| 1 | Delphi methodology in healthcare research:How to decide its appropriateness显示文摘The Delphi technique is a systematic process of forecasting using the collective opinion of panel members.The structured method of developing consensus among panel members using Delphi methodology has gained acceptance in diverse fields of medicine.The Delphi methods assumed a pivotal role in the last few decades to develop best practice guidance using collective intelligence where research is limited,ethically/logistically difficult or evidence is conflicting.However,the attempts to assess the quality standard of Delphi studies have reported significant variance,and details of the process followed are usually unclear.We recommend systematic quality tools for evaluation of Delphi methodology;identification of problem area of research,selection of panel,anonymity of panelists,controlled feedback,iterative Delphi rounds,consensus criteria,analysis of consensus,closing criteria,and stability of the results.Based on these nine qualitative evaluation points,we assessed the quality of Delphi studies in the medical field related to coronavirus disease 2019.There was inconsistency in reporting vital elements of Delphi methods such as identification of panel members,defining consensus,closing criteria for rounds,and presenting the results.We propose our evaluation points for researchers,medical journal editorial boards,and reviewers to evaluate the quality of the Delphi methods in healthcare research. | Prashant Nasa Ravi Jain Deven Juneja | 2021 | World Journal of Methodology2021，11，4： | 21 |
| 2 | Severe sepsis and septic shock in the elderly:An overview显示文摘The incidence of severe sepsis and septic shock is increasing in the older population leading to increased admissions to the intensive care units(ICUs). The elderly are predisposed to sepsis due to co-existing comorbidities, repeated and prolonged hospitalizations, reduced immunity, functional limitations and above all due to the effects of aging itself. A lower threshold and a higher index of suspicion is required to diagnose sepsis in this patient population because the initial clinical picture may be ambiguous, and aging increases the risk of a sudden deterioration in sepsis to severe sepsis and septic shock. Management is largely based on standard international guidelines with a few modifications. Age itself is an independent risk factor for death in patients with severe sepsis, however, many patients respond well to timely and appropriate interventions. The treatment should not be limited or deferred in elderly patients with severe sepsis only on the grounds of physician prejudice, but patient and family preferences should also be taken into account as the outcomes are not dismal. | 4 | Exploring point-of-care transformation in diabetic care:A quality improvement approach显示文摘Objective:This quality improvement project evaluated the impact of a point-of-care(POC)HbA1c stat lab intervention and a nurse-assisted expanded visit implemented among patients with uncontrolled type 2 diabetes(T2D)at a community health center in Houston,TX.Methods:This was a before-and-after POC intervention among adult patients who received primary care services between 1 July 2014 and 31 December 2014(baseline visit)and who had at least one 3-month follow-up visit.Results:Three hundred eighty-seven patients were included in the study.The majority were<60 years of age(72.1%),female(60.5%),and Hispanic(63%),followed by black(16.5%)and Asian(11.1%).Almost 87%of the patients had uncontrolled T2D(HbA1c>9%)at baseline,with the highest average levels among Hispanic(10.9%)and black(10.7%)patients.There was a significant difference in the HbA1c level before(mean=10.65,SD=1.9291)and after(mean=9.25,SD=1.8187)intervention.The absolute reduction in the level of HbA1c was 1.4%(t=12.834,p<0.001),corresponding to a 13%overall percentage decrease from baseline.Conclusion:There is a distinct advantage in using a stat HbA1c lab when combined with shared POC visits to assist patients with uncontrolled T2D in lowering the HbA1c,improving self-management,and reducing long-term costs. | Malvika Juneja Maria C.Mejia de Grubb Haijun Wang Kiara Spooner Roger J.Zoorob | 2015 | Family Medicine and Community Health2015，3，2： | 2 |
