| 1 | Gallbladder cancer epidemiology, pathogenesis and molecular genetics: Recent update显示文摘Gallbladder cancer is a malignancy of biliary tract which is infrequent in developed countries but common in some specific geographical regions of developing countries. Late diagnosis and deprived prognosis are major problems for treatment of gallbladder carcinoma. The dramatic associations of this orphan cancer with various genetic and environmental factors are responsible for its poorly defined pathogenesis. An understanding to the relationship between epidemiology, molecular genetics and pathogenesis of gallbladder cancer can add new insights to its undetermined pathophysiology. Present review article provides a recent update regarding epidemiology, pathogenesis, and molecular genetics of gallbladder cancer. We systematically reviewed published literature on gallbladder cancer from online search engine Pub Med(http://gffzz16a62c593b5540cehnv69kwpqnup660ow.ffgz.tsg.suse.edu.cn/pubmed). Various keywords used for retrieval of articles were Gallbladder, cancer Epidemiology, molecular genetics and bullion operators like AND, OR, NOT. | 3 | Diagnosis of Helicobacter pylori : What should be the gold standard?显示文摘Since the discovery of Helicobacter pylori(H. pylori) in 1983, numerous detection methods for the presence of the bacterium have been developed. Each one of them has been associated with advantages and disadvantages. Noninvasive tests such as serology, 13 C urea breath test(UBT) and stool antigen tests are usually preferred by the clinicians. Serology has its own limitation especially in endemic areas while 13 C UBT is technically very demanding. The stool antigen detection method, although specific, is usually associated with poor sensitivity. The 13 C UBT is believed to be specific, but with present revelation of the fact that stomach is colonized by many other urease producing bacteria makes it questionable. Histology, culture, rapid urease test and polymerase chain reaction(PCR) are the tests which are carried out on antral biopsies collected by invasive means. Histology has been proposed to be very sensitive and specific but the question is how by simply looking the morphology of the bacteria in the microscope, one can claim that the curved bacterium is exclusively H. pylori. Rapid urease test(RUT), the doctor's test, is also challenged because the presence of other urease producing bacteria in the stomach cannot be denied. Moreover, RUT has been reported with poor sensitivity specially, when density of the bacterium is low. Isolation of H. pylori is essential to investigate its growth requirements, antibiotic susceptibility testing, studying virulence factor to develop vaccine and many more explorations. It has also got several disadvantages i.e., special condition for transporting, media, incubation and few days waiting for the colonies to appear, apart from the speed essentially needed to process the specimens. Till date, majority of the microbiological laboratories in the world are not equipped and trained to isolate such fastidious bacterium. The option left is PCR methods to detect H. pylori 's DNA in gastric mucosa, gastric juice, saliva, dental plaques and environmental specimens. There are speculations for false positivity due to detection of non-pylori Helicobacters due to genetic sharing; and false negativity due to low bacterial counts and presence of PCR inhibitors. However, specimen collection, transportation and processing do not require speed and special conditions. PCR based diagnosis may be considered as gold standard by designing primers extremely specific to H. pylori and targeting at least more than one conserved genes. Similarly specificity of PCR may be improved by use of internal Primers. Further, nested PCR will take care of false negatives by countering the effect of PCR inhibitors and low bacterial counts. | 4 | Anterior resection for rectal carcinoma-risk factors for anastomotic leaks and strictures显示文摘AIM:To determine the incidence and factors responsible for anastomotic leaks and stricture following anterior resection(AR)and its subsequent management.METHODS:Retrospective analysis of data from 108 patients with rectal carcinoma who underwent AR or low anterior resection(LAR)to identify the various