| 1 | Efficacy and safety of over-the-scope clip: Including complications after endoscopic submucosal dissection显示文摘AIM: To retrospectively review the results of over-thescope clip (OTSC) use in our hospital and to examine the feasibility of using the OTSC to treat perforations after endoscopic submucosal dissection (ESD). METHODS: We enrolled 23 patients who presented with gastrointestinal (GI) bleeding, fistulae and perforations and were treated with OTSCs (Ovesco Endoscopy GmbH, Tuebingen, Germany) between November 2011 and September 2012. Maximum lesion size was defined as lesion diameter. The number of OTSCs to be used per patient was not decided until the lesion was completely closed. We used a twin grasper (Ovesco Endoscopy GmbH, Tuebingen, Germany) as a grasping device for all the patients. A 9 mm OTSC was chosen for use in the esophagus and colon, and a 10 mm device was used for the stomach, duodenum and rectum. The overall success rate and complications were evaluated, with a particular emphasis on patients who had undergone ESD due to adenocarcinoma. In technical successful cases we included not only complete closing by using OTSCs, but also partial closing where complete closure with OTSCs is almost difficult. In overall clinical successful cases we included only complete closing by using only OTSCs perfectly. All the OTSCs were placed by 2 experienced endoscopists. The sites closed after ESD included not only the perforation site but also all defective ulcers sites.RESULTS: A total of 23 patients [mean age 77 years (range 64-98 years)] underwent OTSC placement during the study period. The indications for OTSC placement were GI bleeding (n = 9), perforation (n = 10), fistula (n = 4) and the prevention of post-ESD duodenal artificial ulcer perforation (n = 1). One patient had a perforation caused by a glycerin enema, after which a fistula formed. Lesion closure using the OTSC alone was successful in 19 out of 23 patients, and overall success rate was 82.6%. A large lesion size (greater than 20 mm) and a delayed diagnosis (more than 1 wk) were the major contributing factors for the overall unsuccessful clinical cases. The location of the unsuccessful lesion was in the stomach. The median operation time in the successful cases was 18 min, and the average observation time was 67 d. During the observation period, none of the patients experienced any complications associated with OTSC placement. In addition, we successfully used the OTSC to close the perforation site after ESD in 6 patients. This was a single-center, retrospective study with a small sample size. | 3 | The utility of point-of-care ultrasound in the assessment of volume status in acute and critically ill patients显示文摘BACKGROUND:Volume resuscitation has only been demonstrated to be effective in approximately fifty percent of patients.The remaining patients do not respond to volume resuscitation and may even develop adverse outcomes(such as acute pulmonary edema necessitating endotracheal intubation).We believe that point-of-care ultrasound is an excellent modality by which to adequately predict which patients may benefi t from volume resuscitation.DATA RESOURCES:We performed a search using PubMed,Scopus,and MEDLINE.The following search terms were used:fluid responsiveness,ultrasound,non-invasive,hemodynamic,fluid challenge,and passive leg raise.Preference was given to clinical trials and review articles that were most relevant to the topic of assessing a patient’s cardiovascular ability to respond to intravenous fl uid administration using ultrasound.RESULTS:Point-of-care ultrasound can be easily employed to measure the diameter and collapsibility of various large vessels including the inferior vena cava,common carotid artery,subclavian vein,internal jugular vein,and femoral vein.Such parameters are closely related to dynamic measures of fluid responsiveness and can be used by providers to help guide fluid resuscitation in critically ill patients.CONCLUSION:Ultrasound in combination with passive leg raise is a non-invasive,costand time-effective modality that can be employed to assess volume status and response to fluid resuscitation.Traditionally sonographic studies have focused on the evaluation of large veins such as the inferior vena cava,and internal jugular vein.A number of recently published studies also demonstrate the usefulness of evaluating large arteries to predict volume status. | Ali Pourmand Matthew Pyle David Yamane Kazi Sumon Sarah E.Frasure | 2019 | World Journal of Emergency Medicine2019,10,4: | 18 |

