| 1 | Comparison of selected inflammation-based prognostic markers in relapsed or refractory metastatic colorectal cancer patients显示文摘AIM: To investigate the impact of systemic inflammationbased prognostic markers on overall survival in relapsed/refractory metastatic colorectal cancer(m CRC) patients. METHODS: To investigate prognostic markers in m CRC patients,this study was performed with patients who have experienced relapsed/refractory m CRC with standard chemotherapy or were inapplicable to conventional treatment modality because of poor performance status,age,or comorbidity. We reviewed the medical records of 177 m CRC patients managed with Korean Medicine(KM) treatment modality using an anticancer agent of Rhus verniciflua Stokes extract from June 2006 to April 2013. The clinicopathologic characteristics,laboratory test,the systemic inflammation markers including the modified Glasgow prognostic score(m GPS),neutrophil lymphocyte ratio(NLR),platelet lymphocyte ratio(PLR),lymphocyte monocyte ratio(LMR),and prognostic nutritional index(PNI) were analyzed. The overall survival of patients was calculated with the Kaplan-Meier method and the statistical significance was compared using with the log-rank test. To compare the impact of systemic inflammation based markers,the hazard ratio(HR) of m GPS,NLR,PLR,LMR,and PNI for overall survival were evaluated with the Cox proportional hazards regression.RESULTS: The majority of m CRC patients had relapsed/refractory to standard chemotherapy; 128 patients(72.3%) had undergone more than second line chemotherapy,and the median time from diagnosis of m CRC to initiation of KM was 9.4 mo. The median overall survival of enrolled patients was 8.3 mo. On univariate analyses,the inflammation markers of higher m GPS(P < 0.001),NLR ≥ 5(P < 0.001),PLR > 300(P = 0.004),LMR ≤ 3.4(P < 0.001),and PNI ≤ 45.3(P = 0.001) were significantly associated with decreased survival time. On stepwise multivariate proportional hazards model,m GPS at 2 vs 0(HR = 3.212,95%CI: 1.437-7.716,P = 0.004),and LMR ≤ 3.4(HR = 1.658,95%CI: 1.092-2.518,P = 0.018) as independent predictors associated with poor overall survival along with carbohydrate antigen 19-9(HR = 1.482,95%CI: 1.007-2.182,P = 0.046),AST ≥ 40(HR = 2.377,95%CI: 1.359-4.155,P = 0.002),and the treatment duration for KM less than 2.9 mo(HR = 1.718,95%CI: 1.160-2.543,P = 0.007).CONCLUSION: These results indicate that the inflammatory markers,m GPS and LMR are independent prognostic factors for predicting overall survival in relapsed/refractory m CRC patients. | Anna Song Wankyu Eo Sookyung Lee | 2015 | World Journal of Gastroenterology2015,21,43: | 8 |
| 2 | Absolute monocyte and lymphocyte count prognostic score for patients with gastric cancer显示文摘AIM:To measure the prognostic significance of absolute monocyte count/absolute lymphocyte count prognostic score(AMLPS) in patients with gastric cancer.METHODS:We retrospectively examined the combination of absolute monocyte count(AMC) and absolute lymphocyte count(ALC) as prognostic variables in a cohort of 299 gastric cancer patients who underwent surgical resection between 2006 and 2013 and were followed at a single institution.Both AMC and ALC were dichotomized into two groups using cut-off points determined by receiving operator characteristic curve analysis.An AMLPS was generated,which stratified patients into three risk groups:low risk(both low AMC and high ALC),intermediate risk(either high AMC or low ALC),and high risk(both high AMC and low ALC).The primary objective of the study was to validate the impact of AMLPS on both disease-free survival(DFS) and overall survival(OS),and the second objective was to assess the AMLPS as an independent prognostic factor for survival in comparison with known prognostic factors.RESULTS:Using data from the entire cohort,the most discriminative cut-off values of AMC and ALC selected on the receiver operating characteristic curve were 672.4/μL and 1734/μL for DFS and OS.AMLPS risk groups included 158(52.8%) patients in the lowrisk,128(42.8%) in the intermediate-risk,and 13(4.3%) in the high-risk group.With a median followup of 37.2 mo(range:1.7-91.4 mo),five-year DFS rates in the low-,intermediate-,and high-risk groups were 83.4%,78.7%,and 19.8%,respectively.And fiveyear OS rates in the low-,intermediate-,and high-risk groups were 89.3%,81.1%,and 14.4%,respectively.On multivariate analysis performed with patient- and tumor-related factors,we identified AMLPS,age,and pathologic tumor-node-metastasis stage as the most valuable prognostic factors impacting DFS and OS.CONCLUSION:AMLPS identified patients with a poor DFS and OS,and it was independent of age,pathologic stage,and various inflammatory markers. | Wan Kyu Eo Da Wun Jeong Hye Jung Chang Kyu Yeoun Won Sung Il Choi Se Hyun Kim Sung Wook Chun Young Lim Oh Tae Hwa Lee Young Ok Kim Ki Hyung Kim Yong Il Ji Ari Kim Heung Yeol Kim | 2015 | World Journal of Gastroenterology2015,21,9: | 6 |
