| 1 | Atrial fibrillation in obstructive sleep apnea显示文摘Atrial fibrillation(AF) is a common arrhythmia with rising incidence.Obstructive sleep apnea(OSA) is prevalent among patients with AF.This observation has prompted significant research in understanding the relationship between OSA and AF.Multiple studies support a role of OSA in the initiation and progression of AF.This association has been independent of obesity,body mass index and hypertension.Instability of autonomic tone and wide swings in intrathoracic pressure are seen in OSA.These have been mechanistically linked to initiation of AF in OSA patients by lowering atrial effective refractory period,promoting pulmonary vein discharges and atrial dilation.OSA not only promotes initiation of AF but also makes management of AF difficult.Drug therapy and electrical cardioversion for AF are less successful in presence of OSA.There has been higher rate of early and overall recurrence after catheter ablation of AF in patients with OSA.Treatment of OSA with continuous positive airway pressure has been shown to improve control of AF.However,additional studies are needed to establish a stronger relationship between OSA treatment and success ofAF therapies.There should be heightened suspicion of OSA in patients with AF.There is a need for guidelines to screen for OSA as a part of AF management. | Sandeep K Goyal Abhishek Sharma | 2013 | World Journal of Cardiology2013，5，6： | 16 |
| 2 | Hepatocellular carcinoma in non-alcoholic steatohepatitis:Current knowledge and implications for management显示文摘With the prevalence of hepatitis C virus expected to decline, the proportion of hepatocellular carcinoma(HCC) related to non-alcoholic steatohepatitis(NASH) is anticipated to increase exponentially due to the growing epidemic of obesity and diabetes. The annual incidence rate of developing HCC in patients with NASH-related cirrhosis is not clearly understood with rates ranging from 2.6%-12.8%. While multiple new mechanisms have been implicated in the development of HCC in NASH; further prospective long-term studies are needed to validate these findings. Recent evidence has shown a significant proportion of patients with non-alcoholic fatty liver disease and NASH progress to HCC in the absence of cirrhosis. Liver resection and transplantation represent curative therapeutic options in select NASHrelated HCC patients but have placed a significant burden to our healthcare resources and utilization. Currently NASH-related HCC is the fastest growing indication for liver transplant in HCC candidates. | 3 | Hepatocellular carcinoma after Iocoregional therapy:Magnetic resonance imaging findings in falsely negative exams显示文摘AIM:To elucidate causes for false negative magnetic resonance imaging(MRI)exams by identifying imaging characteristics that predict viable hepatocellular carcinoma(HCC)in lesions previously treated with locoregional therapy when obvious findings of recurrence are absent.METHODS:This retrospective institutional review board-approved and Health Insurance Portability and Accountability Act-compliant study included patients who underwent liver transplantation at our center between 1/1/2000 and 12/31/2012 after being treated for HCC with locoregional therapy.All selected patients had a contrast-enhanced MRI after locoregional therapy within 90 d of transplant that was prospectively interpreted as without evidence of residual or recurrenttumor.Retrospectively,2 radiologists,blinded to clinica and pathological data,independently reviewed the pre transplant MRIs for 7 imaging features.Liver explan histopathology provided the reference standard,with clinically significant tumor defined as viable tumor≥1.0cm in maximum dimension.Fisher’s exact test was firs performed to identify significant imaging features.RESULTS:Inclusion criteria selected for 42 patients with 65 treated lesions.Fourteen of 42 patients(33%and 16 of 65 treated lesions(25%)had clinically significant viable tumor on explant histology.None o the 7 imaging findings examined could reliably and reproducibly determine which treated lesion had viable tumor when the exam had been prospectively read as without evidence of viable HCC.CONCLUSION:After locoregional therapy some treated lesions that do not demonstrate any MRI evidence o HCC will contain viable tumor.As such even patients with a negative MRI following treatment should receive regular short-term imaging surveillance because some have occult viable tumor.The possibility of occult tumo should be a consideration when contemplating any action which might delay liver transplant. | David Becker-Weidman Jesse M Civan Sandeep P Deshmukh Christopher G Roth Steven K Herrine Laurence Parker Donald G Mitchell | 2016 | World Journal of Hepatology2016，8，16： | 2 |
