1模糊证据权方法在镇沅(老王寨)地区金矿资源评价中的应用显示文摘采用模糊证据权方法和GeoDASGIS技术开展了镇沅(老王寨)及其邻区的金矿资源潜力评价.分别采用GeoDASGIS软件提供的局部奇异性分析技术、S-A异常分解技术、主成分分析技术、证据权、模糊证据权等技术对相关地球化学元素进行了系统的处理和分析.应用主成分分析方法确定了可能的2种不同成矿类型,并采用主成分得分确定了组合异常点,在此基础上分别采用普通证据权和模糊证据权方法编制了成矿后验概率图,圈定了有利成矿地段.对比普通证据权方法与模糊证据权方法所得结果表明,模糊证据权方法可减小图层离散化造成的有用信息损失,提高预测结果精度.成秋明 陈志军 Ali Khaled 2007地球科学(中国地质大学学报)2007,32,2:55

3Postoperative outcomes after open splenectomy versus laparoscopic splenectomy in cirrhotic patients:a meta-analysis显示文摘BACKGROUND: Laparoscopic splenectomy is considered the gold standard for resecting normal-to-moderately bigger spleens in benign conditions, and in addition could be tried for patients with malignant splenic disorders. However, the safety of laparoscopic splenectomy in patients with hypersplenism is not well-known. This study aimed to investigate the efficacy and safety of laparoscopic splenectomy for patients with hypersplenism secondary to liver cirrhosis by comparing with the open splenectomy.DATA SOURCES: Several databases were searched to identify comparative studies fulfilling the predefined selection criteria from January 2000 to June 2015. The subsequent key words were utilized for browsing 'laparoscopy' or 'laparoscopic', 'open', 'splenectomy', and 'liver cirrhosis'. Studies evaluating laparoscopic and open splenectomy for patients with liver cirrhosis were incorporated. Two evaluators personally strained the title and abstract of each publication. Citations with contemplated compliance within our eligibility criteria underwent compressed review. Meta-analysis was carried out according to the recommendations of the Cochrane Collaboration software(review manager 5.1).RESULTS: Seven studies containing 509 patients were included. Compared with the open splenectomy group, patients in thelaparoscopic splenectomy group had significantly less intraoperative blood loss(MD=210.30; 95% CI: 11.28-409.32; P=0.04), longer operative time(MD=-31.58; 95% CI:-53.34--9.82; P=0.004), shorter duration of postoperative hospital stay(MD=3.41; 95% CI: 2.39-4.43; P<0.01), lower incidence of postoperative complications(RR=1.34; 95% CI: 0.88-2.01; P=0.17), and decreased liver damage [ALT(MD=8.52; 95% CI: 0.19- 16.85; P=0.05) and total bilirubin(MD=5.12; 95% CI: 0.37-9.87; P=0.03)].CONCLUSION: Hypersplenism secondary to cirrhosis and portal hypertension should not be a contraindication for laparoscopic splenectomy.Khaled Al-raimi Shu-Sen Zheng 2016Hepatobiliary & Pancreatic Diseases International2016,15,1:23

6Anterior cruciate ligament reconstruction best practice: A review of graft choice显示文摘There is much literature about differing grafts used in anterior cruciate ligament(ACL) reconstruction. Much of this is of poor quality and of a low evidence base. We review and summarise the literature looking at the four main classes of grafts used in ACL reconstruction; bone-patella tendon-bone, hamstrings, allograft and synthetic grafts. Each graft has the evidence for its use reviewed and then compared, where possible, to the others. We conclude that although there is no clear 'best' graft, there are clear differences between the differing graft choices. 7Ciliary parathyroid hormone signaling activates transforming growth factor-βto maintain intervertebral disc homeostasis during aging显示文摘Degenerative disc disease(DDD) is associated with intervertebral disc degeneration of spinal instability. Here, we report that the cilia of nucleus pulposus(NP) cells mediate mechanotransduction to maintain anabolic activity in the discs. We found that mechanical stress promotes transport of parathyroid hormone 1 receptor(PTH1 R) to the cilia and enhances parathyroid hormone(PTH) signaling in NP cells. PTH induces transcription of integrin α_vβ_6 to activate the transforming growth factor(TGF)-β-connective tissue growth factor(CCN2)-matrix proteins signaling cascade. Intermittent injection of PTH(iPTH) effectively attenuates disc degeneration of aged mice by direct signaling through NP cells, specifically improving intervertebral disc height and volume by increasing levels of TGF-β activity, CCN2, and aggrecan. PTH1 R is expressed in both mouse and human NP cells. Importantly,knockout PTH1 R or cilia in the NP cells results in significant disc degeneration and blunts the effect of PTH on attenuation of aged discs. 8Recent biological trends in management of fracture non-union显示文摘Bone regeneration is a complex, well-orchestrated physiological process of bone formation, which can be seen during normal fracture healing, and is involved in continuous remodelling throughout adult life. Currently,there is a plethora of different strategies to augment the impaired or 'insufficient' bone-regeneration process, including the 'gold standard' autologous bone graft, free fibula vascularised graft, allograft implantation, and use of growth factors, osteoconductive scaffolds, osteoprogenitor cells and distraction osteogenesis. Improved 'local' strategies in terms of tissue engineering and gene therapy, or even 'systemic' enhancement of bone repair, are under intense investigation, in