1Laparoscopic Partial Nephrectomy With Segmental Renal Artery Clamping:Technique and Clinical Outcomes显示文摘Background:Warm ischemic injury is one of the most important factors affecting renal function in partial nephrectomy(PN).The technique of segmental renal artery clamping emerges as an alternative to conventional renal artery clamping for renal hilar control.Objective:To evaluate the feasibility and efficiency of laparoscopic PN(LPN) with segmental renal artery clamping in comparison with the conventional technique.Design,setting,and participants:A total of 75 patients underwent LPN from June 2007 to November 2009.All patients had T1a or T1b tumor in one kidney and a normal contralateral kidney.Thirty-seven patients underwent surgeries with main renal artery clamping,and 38 underwent surgeries with segmental artery clamping.Intervention:All procedures were performed by the same laparoscopic surgeon.Measurements:Blood loss,operation time,warm ischemia(WI) time,and complications affected renal function before and after operation were recorded.Results and limitations:All LPNs were completed without conversion to open surgery or nephrectomy.The novel technique slightly increased WI time(p < 0.001) and intraoperative blood loss(p = 0.006),while it provided better postoperative affected renal function(p < 0.001) compared with the conventional technique.The total complication rate was 12%.Among the 38 cases where segmental renal artery clamping was performed,7 had to convert to the conventional method.Tumor size and location influenced the number of clamped segmental arteries.Long-term postoperative renal function is still awaited.Conclusions:LPN with segmental artery clamping is safe and feasible in clinical practice.It minimizes the intraoperative WI injury and improves early postoperative affected renal function compared with main renal artery clamping.(C) 2010 European Association of Urology.Published by Elsevier B.V.All rights reserved.Shao,P.F. 3Pollen-inferred Holocene vegetation and climate histories in Taro Co,southwestern Tibetan Plateau显示文摘A 310-cm-long sediment core,covering the last10,200 years,was collected from Taro Co on the southwestern Tibetan Plateau and analyzed for pollen,grain size and total inorganic carbon content.The pollen data showed that vegetation changed from alpine steppe to alpine meadow during 10,200–8,900 cal a BP,to alpine steppe dominated by Artemisia during 8,900–7,400 cal a BP,to alpine meadow during 7,400–3,300 cal a BP and to alpine steppe after 3,300 cal a BP.Correspondingly,the pollen,grain size and total inorganic carbon content results revealed climatic change in this area over four stages.The initial stage was from 10,200 to 8,900 cal a BP,during which the climate changed from cold-dry to warm-humid.The second stage(8,900–7,400 cal a BP)was characterized by a warm and dry climate.However,at approximately7,400 cal a BP,the climate began to become cold and humid,which continued until 3,300 cal a BP.The last stage,from 3,300 cal a BP to present,was characterized as cold and increasingly arid.Climatic events of the early and mid-late Holocene showed that the area was significantly affected by the westerlies.However,the mid-Holocene climate in Taro Co was controlled by the Indian monsoon.The mid-late Holocene depositional environment record of Taro Co was very important to further elaborate the degree of influence by the westerlies or Indian monsoon.Qingfeng Ma Liping Zhu Xinmiao L Yun Guo Jianting Ju Junbo Wang Yong Wang Lingyu Tang 2014Chinese Science Bulletin2014,59,31:7
4The onset and advance of the Asian summer monsoon显示文摘Based on the daily outgoing longwave radiation (OLR) data from National Oceanic and At- mospheric Administration (NOAA) satellites, the Cli- mate Prediction Center’s merged analysis of pre- cipitation (CMAP) data and the National Centers for Environmental Prediction/National Center for At- mospheric Research (NCEP/NCAR) reanalysis dataset, the mean intraseasonal variability of the Asian summer monsoon (ASM) is investigated by using power spectrum analysis, band-pass filter, and diagnostic analyses. The processes of the onset and advance of monsoon over the southern part of Indo- china Peninsula, the east coast of Bay of Bengal, the South China Sea and the Indian subcontinent are explored. It is found that there is an abrupt change in OLR, precipitation and zonal wind during the onset and