5推动消除疼痛治疗差异的国家议题:美国卫生政策、教育、医疗实践和研究指南(简版)显示文摘背景:疼痛会给个人和社会带来沉重负担。任何有关疼痛的法案都必须重点关注如何消除美国医疗体系内广泛存在的疼痛治疗差异问题。目标:本文主要关注与美国疼痛治疗差异性有关的,并有可能进行政策干预的宏观因素。方法:我们明确了可以实现疼痛治疗公平性的重要机会,特别是由近期美国医疗系统立法改革引出的机会。一个进取性的政策、倡议和研究议程由5个部分构成:①结构/系统;②政策和倡议;③医疗从业人员;④医疗服务提供者;以及⑤研究相关性。结果:疼痛治疗的不公平性是一个重要的卫生政策问题,但是却常常被忽视。平价医疗法案(ACA)中的许多直接和间接条款以及其他利用ACA法案推动的全国性计划都为实现疼痛治疗公平性提供了机会。这些机会包括对保险、社会、医疗服务提供者、立法者进行的教育改革,对初级医疗机构和疼痛专科医生培训方面进行改革、提高医疗从业人员的多样性、实现数据采集的种族/民族统一性、着重强调以患者为中心的转归研究,以及鼓励在疗效比较研究模式中关注疼痛治疗的差异性。结论:最近在ACA法案框架内发起的全国性立法倡议将会在多个层面上影响财政资金的流向,来解决医疗系统内广泛存在的治疗差异性问题。对疼痛治疗界来说,这是个绝佳的机会,可以首先利用这些全国性倡议,推进落实一项共同议题来解决疼痛治疗差异性的问题。Meghani SH Polomano RC Tait RC Vallerand AH Anderson KO Gallagher RM 2014中国疼痛医学杂志2014,20,2:1