an effort to overcome the limitations of the current methods, to produce bone-graft substitutes with biomechanical properties that are as identical to normal bone as possible, to accelerate the overall regeneration process, or even to address systemic conditions, such as skeletal disorders and osteoporosis. An improved understanding of the molecular and cellular events that occur during bone repair and remodeling has led to the development of biologic agents that can augment the biological microenvironment and enhance bone repair. Orthobiologics, including stem cells, osteoinductive growth factors, osteoconductive matrices, and anabolic agents, are available clinically for accelerating fracture repair and treatment of compromised bone repair situations like delayed unions and nonunions. 10Abdominal applications of diffusion-weighted magnetic resonance imaging: Where do we stand?显示文摘Diffusion-weighted imaging (DWI) is one of the magnetic resonance imaging (MRI) sequences providing qualitative as well as quantitative information at a cellular level. It has been widely used for various applications in the central nervous system. Over the past decade, various extracranial applications of DWI have been increasingly explored, as it may detect changes even before signal alterations or morphological abnormalities become apparent on other pulse sequences. Initial results from abdominal MRI applications are promising, particularly in oncological settings and for the detection of abscesses. 11Reversibility of minimal hepatic encephalopathy following liver transplantation in Egyptian cirrhotic patients显示文摘AIM To evaluate the reversibility of minimal hepatic encephalopathy(MHE) following liver transplantation(LT) in Egyptian cirrhotic patients. METHODS This prospective study included twenty patients with biopsy-proven liver cirrhosis listed for LT and twenty ageand sex-matched healthy control subjects. All underwent neuro-psychiatric examination, laboratory investigations, radiological studies and psychometric tests including trail making test A(TMT A), TMT B, digit symbol test and serial dotting test. The psychometric hepatic encephalopathy score(PHES) was calculated for patients to diagnose MHE. Psychometric tests were repeated six months following LT in the cirrhotic patient group. RESULTS Before LT, psychometric tests showed highly significant deficits in cirrhotic patients in comparison to controls(P < 0.001). There was a statistically significant improvement in test values in the patient group after LT; however, their values were still significantly worse than those of the controls(P < 0.001). The PHES detected MHE in 16 patients(80%) before LT with a median value of -7 ± 3.5. The median PHES value was significantly improved following LT, reaching-4.5 ± 5(P < 0.001), and the number of patients with MHE decreased to 11(55%). The pre-transplant model for end-stage liver disease(MELD) score ≥ 15 was significantly related to the presence of post-transplant MHE(P = 0.005). 14Distribution and ecological risk assessment of some heavy metals in coastal surface sediments along the Red Sea,Egypt显示文摘The ecological risk assessment for Al,Zn,Cu,Ni,V,Pb,Cd,and Hg in surface sediment collected from the Egyptian Red Sea coast was evaluated using the Geo-accumulation Index(I_(geo)).Sediment Enrichment Factor(SEF) and Potential Ecological Risk Index(PERI) methods.The predominant heavy metal,aluminum,showed high concentrations along both of Aqaba Gulf(4378.8 ± 2554.1 μg/g) and southern part of the Red Sea(2972.8 + 1527.5 μg/g).while it recorded the lowest concentration in Suez Gulf(829.7 ± 398.2 μg/g).The determined heavy metal concentrations had the order of Al > Zn >-Ni > V > Pb > Cu > Cd > Hg.The statistical analyses showed some correlations among the heavy metals contents.Several international sediment quality guidelines were used to estimate the quality of the collected sediments.Interestingly,the recorded average heavy metals concentrations were lower than those of the permissible contents for sediment quality guidelines.The Geo-accumulation index calculations(I_(geo)) proved that the investigated region could be classified as an unpolluted area.Sediment Enrichment Factor(K_(SEF)) study showed high values in Suez Gulf region.The single pollution index analysis of heavy metals in the sediments(C^i_f) indicated that Al,Zn,V,and Pb were of natural origin,while Ni,Cd and Hg were seriously affected by human activities.Interestingly,amongst,all the determined heavy metals,Cd and Hg gave moderate ecological risk indicators.Ahmed El Nemr Ghada F.El-Said Azza Khaled Safaa Ragab 2016International Journal of Sediment Research2016,31,2:10