| 4 | Performance studies on an oscillating water column employing a Savonius rotor显示文摘A new oscillating water column (OWC) design is proposed in this study to incorporate a simpler Savonius type turbine. Conventional OWC devices employ a bi-directional turbine such as a Wells or an Impulse turbine to extract energy from the air. The disadvantages of the Wells turbine include its inability to self start and stalling. The Savonius turbine is much cheaper and is an effective option at low Reynolds numbers. In the current rectangular OWC device, unlike the circular OWC, the width of entry of the capture chamber can be increased without being influenced by the diameter at the turbine section. To improve its primary capture efficiency, the front and rear walls of the OWC are inclined to minimize reflection. The Savonius rotor characteristics are studied with respect to the change in frequency of the incoming waves. The rotor rpm is sensitive to wave period and primary conversion efficiency while changes in depth only affect the rotor rpm at lower frequencies. | 5 | Unique case of oligoastrocytoma with recurrence and grade progression:Exhibiting differential expression of high mobility group-A1 and human telomerase reverse transcriptase显示文摘Mixed gliomas, primarily oligoastrocytomas, account for about 5%-10% of all gliomas. Distinguishing oligoastrocytoma based on histological features alone has limitations in predicting the exact biological behavior, necessitating ancillary markers for greater specificity. In this case report, human telomerase reverse transcriptase(hT ERT) and high mobility group-A1(HMGA1); markers of proliferation and stemness, have been quantitatively analyzed in formalin-fixed paraffin-embedded tissue samples of a 34 years old patient with oligoastrocytoma. Customized florescence-based immunohistochemistry protocol with enhanced sensitivity and specificity is used in the study. The patient presented with a history of generalized seizures and his magnetic resonance imaging scans revealed infiltrative ill-defined mass lesion with calcified foci within the left frontal white matter, suggestive of glioma. He was surgically treated at our center for four consecutive clinical events. Histopathologically, the tumor was identified as oligoastrocytoma-grade Ⅱ followed by two recurrence events and final progression to grade Ⅲ. Overall survival of the patient without adjuvant therapy was more than 9 years. Glial fibrillary acidic protein, p53, Ki-67, nuclear atypia index, pre-operative neutrophillymphocyte ratio, are the other parameters assessed. Findings suggest that hT ERT and HMGA1 are linked to tumor recurrence and progression. | 6 | Novel molecular panel for evaluating systemic inflammation and survival in therapy naïve glioma patients显示文摘BACKGROUND Inflammation is crucial to tumor progression.A traumatic event at a specific site in the brain activates the signaling molecules,which triggers inflammation as the initial response within the tumor and its surroundings.The educated immune cells and secreted proteins then initiate the inflammatory cascade leading to persistent chronic inflammation.Therefore,estimation of the circulating inflammatory indicators kynurenine(KYN),interleukin-6(IL-6),tissue-inhibitor of matrix-metalloproteinase-1 and human telomerase reverse transcriptase(hTERT)along with neutrophil-lymphocyte ratio(NLR)has prognostic value.AIM To assess the utility of chosen inflammatory marker panel in estimating systemic inflammation.METHODS The chosen markers were quantitatively evaluated in 90 naive,molecularly subtyped plasma samples of glioma.A correlation between the markers and confounders was assessed to establish their prognostication power.Follow-up on the levels of the indicators was done 3-mo post-surgery.To establish the validity of circulating KYN,it was also screened qualitatively by dot-immune-assay and by immunofluorescence-immunohistochemistry in tumor tissues.RESULTS Median values of circulating KYN,IL-6,hTERT,tissue-inhibitor of matrixmetalloproteinase-1 and NLR in isocitrate-dehydrogenase-mutant/wildtype and within the astrocytic sub-groups were estimated,which differed from controls,reaching statistical significance(P<0.0001).All markers negatively correlated with mortality(P<0.0001).Applying combination-statistics,the panel of KYN,IL-6,hTERT and NLR achieved higher sensitivity and specificity(>90%)than standalone markers,to define survival.The inflammatory panel could discriminate between WHO grades,and isocitrate-dehydrogenase-mutant/wildtype and define differential survival between astrocytic isocitrate-dehydrogenase-mutant/wildtype.Therefore,its assessment for precise disease prognosis is indicated.Association of KYN with NLR,IL-6 and hTERT was significant.Cox-regression described KYN,IL-6,NLR,and hTERT as good prognostic markers,independent of confounders.Multivariate linear-regression analysis confirmed the association of KYN and hTERT with inflammation marker IL-6.There was a concomitant significant decrease in their levels in a 3-mo follow-up.CONCLUSION The first evidence-based study of circulating-KYN in molecularly defined gliomas,wherein the tissue expression was found to be concomitant with plasma levels.A non-invasive model for assessing indicators of chronic systemic inflammation is proposed. | Puneet Gandhi Richa Shrivastava Nitin Garg Sandeep K Sorte | 2021 | World Journal of Clinical Oncology2021，12，10： | 2 |
