1Experimental study of“Tong Xia”purgative method in ameliorating lung injury in acute necrotizing pancreatitis显示文摘AIM To investigate the role of tumor necrosisfactor(TNF)in lung injury during acutenecrotizing pancreatitis(ANP),and thetherapeutic effect of'Tong Xia'purgativemethod in minimizing the severity of lung injury.METHODS Fourteen canines were randomlydivided into 3 groups:the'Tong Xia'treatmentgroup(n = 5)using Dachengqitang;salinecontrol group(n = 5),and the sham operationgroup(n = 4).TNF activity in serum and inbronchoalveolar lavage fluid(BALF),the serumendotoxin levels were measured,and theseverity of lung injury evaluated.RESULTS Elevation of TNF activity was moreprominent in BALF than in serum.TNF activity inserum at 6 and 12 hours and in BALI:wassignificantly decreased in the'Tong Xia'treatment group than in the saline control one(q=21.11,q=12.07,q=9.03,respectively,P<0.01)and the lung injury was significantlyalleviated at 12 hours as compared with that inthe saline group,manifested as amelioration otthe lung wet/ dry weight ratio,decrease inprotein concentration and neutrophils count inBALF,and improvement of pulmonaryinflammatory changes.A positive correlationwas demonstrated between serum TNF activity and endotoxin level.CONCLUSION Hypersecretion of TNF is shownto be one of the major causes of lung injuryduring ANP;'Tong Xia'purgative method couldalleviate the degree of lung injury mediated byTNF.Xia Q Jiang JM Gong X Chen GY Li L Huang ZW 2000World Journal of Gastroenterology2000,6,1:31

3First-line eradication of Helicobacter pylori:Are the standard triple therapies obsolete? A different perspective显示文摘Studies concerning the eradication of Helicobacter pylori have resulted in a proliferation of meta-analyses. To date, there are 303 meta-analyses cited in PubMed, 113 dealing with the therapy of the infection. A chronological analysis of the results of meta-analyses performed between 1998 and 2010 shows that first-line standard triple therapies achieved eradication rates on an intention-to-treat basis of around 80%; prolonging treatment to 14, but not 10 d should improve the results. The proton pump inhibitors have a similar efficiency, and giving a double dose is more efficient than the standard doses of these drugs. Triple and quadruple therapies proved to be equivalent. Based on meta-analytical data, the decrease in efficiency over time cannot be substantiated: eradication rates < 80% followed from the introduction of triple therapies. As alternatives, ranitidine bismuth citrate-, levofloxacinor furazolidone-based therapies were shown to obtain the same eradication rates as standard triple regimens. Sequential therapies and quadruple non-bismuth-based therapies were superior to standard triple therapies but their use is limited to certain countries. In the author's opinion, and from a meta-analytical viewpoint, standard triple therapies cannot yet be considered obsolete. 6Metabolic consequences of Helicobacter pylori infection and eradication显示文摘Helicobacter pylori(H.pylori)is still the most prevalent infection of the world.Colonization of the stomach by this agent will invariably induce chronic gastritis which is a low-grade inflammatory state leading to local complications(peptic ulcer,gastric cancer,lymphoma)and remote manifestations.While H.pylori does not enter circulation,these extragastric manifestations are probably mediated by the cytokines and acute phase proteins produced by the inflammed mucosa.The epidemiologic link between the H.pylori infection and metabolic changes is inconstant and controversial.Growth delay was described mainly in low-income regions with high prevalence of the infection,where probably other nutritional and social factors contribute to it.The timely eradication of the infection will lead to a more healthy development of the young population,along with preventing peptic ulcers and gastric cancer An increase of total,low density lipoprotein and high density liporotein cholesterol levels in some infected people creates an atherogenic lipid profile which could promote atherosclerosis with its complications,myocardial infarction,stroke and peripheral vascular disease.Well designed and adequately powered long-term studies are required to see whether eradication of the infection will prevent these conditions.In case of glucose metabolism,the most consistent association was found between H.pylori and insulin resistance:again,proof that eradication prevents this common metabolic disturbance is expected.The results of eradication with standard regimens in diabetics are significantly worse than in non-diabetic patients,thus,more active regimens must be found to obtain better results.Successful eradication itself led to an increase of body mass index and cholesterol levels in some populations,while in others no such changes were encountered.Uncertainities of the metabolic consequences of H.pylori infection must be clarified in the future.Gy?rgy Miklós Buzás 2014World Journal of Gastroenterology2014,20,18:20