advance of the ASM. 5Hydrodynamic process of Tibetan Plateau lake revealed by grain size:Case study of Pumayum Co显示文摘Under the background of global warming,some lakes on the Tibetan Plateau(TP) are potentially sensitive to temperature change.With a case study of Pumayum Co,where glacier meltwater is important to supply(we call this a glacier-fed lake hereafter),we analyze the sensitivity of lake sediment grain size to temperature change.This is done by resolving the modern hydrodynamic process,coupled with comparison of paleoclimatic proxies.According to the spatial distributions of parameters,percentage of grain size and the grain size frequency distribution curve,hydrodynamic processes are analyzed.Five clastic sedimentation types are thereby discriminated.In the open lake area,suspended load transport is the main transport agent.Grain Size Trend Analysis(GSTA),a sediment dynamics model,reveals a trend toward eastward transport.This indicates that the largest and glacier-fed river,the Jiaqu River,influences the entire lake(not just the subsurface alluvial fan),and that lake sediment grain size may serve as a temperature indicator.Time series comparison between grain size of a short core from the central lake and meteorological data confirms this temperature indication,which in turn shows reliability of the method discriminating the hydrodynamic process.This case study will improve the ability of paleoclimatic reconstruction using lake sediment in glacier-fed lakes on the Tibetan Plateau.JU JianTing ZHU LiPing FENG JinLiang WANG JunBo WANG Yong XIE ManPing PENG Ping ZHEN XiaoLin LÜXinMiao 2012Chinese Science Bulletin2012,57,19:7
6Assessment of scoring systems for acute-on-chronic liver failure at predicting short-term mortality in patients with alcoholic hepatitis显示文摘AIM To assess the performance of proposed scores specific for acute-on-chronic liver failure in predicting shortterm mortality among patients with alcoholic hepatitis.METHODS We retrospectively collected data from 264 patients with clinically diagnosed alcoholic hepatitis from January to December 2013 at 21 academic hospitals in Korea. The performance for predicting short-term mortality was calculated for Chronic Liver FailureSequential Organ Failure Assessment(CLIF-SOFA), CLIF Consortium Organ Failure score(CLIF-C OFs), Maddrey'sdiscriminant function(DF), age, bilirubin, international normalized ratio and creatinine score(ABIC), Glasgow Alcoholic Hepatitis Score(GAHS), model for end-stage liver disease(MELD), and MELD-Na.RESULTS Of 264 patients, 32(12%) patients died within 28 d. The area under receiver operating characteristic curve of CLIF-SOFA, CLIF-C OFs, DF, ABIC, GAHS, MELD, and MELD-Na was 0.86(0.81-0.90), 0.89(0.84-0.92), 0.79(0.74-0.84), 0.78(0.72-0.83), 0.81(0.76-0.86), 0.83(0.78-0.88), and 0.83(0.78-0.88), respectively, for 28-d mortality. The performance of CLIF-SOFA had no statistically significant differences for 28-d mortality. The performance of CLIF-C OFs was superior to that of DF, ABIC, and GAHS, while comparable to that of MELD and MELD-Na in predicting 28-d mortality. 7Evolutionary trace analysis of eukaryotic DNA topoisomerase I superfamily: Identification of novel antitumor drug binding site显示文摘The studies of novel inhibitors of DNA topoisomerase I (Topo I) have already be-come very promising in cancer chemotherapy. Identifying the new drug-binding residues is playing an important role in the design and optimization of Topo I inhibitors. The designed com-pounds may have novel scaffolds, thus will be helpful to overcome the toxicities of current camptothecin (CPT) drugs and may provide a solution to cross resistance with these drugs. Mul-tiple sequence alignments were performed on eukaryotic DNA topoisomerase I superfamily and thus the evolutionary tree was constructed. The Evolutionary Trace method was applied to iden-tify functionally important residues of human Topo I. It has been demonstrated that class-specific hydrophobic residues Ala351, Met428, Pro431 are located around the 7,9-position of CPT, indi-cating suitable substitution of hydrophobic group on CPT will increase antitumor activity. The conservative residue Lys436 in the superfamily is of particular interest and new CPT derivatives designed based on this residue may greatly increase water solubility of such drugs. It has also been demonstrated that the residues Asn352 and Arg364 were conservative in the superfamily, whose mutation will render CPT resistance. As our molecular docking studies demonstrated they did not make any direct interaction with CPT, they are important drug-binding site residues for future design of novel non-camptothecin lead compounds. 