preoperative,operative,and post operative factors that might have influence on anastomotic leaks and strictures.RESULTS:There were 68 males and 40 females with an average of 47 years(range 21-75 years).The median distance of the tumor from the anal verge was 8 cm(range 3-15 cm).Sixty(55.6%)patients underwent handsewn anastomosis and 48(44.4%)were stapled.The median operating time was 3.5 h(range2.0-7.5 h).Sixteen(14.6%)patients had an anastomotic leak.Among these,11 patients required reexploration and five were managed expectantly.The anastomotic leak rate was similar in patients with and without diverting stoma(8/60,13.4%with stoma and 8/48;16.7%without stoma).In 15(13.9%)patients,resection margins were positive for malignancy.Ninteen(17.6%)patients developed anastomotic strictures at a median duration of 8 mo(range 3-20 mo).Among these,15 patients were successfully managed with per-anal dilatation.On multivariate analysis,advance age(>60 years)was the only risk factor for anastomotic leak(P=0.004).On the other hand,anastomotic leak(P=0.00),mucin positive tumor(P =0.021),and lower rectal growth(P=0.011)were found as risk factors for the development of an anastomotic stricture.CONCLUSION:Advance age is a risk factor for an anastomotic leak.An anastomotic leak,a mucin-secreting tumor,and lower rectal growth predispose patients to develop anastomotic strictures. | Ashok Kumar Ram Daga Paari Vijayaragavan Anand Prakash Rajneesh Kumar Singh Anu Behari Vinay K Kapoor Rajan Saxena | 2011 | World Journal of Gastroenterology2011,17,11: | 28 |

| 6 | Computer vision-based limestone rock-type classification using probabilistic neural network显示文摘Proper quality planning of limestone raw materials is an essential job of maintaining desired feed in cement plant. Rock-type identification is an integrated part of quality planning for limestone mine. In this paper, a computer vision-based rock-type classification algorithm is proposed for fast and reliable identification without human intervention. A laboratory scale vision-based model was developed using probabilistic neural network(PNN) where color histogram features are used as input. The color image histogram-based features that include weighted mean, skewness and kurtosis features are extracted for all three color space red, green, and blue. A total nine features are used as input for the PNN classification model. The smoothing parameter for PNN model is selected judicially to develop an optimal or close to the optimum classification model. The developed PPN is validated using the test data set and results reveal that the proposed vision-based model can perform satisfactorily for classifying limestone rocktypes. Overall the error of mis-classification is below 6%. | 7 | Differential expression of interleukin-1/Toll-like receptor signaling regulators in microscopic and ulcerative colitis显示文摘AIM:To investigate Toll-like receptor(TLR)signaling regulators in microscopic and ulcerative colitis patients.METHODS:Total RNA and microRNA were isolated from fresh frozen colonic biopsies of non-inflamed controls and patients with active or in-remission collagenous colitis(CC),lymphocytic colitis(LC),or ulcerative colitis(UC).We compared expressions of interleukin-1receptor-associated kinase(IRAK)-2,IRAK-M,interleukin(IL)-37,microRNA(miR)-146a,miR-155,and miR-21 using quantitative real time reverse transcription polymerase chain reaction.RESULTS:IRAK-M expression was increased in LC patients with active disease in histopathological remission(LC-HR;P=0.02)and UC patients(P=0.01),but no differences in IRAK-2 expression were detected compared to controls.miR-146a,-155 and-21 expressions were increased in LC-HR(P=0.04,0.07,and 0.004)and UC(P=0.02,0.04 and 0.03)patients.miR-146a and miR-21 expressions were significantly enhanced in UC patients compared to UC remission(UC-R;P=0.01and 0.04).Likewise,active CC patients showed significantly increased expression of miR-155(P=0.003)and miR-21(P=0.006).IL-37 expression was decreased in both CC(P=0.03)and LC(P=0.04)patients with a similar trend in UC patients but not statistically significant,whilst it was increased in UC-R patients compared to controls(P=0.02)and active UC(P=0.001).CONCLUSION:The identification of differentially expressed miRNAs,IL-37,and IRAK-M suggests different pathophysiologic mechanisms in various disease stages in LC,CC,and UC. | Sezin Gunaltay Nils Nyhlin Ashok Kumar Kumawat Curt Tysk Johan Bohr Olof Hultgren Elisabeth Hultgren Hornquist | 2014 | World Journal of Gastroenterology2014,20,34: | 11 |
