1Update on the management of gastrointestinal varices显示文摘Cirrhosis of liver is a major problem in the western world. Portal hypertension is a complication of cirrhosis and can lead to a myriad of pathology of which include the development of porto-systemic collaterals. Gastrointestinal varices are dilated submucosal veins, which often develop at sites near the formation of gastroesophageal collateral circulation. The incidence of varices is on the rise due to alcohol and obesity. The most significant complication of portal hypertension is life-threatening bleeding from gastrointestinal varices, which is associated with substantial morbidity and mortality. In addition, this can cause a significant burden on the health care facility. Gastrointestinal varices can happen in esophagus, stomach or ectopic varices. There has been considerable progress made in the understanding of the natural history, pathophysiology and etiology of portal hypertension. Despite the development of endoscopic and medical treatments, early mortality due to variceal bleeding remains high due to significant illness of the patient. Recurrent variceal bleed is common and in some cases, there is refractory variceal bleed. This article aims to provide a comprehensive review of the management of gastrointestinal varices with an emphasis on endoscopic interventions, strategies to handle refractory variceal bleed and newer endoscopic treatment modalities. 2Autologous CD34^+ and CD133^+ stem cells transplantation in patients with end stage liver disease显示文摘AIM:To assess the utility of an autologous CD34 + and CD133 + stem cells infusion as a possible therapeutic modality in patients with end-stage liver diseases.METHODS:One hundred and forty patients with endstage liver diseases were randomized into two groups.Group 1,comprising 90 patients,received granulocyte colony stimulating factor for five days followed by autologous CD34 + and CD133 + stem cell infusion in the portal vein.Group 2,comprising 50 patients,received regular liver treatment only and served as a control group.RESULTS:Near normalization of liver enzymes and improvement in synthetic function were observed in 54.5% of the group 1 patients;13.6% of the patients showed stable states in the infused group.None of the patients in the control group showed improvement.No adverse effects were noted.CONCLUSION:Our data showed that a CD34 + and CD133 + stem cells infusion can be used as supportive treatment for end-stage liver disease with satisfactory tolerability.Hosny Salama Abdel-Rahman N Zekri Abeer A Bahnassy Eman Medhat Hanan A Halim Ola S Ahmed Ghada Mohamed Sheren A Al Alim Ghada M Sherif 2010World Journal of Gastroenterology2010,16,42:16
3Gastroesophageal reflux disease, obesity and laparoscopic sleeve gastrectomy: The burning questions显示文摘Obesity is a global health epidemic with considerable economic burden.Surgical solutions have become increasingly popular following technical advances leading to sustained efficacy and reduced risk.Sleeve gastrectomy accounts for almost half of all bariatric surgeries worldwide but concerns regarding its relationship with gastroesophageal reflux disease(GERD)has been a topic of debate.GERD,including erosive esophagitis,is highly prevalent in the obese population.The role of pre-operative endoscopy in bariatric surgery has been controversial.Two schools of thought exist on the matter,one that believes routine upper endoscopy before bariatric surgery is not warranted in the absence of symptoms and another that believes that symptoms are poor predictors of underlying esophageal pathology.This debate is particularly important considering the evidence for the association of laparoscopic sleeve gastrectomy(LSG)with de novo and/or worsening GERD compared to the less popular Roux-en-Y gastric bypass procedure.In this paper,we try to address 3 burning questions regarding the inter-relationship of obesity,GERD,and LSG:(1)What is the prevalence of GERD and erosive esophagitis in obese patients considered for bariatric surgery?(2)Is it necessary to perform an upper endoscopy in obese patients considered for bariatric surgery?And(3)What are the long-term effects of sleeve gastrectomy on GERD and should LSG be done in patients with pre-existing GERD?Halim Bou Daher Ala I Sharara 2019World Journal of Gastroenterology2019,25,33:6