8Diagnosis and surgical management of breast cancer metastatic to the spine显示文摘Breast cancer is the most common malignancy and the second leading cause of death in Western women. Breast cancer most commonly metastasizes to the bone and has a particular affinity with the spine, accounting for 2/3 of osseous metastases discovered. With significant improvements in cancer therapies, the number of patients at risk for symptomatic spinal metastases is likely to increase. Patients may suffer from intractable pain and neurological dysfunction, negatively influencing their quality of life. Timely diagnosis of patients is crucial and has been aided by several breakthrough advances in imaging techniques which aid in detection, staging, and follow-up of bone metastases. Breast metastases are usually responsive to hormonal therapy and pharmacologic interventions, but skeletal metastases often require surgical intervention. The treatments are palliative but goals include the preserving or restoring neurologic function, ensuring spinal stability, and relieving pain. Advances in surgical techniques and instrumentation have allowed more effective decompres-sion and stabilization of the spine, and with the support of recent evidence the trend has shifted towards using more advanced surgical options in appropriately selected patients. 9Linagliptin alleviates fatty liver disease in diabetic db/db mice显示文摘AIM To study the effects of linagliptin on the structural signs of non-alcoholic fatty liver disease(NAFLD) in db/db mice. METHODS Male diabetic db /db mice(BKS.Cg-Dock7m+/+Leprdb/J) aged 10 wk received the dipeptidyl peptidase 4(DPP4) inhibitor linagliptin(10 mg/kg) or saline as a placebo once per day by gavage for 8 wk. Intact db/db mice served as controls. Structural changes in the liver were analyzed from light and electron microscopic images of sections from intact, placebo-treated and linagliptin-treated animals. We estimated the changes in hepatocytes, sinusoidal cells, liver microvasculature and lymphatic roots. Hepatic staining for lymphatic vessel endothelial hyaluronan receptor-1(LYVE-1) was assessed by immunohistochemistry. RESULTS In 18-wk-old diabetic mice, liver steatosis(predominantly microvesicular and mediovesicular steatosis) was accompanied by dilation of the roots of the lymphatic system, interlobular blood vessels and bile canaliculi. Compared to saline-treated mice, linagliptin-treated mice exhibited a reduction in the mean numeral densities of hepatocytes with lipid droplets(92.4% ± 1.7% vs 64.9% ± 5.8% per field of view, P = 0.0002) and a lower proportion of hepatocytes with a high density of lipid droplets(20.7% ± 3.6% vs 50.4% ± 3.1%, P = 0.0007). We observed heterogeneous hepatocytes and relatively preserved cell structures in the linagliptin group. Dilation of blood and lymphatic vessels, as well as ultrastructural changes in the hepatocyte endoplasmic reticulum and mitochondria, were alleviated by linagliptin treatment. In intact and placebo-treated mice, immunohistochemical staining for LYVE-1 was observed in the endothelial cells of interlobular lymphatic vessels and on the membranes of some endothelial sinusoidal cells. We observed an enlarged LYVE-1 reaction area in linagliptin-treated mice compared to intact and placebo-treated mice. The improvement in the structural parameters of the liver in linagliptin-treated mice was independent to changes in the plasma glucose levels. 10Model for end-stage liver disease-Na score or Maddrey discrimination function index, which score is best?显示文摘AIM: To compare the ability of model for end-stage liver disease(MELD)-Na and Maddrey discrimination function index(DFI) to predict mortality at 30 and 90 d in patients with alcoholic hepatitis(AH).METHODS: We prospectively assessed 52 patients with AH. Demographic, clinical and laboratory parameters were obtained. MELD-Na and Maddrey DFI were calculated on admission. Short-term mortality was assessed at 30 and 90 d. Receiver operating characteristic curve analysis was performed. RESULTS: Thirty-day and 90-d mortality was 44% and 58%, respectively. In the univariate analysis, sodium levels was associated with mortality at 30 and 90 d(P = 0.001 and P = 0.03). Child stage, encephalopathy, ascites, or types of treatment were not associated with mortality. MELD-Na was the only predictive factor for mortality at 90 d. For 30-d mortality area under the curve(AUC) was 0.763(95%CI: 0.63-0.89) for Maddrey DFI and 0.784 for MELD-Na(95%CI: 0.65-0.91, P = 0.82). For 90-d mortality AUC was 0.685(95%CI: 0.54-0.83) for Maddrey DFI and 0.8710 for MELD-Na(95%CI: 0.76-0.97, P = 0.041). CONCLUSION: AH is associated with high shortterm mortality. 