| 2 | New Trends in Olefin Production显示文摘Most olefins (e.g., ethylene and propylene) will continue to be produced through steam cracking (SC) ofhydrocarbons in the coming decade. In an uncertain commodity market, the chemical industry is investingvery little in alternative technologies and feedstocks because of their current lack of economic viability,despite decreasing crude oil reserves and the recognition of global warming. In this perspective, some of themost promising alternatives are compared with the conventional SC process, and the major bottlenecks ofeach of the competing processes are highlighted. These technologies emerge especially from the abundanceof cheap propane, ethane, and methane from shale gas and stranded gas. From an economic point of view,methane is an interesting starting material, if chemicals can be produced from it. The huge availability ofcrude oil and the expected substantial decline in the demand for fuels imply that the future for proventechnologies such as Fischer-Tropsch synthesis (FFS) or methanol to gasoline is not bright. | 3 | Classification and management of bile duct injuries显示文摘To review the classification and general guidelines for treatment of bile duct injury patients and their long term results. In a 20-year period, 510 complex circumferential injuries have been referred to our team for repair at the Instituto Nacional de Ciencias Médicasy Nutrición 'Salvador Zubirán' hospital in Mexico City and 198 elsewhere (private practice). The records at the third level Academic University Hospital were analyzed and divided into three periods of time: GⅠ-1990-99 (33 cases), G Ⅱ2000-2004 (139 cases) and GⅢ2004-2008 (140 cases). All patients were treated with a Roux en Y hepatojejunostomy. A decrease in using transanastomotic stents was observed (78% vs 2%, P = 0.0001). Partial segment Ⅳ and Ⅴ resection was more frequently carried out (45% vs 75%, P = 0.2) (to obtain a high bilioenteric anastomosis). Operative mortality (3% vs 0.7%, P = 0.09), postoperative cholangitis (54% vs 13%, P = 0.0001), anastomosis strictures (30% vs 5%, P = 0.0001), short and long term complications and need for reoperation (surgical or radiological) (45% vs 11%, P = 0.0001) were significantly less in the last period. The authors concluded that transition to a high volume center has improved long term results for bile duct injury repair. | 4 | The Immune System and the Role of Inflammation in Perinatal Depression显示文摘Major depression during pregnancy is a common psychiatric disorder that arises from a complex and multifactorial etiology. Psychosocial stress, sex, hormones, and genetic vulnerability increase the risk for triggering mood disorders. Microglia and toll-like receptor 4 play a crucial role in triggering wide and varied stress-induced responses mediated through activation of the inflammasome; this leads to the secretion of inflammatory cytokines, increased serotonin metabolism, and reduction of neurotransmitter availability along with hypothalamic–pituitary–adrenal axis hyperactivity. Dysregulation of this intricate neuroimmune communication network during pregnancy modifies the maternal milieu, enhancing the emergence of depressive symptoms and negative obstetric and neuropsychiatric outcomes. Although several studies have clearly demonstrated the role of the innate immune system in major depression, it is still unclear how the placenta, the brain, and the monoaminergic and neuroendocrine systems interact during perinatal depression. Thus, in the present review we describe the cellular and molecular interactions between these systems in major depression during pregnancy, proposing that the same stress-related mechanisms involved in the activation of the NLRP3 inflammasome in microglia and peripheral myeloid cells in depressed patients operate in a similar fashion in the neuroimmune placenta during perinatal depression. | 5 | Availability and Quality of Emergency Obstetric Care, an Alternative Strategy to Reduce Maternal Mortality: Experience of Tongji Hospital, Wuhan, China显示文摘The burden of maternal mortality (MM) and morbidity is especially high in Asia. However, China has made significant progress in reducing MM over the past two decades, and hence maternal death rate has declined considerably in last decade. To analyze availability and quality of emergency obstetric care (EmOC) received by women at Tongji Hospital, Wuhan, China, this study retrospectively analyzed various pregnancyrelated complications at the hospital