4The effects of plant density of Melastoma malabathricum on the erosion rate of slope soil at different slope orientations显示文摘Malaysia's cut slopes,especially for road lines accommodation,are prone to erosions and landslides.These problems mainly occur due to lack of vegetation cover and strong erosive forces.In addition,the topography factors have also become a major factor affecting soil degradation.Thus,this study is aimed at determining the effects of planting density of a selected species,namely Melastoma malabathricum;one,two,and three seedlings per box,on the erosion rate at selected slopes of different orientation(morning and evening sun) at the Guthrie Corridor Expressway,Selangor.In six months of observation,treatment with three seedlings/box on the morning sun slope showed a lower erosion rate by 69.2%than those with the same treatment on the evening sun slope.In addition,the treatment of the three seedlings recorded at month six(final observation) had the highest reduction of soil saturation level(STL),by 23.6%.Furthermore,the physiological values of the species studied,grown on the morning sun slope,were higher in terms of stomatal conductance and photosynthetic rate by 12.1%and 15.8%(three seedlings/box),respectively.The overall results showed that plant density was inversely related to the STL and erosion rate on the slope.In conclusion,a planting density of three seedlings/box and morning sun orientation gave positive effects on the plant's physiological performance of the slope,reducing the STL,as well as alleviating the erosion rate of slope soils.Aimee Halim Osman Normaniza 2015International Journal of Sediment Research2015,30,2:6
5Comparative analysis of some selected macronutrients of soil in orange orchard and degraded forests in Chittagong Hill Tracts,Bangladesh显示文摘Status of organic carbon (C), total nitrogen (N), available potassium (K), calcium (Ca) and phosphorus (P) in three different depths (0-5 cm, 5-15 cm and 15?30 cm) on two hill slopes of 35% and 55% in orange orchard cultivated by the Mro tribe of Chittagong Hill Tracts (CHTs) were evaluated and compared with those in degraded bush forests, through digging three profiles in each land use. The content of all the five nutrients was found to be higher in the soil of orange orchard than in the soil of forest. But the variation was not consistent for both the slopes. The content varied depth wise also, having the highest value in surface soil in case of both the land uses on both the slopes. A mean available K content was significantly higher in orange orchard than in forest on 55% slope, while it was lower on 35% slope. Surface soil contained the nutrients of K and Ca with the amount of 0.2905-mg·g^(-1) soil and 3.025-mg·g^(-1)soil respectively in the orchard, while 0.1934-mg·g^(-1) soil and 1.6083-mg·g^(-1) soil were respectively in the forest. Organic carbon and total nitrogen were found more or less similar in surface soil on both the land uses showing a slight difference. Available P was found only in orange orchard, and in forest it was too little in amount to detect by the spectrophotometer. The degraded forests were poor in nutrient content due to high rate of soil erosion, which would be possible to be improved by bringing it under tree cover as proved by the adaptation of orange orchard there.Mohammad Shaheed Hossain Chowdhury Shampa Biswas Md. 6Comparative evaluation of physical properties in soils of orange orchard and bushy forest in chittagong hill tracts, Banglandesh显示文摘对孟加拉国丘陵地带Mro部落经营的桔园与灌木林35%和55%两种坡地土壤物理性质进行了比较与评价。每一块用地内挖掘3个土壤剖面采集不同深度(0-5cm,5-15cm and 15-30cm)的土壤样品,测定土壤水分、有机质含量和土壤密度。最大持水量、田间持水量、干或湿土壤容重和土壤孔隙度的测定仅限于表层土壤。两种坡地桔园3个深度的土壤水分含量均显著高于灌木林地相同深度的土壤水分。桔园在55%坡地内土壤有机质含量低于林地含量值,而在35%坡地内的含量值则高于林地值。两块样地内上述两项测定值均在表层土壤中最高,且随土层深度的增加而降低。与灌木林地比较,桔园地表层土壤最大持水量、孔隙度和3个不同深度的土壤密度均较低。两块用地内不同坡地的表层土田间持水量有所差异,且桔园地干或湿表层土壤容重高于林地内的数值。表5参23。Mohammad Shaheed Hossain Chowdhury Md. 7Influence of diversion angle on water and sediment flow into diversion channel显示文摘Accumulation of the sediment in the stream of the diversion channels adversely affects its operational systems.Diversion channels are often constructed perpendicular to the main river.In this study,the water flow and sediment transport in the diversion channel with different angles were investigated in an attempt to maximize water discharge and minimize sediment discharge.A physical model with movable bed was used to simulate water and sediment flow with five diversion angles(θ)between(30°-90°).Moreover,three bed width ratios(Br)(the relation between diversion to main channel bed width)between 30%and 50%and five total discharges between(7.25 L/s to 12.25 L/s)were considered for each case of(θ).The results showed,up to 10%,increasing in proportion discharge ratios for 30 and 45 diversion angles compared with 90° diversion angle.The results also showed that the lowest diversion sediment concentration was provided by the(θ)of 30°.Across all scenarios,the average proportion concentration reduction was 64%,compared with 90diversion angle.Closer observation of the diversion system mechanism confirmed that decreased(θ)result in decreased sediment concentrations in the diversion channels.In conclusion,the diversion channel water and sediment discharge could be effectively managed by changing the(θ)to 30° or 45° instead of 90°.Nashwan Kamalaldeen Alomari Badronnisa Yusuf Thamer Ahmad Mohammad Abdul Halim Ghazali 2020International Journal of Sediment Research2020,35,6:4