11Red meat intake and the risk of endometrial cancer:Metaanalysis of observational studies显示文摘AIM:To evaluate whether red meat intake is related to the risk of endometrial cancer(EC) using meta-analysis.METHODS:We searched Pub Med,EMBASE,and the Cochrane Library up to June 2013,using common keywords related to red meat and EC.Case-control studies and cohort studies comparing the risk of endometrial cancer among categories by the amount of intake were included.Eleven case-control studies and five cohort studies met our criteria.We performed a conventional and a dose-response meta-analysis of case-control studies using the Der Simonian-Laird method for random-effects.For cohort studies we performed a conventional meta-analysis.Publication bias was evaluated using Egger's test.RESULTS:In the meta-analysis of 11 case-control studies including 5419 cases and 12654 controls,higher red meat consumption was associated with an increased risk of EC [summary relative risk(SRR) = 1.43,95%CI:1.15-1.79;I2 = 73.3% comparing extreme intake categories).In a dose-response analysis,for red meat intake of 100 g/d,SRR was 1.84(95%CI:1.64-2.05).In contrast,in the meta-analysis of five prospective studies including a total of 2549 cases among 247746 participants,no significant association between red meat intake and EC risk(SRR = 0.97,95%CI:0.85-1.11;I2 = 4.9% comparing extreme intake categories) was observed.CONCLUSION:Our meta-analysis found a significantlinear association between red meat intake and EC risk based on case-control studies but this was not confirmed in prospective studies.Woong Ju Na Na Keum Dong Hoon Lee Yun Hwan Kim Seung Cheol Kim Eric L Ding Eunyoung Cho 2015World Journal of Meta-Analysis2015,3,2:2

13Analysis of influence of observation operator on sequential data assimilation through soil temperature simulation with common land model显示文摘An observation operator is a bridge linking the system state vector and observations in a data assimilation system. Despite its importance, the degree to which an observation operator influences the performance of data assimilation methods is still poorly understood. This study aimed to analyze the influences of linear and nonlinear observation operators on the sequential data assimilation through soil temperature simulation using the unscented particle filter(UPF) and the common land model. The linear observation operator between unprocessed simulations and observations was first established. To improve the correlation between simulations and observations, both were processed based on a series of equations. This processing essentially resulted in a nonlinear observation operator. The linear and nonlinear observation operators were then used along with the UPF in three assimilation experiments: an hourly in situ soil surface temperature assimilation, a daily in situ soil surface temperature assimilation, and a moderate resolution imaging spectroradiometer(MODIS) land surface temperature(LST) assimilation. The results show that the filter improved the soil temperature simulation significantly with the linear and nonlinear observation operators. The nonlinear observation operator improved the UPF's performance more significantly for the hourly and daily in situ observation assimilations than the linear observation operator did, while the situation was opposite for the MODIS LST assimilation. Because of the high assimilation frequency and data quality, the simulation accuracy was significantly improved in all soil layers for hourly in situ soil surface temperature assimilation, while the significant improvements of the simulation accuracy were limited to the lower soil layers for the assimilation experiments with low assimilation frequency or low data quality.Xiao-lei Fu Zhong-bo Yu Yong-jian Ding Ying Tang Hai-shen Lü Xiao-lei Jiang Qin Ju 2018Water Science and Engineering2018,11,3:2
