2Clinical characteristics and management of patients with fat embolism syndrome in level I Apex Trauma Centre显示文摘Purpose: Fat embolism syndrome (FES) is systemic manifestation of fat emboli in the circulation seen mostly after long bone fractures. FES is considered a lethal complication of trauma. There are various case reports and series describing FES. Here we describe the clinical characteristics, management in ICU and outcome of these patients in level I trauma center in a span of 6 months. Methods: In this prospective study, analysis of all the patients with FES admitted in our polytrauma intensive care unit (ICU) of level I trauma center over a period of 6 months (from August 2017 to January 2018) was done. Demographic data, clinical features, management in ICU and outcome were analyzed. Results: We admitted 10 cases of FES. The mean age of patients was 31.2 years. The mean duration from time of injury to onset of symptoms was 56 h. All patients presented with hypoxemia and petechiae but central nervous system symptoms were present in 70% of patients. The mean duration of mechanical ventilation was 11.7 days and the mean length of ICU stay was 14.7 days. There was excellent recovery among patients with no neurological deficit. Conclusion: FES is considered a lethal complication of trauma but timely management can result in favorable outcome. FES can occur even after fixation of the fracture. Hypoxia is the most common and earliest feature of FES followed by CNS manifestations. 6Clinical characteristics of COVID-19 patients who underwent tracheostomy and its effect on outcome:A retrospective observational study显示文摘BACKGROUND The exponential rise in Coronavirus disease 2019(COVID-19)cases has resulted in an increased number of patients requiring prolonged ventilatory support and subsequent tracheostomy.With the limited availability of literature regarding the outcomes of COVID-19 patients with tracheostomy,we attempted to study the clinical characteristics and multiple parameters affecting the outcomes in these patients.AIM To determine all-cause mortality following tracheostomy and its association with various risk factors in COVID-19 patients.METHODS This retrospective study included 73 adult COVID-19 patients admitted to the ICU between 1 April,2020 and 30 September,2021 who underwent tracheostomy as a result of acute respiratory failure due to COVID-19.The data collected included demographics(age,sex),comorbidities,type of oxygen support at admission,severity of COVID-19,complications,and other parameters such as admission to tracheostomy,intubation to tracheostomy,ICU stay,hospital stay,and outcome.RESULTS This study included 73 adult patients with an average age of 52±16.67 years,of which 52%were men.The average time for admission to tracheostomy was 18.12±12.98 days while intubation to tracheostomy was 11.97±9 days.The mortality rate was 71.2%and 28.8%of patients were discharged alive.The mean duration of ICU and hospital stay was 25±11 days and 28.21±11.60 days,respectively.Greater age,severe COVID-19,mechanical ventilation,shock and acute kidney injury were associated with poor prognosis;however,early tracheostomy in intubated patients resulted in better outcomes.CONCLUSION Patients with severe COVID-19 requiring mechanical ventilation have a poor prognosis but patients with early tracheostomy may benefit with no added risk.We recommend that the timing of tracheostomy be decided on a case-by-case basis and a well-designed randomised controlled trial should be performed to elucidate the potential benefit of early tracheostomy in such patients.Yudhyavir Singh Kapil Dev Soni Abhishek Singh Nikita Choudhary Fahina Perveen Richa Aggarwal Nishant Patel Shailendra Kumar Anjan Trikha 2022World Journal of Virology2022,11,6:0
7Increase in optic nerve sheath diameter predicts early cerebral involvement in fat embolism syndrome显示文摘Early diagnosis of cerebral fat embolism in a patient with contradiction to MRI is challenging.Here we report an interesting case,where the raised optic nerve sheath diameter helped us to predict the early cerebral involvement with fat emboli in a left femoral shaft fracture patient.MRI scan could not be performed due to the presence of a metallic implant in the patient from a previous surgery.He was later diagnosed as an atypical presentation of fat embolism syndrome.Optic nerve sheath monitoring also helped us to guide further management of the patient.Arnab Banerjee Richa Aggarwal Kapil dev Soni Anjan Tirkha 2021Chinese Journal of Traumatology2021,24,3:0
8A Comprehensive Review on Preclinical Safety and Toxicity of Medicinal Plants显示文摘Background:Globally,80%people use plant-derived products for treating or preventing diseases.One prevalent perception about medicinal plants is that they are safe and devoid of adverse effects.However,approximately 1,50,000 plants contain toxic substances.Objective:The present review focuses on medicinal plant extracts/fractions toxicity assessments made in preclin-ical models by oral route.Methods:Detail studies were searched from databases including PubMed and Google Scholar.A manual reference screening of the selected studies was done to identify relevant articles,with no language restriction being imposed at the time of searching.Results:The studies included were performed in rodents,and the test substances were administered orally.Our search revealed 33 widely used plants or products with significant toxicity,and phytochemicals from these plants have been summarized.Through a systematic review,we identified a plethora of medicinal plant extracts reporting safety and toxicity concerns.Conclusion:In the future,preclinical toxicokinetic studies of herbs and the determination of their no-observed-adverse-effect levels are required for a complete safety assessment.Finally,the interaction of herbs with com-monly used/over-the-counter drugs in terms of the latter’s metabolic profile should be undertaken.Madhav Nilakanth Mugale Kapil Dev Bhumika S.More Vaishali Sunil Mishra Kaveri R.Washimkar Kishan Singh Rakesh Maurya Srikanta Kumar Rath Debprasad Chattopadhyay Naibedya Chattopadhyay 2024Clinical Complementary Medicine and Pharmacology2024,4,1:0
9In vivo detection of metabolic^(2)H-incorporation upon ingestion of^(2)H_(2)O显示文摘Objective:The aim of this work was to explore the feasibility of in vivo and non-invasive monitoring of deuterium/hydrogen(^(2)H/1H)exchange at the metabolic level upon exposure to heavy water(^(2)H_(2)O).Methods:The healthy female mice were randomly assigned to two groups after day 0 when both mice received standard drinking water.The treated mouse was fed with^(2)H_(2)O(80%,v/v)and the control mouse fed with standard drinking water(H2O)over next 13days.Real-time mass spectrometric analysis of volatile metabolism emitted through breathing and the skin was performed on days 1,2,3,10,12,and 13.Animal experiment was approved by the Laboratory Animal Ethics Committee of Jinan University(approval No.20161117163322)on October 29,2021.Results:We observed a replacement of 1H by^(2)H in 52 mass spectral features(60^(2)H/1H isotopologue pairs)for the mouse fed with^(2)H_(2)O,but not for the control mouse.These included pyruvic acid and lactic acid,lysine and methyl-lysine as well as short-chain fatty acids comprising acetic acid,propionic acid,butyric acid and valeric acid.Conclusion:Secondary electrospray ionization-high resolution mass spectrometry allows monitoring in vivo^(2)H-incorporation of metabolites in a non-invasive and real-time setup and opens new opportunities to use^(2)H tracing to extend current metabolic studies,especially those with a focus on anaerobic glycolysis,lysine methylation and gut microbiome via monitoring of short-chain fatty acids.Kim Arnold Xing Chen Hui Zhang Kapil Dev Singh Zhihong Yin Yao Yao Tiangang Luan Pablo Sinues Xue Li 2022Journal of Bio-X Research2022,5,2:0