| 3 | Nuclear imaging for functional evaluation and theragnosis in liver malignancy and transplantation显示文摘Currently,nuclear imaging such as positron emission tomography(PET)and single photon emission computed tomography(SPECT)is increasingly used in the management of liver malignancy.18F-fluorodeoxyglucose(FDG)-PET is the most widely used nuclear imaging in liver malignancy as in other cancers,and has been reported to be effective in diagnosis,response monitoring,recurrence evaluation,and prognosis prediction.Other PET imaging such as 11C-acetate PET is also used complementarily to FDG-PET in diagnosis of liver malignancy.Additionally,image-based evaluation of regional hepatic function can be performed using nuclear imaging.Those imaging modalities are also effective for candidate selection,treatment planning,and perioperative evaluation in liver surgery and transplantation.Recently,nuclear imaging has been actively adopted in the transarterial radioembolization therapy of liver malignancy,according to the concept of theragnosis.With the development of new hybrid imaging technologies such as PET/magnetic resonance imaging and SPECT/CT,nuclear imaging is expected to be more useful in the management of liver malignancy,particularly regarding liver surgery and transplantation.In this review,the efficacy and roles of nuclear imaging methods in diagnosis,transplantation and theragnosis are discussed. | Jae Seon Eo Jin Chul Paeng Dong Soo Lee | 2014 | World Journal of Gastroenterology2014,20,18: | 3 |

| 11 | Gouty tenosynovitis with compartment syndrome in the hand: A case report显示文摘BACKGROUND Gout is a common type of inflammatory arthritis caused by the deposition of monosodium urate crystals in the joints and surrounding tissues.It typically appears with abrupt and intense pain,redness,and swelling in the affected joint.It frequently targets the lower extremities,such as the big toe.However,rarely,gout can manifest in atypical locations,including the hands,leading to an uncommon presentation known as gouty tenosynovitis.However,it can result in significant morbidity owing to the potential for severe complications,such as myonecrosis and compartment syndrome.CASE SUMMARY An 82-year-old male patient with a history of hypertension,cerebral infarction,Parkinson's disease,and recurrent gout attacks sought medical attention because of progressive pain and swelling in the right hand.Imaging findings revealed forearm swelling,raising concerns of possible tenosynovitis,bursitis,septic arthritis,and compartment syndrome.A fasciotomy was performed to decompress the patient’s hands and forearms.The procedure revealed diffuse tenosynovitis,tophi with a pus-like discharge surrounding the carpal tunnel,and involvement of the flexor and extensor tendon sheaths.However,microbiological investigations,including Gram staining,acid-fast bacilli,tuberculosis,and nontuberculous mycobacteria,yielded negative results.The patient was ultimately diagnosed with a severe gouty attack with compartment syndrome and myonecrosis.Septic arthritis and infectious flexor tenosynovitis were ruled out.Serial debridement and inflammation control were initiated,followed by staged closure with a skin graft.CONCLUSION Septic-like complications can occur in the absence of infection in severe gout attacks with pus-like discharges due to compartment syndrome and myonecrosis.Cultures can be used to differentiate between gouty attacks,septic arthritis,and infectious tenosynovitis.Involvement of the flexor and extensor muscles,as in this case,is rare.This study contributes to the literature by reporting a rare case of successful fasciotomy and serial debridement in an elderly patient with multiple comorbidities. | Dong Yun Lee SuRak Eo SooA Lim Jung Soo Yoon | 2023 | World Journal of Clinical Cases2023,11,30: | 1 |
