1程序化清单查房系统在ICU的应用效果:一项前瞻性观察性研究显示文摘目的评估程序化清单查房系统(CERTAIN)在重症加强治疗病房(ICU)中的应用效果。方法采用前瞻性观察性研究方法,选择2018年1月至7月安徽省六安市第二人民医院ICU收治的100例患者(年龄>18岁、ICU住院时间>72h)进行研究。采用便利抽样法,以2018年1月至4月入院的50例患者作为对照组,由3名未告知研究计划的高、中级医师使用普通查房系统查房;2018年3月底,该3名医师通过美国梅奥诊所远程教学培训CERTAIN的使用方法并获得认证,对2018年3月至7月入组的50例患者(作为观察组)使用CERTAIN清单进行查房。CERTAIN清单包含了20个条目,涉及每日危重患者查房所需要涵盖的范围,临床医生需要对每个条目进行量化。收集资料,对比分析两组患者ICU住院时间、中心静脉导管(CVC)留置时间、尿管留置时间、机械通气时间、药物使用率、ICU病死率、不良事件发生率等。采用log-rank单因素比较和Cox回归多因素分析影响患者ICU死亡的独立因素。结果与对照组比较,观察组患者ICU住院时间显著缩短(d:8.68±4.84比13.64±9.37),尿管留置时间(d:8.16±5.29比13.32±9.31)和机械通气时间(d:3.46±4.14比6.62±9.57)也明显缩短,胰岛素使用率(34.0%比56.0%)、ICU病死率(2.0%比14.0%)明显下降(均P<0.05)。此外,CERTAIN清单的使用可使医师的查房效率显著提高,查房时间由普通查房系统的(8.00±0.45)min缩短到(5.00±0.33)min(t=9.312,P<0.01)。Log-rank单因素比较显示,CERTAIN清单的应用可以降低ICU病死率(χ2=3.898,P=0.048),但是多元Cox回归分析显示,CERTAIN清单的应用并不是降低ICU病死率的一个独立因素[优势比(OR)=1.001,P=0.922]。结论CERTAIN清单应用在ICU患者中效果显著,可在我国重症医学科室进一步推广。闻雪松 邵敏 Kianoush B Banaei Kashani 2018中华危重病急救医学2018,30,12:6
2Effects of TIG Surface Melting and Chromium Surface Alloying on Microstructure, Hardness and Wear Resistance of ADI显示文摘Microhardness and wear resistance of different microstructures formed by TIG(tungsten inert gas) surface melting and chromium surface alloying(using ferrochromium)of ADI(austempered ductile iron)were studied. Sur- face melting resulted in the formation of a ledeburitic structure in the melted zone, and this structure has a hardness up to 896 HV as compared to 360 HV in that of ADI. Moreover, chromium surface alloying resulted in the forma- tion of different structures including:(1)a hypereutectic structure consisting of primary(Fe,Cr)_7 C_3 carbides and a eutectic matrix of transformed austenite(into martensite and retained austenite),as also (Fe,Cr)_7C_3 carbides, with a hardness of 1 078 HV;(2) a hypoeutectic structure consisting of the same eutectic along with transformed primary austenite, with a hardness of 755 HV; and (3) a ledeburitic structure with an acicular morphology and a hardness of 896 HV. The results also indicated that surface melting reduced the wear rate of the ADI by approximately 37%. 3Imperialistic Competitive Algorithm:A metaheuristic algorithm for locating the critical slip surface in 2-Dimensional soil slopes显示文摘In this study, Imperialistic Competitive Algorithm(ICA) is utilized for locating the critical failure surface and computing the factor of safety(FOS) in a slope stability analysis based on the limit equilibrium approach. The factor of safety relating to each trial slip surface is calculated using a simplified algorithm of the Morgenstern-Price method, which satisfies both the force and the moment equilibriums. General slip surface is considered non-circular in this study that is constituted by linking random straight lines.To explore the performance of the proposed algorithm, four benchmark test problems are analyzed. The results demonstrate that the present techniques can provide reliable, accurate and efficient solutions for locating the critical failure surface and relating FOS. 4薰衣草水提取物能改善阿尔茨海默大鼠的空间学习和记忆能力(英文)显示文摘目的阿尔茨海默病(Alzheimer's disease, AD)是主要的神经退行性疾病之一,其特征主要表现为痴呆,包括学习与记忆的衰退。本研究旨在探索薰衣草(唇形科,薰衣草属)水提取物对AD大鼠空间学习和记忆的影响。方法 Wistar大鼠分为对照组和AD 组,通过给大鼠脑室注射10 ?g A?1-42 建立AD模型。20天后,两组大鼠进行为期5天的水迷宫空间记忆采集实验(每天4次),紧接着进行一次空间探索实验。实验结束后,分别给予AD组和对照组大鼠不同剂量的薰衣草水提取物(50、100、200 mg/kg) 或 0.4 mL/kg 蒸馏水,每天一次共20天。随后,重复上述水迷宫实验。结果在水迷宫实验的第一阶段,AD组大鼠找到平台的潜伏期显著高于对照组大鼠,而在第二阶段,接受蒸馏水注射的对照和AD大鼠找到平台的潜伏期没有差异,说明水迷宫运动本身能提高AD大鼠的空间学习能力。在第二阶段空间记忆采集实验中,与接受蒸馏水注射的同类大鼠相比,接受薰衣草水提取物注射的对照和AD大鼠找到平台的潜伏期显著降低。此外,200 mg/kg 薰衣草水提取物能显著提高对照和AD大鼠的空间探索能力。结论薰衣草水提取物能有效逆转AD 大鼠空间学习功能的损伤。Masoud Soheili Kashani Mostafa Rezaei Tavirani Sayyed Alireza Talaei Mahmoud Salami 2011Neuroscience Bulletin2011,27,2:2
5Contrast-induced acute kidney injury in kidney transplant recipients: A systematic review and meta-analysis显示文摘AIM To evaluate the incidence of contrast-induced acute kidney injury(CIAKI) in kidney transplant recipients. METHODS A literature search was performed using MEDLINE, EMBASE, and the Cochrane Database of Systematic Reviews from the inception of the databases through July 2016. Studies assessing the incidence of CIAKI in kidney transplant recipients were included. We applied a randomeffects model to estimate the incidence of CIAKI.RESULTS Six studies of 431 kidney transplant recipients were included in the analyses to assess the incidence of CIAKI in kidney transplant recipients. The estimated incidence of