| 1 | Changing trends in management of gestational diabetes mellitus显示文摘Gestational diabetes mellitus(GDM)is on the rise globally.In view of the increasing prevalence of GDM and fetal and neonatal complications associated with it,there is a splurge of research in this field and management of GDM is undergoing a sea change.Trends are changing in prevention,screening,diagnosis,treatment and future follow up.There is emerging evidence regarding use of moderate exercise,probiotics and vitamin D in the prevention of GDM.Regarding treatment,newer insulin analogs like aspart,lispro and detemir are associated with better glycemic control than older insulins.Continuous glucose monitoring systems and continuous subcutaneous insulin systems may play a role in those who require higher doses of insulin for sugar control.Evidence exists that favors metformin as a safer alternative to insulin in view of good glycemic control and better perinatal outcomes.As the risk of developing GDM in subsequent pregnancies and also the risk of overt diabetes in later life is high,regular assessment of these women is required in future.Lifestyle interventions or metformin should be offered to women with a history of GDM who develop pre-diabetes.Further studies are required in the field of prevention of GDM for optimizing obstetric outcome. | Gunasekaran Kala Poomalar | 2015 | World Journal of Diabetes2015,6,2: | 32 |
| 2 | Community Responses to Mountain Tourism:A Case in Bhyundar Valley,Indian Himalaya显示文摘Tourism industry has become an important part of Indian economy,and tourism development has been,however,concentrated to a few areas. Accumulation of tourism activities has reached the critical stage in most of the tourism centers,which pose serious threats to the natural environment. The Bhyundar Valley of Nanda Devi Biosphere Reserve (NDBR) in Uttarakhand State of the Indian Himalaya is a burning example of such problems. Land use/cover changes,deforestation and accumulation of garbage are some of the negative impacts of unregulated tourism in the valley. Unsustainable activities of tourists and villagers in the valley make such problems very severe. To deal with the ongoing problems and ensure community involvement in tourism management and environmental conservation,the Biosphere Reserve Authority introduced ecotourism in the form of Eco Development Committees (EDCs) in the Bhyundar Valley. Study reveals that many problems associated with tourism have been minimized to some extent with the help of EDCs but few are still prevailing in valley. | 3 | Abnormal liver enzymes: A review for clinicians显示文摘Liver biochemical tests are some of the most commonly ordered routine tests in the inpatient and outpatient setting,especially with the automatization of testing in this technological era.These tests include aminotransferases,alkaline phosphatase,gamma-glutamyl transferase,bilirubin,albumin,prothrombin time and international normalized ratio(INR).Abnormal liver biochemical tests can be categorized based on the pattern and the magnitude of aminotransferases elevation.Generally,abnormalities in aminotransferases can be classified into a hepatocellular pattern or cholestatic pattern and can be further sub-classified based on the magnitude of aminotransferase elevation to mild[<5×upper limit of normal(ULN)],moderate(>5-<15×ULN)and severe(>15×ULN).Hepatocellular pattern causes include but are not limited to;non-alcoholic fatty liver disease/non-alcoholic steatohepatitis,alcohol use,chronic viral hepatitis,liver cirrhosis(variable),autoimmune hepatitis,hemochromatosis,Wilson’s disease,alpha-1 antitrypsin deficiency,celiac disease,medication-induced and ischemic hepatitis.Cholestatic pattern causes include but is not limited to;biliary pathology(obstruction,autoimmune),other conditions with hyperbilirubinemia(conjugated and unconjugated).It is crucial to interpret these commonly ordered tests accurately as appropriate further workup,treatment and referral can greatly benefit the patient due to prompt treatment which can improve the natural history of several of the diseases mentioned and possibly reduce the risk of progression to the liver cirrhosis. | M Ammar Kalas Luis Chavez Monica Leon Pahnwat Tonya Taweesedt Salim Surani | 2021 | World Journal of Hepatology2021,13,11: | 5 |
| 4 | Helicobacter pylori infection and other bacteria in pancreatic cancer and autoimmune pancreatitis显示文摘Helicobacter pylori(H.pylori)is an infectious agent influencing as much as 50%of the world’s population.It is the causative agent for several diseases,most especially gastric and duodenal peptic ulcer,gastric adenocarcinoma and mucosaassociated lymphoid tissue lymphoma of the stomach.A number of other,extragastric manifestations also are associated with H.pylori infection.These include neurological disorders,such as Alzheimer’s disease,demyelinating multiple sclerosis and Parkinson’s disease.There is also evidence for a relationship between H.pylori infection and such dermatological diseases as psoriasis and rosacea as well as a connection with infection and open-angle glaucoma.Generally little is known about the relationship between H.pylori infection and diseases of the pancreas.Most evidence about H.pylori and its potential role in the development of pancreatic diseases concerns pancreatic adenocarcinoma and autoimmune forms of chronic pancreatitis.There is data(albeit not fully consistent)indicating modestly increased pancreatic cancer risk in H.pylori-positive patients.The pathogenetic mechanism of this increase is not yet fully elucidated,but several theories have been proposed.Reduction of antral Dcells in H.pylori-positive patients causes a suppression of somatostatin secretion that,in turn,stimulates increased secretin secretion.That stimulates pancreatic growth and thus increases the risk of carcinogenesis.Alternatively,H.pylori,as a part of microbiome dysbiosis and the so-called oncobiome,is proven to be associated with pancreatic