| 1 | A case of asymptomatic intraductal papillary neoplasm of the bile duct without hepatolithiasis显示文摘A 65-year-old woman was found to have dilatation of the intrahepatic bile duct in the right anterior segment during a general health. Laboratory data were within normal ranges and no solid mass was detected in her abdominal computer tomography (CT) or nuclear magnetic resonance imaging (MRI). However, endoscopic retrograde cholangiopancreatography (ERCP) demonstrated an obstruction of the right bile duct. Intraoperative cholangiography showed stenosis of the intrahepatic bile duct in the anterior inferior segment (B5) and narrowness of the intrahepatic bile duct in the anterior superior segment (B8), so that we strongly suspected intrahepatic cholangiocarcinoma (ICC). Histologically, surgically resected liver specimens, without tumor mass by macroscopic observation, showed intraductal papillary proliferation with fibrovascular cores and intraductal spreading of carcinoma in situ throughout a considerable area, especially in bile ductules around the peripheral small portal area. Furthermore, the immunohistochemical profile of the tumor (MUC5AC+/CK7+) was compatible with an intraductal papillary neoplasm of the bile duct (IPN-B). Consequently, this case was diagnosed as IPN-B with spreading CIS, stageⅠ(pT1, pN0, P0, H1, M0). | 2 | Extract of Yokukansan improves anxiety-like behavior and increases serum brain-derived neurotrophic factor in rats with cerebral ischemia combined with amyloid-β42 peptide显示文摘OBJECTIVE: To examine the effects of Yokukansan(YKS) extract on two endogenous modulators of anxiety, brain-derived neurotrophic factor(BDNF)and serotonin (5-HT)_(2A) receptors pharmacologically, in the ischemic rat model of dementia.METHODS: The cerebral ischemia(CI) was induced by bilateral occlusion of the vertebral and common carotid arteries(4-vessel occlusion ischemia). The CI was combined with the amyloid-β42 peptide(A42) injected intracerebroventricularly, and referreβdto as CI+Aβ. Anxiety-like behaviors were assessed by elevated plus maze(enclosed arm), light/dark transition test(dark chamber), and open-field test.Wet-dog shakes were induced by the 5-HT_(2A) receptor agonist 2, 5-dimethoxy-4-iodoamphetamine(DOI). The concentration of BDNF in serum was determined by enzyme-linked immuno sorbent assay.RESULTS: CI + Aβ increased anxiety, as demonstrated by the increase of time spent in the enclosed arms and dark chambers, and locomotion in the outer zone of the open field(thigmotaxis). CI + Adecreased the serum concentration of BDNF. | 3 | Spontaneous regression of gastric gastrinoma after resection of metastases to the lesser omentum:A case report and review of literature显示文摘BACKGROUND Gastric gastrinoma and spontaneous tumor regression are both very rarely encountered.We report the first case of spontaneous regression of gastric gastrinoma.CASE SUMMARY A 37-year-old man with a 9-year history of chronic abdominal pain was referred for evaluation of an 8 cm mass in the lesser omentum discovered incidentally on abdominal computed tomography.The tumor was diagnosed as grade 2 neuroendocrine neoplasm(NEN)on endoscopic ultrasound-guided fine-needle aspiration.Screening esophagogastroduodenoscopy revealed a 7 mm red polypoid lesion with central depression in the gastric antrum,also confirmed to be a grade 2 NEN.Laparoscopic removal of the abdominal mass confirmed it to be a metastatic gastrinoma lesion.The gastric lesion was subsequently diagnosed as primary gastric gastrinoma.Three months later,the gastric lesion had disappeared without treatment.The patient remains symptom-free with normal fasting serum gastrin and no recurrence of gastrinoma during 36 mo of follow-up.CONCLUSION Gastric gastrinoma may arise as a polypoid lesion in the gastric antrum.Spontaneous regression can rarely occur after biopsy. | Takeshi Okamoto Takaaki Yoshimoto Nobuyuki Ohike Aoi Fujikawa Takayoshi Kanie Katsuyuki Fukuda | 2021 | World Journal of Gastroenterology2021，27，1： | 2 |