8Correlation between the skull base fracture and the incidence of intracranial hemorrhage in patients w让h traumatic brain injury显示文摘Purpose:A head injury(HI)may cause a skull fracture,which may or may not be associated with injury to the brain.In essence,a skull base fracture(SBF)is a linear fracture at the base of the skull.Loss of consciousness and Glasgow coma score(GCS)may vary depending on an associated intracranial pathology.The pathomechanism is believed to be caused by high energy impact directly to the mastoid and supraorbital bone or indirectly from the cranial vault.Aim of this study is to define the correlation between SBF and intracranial hemorrhage(ICH)in patients with HI.Methods:Analysis of data obtained from a retrospective review of medical records and from a systematized database pertaining to diagnostic criteria of SBF patients based only on clinical symptoms associated with ICH caused by HI treated in the Department of Neurosurgery at Dr.Hasan Sadikin Hospital,Bandung,Indonesia from January 1,2012 to December 31,2017.The exclusion criteria in eluded age less than 15 years and no head computed tomography(CT)scan examination provided.Results:A total of 9006 patients were in eluded into this study in which they were divided into 3 groups:group 1,HI with no ICH;group 2,HI with single ICH and group 3,HI with multiple ICH.In all the SBF cases,SBF at anterior fossa accounted for 69.40%of them,which were mostly accompanied with mild HI(64.70%).Severity of HI and site of SBF correlated with the existence of traumatic brain lesions on CT scan,thus these factors were able to predict whether there were traumatic brain lesions or not.Most of the patients with epidural hemorrhage(EDH)has single traumatic lesion on CT scan,whereas most of the patients with cerebral contusion(CC)has multiple traumatic lesions on CT scan.On patients with both traumatic brain injury and SBF,most of the patients with anterior fossa SBF has EDH;whereas most of the patients with middle fossa SBF were accompanied with CC.Surgery was not required for most of the patients with SBF.Conclusion:SBFs were strongly correlated with traumatic ICH lesions patients with anterior fossa SBF were more likely to suffer EDH whereas with middle fossa SBF were more likely to suffer CC.Ahmad Faried Danny Halim Ingrid Ayke Widjaya Rendy Febrian Badri Syailendra Fii Sulaiman Muhammad Z.Arifin 2019Chinese Journal of Traumatology2019,22,5:3
9Effect of Al Doping on Structural, Electrical, Optical and Photoluminescence Properties of Nano-Structural ZnO Thin Films显示文摘The nano-structural Al-doped ZnO thin films of different morphologies deposited on glass substrate were successfully fabricated at substrate temperature of 350 C by an inexpensive spray pyrolysis method. The structural, electrical, optical and photoluminescence properties were investigated. X-ray diffraction study revealed the crystalline wurtzite (hexagonal) structure of the films with nano-grains. Scanning electron mi- croscopy (SEM) micrographs indicated the formation of a large variety of nano-structures during film growth. The spectral absorption of the films occurred at the absorption edge of ~410 nm. In the present study, the optical band gap energy 3.28 eV of ZnO decreased gradually to 3.05 eV for 4 mol% of Al doping. The deep level activation energy decreased and carrier concentrations increased substantially with increasing doping. Exciting with the energy 3.543 eV (λ=350 nm), a narrow and a broad characteristic photoluminescence peaks that correspond to the near band edge (NBE) and deep level emissions (DLE), respectively emerged.M. 12Performance Assessment of Nanocellulose Hydroxypropyl Methyl Cellulose Composite on Role of Nano-CaCO_(3) for the Preservation of Paper Documents显示文摘Deacidification and self-cleaning are important for the preservation of paper documents.In this study,nano-CaCO_(3) was used as a deacidification agent and stabilized by nanocellulose(CNC)and hydroxypropyl methylcellulose(HPMC)to form a uniform dispersion.Followed by polydimethylsiloxane(PDMS)treatment and chemical vapor deposition(CVD)of methyltrimethoxysilane(MTMS),a hydrophobic coating was constructed for self-cleaning purposes.The pH value of the treated paper was approximately 8.20,and the static contact angle was as high as 152.29°.Compared to the untreated paper,the tensile strength of the treated paper increased by 12.6%.This treatment method endows the paper with a good deacidification effect and self-cleaning property,which are beneficial for its long-term preservation.Xiaochun Ma Altaf Halim Xiaohong Li Huiming Fan Shiyu Fu 2022Paper And Biomaterials2022,7,2:2