from 2000 to 2009. Two baseline periods of equal length were used for the comparison of variables. A total of 11 223 obstetric complications leading to MM were identified on a total of 15 730 hospitalizations, either 71.35% of all activities. No maternal death was recorded. Mean age of women was 29.31 years with a wide range of 14-52 years. About 96.26% of women had higher levels of schooling, university degrees and above and received the education of secondary school or college. About 3.74% received primary education at period two (P2) from 2005 to 2009, which was significantly higher than that of period one (P1) from 2000 to 2004 (P<0.05) (OR: 0.586; 95% CI: 0.442 to 0.776). About 65.69% were employed as skilled or professional workers at P2, which was significantly higher than that of P1 (P<0.05). About 34.31% were unskilled workers at P2, which was significantly higher than that of P1 (P<0.05). Caesarean section was performed for 9,930 women (88.48%) and the percentage of the procedure increased significantly from 19.25% at P1 to 69.23% at P2 (P<0.05). We were led to conclude that, despite the progress, significant gaps in the performance of maternal health services between rural and urban areas remain. However, MM reduction can be achieved in China. | 8 | Assessment of vascular invasion in gastric cancer: A comparative study显示文摘AIM: To evaluate and compare detection of lymphatic and blood vessel invasion (LVI and BVI) by hematox-ylin-eosin (HE) and immunohistochemistry (IHC) in gastric cancer specimens, and to correlate with lymph node status. METHODS: IHC using D2-40 (a lymphatic endothelial marker) and CD34 (a pan-endothelial marker) was performed to study LVI and BVI in surgical specimens froma consecutive series of 95 primary gastric cancer cases. The results of the IHC study were compared with the detection by HE using McNemar test and kappa index. The morphologic features of the tumors and the presence of LVI and BVI were related to the presence of lymph node metastasis. A χ2 test was performed to obtain associations between LVI and BVI and other prognostic factors for gastric cancer. RESULTS: The detection rate of LVI was considerably higher than that of BVI. The IHC study identified eight false-positive cases and 13 false-negative cases for LVI, and 24 false-positive cases and 10 false-negative cases for BVI. The average Kappa value determined was moderate for LVI (k=0.50) and low for BVI (k=0.20). Both LVI and BVI were statistically associated with the presence of lymph node metastasis (HE: P=0.001, P=0.013, and IHC: P=0.001, P=0.019). The mor-phologic features associated with LVI were location of the tumor in the distal third of the stomach (P=0.039), Borrmann's macroscopic type (P=0.001), organ inva-sion (P=0.03) and the depth of tumor invasion (P=0.001). The presence of BVI was related only to the depth of tumor invasion (P=0.003). | 9 | Visualization of Disciplinary Profiles: Enhanced Science Overlay Maps显示文摘Purpose: The purpose of this study is to modernize previous work on science overlay maps by updating the underlying citation matrix, generating new clusters of scientific disciplines, enhancing visualizations, and providing more accessible means for analysts to generate their own maps.Design/methodology/approach: We use the combined set of 2015 Journal Citation Reports for the Science Citation Index (n of journals = 8,778) and the Social Sciences Citation Index (n=3,212) for a total of 11,365 journals. The set of Web of Science Categories in the Science Citation Index and the Social Sciences Citation Index increased from 224 in 2010 to 227 in 2015. Using dedicated software, a matrix of 227 × 227 cells is generated on the basis of whole-number citation counting. We normalize this matrix using the cosine function. We first develop the citing-side, cosine-normalized map using 2015 data and VOSviewer visualization with default parameter values. A routine for making overlays on the basis of the map('wc15.exe') is available at http://gffzzcc9cb02d7b4f4bb1hvu60uk0nq5xw6wvq.ffgz.tsg.suse.edu.cn/wc15/index.htm.Findings: Findings appear in the form of visuals throughout the manuscript. | 11 | Hazardous elements and amorphous nanoparticles in historical estuary coal mining area显示文摘In Brazil, intense coal exploitation activities have led to environmental deterioration, including soil mortification, water contamination, loss of ecosystem, and atmospheric contamination. In addition,considerable quantities of sulfur-rich residues