| 11 | Real-Time Safety Helmet Detection Using Yolov5 at Construction Sites显示文摘The construction industry has always remained the economic and social backbone of any country in the world where occupational health and safety(OHS)is of prime importance.Like in other developing countries,this industry pays very little,rather negligible attention to OHS practices in Pakistan,resulting in the occurrence of a wide variety of accidents,mishaps,and near-misses every year.One of the major causes of such mishaps is the non-wearing of safety helmets(hard hats)at construction sites where falling objects from a height are unavoid-able.In most cases,this leads to serious brain injuries in people present at the site in general and the workers in particular.It is one of the leading causes of human fatalities at construction sites.In the United States,the Occupational Safety and Health Administration(OSHA)requires construction companies through safety laws to ensure the use of well-defined personal protective equipment(PPE).It has long been a problem to ensure the use of PPE because round-the-clock human monitoring is not possible.However,such monitoring through technological aids or automated tools is very much possible.The present study describes a systema-tic strategy based on deep learning(DL)models built on the You-Only-Look-Once(YOLOV5)architecture that could be used for monitoring workers’hard hats in real-time.It can indicate whether a worker is wearing a hat or not.The proposed system usesfive different models of the YOLOV5,namely YOLOV5n,YOLOv5s,YOLOv5 m,YOLOv5l,and YOLOv5x for object detection with the support of PyTorch,involving 7063 images.The results of the study show that among the DL models,the YOLOV5x has a high performance of 95.8%in terms of the mAP,while the YOLOV5n has the fastest detection speed of 70.4 frames per second(FPS).The proposed model can be successfully used in practice to recognize the hard hat worn by a worker. | Kisaezehra Muhammad Umer Farooq Muhammad Aslam Bhutto Abdul Karim Kazi | 2023 | Intelligent Automation & Soft Computing2023,,4: | 2 |

| 16 | Difficulty in differentiating two cases of sigmoid stenosis by diverticulitis from cancer显示文摘The incidence of colonic diverticulosis with or without diverticulitis has increased in the Japanese population due to the modernization of food and aging.The rate of diverticulitis in colon diverticulosis ranges from 8.1% to 9.6%.However,few cases of stenosis due to diverticulitis have been reported.These reports suggest that the differentiation between sigmoid diverticulitis and colon cancer is difficult.This report describes two cases of colon stenosis due to diverticulitis that were difficult to differentiate from colon cancer.Case 1 was a 70-year-old woman with narrowed stools for 1 month who underwent colonofiberscopy (CFS).CFS revealed a diverticulum and circumferential stenosis in the sigmoid colon.Barium enema revealed a marked,hourglassshaped,2-cm circumferential stenosis in the sigmoid colon.Fluorodeoxyglucose (FDG)-positron emission tomography computed tomography (CT) revealed an increased FDG uptake at the affected portion of the sigmoid colon.Sigmoid colon cancer was suspected,and laparoscopic sigmoidectomy was performed.Pathological examination demonstrated active inflammation with no evidence of malignancy.Case 2 was a 50-yearold man who presented to a nearby clinic with reduced stool output despite the urge to defecate.CFS detected severe stenosis in the sigmoid colon approximately 25 cm from the dentate line.Contrast-enhanced abdominal CT revealed multiple diverticula,wall thickening,and swelling of the lymph nodes around the peritoneal aorta and the inferior mesenteric artery.A partial sigmoidectomy was performed.Pathological examination of the resected specimen revealed no changes in the mucosal epithelial surface,but a marked infiltration of inflammatory cells was observed. | Noriko Nishiyama Hirohito Mori Hideki Kobara Kazi Rafiq Shintarou Fujihara Mitsuyoshi Kobayashi Tsutomu Masaki | 2012 | World Journal of Gastroenterology2012,18,27: | 2 |