16Constipation-predominant irritable bowel syndrome: A review of current and emerging drug therapies显示文摘Irritable bowel syndrome(IBS) is a highly prevalent medical condition that adversely affects patient quality of life and constitutes a significant economic burden on healthcare resources. A large proportion of patients suffer from the constipation subtype of IBS(IBS-C), most commonly afflicting older individuals and those with a lower socioeconomic status. Conventional pharmacologic and nonpharmacologic treatment options have limited efficacies and/or significant adverse events, which lead to increased long-term health care expenditures. Failure to effectively treat IBS-C patients over the past decades has largely been due to a poor understanding of disease pathophysiology, lack of a global view of the patient, and an inappropriate selection of patients and treatment endpoints in clinical trials. In recent years, however, more effective and safer drugs have been developed for the treatment of IBS-C. The advancement in the area of pharmacologic treatment is based on new knowledge of the pathophysiologic basis of IBS-C and the development of drugs with increased selectivity within pharmacologic classes with recognized efficacies. This narrative review covers the spectrum of available drugs and their mechanisms of action, as well as the efficacy and safety profiles of each as determined in relevant clinical trials that have investigated treatment options for IBS-C and chronic constipation. 17Simultaneous effects of magnetic field and space porosity on compressible Maxwell fluid transport induced by a surface acoustic wave in a microchannel显示文摘Peristaltic motion induced by a surface acoustic wave of a viscous, compressible and electrically conducting Maxwell fluid in a confined parallel-plane microchannel through a porous medium is investigated in the presence of a constant magnetic field. The slip velocity is considered and the problem is discussed only for the free pumping case. A perturbation technique is employed to analyze the problem in terms of a small amplitude ratio. The phenomenon of a 'backward flow' is found to exist in the center and at the boundaries of the channel. In the second order approximation, the net axial velocity is calculated for various values of the fluid parameters. Finally, the effects of the parameters of interest on the mean axial velocity, the reversal flow, and the perturbation function are discussed and shown graphically. We find that in the nonNewtonian regime, there is a possibility of a fluid flow in the direction opposite to the propagation of the traveling wave. 18Mechanosignaling activation of TGFβmaintains intervertebral disc homeostasis显示文摘Intervertebral disc(IVD) degeneration is the leading cause of disability with no disease-modifying treatment.IVD degeneration is associated with instable mechanical loading in the spine, but little is known about how mechanical stress regulates nucleus notochordal(NC) cells to maintain IVD homeostasis. Here we report that mechanical stress can result in excessive integrin α_vβ_6-mediated activation of transforming growth factor beta(TGFβ), decreased NC cell vacuoles, and increased matrix proteoglycan production, and results in degenerative disc disease(DDD). Knockout of TGFβ type II receptor(TβRII) or integrin α_v in the NC cells inhibited functional activity of postnatal NC cells and also resulted in DDD under mechanical loading.Administration of RGD peptide, TGFβ, and α_vβ_6-neutralizing antibodies attenuated IVD degeneration. Thus,integrin-mediated activation of TGFβ plays a critical role in mechanical signaling transduction to regulate IVD cell function and homeostasis. 19Diffusion-weighted magnetic resonance imaging and micro-RNA in the diagnosis of hepatic fibrosis in chronic hepatitis C virus显示文摘BACKGROUND Diffusion-weighted magnetic resonance imaging has shown promise in the detection and quantification of hepatic fibrosis. In addition, the liver has numerous endogenous micro-RNAs(miRs) that play important roles in the regulation of biological processes such as cell proliferation and hepatic fibrosis.AIM To assess diffusion-weighted magnetic resonance imaging and miRs in diagnosing and staging hepatic fibrosis in patients with chronic hepatitis C.METHODS This prospective study included 208 patients and 82 age-and sex-matched controls who underwent diffusion-weighted magnetic resonance imaging of the abdomen, miR profiling, and liver biopsy. Pathological scoring was classified according to the METAVIR scoring system. The apparent diffusion coefficient (ADC) and miR were calculated and correlated with pathological scoring.RESULTS The ADC value decreased significantly with the progression of fibrosis, from controls(F0) to patients with early fibrosis(F1 and F2) to those with late fibrosis(F3 and F4)(median 1.92, 1.53, and 1.25 × 10^(-3) mm^2/s, respectively)(P = 0.001).The cut-off ADC value used to differentiate patients from controls was 1.83 × 10^(-3) mm^2/s with an area under the curve(AUC) of 0.992. Combining ADC and miR-200 b revealed the highest AUC(0.995) for differentiating patients from controls with an accuracy of 96.9%. The cut-off ADC used to differentiate early fibrosis from late fibrosis was 1.54 × 10^(-3) mm^2/s with an AUC of 0.866. The combination of ADC and miR-200 b revealed the best AUC(0.925) for differentiating early fibrosis from late fibrosis with an accuracy of 80.2%.
