2Effect of acupotomy versus electroacupuncture on ethology and morphology in a rabbit model of knee osteoarthritis显示文摘OBJECTIVE: To evaluate the treatment effect of acupotomy(Apo) in a rabbit model of knee osteoarthritis(KOA), compare the results of Apo versus electroacupuncture(E-Apu) on ethology, morphology, and structure of the articular cartilage surface in a rabbit model of KOA, and analyze the differences in the treatment effects of Apo versus E-Apu.METHODS: Twenty-eight male New Zealand white rabbits were randomized into four groups: normal control, blank model, Apo, and E-Apu(n = 7 in each group). Except for the normal control group, the left hindlegs of all rabbits were fixed in an extended position for 5 weeks to establish the KOA model.The passive range of motion(PROM) and Lequesne index were measured before and after the establishment of the KOA model to assess the ethology in all groups. Safranin O-fast green staining and the Mankin score were used to assess the morphological cartilaginous changes to compare the effect of Apo versus E-Apu on the degeneration of articular cartilage, and to identify which therapy was superior in treating KOA.RESULTS: Compared with before the establishment of the KOA model, the Lequesne index of the KOA model rabbits was significantly increased(P <0.01), while the PROM was significantly decreased(P < 0.01). The articular cartilaginous tissue in the three model groups exhibited pathological variations in the form of laminar derangement of cartilage cells, and so the Mankin score was significantly increased compared with the control group(P <0.01). At 1 week after the final treatment session,compared with the blank model group, both the Apo and E-Apu groups showed a significant decrease in the Lequesne index(P < 0.01), and attenuation in the degree of morphologic pathological changes(P < 0.05); The Apo improved the PROM significantly compared with the blank model group(P < 0.05), while the E-Apu had no effect(P > 0.05).Furthermore, compared with the E-Apu group, the Apo group had a significantly lower Lequesne index(P < 0.05), and a significantly greater PROM(P <0.05).CONCLUSION: In a rabbit model of KOA, both Apo and E-Apu reduce disorders of ethology and morphology, and improve the condition of the articular cartilage. 3Evaluation of patients with dry eye disease for conjunctival Chlamydia trachomatis and Ureaplasma urealyticum显示文摘AIM:To determine the possibility of the development of dry eye disease(DED) as a result of persistent infection with Chlamydia trachomatis and Ureaplasma urealyticum in the conjunctiva of patients.METHODS: This study was conducted on 58 patients of age range 20-50 y,diagnosed with DED confirmed by Schirmer I test and tear breakup time.The non-dry eye control group included 27 subjects of the same age.Ocular specimens were collected as conjunctival scrapings and swabs divided into three groups: the first used for bacterial culture,the second and third taken to detect Chlamydia trachomatis and Ureaplasma urealyticum by direct fluorescent antibody(DFA) assay and polymerase chain reaction(PCR) method. RESULTS: Chlamydia trachomatis was detected in 65.5% and 76% of DED patients by DFA and PCR methods respectively.Ureaplasma urealyticum was found in 44.8% of DED infected patients using the PCR method.Both organisms were identified in only 37.9% of DED patients found to be infected.Control subjects had a 22%detection rate of Chlamydia trachomatis by DFA assay versus a 7% detection rate by PCR; while Ureaplasma urealyticum was detected in 3.7% of the controls by PCR method.The conjunctival culture revealed that gram positive microorganisms represented 75% of isolates with coagulase negative Staphylococci the most common(50%) followed by Staphylococcus aureus(20%),whereas gram negative microorganisms occurred in 25% of cases,isolating Moraxella spp.as the most frequent organism. CONCLUSION: Our results tend to point out that Chlamydia trachomatis and Ureaplasma urealyticum were detected in a moderate percentage of patients with DED,and could be a fair possibility for its development.PCR is more reliable in detecting Chlamydia trachomatis than DFA technique.The presence of isolated conjunctival bacterial microflora can be of some potential value.Maha Mohssen Abdelfattah Rania Abdelmonem Khattab Magda H.Mahran Ebrahim S.Elborgy 2016International Journal of Ophthalmology(English edition)2016,9,10:6
4Targeting host factors:A novel rationale for the management of hepatitis C virus显示文摘Hepatitis C is recognized as a major threat to global public health.The current treatment of patients with chronic hepatitis C is the addition of ribavirin to interferon-based therapy which has limited eff icacy, poor tolerability, and signif icant expense.New treatment options that are more potent and less toxic are much needed.Moreover, more effective treatment is an urgent priority for those who relapse or do not respond to current regimens.A major obstacle in combating hepatitis C virus(HCV) infection is that the fidelity of the viral replication machinery is notoriously low, thus enabling the virus to quickly develop mutations that resist compounds targeting viral enzymes.Therefore, an approach targeting the host cofactors, which are indispensable for the propagation of viruses, may be an ideal target for the development of antivira agents because they have a lower rate of mutation than that of the viral genome, as long as they have no side effects to patients.Drugs targeting, for example, receptors of viral entry, host metabolism or nuclear receptors, which are factors required to complete the HCV life cycle, may be more effective in combating the vira infection.Targeting host cofactors of the HCV life cycle is an attractive concept because it imposes a higher genetic barrier for resistance than direct antiviral compounds.However the principle drawback of this strategy is the greater potential for cellular toxicity.Mahmoud Aboelneen Khattab 2009World Journal of Gastroenterology2009,15,28:5