13Vital roles of stem cells and biomaterials in skin tissue engineering显示文摘Tissue engineering essentially refers to technology for growing new human tissue and is distinct from regenerative medicine. Currently, pieces of skin are already being fabricated for clinical use and many other tissue types may be fabricated in the future.Tissue engineering was first defined in 1987 by the United States National Science Foundation which critically discussed the future targets of bioengineering research and its consequences. The principles of tissue engineering are to initiate cell cultures in vitro, grow them on scaffolds in situ and transplant the composite into a recipient in vivo. From the beginning, scaffolds have been necessary in tissue engineering applications. Regardless, the latest technology has redirected established approaches by omitting scaffolds. Currently, scientists from diverse research institutes are engineering skin without scaffolds. Due to their advantageous properties, stem cells have robustly transformed the tissue engineering field as part of an engineered bilayered skin substitute that will later be discussed in detail. Additionally, utilizing biomaterials or skin replacement products in skin tissue engineering as strategy to successfully direct cell proliferation and differentiation as well as to optimize the safety of handling during grafting is beneficial. 15孟加拉国Bhanugach西部保育林用地类型变化(英文)显示文摘应用遥感和地理信息系统调查了孟加拉国Sylhet林业分局所辖的Bhanugach西部保育林从1988年到2006年间用地类型的变化。应用了监督和非监督两种方法,用全球定位系统搜集了地面控制点。地理信息系统软件制成地图。结果表明,植被覆盖从1988年的1826hm2大幅地降低到1996年的1714.85hm2,随着协同管理措施的实施,植被覆盖自1996年后逐年增长到2006年的1847.83hm2。在没有其他类型的用地转变成裸地的情况下,裸地变化与植被覆盖间成反比。18年间水体面积从1988年的307.67hm2增加到1996年379.53hm2,96-97年间略有下降,1997年以后直到2006年基本保持不变。本文还提出了将遥感技术和地理信息系统应用于用地类型变化及地理信息系统数据库创建的一些建议。Md. 16ANALYTICAL ANALYSIS OF INTERFACIAL STRESSES IN FRP-RC HYBRID BEAMS WITH TIME-DEPENDENT DEFORMATIONS OF RC BEAM显示文摘In this paper, the effect of time-dependent deformations (such as shrinkage and creep) on the interfacial stresses between an RC beam and FRP plate is presented. For this end, a closed-form solution for such stresses in externally FRP plated RC beams including creep and shrinkage effects is presented. The developed model is formulated to predict the interfacial stresses at time ‘t’, in which the RC beams have been already subjected to creep and shrinkage effects. The adherend shear deformations have been included in the present theoretical analysis by assuming a parabolic shear stress through the thickness of the RC beam and the FRP panel. Contrary to some existing studies, the assumption that both RC beam and FRP panel have the same curvature is not used in the present investigation. 17Theoretical Approach to Change Blast Furnace Regime With Natural Gas Injection显示文摘The operation of blast furnace using natural gas and oxygen enriched blast(composite blast technology)is considered in many countries to be standard operation for a modern blast furnace particularly in certain countries with cheap and stable supply of natural gas.The theoretical flame temperature(TFT)of combustion and the degree of direct reduction of iron oxides(rd)are considered as the main controlling parameters of composite blast technology.The calculated values of these parameters are mainly dependent on the amount of air blast consumption.This amount of air blast is measured before entering into blast stoves.Actually,some of air blast is lost through valves of air stoves.Consequently,the real volume of air blast in the furnace is less than the recorded value by amounts of 5%-15% which is not considered in the estimation of rd and TFT.The purpose is to analyze the different methods for estimation of air blast inside the blast furnaces and develop a theoretical model to calculate air blast consumption with high accuracy.Based on the calculation of air blast consumption,a complete roadmap is demonstrated to change the operation regime parameters of blast furnaces working on composite blast technology.K S Abdel Halim 2013Journal of Iron and Steel Research(International)2013,20,9:2
