. Taking advantage of the methodology and basic principles of plate convergence anatomy and synthesizing previous studies together with our recent research, we may gain new insights into the evolution of the Himalayan orogeny.(1) The Yarlung-Zangbo ophiolite is composed of multiple tectonic units rather than a single terminal suture zone, and a group of different tectonic units were juxtaposed against each other in the backstop of the Gangdese forearc.(2) The Tethyan Himalayan Sequence(THS) contains mélanges with typical block-in-matrix structures, uniform southwards paleocurrents and age spectra of detrital zircons typical of Eurasia continent. All of these facts indicate that the THS belonged to Eurasia plate before the terminal collision, emplaced in the forearc of the Gangdese arc.(3) The Greater Himalayan Crystalline Complex(GHC) and Lesser Himalayan Sequence(LHS) comprise complex components including eclogites emplaced into the GHC and the upper part of the LHS. Judging from the fact that HP-UHP metamorphic rocks are exhumed and emplaced in the upper plate, the GHC and the upper part of the LHS where eclogite occur should be assigned to the upper plate, lying above the terminal subduction zone surface. It is the very surface along which the continuous subduction of the India subcontinent occurred, therefore acting as the terminal, cryptic suture. From the suture further southward, the bulk rock associations of the LHS and Sub-Himalayan Sequence(Siwalik) show little affinity of mélange, probably belonging to the foreland system of the India plate. By the anatomy of tectonic features of all the tectonic units in the Himalayan orogen as well as the ages of the subduction-accretion related deformation, we conclude that the terminal India-Eurasia collision occurred after 14 Ma, the timing of the metamorphism of the eclogites emplaced into the upper plate. The development of rifts stretching in N-S direction in Tibet and tectonic events with the transition from sinistral to dextral movements in shear zones, such as the Ailaoshan fault in East Tibet, can coordinately reflect the scale and geodynamic influence of the India-Eurasia convergence zone.By conducting a detailed anatomy of the southern Himalayas, we propose a new model for the final collision-accretion of the Himalayan orogeny. Our study indicates that the anatomy of structures, composition, and tectonic nature is the key to a better understanding of orogenic belts, which may apply to all the orogenic belts around the world. 17YAP promotes osteogenesis and suppresses adipogenic differentiation by regulatingβ-catenin signaling显示文摘YAP(yes-associated protein) is a transcriptional factor that is negatively regulated by Hippo pathway, a conserved pathway for the development and size control of multiple organs. The exact function of YAP in bone homeostasis remains controversial. Here we provide evidence for YAP's function in promoting osteogenesis, suppressing adipogenesis, and thus maintaining bone homeostasis.YAP is selectively expressed in osteoblast(OB)-lineage cells. Conditionally knocking out Yap in the OB lineage in mice reduces cell proliferation and OB differentiation and increases adipocyte formation, resulting in a trabecular bone loss. Mechanistically, YAP interacts with β-catenin and is necessary for maintenance of nuclear β-catenin level and Wnt/β-catenin signaling. Expression of β-catenin in YAP-deficient BMSCs(bone marrow stromal cells) diminishes the osteogenesis deficit. 18Evaluation of soil fertility of the shelter-forest land along the Tarim Desert Highway显示文摘To study the changes of soil fertility of the shelter-forest land along the Tarim Desert Highway, soils from the forest land were collected at the layers of 0―10 cm, 10―20 cm, 20―30 cm. Different soil fertility parameters were measured, and quantitative evaluation of soil fertility was performed by the soil integrated fertility index (IFI). The main results show that the construction of the shelter forest along the Tarim Desert Highway improved the soil physical structure, increased soil porosity and enhanced wa- ter-holding capacity. With the increase of plantation time of the shelter forest, soil microbial biomass C, N, P and the activities of six types of enzyme were enhanced, which promoted the accumulation and transformation of soil nutrients of the forest land. Consequently, the soil nutrients in 12-year-old forest land were much higher than in the newer ones and drifting sand. However, soil salt content of the older forest land was higher owing to the drip-irrigation with salt water. Through the comprehensive evalua- tion, we found that soil fertility index in the forest land was enhanced with the forest age, and it had close correlations with the growth indices of the forest trees. In summary, construction of the shelterforest along the Tarim Desert Highway accelerated the improvement of aeolian soil in the forest land, and the soil fertility improved year by year. 20Immediate Effects of Hegu Needling on Adhesive Scapulohumeral Periarthritis显示文摘Objective:To observe the immediate effects of Hegu needling at Ashi points in patients with adhesive scapulohumeral periarthritis (SP). Methods:A total of 60 SP cases were randomized into a treatment group and a control group, 30 in each group. Cases in the treatment group were treated with Hegu needling at Ashi points and normal needling at other local points. Cases in the control group were treated with normal needling at local points alone. The therapeutic efficacy was then evaluated and analyzed using short-form of McGill pain questionnaire (SF-MPQ) and Japanese Orthopaedic Association (JOA) shoulder scoring system. Results:Hegu needling at Ashi points can substantially alleviate pain in patients with adhesive SP, showing a statistically significant difference when compared with the control group (P<0.01 or P<0.05). In addition, cases in the treatment group obtained better effects in abductor muscle strength, endurance, arm raising, internal rotation, external rotation, activities of daily living (ADL) and joint stability than those in the control group (P<0.01 or P<0.05).