CIAKI and CIAKI-requiring dialysis were 9.6%(95%CI: 4.5%-16.3%) and 0.4%(95%CI: 0.0%-1.2%), respectively. A sensitivity analysis limited only to the studies that used low-osmolar or iso-osmolar contrast showed the estimated incidence of CIAKI was 8.0%(95%CI: 3.5%-14.2%). The estimated incidences of CIAKI in recipients who received contrast media with cardiac catheterization, other types of angiogram, and CT scan were 16.1%(95%CI: 6.6%-28.4%), 10.1%(95%CI: 4.2%-18.0%), and 6.1%(95%CI: 1.8%-12.4%), respectively. No graft losses were reported within 30 d post-contrast media administration. However, data on the effects of CIAKI on long-term graft function were limited.CONCLUSION The estimated incidence of CIAKI in kidney transplant recipients is 9.6%. 8The Behavior of Rectangular and Circular Reinforced Concrete Columns Under Biaxial Multiple Excitation显示文摘The aim of this study is to investigate the dynamic performance of rectangular and circular reinforced concrete(RC)columns considering biaxial multiple excitations.For this purpose,an advanced nonlinear finite element model which can simulate various features of cyclic degradation in material and structural components is used.The implemented nonlinear fiber beam-column model accounts for inelastic buckling and low-cycle fatigue degradation of longitudinal reinforcement and can simulate multiple failure modes of RC columns under dynamic loading.Hypothetical rectangular and circular columns are used to investigate the failure modes of RC columns.A detailed ground motion selection is implemented to generate real mainshock and aftershocks.It was found that multiple excitations due to aftershock has the potential of increasing the damage of the RC columns and longitudinal reinforcements are significantly affected low-cycle fatigue.Also,it was found that rectangular column is more sensitive to accumulative damage due to cyclic fatigue.This study increases the accuracy of structural analysis of RC columns and consequently improves understanding the failure modes of RC columns with different cross-sectional shapes.Mohammad Reza Salami Ebrahim Afsar Dizaj Mohammad Mehdi Kashani 2019Computer Modeling in Engineering & Sciences2019,,9:2

10An investigation into the effects of particle texture,water content and parallel plates' diameters on rheological behavior of fine sediment显示文摘Siltation,a phenomenon resulted from the presence of fine particles in an aqueous environment,dominated by silt and clay,is a known and common environmental issue worldwide.The accumulation of fine sediments engenders murky water with low oxygen levels,which leads to the death of aquatic life.Thus,investigating the physical and mechanical properties of fine sediment by rheological methods has expanded.Rheology is the science of deformation and flow of matter in stress.This survey investigates the rheological behavior of six samples of soil as the fine particles structure(D < 63 μm) from different regions of Malaysia by using a rotational rheometer with a parallel-plate measuring(using two sizes:25 mm and 50 mm) device to explore the flow and viscoelastic properties of fine particles.The samples were examined in two rheological curve and amplitude sweep test methods to investigate the effect of water content ratio,texture,and structure of patticles on rheological properties.It was found that the content of fine sand,clay,and silt had an effect on the stiffness,structural stability,and shear behavior.Thus,the pseudoplastic and viscoelastic behaviot are respectively shown.Moreover,the amount of fine sediments present in water i.e.the concentration of these particles,has a direct effect on the rheological curve.A reduction in viscosity of samples with higher concentrations of water has been observed.As a consequence,a considerable quantity of fine sediments are distributed within the water body and remain suspended over the time.As a result,the sedimentation rate slows down.It needs to be asserted that the stotage modulus G′,loss modulus G″,and yield point can vary depending on particle type.The G′ and G″were instigated for samples(70%and 45%concentrations) that demonstrated viscoelastic characteristics using the same rotational rheometer with a parallel-plate measuring device.Masoumeh Moayeri Kashani Lai Sai Hin Shaliza Binti Ibrahim Nik Meriam Binti Nik Sulaiman Fang Yenn Teo 2016International Journal of Sediment Research2016,31,2:2