| 8 | Colorectal carcinogenesis:Insights into the cell death and signal transduction pathways:A review显示文摘Colorectal carcinogenesis(CRC) imposes a major health burden in developing countries. It is the third major cause of cancer deaths. Despite several treatment strategies, novel drugs are warranted to reduce the severity of this disease. Adenomatous polyps in the colon are the major culprits in CRC and found in 45% of cancers, especially in patients 60 years of age. Inflammatory polyps are currently gaining attention in CRC, and a growing body of evidence denotes the role of inflammation in CRC. Several experimental models are being employed to investigate CRC in animals, which include the APC^(min/+) mouse model, Azoxymethane, Dimethyl hydrazine, and a combination of Dextran sodium sulphate and dimethyl hydrazine. During CRC progression, several signal transduction pathways are activated. Among the major signal transduction pathways are p53, Transforming growth factor beta, Wnt/β-catenin, Delta Notch, Hippo signalling, nuclear factor erythroid 2-related factor 2 and Kelch-like ECH-associated protein 1 pathways. These signalling pathways collaborate with cell death mechanisms, which include apoptosis, necroptosis and autophagy, to determine cell fate. Extensive research has been carried out in our laboratory to investigate these signal transduction and cell death mechanistic pathways in CRC. | 11 | Fabrication of Bi_2O_3/TiO_2nanocomposites and their applications to the degradation of pollutants in air and water under visible-light显示文摘A nanoheterojunction composite photocatalyst Bi2O3/TiO2working under visible-light(λ 420 nm) was prepared by combining two semiconductors Bi2O3 and TiO2 varying the Bi2O3/TiO2molar ratio. Maleic acid was employed as an organic binder to unite Bi2O3 and TiO2 nanoparticles. The SEM, TEM, XRD and diffuse reflectance spectra were utilized to characterize the prepared Bi2O3/TiO2nanoheterojunction. The nanocomposite exhibited unusual high photocatalytic activity in decomposing 2-propanol in gas phase and phenol in aqueous phase and, evolution of CO2 under visible light irradiation while the end members exhibited low photocatalytic activity. The composite was optimized to 5 mol% Bi2O3/TiO2. | 12 | WISH:wavefront imaging sensor with high resolution显示文摘Wavefront sensing is the simultaneous measurement of the amplitude and phase of an incoming optical field.Traditional wavefront sensors such as Shack-Hartmann wavefront sensor(SHWFS)suffer from a fundamental tradeoff between spatial resolution and phase estimation and consequently can only achieve a resolution of a few thousand pixels.To break this tradeoff,we present a novel computational-imaging-based technique,namely,the Wavefront Imaging Sensor with High resolution(WISH).We replace the microlens array in SHWFS with a spatial light modulator(SLM)and use a computational phase-retrieval algorithm to recover the incident wavefront.This wavefront sensor can measure highly varying optical fields at more than 10-megapixel resolution with the fine phase estimation.To the best of our knowledge,this resolution is an order of magnitude higher than the current noninterferometric wavefront sensors.To demonstrate the capability of WISH,we present three applications,which cover a wide range of spatial scales.First,we produce the diffraction-limited reconstruction for long-distance imaging by combining WISH with a large-aperture,low-quality Fresnel lens.Second,we show the recovery of high-resolution images of objects that are obscured by scattering.Third,we show that WISH can be used as a microscope without an objective lens.Our study suggests that the designing principle of WISH,which combines optical modulators and computational algorithms to sense high-resolution optical fields,enables improved capabilities in many existing applications while revealing entirely new,hitherto unexplored application areas. | Yicheng Wu Manoj Kumar Sharma Ashok Veeraraghavan | 2019 | Light(Science & Applications)2019,8,1: | 6 |