are left behind in the mining area; these residues pose grave environmental issues as they undergo sulfide oxidation reactions. When sulfur oxides come in contact with water, extreme acid leachate is produced with great proportions of sulfate, and hazardous elements(HEs), which are identified as coal drainage(CMD). CMD is an environmental pollution challenge, particularly in countries with historic or active coal mines. To prevent CMD formation or its migration, the source must be controlled; however, this may not be feasible at many locations. In such scenarios, the mine water should be collected, treated, and discharged. In this study, data from 2005 to2010 was gathered on the geochemistry of 11 CMD discharges from ten different mines. There are several concerns and questions on the formation of nanominerals in mine acid drainage and on their reactions and interfaces. The detailed mineralogical and geochemical data presented in this paper were derived from previous studies on the coal mine areas in Brazil. Oxyhydroxides, sulfates, and nanoparticles in these areas possibly go through structural transformations depending on their size and formation conditions. The geochemistry of Fe-precipitates(such as jarosite, goethite, and hematite) existent in the CMD-generating coal areas and those that could be considered as a potential source of hazardous elements(HEs)(e.g., Cr) were also studied because these precipitates are relatively stable in extremely low pH conditions. To simplify and improve poorly ordered iron, strontium, and aluminum phase characterization, field emission scanning electron microscopy(FE-SEM), high-resolution transmission electron microscopy(HR-TEM), micro-Raman spectroscopy, and X-ray diffraction(XRD) and sequential extraction(SE) studies were executed on a set CMD samples from the Brazilian mines. In the case of the stomach, there are highly sensitive and specific methods for assessing gastric motility and emptying (GME). However, these methods are invasive, expensive and/or not technically feasible for all clinicians and patients. We present a summary of the most relevant international information on non-invasive methods and techniques for clinically evaluating GME. We particularly emphasize the potential of gastric electrical bioimpedance (EBI). EBI was initially used mainly in gastric emptying studies and was essentially abandoned in favor of techniques such as electrogastrography and the gold standard, scintigraphy. The current research evaluating the utility of gastric EBI either combines this technique with other frequently used techniques or uses new methods for gastric EBI signal analysis. In this context, we discuss our results and those of other researchers who have worked with gastric EBI. In this review article, we present the following topics: (1) a description of the oldest methods and procedures for evaluating GME; (2) an explanation of the methods currently used to evaluate gastric activity; and (3) a perspective on the newest trends and techniques in clinical and research GME methods. | 15 | Effect of psychological stress on gastric motility assessed by electrical bio-impedance显示文摘AIM:To evaluate gastric motility using electrical bioimpedance(EBI) and gastric changes as a result of stress induced by psychological tests.METHODS:A group of 57 healthy women,aged 40-60 years,was recruited,and a clinical history and physical examination were performed.The women were free from severe anxiety,chronic or acute stress,severe depression,mental diseases and conditions that affect gastric activity.The women were evaluated under fasting conditions,and using a four-electrode configuration,the gastric signals were obtained through a BIOPAC MP-150 system.The volunteers were evaluated using the following paradigm:basal state,recording during the Stroop Test,intermediate resting period,recording during the Raven Test,and a final resting period.We analyzed the relative areas of the frequency spectrum:A1(1-2 cpm),A2(2-4 cpm),A3(4-8 cpm),and A4(8-12 cpm),as well as the median of area A2 + A3.The data were analyzed by an autoregressive method using a Butterworth filter with MatLab and Origin.Analysis of variance(ANOVA) and Friedman ANOVA(for nonparametric variables) were performed;in addition,pairs of groups were compared using the T dependent and Wilcoxon T tests.RESULTS:The results of the main values of area A2 were not significantly different comparing the five steps of the experimental paradigm.Nevertheless,there was a