7Changes of the cytokine profile in inflammatory bowel diseases显示文摘Cytokines are indispensable signals of the mucosaassociated immune system for maintaining normal gut homeostasis.An imbalance of their profile in favour of inflammation initiation may lead to disease states,such as that is observed in inflammatory bowel diseases(IBD).Although Crohn's disease(CD) is often described as a prototype of T-helper 1-type diseases,and ulcerative colitis(UC) is traditionally viewed as a T-helper 2-mediated condition,the classic paradigm,which categorises cytokines into pro-and anti-inflammatory groups,has recently been changed.The inflammation regulatory pathways may not be mutually exclusive as individual cytokines can have diverse and even opposing functions in various clinical and immunological settings.None the less there are many common immunological responses in IBD that are mediated by cytokines.Although they regulate and influence the development,course and recurrence of the inflammatory process,the concrete pathogenic role of these small signaling molecules is sometimes not unambiguous in the subtypes of the disease.Our aim is to review the current information about pro-and anti-inflammatory effects of traditionally studied and recently discovered cytokines in the pathogenesis of UC and CD.The better understanding of their production and functional activity may lead to the development of new therapeutic modalities.Gyrgyi Mzes Béla Molnár Zsolt Tulassay Ferenc Sipos 2012World Journal of Gastroenterology2012,18,41:16
8Molecular organic geochemical evidence for paleoenvironmental changes at 11.87—12.28 m in GS-1 sedimentary core,Gucheng Lake,East China显示文摘Organic lipids with different occurrence modes in a sedimentary core in a subtropicalfreshwater lake, Gucheng Lake, were sequentially extracted, separated and analyzed. Thedown-core variations of OEP index of alkanes, C18:2/C18:0 ratios of fatty acids, ratios of boundfatty acids to free fatty acids, thermo-maturity indices of hopanoid hydrocarbons and relativeconcentrations of sedimentary minerals revealed an evident increasing in allochthonous inputfrom rock detritus below 12.28 m in the sedimentary profile. The distribution pattern of tightly-bound β-hydroxy fatty acids indicated a difference in bacterial communities above and below theboundary, as well as the gammacerane index may assign a significant change in lake watersalinity. 9Epithelial toll-like receptor 9 signaling in colorectal inflammation and cancer: Clinico-pathogenic aspects显示文摘Toll-like receptors (TLRs) recognize specific motifs which are frequently present in bacteria, fungi, prokaryotes and viruses. Amongst TLRs, TLR9 can be activated by such bacterial or viral DNA fragments, immunoglobulin-DNA complexes or synthetic oligonucleotides, which all contain unmethylated cytosineguanine nucleotide sequences (CpGs). Emerging data indicate that TLR9 signaling has a role in, and may influence, colorectal carcinogenesis and colonic inflammation. CpGs are classified into three groups according to their influence on both the antigen-specific humoraland cellular immunity, and the production of type 1 interferons and proinflammatory cytokines. TLR9 activation via CpGs may serve as a new therapeutic target for several cancerous and various inflammatory conditions. Due to its probable anti-cancer effects, the application possibilities of TLR9-signaling modulation may be extremely diverse even in colorectal tumors. In this review we aimed to summarize the current knowledge about TLR-signaling in the pathogenesis and therapy of inflammatory bowel diseases and colorectal cancer. Due to the species-specific differences in TLR9 expression, however, one must be careful in translating the animal model data into the human system, because of the differences between CpG-oligodeoxynucleotide-responsive cells. 13Synthesis and Properties of Cerium Oxide Nanometer Powders by Pyrolysis of Amorphous Citrate显示文摘CeO2 nanometer powders of different sizes were prepared at low temperature by pyrolysis of amorphous citrate. XRD patterns show that CeO2 is cubic in structure, space group Oh5-FM3M·TEM indicates that the prepared CeO2 is spherical in shape, and the particle size distribution is in narrow range. It was found that calcination temperature is a more important factor affecting the crystallite size of CeO2 than calcining time, the smaller the particle. the bigger the crystallattice distortion, the worse the crystal growth. Solubility test of CeO2 in nitric acid reveals that the surface activity of CeO2 decreases with the increasing particle sizes. 15Therapeutic aspects of c-MYC signaling in inflammatory and cancerous colonic diseases显示文摘Colonic inflammation is required to heal infections, wounds, and maintain tissue homeostasis. As the seventh hallmark of cancer, however, it may affect all phases of tumor development, including tumor initiation, promotion, invasion and metastatic dissemination, and also evasion immune surveillance. Inflammation acts as a cellular stressor and may trigger DNA damage or genetic instability, and, further, chronic inflammation can provoke genetic mutations and epigenetic mechanisms that promote malignant cell transformation. Both sporadical and colitis-associated colorectal carcinogenesis are multi-step, complex processes arising from the uncontrolled proliferation and spreading of malignantly transformed cell clones with the obvious ability to evade the host's protective immunity. In cells upon DNA damage several protooncogenes, including c-MYC are activated in parelell with the inactivation of tumor suppressor genes. The target genes of the c-MYC protein participate in different cellular functions, including cell cycle, survival, protein synthesis, cell adhesion, and microRNA expression. The transcriptional program regulated by c-MYC is context dependent, therefore the final cellular response to elevated c-MYC levels may range from increased proliferation to augmented apoptosis. Considering physiological intestinal homeostasis, c-MYC displays a fundamental role in the regulation of cell proliferation and crypt cell number. However, c-MYC gene is frequently deregulated in inflammation, and overexpressed in both sporadic and colitis-associated colon adenocarcinomas. Recent results demonstrated that endogenous c-MYC is essential for efficient induction of p53-dependent apoptosis following DNA damage, but c-MYC function is also involved in and regulated by autophagy-related mechanisms, while its expression is affected by DNA-methylation, or histone acetylation. Molecules directly targeting c-MYC, or agents acting on other genes involved in the c-MYC pathway could be selected for combined regiments. However, due to its context-dependent cellular function, it is clinically essential to consider which cytotoxic drugs are used in combination with c-MYC targeted agents in various tissues. 