| 1 | Clinical importance of aspirin and clopidogrel resistance显示文摘Aspirin and clopidogrel are important components of medical therapy for patients with acute coronary syndromes, for those who received coronary artery stents and in the secondary prevention of ischaemic stroke. Despite their use, a significant number of patients experience recurrent adverse ischaemic events. Interindividual variability of platelet aggregation in response to these antiplatelet agents may be an explanation for some of these recurrent events, and small trials have linked 'aspirin and/or clopidogrel resistance', as measured by platelet function tests, to adverse events. We systematically reviewed all available evidence on the prevalence of aspirin/clopidogrel resistance, their possible risk factors and their association with clinical outcomes. We also identified articles showing possible treatments. After analyzing the data on different laboratory methods, we found that aspirin/clopidogrel resistance seems to be associated with poor clinical outcomes and there is currently no standardized or widely accepted definition of clopidogrel resistance. | 2 | A study of solid ramjet fuel containing boron–magnesium mixtures显示文摘Solid fuel ramjets (SFRJ) are known for their operational simplicity and high specific impulse. The performance of the SFRJ propulsion system is directly tied to the energy density and combustion behavior of the fuel. A typical solid fuel used in a ramjet application is a collection of metal particles suspended in a polymeric binder. Boron is the ubiquitous candidate when considering metal additives for fuels due to an impressive 122.5 kJ/cm3 energy density. However, boron requires long residence times in combustors due to its high melting and boiling points. Magnesium appears to be a natural complement to boron;while possessing a lower energy density (42.1 kJ/cm3), it burns with a high flame temperature and readily reacts in combustion with a low melting point. In this study, several HTPB–boron–magnesium fuels are studied on a small scale to evaluate performance for ramjet application. Holography experiments are conducted, as well as laser ignition tests, to study particle behavior just above the fuel surface. Small, center-perforated fuel grains are examined in a direct-connect SFRJ test stand configuration to measure ignition temperatures and performance parameters. | 3 | Pulmonary embolization as primary manifestation of hepatocellular carcinoma with intracardiac penetration: A case report显示文摘Intracardiac manifestation of hepatocellular carcinoma (HCC) is a rare condition and an uncommon finding even at autopsy. Pulmonary tumor embolism as a presenting feature of HCC has been published only twice previously.In our case report, a 63-year-old man presented with high fever and six episodes of recurrent pneumonias during the last half year. Echocardiography was performed, a solid mass was found in the right atrium. Transesophageal echocardiography proved a tumor mass in the inferior vena cava (ⅣC) extending into the right atrium, abdominal ultrasound revealed tumor mass in the ⅣC and a solid tumor in the liver. Combined liver and heart surgery was attempted in order to remove the tumor mass from both the liver and the right atrium. Acute cor pulmonale occurred during tumor removal from the right atrium and the patient expired. In addition to local factors the possibility of embolization should arise in the background of recurrent pneumonia. Occult carcinoma must be included in possible causes of recurrent pulmonary embolism. Searching for primary malignancy should include HCC as frequent cause of hypercoagulability. | 4 | Accelerating CFD–DEM simulation of processes with wide particle size distributions显示文摘Size-reduction systems have been extensively used in industry for many years. Nevertheless, reliable engineering tools to be used to predict the comminution of particles are scarce. Computational fluid dynamics(CFD)–discrete element model(DEM) numerical simulation may be used to predict such a complex phenomenon and therefore establish a proper design and optimization model for comminution systems.They may also be used to predict attrition in systems where particle attrition is significant. Therefore,empirical comminution functions(which are applicable for any attrition/comminution process), such as:strength distribution, selection, equivalence, breakage, and fatigue, have been integrated into the threedimensional CFD–DEM simulation tool. The main drawback of such a design tool is the long computational time required owing to the large number of particles and the minute time-step required to maintain a steady solution while simulating the flow of particulate materials with very fine particles.The present study developed several methods to accelerate CFD–DEM simulations: reducing the number of operations carried out at the single-particle level, constructing a DEM grid detached from the CFD grid enabling a no binary search, generating a sub-grid within the DEM grid to enable a no binary search for fine particles, and increasing the computational time-step and eliminating the finest particles in the simulation while still tracking their contribution to the process.The total speedup of the simulation process without the elimination of the finest particles was a factor of about 17.
