| 1 | 中国儿童15项常用临床检验指标的生物参考区间研究显示文摘目的建立不同年龄、性别儿童的临床常用检验指标的生物参考区间。方法横断面多中心研究。由全国15个省、市(北京、广东、湖北、江苏、福建、陕西、山东、浙江、湖南、河南、天津、上海、重庆、四川、吉林)20家医学中心合作完成,自2013年6月至2014年12月招募6月龄-17岁的表观正常儿童作为研究对象,通过问卷调查、体格检查和腹部超声等筛选、建立"参考人群";留取研究对象空腹血液标本,送中心实验室进行检测,检测指标包括性激素(雌二醇、睾酮、黄体生成素、卵泡刺激素,化学发光微粒子免疫检测法)、肝功能(丙氨酸转氨酶,加磷酸吡哆醛、不加磷酸吡哆醛试剂速率法两种方法同时检测)、肾功能(肌酐,碱性苦味酸法、酶法两种方法检测;胱抑素C,免疫透射比浊法)、免疫功能(免疫球蛋白A、免疫球蛋白G、免疫球蛋白M、补体C3、补体C4,免疫透射比浊法)、碱性磷酸酶(磷酸对硝基苯法)、尿酸(尿酸酶法)和肌酸激酶(酶法)。将"参考人群"上述15项指标的检测数据按照性别、年龄进行分层统计分析,建立中国儿童参考区间。
结果共招募2 259名儿童完成现场调查,筛选"表观正常儿童"1 648名作为"参考人群";其中男830名、女818名,年龄(7.4±4.9)岁。建立了15项检验指标的生物参考区间,各指标参考区间随性别、年龄变化的趋势如下:(1)性激素:随年龄增长逐渐上升,卵泡刺激素在各年龄组均存在性别差异;(2)肝功能:2岁以下儿童的血丙氨酸转氨酶相对较高,5-9岁相对稳定,之后随年龄增长缓慢上升;(3)肾功能:儿童血肌酐随年龄增长持续上升,血胱抑素C在2岁以下相对高;(4)免疫功能:儿童血免疫球蛋白水平随年龄增长逐渐上升,而补体的参考范围相对恒定(年龄、性别差异不明显);(5)其他:碱性磷酸酶和肌酸激酶在生后早期(2岁以下)处于较高水平,肌酸激酶和尿酸的检测值在5-9岁儿童相对稳定。上述指标中,性激素、肌酐、免疫球蛋白、肌酸激酶、碱性磷酸酶和尿酸的参考区间在进入青春期后迅速升高、伴有性别差异。检测丙氨酸转氨酶时,加磷酸吡哆醛试剂速率法的参考区间高于不加磷酸吡哆醛法;苦味酸法检测出的肌酐参考区间高于酶法(特别是9岁及以下儿童)。结论建立了中国儿童基于年龄、性别分层的生物参考区间,涵盖肝肾功能、免疫功能和性激素等15项临床常用检验指标。 | 钟旭辉 丁洁 周建华 余自华 孙书珍 包瑛 毛建华 于力 李志辉 韩子明 宋红梅 蒋小云 刘玉玲 张碧丽 夏正坤 金春华 朱光华 王墨 冯仕品 沈颖 黄松明 马青山 李海霞 王学晶 Ichihara Kiyoshi 姚晨 董冲亚 | 2018 | 中华儿科杂志2018,56,11: | 19 |

| 4 | Mechanical properties of nerve roots and rami radiculares isolated from fresh pig spinal cords显示文摘No reports have described experiments designed to determine the strength characteristics of spinal nerve roots and rami radiculares for the purpose of explaining the complexity of symptoms of medullary cone lesions and cauda equina syndrome. In this study, to explain the pathogenesis of cauda equina syndrome, monoaxial tensile tests were performed to determine the strength characteristics of spinal nerve roots and rami radiculares, and analysis was conducted to evaluate the stress-strain relationship and strength characteristics. Using the same tensile test device, the nerve root and ramus radiculares isolated from the spinal cords of pigs were subjected to the tensile test and stress relaxation test at load strain rates of 0.1, 1, 10, and 100 s–1 under identical settings. The tensile strength of the nerve root was not rate dependent, while the ramus radiculares tensile strength tended to decrease as the strain rate increased. | 5 | Roles of the (pro) renin receptor in the kidney显示文摘Prorenin receptor(PRR) is a multi-functioning protein possessing at least four different roles:(1) working as a receptor for renin and prorenin producing angiotensin I from angiotensinogen thus enhancing the tissue reninangiotensin system;(2) inducing intracellular signals when a ligand binds to PRR;(3) participating in the functions of vacuolar proton ATPase; and(4) constituting the Wnt signaling receptor complex.Here,the roles of PRR in kidney physiology and diabetic conditions as well as recent findings regarding a soluble form of PRR are discussed.We also propose the possible mechanism concerning diabetic nephropathy as 'trade-off hypothesis' from a PRR point of view.In brief,under hyperglycemic conditions,injured podocytes degrade degenerated proteins and intracellular organelles which require V-ATPase and PRR for