adenocarcinoma development via the promotion of cellular proliferation.The role of H.pylori in the inflammation characteristic of autoimmune pancreatitis seems to be explained by a mechanism of molecular mimicry among several proteins(mostly enzymes)of H.pylori and pancreatic tissue.Patients with autoimmune pancreatitis often show positivity for antibodies against H.pylori proteins.H.pylori,as a part of microbiome dysbiosis,also is viewed as a potential trigger of autoimmune inflammation of the pancreas.It is precisely these relationships(and associated equivocal conclusions)that constitute a center of attention among pancreatologists,immunologists and pathologists.In order to obtain clear and valid results,more studies on sufficiently large cohorts of patients are needed.The topic is itself sufficiently significant to draw the interest of clinicians and inspire further systematic research.Next-generation sequencing could play an important role in investigating the microbiome as a potential diagnostic and prognostic biomarker for pancreatic cancer. | Lumir Kunovsky Petr Dite Petr Jabandziev Jiri Dolina Jitka Vaculova Martin Blaho Martina Bojkova Jana Dvorackova Magdalena Uvirova Zdenek Kala Jan Trna | 2021 | World Journal of Gastrointestinal Oncology2021,13,8: | 5 |
| 5 | Predicting the impact of climate change on the distribution of two threatened Himalayan medicinal plants of Liliaceae in Nepal显示文摘Predicting the potential distribution of medicinal plants in response to climate change is essential for their conservation and management. Contributing to the management program, this study aimed to predict the distribution of two threatened medicinal plants, Fritillaria cirrhosa and Lilium nepalense. The location of focal species gathered from herbarium specimen housed in different herbaria and online databases were geo-referenced and checked for spatial autocorrelation. The predictive environmental variables were selected, and MaxE nt software was used to model the current and future distributions of focal species. Four Representative Concentration Pathway(RCP) trajectories of the BCC-CSM1.1 model were used as the future(2050) projection layer. The MaxEnt modelling delineated the potential distribution of F. cirrhosa and L. nepalense. The current suitability is projected towards Central and Eastern Hilly/Mountainous regions. Both species gainmaximum suitability in RCP 4.5 which decline towards other trajectories for L. nepalense. Overall, both the focal species shift towards the north-west, losing their potential habitat in hilly and lower mountainous regions by 2050 across all trajectories. Our results highlight the impact of future climate change on two threatened and valuable species. | 7 | Geoclimatic factors influence the population genetic connectivity of Incarvillea arguta(Bignoniaceae)in the Himalaya-Hengduan Mountains biodiversity hotspot显示文摘Geoclimatic factors related to the uplift of the Himalaya and the Quaternary climatic oscillations in fluence the population genetic connectivity in the Himalaya-Hengduan Mountains(HHM)biodiversity hotspot.Therefore,to explore the relative roles played by these two factors,we examined the population dynamics and dispersal corridors of Incarvillea arguta(Royle)Royle incorporating ensemble species distribution modelling(SDM).Thirty-seven populations were genotyped using plastid chloroplast DNA and low copy nuclear gene(ncpCS)sequences.Phylogeographic analysis was carried out to reveal the genetic structure and lineage differentiation.Ensemble SDMs were carried out for distributional change in the last glacial maximum,present,and future.Finally,the least cost path method was used to trace out possible dispersal corridors.The haplotypes were divided into four clades with strong geographical structure.The late Miocene origin of I.arguta in the western Himalaya ca.7.92 Ma indicates lineage diversification related to the uplift of the HHM.The variability in habitat connectivity revealed by SDM is due to change in suitability since the Pleistocene.A putative dispersal corridor was detected along the drainage systems and river valleys,with strong support in the eastern Hengduan Mountains group.Our results support the signature of geoclimatic influence on population genetic connectivity of I.arguta in the HHM.We proposed that the major drainage systems might have assisted the rapid dispersal of isolated riverine plant species I.arguta in the HHM.The population genetic connectivity,using the fine-tuned ensemble SDMs,enables scientists and policymakers to develop conservation strategies for the species gene pool in the HHM biodiversity hotspots. | Santosh Kumar Rana Dong Luo Hum Kala Rana Alexander Robert O'Neill Hang Sun | 2021 | Journal of Systematics and Evolution2021,59,1: | 3 |

| 10 | Distribution pattern and conservation of threatened medicinal and aromatic plants of Central Himalaya, India显示文摘A study was conducted to examine the distribution pattern of four rhizomatous medicinal and aromatic plant species (MAPs) viz., Angelica glauca, Pleurospermum angelicoides, Rheum emodi and Arne- bia benthamii in different forest stands in Central Himalaya. Results show that A. glauca and P. angelicoides had a higher (50%) frequency at Chipkoan, Garpak and Phagati forest, R. emodi had a higher (60%) frequency at Rishikund, Suki and Himtoli, and A. benthamii had a higher (70%) frequency at Suki and Khambdhar The densities of A. glauca (0.6 plants·m -2 ) and P. angelicoides (0.5 plants·m -2 ) were higher at Chipkoan and Garpak sites than at other micro-sites, while densities of R. emodi (0.8 plants·m -2 ) and A. benthamii (1.0 plants·m -2 ) were higher at Suki and Khambdhar sites. A. glauca had highest total basal covers (TBC) (1.2 cm 2 ·m -2 ) at Chipkoan, P. angelicoides had highest TBC (0.92 cm 2 ·m -2) at Lati kharak site, A. benthamii had the highest TBC (6.48 cm 2 ·m -2 ) atKhambdhar, and R. emodi had highest TBC (4.53 cm 2 ·m -2 ) at Rishikund.
