| 1 | 液体过负荷,基础疾病及启动时机对接受连续性肾脏替代治疗危重症患儿的影响显示文摘目的:确定需要连续性肾脏替代(continuous renal replacement therapy,CRRT)治疗危重症患儿病死因素。设计基于前瞻性注册登记的回顾性观察性研究。场所拥有30张床位的三级、四级转诊PICU。对象接受CRRT的危重症患儿。干预连续性肾脏替代治疗。测量方法与主要结果研究期间接受CRRT治疗的161例患儿总体病死率为36%(n=58),接受体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)治疗患儿病死率为47.5%(28/59),显着高于无需ECMO治疗者[28.4%(29/102),P=0.022]。根据入院诊断,血液肿瘤患儿病死率最高(77.8%),肾脏病患儿则最低(5.6%)。基于多元Logistic回归分析,入院时疾病严重程度评分较高(校正OR 1.49,95%CI 1.18~1.89,P<0.001),血液肿瘤疾病(OR 17.10,95%CI 4.10~72.17,P<0.001),液体过负荷10%~20%(OR 3.83,95%CI 1.33~11.07,P=0.013),>20%(OR 15.03,95%CI 4.03~56.05, P<0.001),及CRRT启动时机(OR 1.01,95%CI 1.00~1.01,P=0.040)与病死率独立相关。本研究人群中,从入住ICU起CRRT启动每延迟1 h,死亡几率增加1%。结论CRRT患儿病死率高,且与基础疾病,疾病严重程度和液体过负荷程度相关。在急性肾损伤及液体过负荷风险较高的危重症患儿中,早期启动CRRT可能降低病死率。 | Cortina G McRae R Hoq M 方伯梁(译者) 钱素云(审校者) | 2019 | 中国小儿急救医学2019,26,4: | 10 |