vesicle internal acidification.Sustained hyperglycemia overproduces PRR molecules,which are transported to the transmembrane and bind to increased serum prorenin in the diabetic condition.This enhances tissue renin-angiotensin system and PRR-mediated mitogen-activated protein kinase signals,resulting in increased injurious molecules such as transforming growth factor-β,cyclooxygenase2,interleukin-1β,and tumor necrosis factor-α ending in diabetic nephropathy progression.Although many findings led us to better PRR understanding,future works should elucidate which PRR functions,of the four discussed here,are dominant in each cell and kidney disease context. | Yoichi Oshima Satoshi Morimoto Atsuhiro Ichihara | 2014 | World Journal of Nephrology2014,3,4: | 3 |
| 6 | Stent placement is effective on both postoperative hepatic arterial pseudoaneurysm and subsequent portal vein stricture:A case report显示文摘To treat postoperative bleeding after hepato-pancreato-biliary surgery, interventional radiology has become essential. We report a case of coincidental pseudoan-eurysm and jejunal varices that were both successfully treated by stent-grafts. After a pancreaticoduodenec-tomy, the patient developed a pseudoaneurysm in the hepatic artery and a stenosis in its periphery. After establishing hepatic arterial flow by placing stent-grafts over both the pseudoaneurysm and the stenosis, the pseudoaneurysm was embolized with microcoils. Nine months later, the patient developed jejunal varices caused by a severe stricture in the main trunk of the portal vein. Percutaneous transhepatic portography was performed and stent-grafts were placed over the stenotic segment. A venoplasty using stent-grafts nor-malized the portal blood flow and the jejunal varices vanished. | 9 | Possible contribution of(pro)renin receptor to development of gestational diabetes mellitus显示文摘(Pro)renin receptor [(P)RR], a receptor for renin and prorenin, was first cloned in 2002. Since then, the pathophysiological roles of(P)RR have been growing concerns.(P)RR binds renin and prorenin, with two important consequences, nonproteolytic activation of prorenin, leading to the tissue renin-angiotensin system activation and the intracellular signalings. It is now also known to play an important role as vacuolar H+-ATPase associated protein, involving in Wnt signaling, main component of embryonic development. Extracellular domain of full-length(P)RR is cleaved in golgi-complex forming soluble(P)RR [s(P)RR]. The s(P)RR is now possible to be measured in human blood and urine. It is now measured in different pathophysiological states, and recent study showed that elevated plasma s(P)RR levels in the early stage of pregnancies are associated with higher incidence of gestational diabetes mellitus later in the pregnancies. Plasma s(P)RR levels of neonates are known to be higher than that of adults. It was also shown that, increased s(P)RR concentrations in cord blood, associated with a lower small for gestational age birth likelihood. These data suggests the involvement of(P)RR in embryo's growth.
