9婴幼儿体外循环期间高氧血症与血浆游离血红蛋白升高的关系显示文摘目的确定儿童体外循环(cardiopulmonarybypass, CPB)期间严重溶血的潜在危险因素, 探讨超生理水平氧与CPB持续时间是否和溶血相关。设计前瞻性观察性研究。场所一家大学附属儿童医院的CICU。对象因心脏手术CPB的>1月且<18岁患儿。干预措施无。测量方法与主要结果采集100例患儿CPB开始/结束时, 以及再灌注后2/24 h的血浆样本, 以评估游离血浆血红蛋白(cell-freeplasmahemoglobin, PHB)水平。CPB前及CPB期间每30 min间隔采集动脉血气。从电子病历中收集人口统计和实验室数据。PHB水平在体外循环结束时达到峰值, 结合珠蛋白水平在所有时间点持续下降。重度溶血(ΔPHB>50 mg/dL)患儿44例。多因素分析显示小年龄(OR/SD 0.45, 95%CI 0.25~0.81)和平均PaO_(2)*CPB时间值较高(31.11, 1.46~664.64)是严重溶血的危险因素。严重溶血与住院/住ICU时间较长, 以及发生急性肾损伤有关。结论本研究发现小年龄, 氧气/CPB共同暴露是溶血危险因素。通过CPB输送氧气是一个容易改变的危险因素, 其产生改变红细胞膜的活性氧物质的作用机制尚需更大规模的前瞻性研究。Gretchen C Bayir H Kochanek PM 方伯梁(译) 钱素云(校) 2022中国小儿急救医学2022,29,5:1