tendency of this A2 region to decrease during the stress tests,with recuperation at the final resting step.When an extended gastric region was considered(1-4 cpm),significant differences with the psychological stress tests were present(F = 3.85,P = 0.005).The A3 region also showed significant changes when the stress psychological tests were administered(F = 7.25,P < 0.001).These differences were influenced by the changes in the adjacent gastric region of A2.The parameter that we proposed in previous studies for the evaluation of gastric motility by electrical bio-impedance(EBI) was the median of the area under the region from 2 to 8 cpm(A2 + A3).The mean values of these frequencies(median of the A2 + A3 area) with the stress test showed significant changes(F = 5.5,P < 0.001).The results of the WilcoxonT test for the A4 area parameter,which is influenced by the breathing response,changed significantly during the Raven stress test(P < 0.05).CONCLUSION:We confirm that the gastric response to acute psychological stress can be evaluated by short-term EBI. | María Raquel Huerta-Franco Miguel Vargas-Luna Juana Berenice Montes-Frausto Ismael Morales-Mata Lorena Ramirez-Padilla | 2012 | World Journal of Gastroenterology2012,18,36: | 5 |

| 17 | Prognostic significance of vascular endothelial growth factor polymorphisms in colorectal cancer patients显示文摘AIM To investigate the associations of the genetic polymor-phisms of vascular endothelial growth factor A(VEGF-A)-1498C>T and-634G>C, with the survival of patients with colorectal cancer(CRC). METHODS A prospective cohort consisting of 131 Brazilians patients consecutively operated on with a curative intention as a result of sporadic colorectal carcinoma was studied. DNA was extracted from peripheral blood and its amplification and allelic discrimination for each genetic polymorphism was performed using the technique of polymerase chain reaction(PCR) in real-time. The real-time PCR technique was used to identify the VEGF-A-1498C>T(rs833031) and-634G>C(rs2010963) polymorphisms. Genotyping was validated for VEGF-A-1498C>T polymorphism in 129 patients and for VEGF-A-634G>C polymorphism in 118 patients. The analysis of association between categorical variables was performed using logistic regression, survival by Kaplan-Meier method and multivariate analysis by the Cox regression method. RESULTS In the univariate analysis there was a significant association(OR = 0.32; P = 0.048) between genotype CC of the VEGF-A-1498C>T polymorphism and the presence of CRC liver metastasis. There was no association between VEGF-A-1498C>T polymorphism and VEGF-A-634G>C polymorphism with further clinical or anatomopathologic variables. The genotype CC of the VEGF-A-1498C>T polymorphism was significantly correlated with the 5-year survival(P = 0.032), but not significant difference(P = 0.27) was obtained with the VEGF-A-634G>C polymorphism with the 5-year survival in the univariate analysis. The genotype CT(HR = 2.79) and CC(HR = 4.67) of the polymorphism VEGF-A-1498C>T and the genotype CC(HR = 3.76) of the polymorphism VEGF-A-634C>G acted as an independent prognostic factor for the risk of death in CRC patients. | 20 | Second-look surgery plus hyperthermic intraperitoneal chemotherapy for patients with colorectal cancer at high risk of peritoneal carcinomatosis:Does it really save lives?显示文摘The treatment of peritoneal carcinomatosis (PC) of colorectal origin with cytoreductive surgery(CRS) plus hyperthermic intraperitoneal chemotherapy (HIPEC) has a 5-year recurrence-free or cure rate of at least 16%, so it is no longer labeled as a fatal disease, and offers prolonged survival for patients with a low peritoneal carcinomatosis index. Metachronous PC of colorectal origin is so predictable that there is a model which has been used to successfully determine the individual risk of each patient. Patients at risk are clearly identified; those with the highest risk have small peritoneal nodules present in the first surgery (70% probability of developing PC), ovarian metastases(60%), perforated tumor onset or intraoperative tumor rupture(50%). Current clinical, biological and imaging techniques still lack sufficient sensitivity to diagnose PC in its initial stages, when CRS plus HIPEC has a greater impact and a higher cure rate. Second-look surgery with HIPEC or prophylactic HIPEC at the time of the first intervention have been proposed as means of preventing and/or anticipating clinical or radiological relapse in at-risk patients.