16Contribution of TLR signaling to the pathogenesis of colitisassociated cancer in inflammatory bowel disease显示文摘In the intestine a balance between proinflammatory and repair signals of the immune system is essential for the maintenance of intestinal homeostasis. The innate immunity ensures a primary host response to microbial invasion, which induces an inflammatory process to localize the infection and prevent systemic dissemination of pathogens. The key elements of this process are the germline encoded pattern recognition receptors inclu-ding Toll-like receptors(TLRs). If pathogens cannot be eliminated, they may elicit chronic inflammation, which may be partly mediated via TLRs. Additionally, chronic inflammation has long been suggested to trigger tissue tumorous transformation. Inflammation, the seventh hallmark of cancer, may affect all phases of tumor development, and evade the immune system. Inflammation acts as a cellular stressor and may trigger DNA damage or genetic instability. Furthermore, chronic inflammation can provoke genetic mutations and epigenetic mechanisms that promote malignant cell transformation. Colorectal cancers in inflammatory bowel disease patients are considered typical examples of inflammation-related cancers. 18Dual color optogenetic control of neural populations using low-noise,multishank optoelectrodes显示文摘Optogenetics allows for optical manipulation of neuronal activity and has been increasingly combined with intracellular and extracellular electrophysiological recordings.Genetically-identified classes of neurons are optically manipulated,though the versatility of optogenetics would be increased if independent control of distinct neural populations could be achieved on a sufficient spatial and temporal resolution.We report a scalable multisite optoelectrode design that allows simultaneous optogenetic control of two spatially intermingled neuronal populations in vivo.We describe the design,fabrication,and assembly of low-noise,multisite/multicolor optoelectrodes.Each shank of the four-shank assembly is monolithically integrated with 8 recording sites and a dualcolor waveguide mixer with a 7×30μm cross-section,coupled to 405 nm and 635 nm injection laser diodes(ILDs)via gradient-index(GRIN)lenses to meet optical and thermal design requirements.To better understand noise on the recording channels generated during diode-based activation,we developed a lumped-circuit modeling approach for EMI coupling mechanisms and used it to limit artifacts to amplitudes under 100μV upto an optical output power of 450μW.We implanted the packaged devices into the CA1 pyramidal layer of awake mice,expressing Channelrhodopsin-2 in pyramidal cells and ChrimsonR in paravalbumin-expressing interneurons,and achieved optical excitation of each cell type using sub-mW illumination.We highlight the potential use of this technology for functional dissection of neural circuits.Komal Kampasi Daniel F.English John Seymour Eran Stark Sam McKenzie Mihály Vöröslakos György Buzsáki Kensall D.Wise Euisik Yoon 2018Microsystems & Nanoengineering2018,4,1:8
19Regulatory T cells in inflammatory bowel diseases and colorectal cancer显示文摘Regulatory T cells(T regs) are key elements in immunological self-tolerance.The number of T regs may alter in both peripheral blood and in colonic mucosa during pathological circumstances.The local cellular,microbiological and cytokine milieu affect immunophenotype and function of T regs.Forkhead box P3+ T regs function shows altered properties in inflammatory bowel diseases(IBDs).This alteration of T regs function can furthermore be observed between Crohn's disease and ulcerative colitis,which may have both clinical and therapeutical consequences.Chronic mucosal inflammation may also influence T regs function,which together with the intestinal bacterial flora seem to have a supporting role in colitis-associated colorectal carcinogenesis.T regs have a crucial role in the immunoevasion of cancer cells in sporadic colorectal cancer.Furthermore,their number and phenotype correlate closely with the clinical outcome of the disease,even if their contribution to carcinogenesis has previously been controversial.Despite knowledge of the clinical relationship between IBD and colitis-associated colon cancer,and the growing number of immunological aspects encompassing sporadic colorectal carcinogenesis,the molecular and cellular links amongst T regs,regulation of the inflammation,and cancer development are still not well understood.In this paper,we aimed to review the current data surrounding the role of T regs in the pathogenesis of IBD,colitis-associated colon cancer and sporadic colorectal cancer.Gyrgyi Mzes Béla Molnár Ferenc Sipos 2012World Journal of Gastroenterology2012,18,40:7