5Role of biomarkers in the prediction and diagnosis of hepatocellular carcinoma显示文摘The prevalence of hepatocellular carcinoma(HCC)has progressively increased in recent years and is now thefifth and the second most common cancer in the World and in Egypt,respectively.Much work has focused in the development of assays for detecting hepatic carcinogensis before the observance of hepatic focal lesions.Particular attention has been directed towards HCC-specific biomarkers for use in the early diagnosis of HCC and in the confirmation of radiological studies.Although a number of biomarkers have been identified,none have been considered reliable indicators of early HCC lesions.This review presents a few of the most relevant HCC biomarkers and suggests improvements to the accuracy of diagnostic assays through their combined use.Furthermore,we present an algorithm for the biomarker-based diagnosis of HCC and highlight its important role in the early prediction of HCC.Mahmoud Khattab Magdy Fouad Elham Ahmed 2015World Journal of Hepatology2015,7,23:5
6Novel non-invasive biological predictive index for liver fibrosis in hepatitis C virus genotype 4 patients显示文摘AIM To investigate the diagnostic ability of a non-invasive biological marker to predict liver fibrosis in hepatitis C genotype 4 patients with high accuracy.METHODS A cohort of 332 patients infected with hepatitis C genotype 4 was included in this cross-sectional study. Fasting plasma glucose, insulin, C-peptide, and angiotensinconverting enzyme serum levels were measured. Insulin resistance was mathematically calculated using the homeostasis model of insulin resistance(HOMA-IR).RESULTS Fibrosis stages were distributed based on Metavir score as follows: F0 = 43, F1 = 136, F2 = 64, F3 = 45 and F4 = 44. Statistical analysis relied upon reclassification of fibrosis stages into mild fibrosis(F0-F) = 179, moderate fibrosis(F2) = 64, and advanced fibrosis(F3-F4) = 89. Univariate analysis indicated that age, log aspartate amino transaminase, log HOMA-IR and log platelet count were independent predictors of liver fibrosis stage(P < 0.0001). A stepwise multivariate discriminant functional analysis was used to drive a discriminative model for liver fibrosis. Our index used cut-off values of ≥ 0.86 and ≤-0.31 to diagnose advanced and mild fibrosis, respectively, with receiving operating characteristics of 0.91 and 0.88, respectively. 7Study of the prevalence and association of ocular chlamydial conjunctivitis in women with genital infection by Chlamydia trachomatis, Mycoplasma genitalium and Candida albicans attending outpatient clinic显示文摘AIM: To determine the association between chlamydial conjunctivitis and genital infection by Chlamydia trachomatis, Mycoplasma genitalium and Candida albicans, in addition to the possible relationship between cultured bacterial pathogens and oculogenital chlamydial infection.METHODS: This study was performed on 100(50 symptomatic and 50 asymptomatic) women attending the Gynecological and Obstetric outpatient clinic of Alzahra hospital, Alazhar University. Simultaneously a conjunctival swab was taken from these patients.Polymerase chain reaction(PCR) was done on DNA extracted from both vaginal and conjunctival swab samples. Culture for both vaginal and conjunctival swabs was also done.RESULTS: Candida albicans was the predominant organism isolated by culture in 20% and 40% of conjunctival and vaginal swabs respectively. By the PCR method, ocular Chlamydia trachomatis was present in 60% of symptomatic women, while genital Chlamydia trachomatis infection was present in 30% of symptomatic women. The results of this method also indicated that 25/50(50%) vaginal swabs were positive with PCR for Candida albicans versus 15/50(30%) were PCR positive in conjunctival swab. Mycoplasma genitalium was present in only 10% of vaginal swabs.Concomitant oculogenital PCR positive results for Chlamydia trachomatis and Candida albicans were 30% and 28% respectively.CONCLUSION: Ocular Chlamydia trachomatis was associated with genital Chlamydia trachomatis in a high percentage of women followed by Candida albicans.Cultured bacterial organisms do not play a role in enhancement of Chlamydia trachomatis infection.Rania Abdelmonem Khattab Maha Mohssen Abdelfattah 2016International Journal of Ophthalmology(English edition)2016,9,8:2

9Expression levels of pro-inflammatory interleukin-8 and certain antimicrobial peptides in concurrent with bacterial conjunctivitis显示文摘AIM:To detect the quantitative expression levels of the pro-inflammatory interleukin-8(IL8),antimicrobial peptides human beta defense-2(HBD2),and human beta defense-3(HBD3)genes in bacterial conjunctivitis.METHODS:The human conjunctival epithelial cells were obtained using the impression cytology technique from healthy controls and patients.The genes expression levels were determined utilizing a reverse transcription quantitative polymerase chain reaction(RT-q PCR).The contribution of causative agent type,the number of isolates and severity of clinical features,in the increase of genes expression was also determined.RESULTS:The RT-q PCR showed that IL8,HBD2,and HBD3 expression increased in bacterial conjunctivitis as compared to healthy control(P<0.001).In gram-negative bacterial conjunctivitis,HBD2 was highly up-regulated(P<0.001)compared to other types of bacterial conjunctivitis.In mixed bacterial conjunctivitis,a direct correlation between HBD2 up-regulation and HBD3 up-regulation was observed(P<0.05).The severity of clinical features was related to the up-regulation of IL8 and HBD2(P<0.05).CONCLUSION:IL8,HBD2,and HBD3 are immuneeffectors in infectious conjunctivitis.HBD2 is active during different bacterial conjunctivitis but is more released with gram-negative bacteria compared to gram-positive bacteria.HBD3 is an obvious defender in different bacterial conjunctivitis.Alaa El-Dien Shawky Hosny Zeinab El-Demerdash El-Bazza Mohammed Abdelhalim Ramadan Maha Ahmed Shafik Mahmoud Ahmed Shafeek Rania Abdelmonem Khattab 2021International Journal of Ophthalmology(English edition)2021,14,5:2