| 13 | 细菌性角膜溃疡的医院流行病学,诱因和微生物诊断(英文)显示文摘目的:确定细菌性角膜溃疡的发病诱因,临床和微生物特性。方法:本研究涉及300例(300眼)临床上疑似微生物感染的角膜溃疡患者。通过病史回顾和裂隙灯检查获得病例资料。采用标准技术完成角膜刮片检查。取部分刮片样本加入100g/L的氢氧化钾液后,以及采用革兰氏和吉姆萨染色,再直接在显微镜下观察是否存在真菌、细菌和棘阿米巴。另一部分刮片样本直接接种在血琼脂、麦康基琼脂、巧克力琼脂和萨布罗琼脂表面。通过角膜刮片检查,细菌性角膜溃疡被确诊为化脓性角膜溃疡浸润和表层上皮细胞缺失伴细菌感染。对患者应用抗菌治疗。结果:在随访期间,因丢失而被排除的有60个病例。在剩下的240人中,被确诊为细菌性角膜溃疡的156人(65%),大多数为男性(102眼),年龄分布14~74岁。角膜溃疡位于角膜中央的96人(61.5%),位于外周的60人(38.5%)。溃疡深度<1/3角膜厚度的82人(52.6%)。前房炎症为:Tyndall现象和浮游细胞1+~2+64人。角膜涂片中分离出细菌的有125人(80%),其中69%为革兰氏阳性,39%为革兰氏阴性。革兰氏阴性菌感染患者伴有重症前房炎症(P=0.003)并且深度>2/3角膜厚度(P=0.001)。分离得到的最常见致病微生物为金黄色葡萄球菌。治疗后40%的患者视力恢复到入院时的水平甚至更好,其余60%的患者预后差。结论:细菌性角膜溃疡是一种严重的眼部感染性疾病,对眼部存在视力威胁,在治疗上仍具挑战性。快速分离细菌和眼部抗生素的应用是治疗此类疾病的关键。 | Ashok Kumar Narsani Shafi Muhammad Jatoi Mahesh Kumar Lohana Syed Asher Dabir Siddiqa Gul Mahtab Alam Khanzada | 2009 | 国际眼科杂志2009,9,3: | 6 |
| 14 | Evaluation of wound healing and antimicrobial potentials of Ixora coccinea root extract显示文摘Objective:To evaluate the wound healing and antimicrobial activity of root extracts of Ixora coccinea(I.coccinea).Methods:To investigate the wound healing efficacy of root extract of I.coccinea Linn,five groups of animals were divided each containing six animals.Two wound models including incision and excision wound models were used in this study.The parameters studied were tensile strength on incision wound model and in terms of wound contraction for excision wound model were compared with standard Nitrofurazone(NFZ) ointment(0.2%w/w). Six extracts(ethanol.aqueous,petroleum ether,benzene,chloroform and ethyl acetate ) of I. coccinea were screened for in vitro growth inhibiting activity against different bacterial strains riz. Staphylococcia aureus.Bacillus pumilius,Enterococcus facialis.Escherichia coli,Salmonella typhi and Pseudomonas aeruginosa and fungi Candida albicans and Aspergillus niger were compared with the standard drugs ciprofloxacin and chloramphenicol for antibacterial and griscofulvin for antifungal screening.The serial dilution and cup(or) well plate methods were used for the antimicrobial study and MIC was determined.Results:The ethanolic extract showed significant (P<0.001) wound healing activity when compared to standard drug NFZ with respect to normal control group.Amongst all.ethanolic extract showed highly significant antibacterial activity against all bacterial strains used in this study when compared to standard.The aqueous extract showed moderate significant inhibition against all bacterial strains when compared to standard. All the extracts were shown negligible activity against the fungal strains used in this study. | 15 | 尾穗苋甲醇提取物对扑热息痛致大鼠肝损伤的保护作用(英文)显示文摘目的:检测尾穗苋(Amaranthus caudatus)甲醇提取物对扑热息痛致大鼠肝损伤的保护作用。方法:5组大鼠分别给予羧甲基纤维素钠、扑热息痛、尾穗苋甲醇提取物(200或400mg/kg)并扑热息痛、水飞蓟素并扑热息痛。给药14d后采集大鼠血样进行血液生化检查,而后处死大鼠,收集各组大鼠肝脏组织样本供组织病理学检查。检测的指标包括肝功能标志物如丙氨酸氨基转移酶(alanine aminotransferase,ALT)、天门冬氨酸氨基转移酶(aspartate aminotransferase,AST)、血清白蛋白(albumin,ALB)、总蛋白(total protein,TP)、总胆红素(total bilirubin,TB)和直接胆红素(direct bilirubin,DB)及氧化损伤标志物如丙二醛(malondialdehyde,MDA)、还原型谷胱甘肽(reduced glutathione,GSH)、总硫醇(total thiols,TT)和过氧化氢酶(catalase,CAT)。结果:与未治疗组相比,浓度为200和400mg/kg的尾穗苋甲醇提取物均能显著改善由扑热息痛引起的大鼠肝损伤,改善扑热息痛导致的ALT、AST、TB和DB水平升高及血清ALB水平降低(P<0.01)。与未治疗组相比,浓度为200和400mg/kg的尾穗苋甲醇提取物能够显著降低扑热息痛导致的MDA及TP水平增高(P<0.01),并显著升高GSH、CAT及TT水平(P<0.01)。大鼠肝脏组织病理学检查的结果也证实了尾穗苋甲醇提取物对大鼠肝脏的保护作用。结论:本研究证实了尾穗苋的甲醇提取物对于扑热息痛引起的大鼠肝损伤具有显著的保护作用,证实了其在民族医学中治疗肝病的药用价值。 | Bagepalli Srinivas Ashok Kumar Kuruba Lakshman Peresandra Avalakondarayppa Arun Kumar Gollapalle Lakshminarayana Shastry Viswantha Veeresh Prabhakar Veerapur Boreddy Shivanadppa Thippeswamy Bachappa Manoj | 2011 | 中西医结合学报2011,9,2: | 5 |

These include smoking, diet, hygiene, drugs, geographical and psychosocial factors. These factors may either increase the risk of or protect against developing this condition and can also affect the course of illness in a positive or negative manner. A number of studies have examined the influence of environmental factors on inflammatory bowel diseases as a whole as well as on ulcerative colitis and Crohn's disease separately. As there are differences in the pathogenesis of ulcerative colitis and Crohn's disease, the effect of environmental factors on their onset and course is not always similar. Some factors have shown a consistent association, while reports on others have been conflicting.
