| 1 | Recurrent thrombotic occlusion of a transjugular intrahepatic portosystemic stent-shunt due to activated protein C resistance显示文摘The transjugular intrahepatic portosystemic stent-shunt (TIPS) has successfully been used in the management of refractory variceal bleeding and ascites in patients with portal hypertension. Major drawbacks are the induction of hepatic encephalopathy and shunt dysfunction. We present a 59-year-old woman with alcoholic liver cirrhosis who received a TIPS because of recurrent bleeding from esophageal varices. Stent occlusion occurred 4 mo after placement of the TIPS. Laboratory testing revealed resistance to activated protein C (APC). Combination therapy with low-dose enoxaparin and clopidogrel could not prevent her recurrent stent occlusion. Finally, therapy with high-dose enoxaparin was sufficient to prevent further shunt complications up to now (follow-up period of 1 year). In conclusion, early occlusion of a TIPS warrants testing for thrombophilia. If risk factors are confirmed,anticoagulation should be intensified. | 2 | Dysregulation of iron and copper homeostasis in nonalcoholic fatty liver显示文摘Elevated iron stores as indicated by hyperferritinemiawith normal or mildly elevated transferrin saturation a n d m o s t l y m i l d h e p a t i c i r o n d e p o s i t i o n a r e a characteristic finding in subjects with non-alcoholic fatty liver disease(NAFLD). Excess iron is observed in approximately one third of NAFLD patients and is commonly referred to as the 'dysmetabolic iron overload syndrome'. Clinical evidence suggests that elevated body iron stores aggravate the clinical course of NAFLD with regard to liver-related and extrahepatic disease complications which relates to the fact that excess iron catalyses the formation of toxic hydroxylradicals subsequently resulting in cellular damage. Iron removal improves insulin sensitivity, delays the onset of type 2 diabetes mellitus, improves pathologic liver function tests and likewise ameliorates NAFLD histology. Several mechanisms contribute to pathologic iron accumulation in NAFLD. These include impaired iron export from hepatocytes and mesenchymal Kupffer cells as a consequence of imbalances in the concentrations of iron regulatory factors, such as hepcidin, cytokines, copper or other dietary factors. | 3 | Treatment of Iron-Loaded Veterinary Sarcoma by Artemisia annua显示文摘Artemisinin,a constituent of Artemisia annua L.,is a well-known antimalarial drug.Artemisinin-type drugs also inhibit cancer growth in vitro and in vivo.Herbal extracts of A.annua inhibit the growth of cancer cell lines.Here,we report on the use of capsules containing powder of Herba Artemisiae annuae to treat pet sarcoma.The surgical tumor removal as standard treatment was supplemented by adjuvant therapy with A.annua.One cat and one dog with fibrosarcoma survived 40 and 37 months,respectively,without tumor relapse.Two other dogs suffering from fibrosarcoma and hemangioendothelial sarcoma also showed complete remission and are still alive after 39 and 26 months,respectively.A.annua was well tolerated without noticeable side effects.These four cases indicate that A.annua may be a promising herbal drug for cancer therapy. | Elmar Breuer Thomas Efferth | 2014 | Natural Products and Bioprospecting2014,4,2: | 4 |
| 4 | Risk Assessment for Tuzla Naval Base Breakwater显示文摘In the applications of reliability-based risk assessment methodology in Turkey, only wave characteristics, tidal range, storm surge, wave set-up, and structural system parameters were included in the past. Tsunami risk was not considered as a major design parameter and thus, was not included in the computation. After a strong earthquake (M w= 7.4) occurred near the town of Golcuk, Izmit Bay, Western Turkey at 00:01 GMT on August 17, 1999, chaotic water movements were observed on the coast of the Marmara Sea. Rubble mound breakwaters demonstrated very little damage, as predicted by the reliability-based risk assessment model developed in this study for the Tuzla Naval Base. For determination of the probability of occurrence under design conditions, which is a function of storm waves, tidal range, storm surge, and tsunami height, the Monte Carlo simulation was interrelated with the β Ⅱ method. | 6 | Importance of intellectual property generated by biomedical research at universities and academic hospitals显示文摘Biomedical research has many different facets.Researchers and clinicians study disease biology and biochemistry to discover novel therapeutic targets,unravel biochemical pathways and identify biomarkers to improve diagnosis,or devise new approaches to clinically manage diseases more effectively.In all instances,the overall goal of biomedical research is to ensure that results thereof(such as a therapy,a device,or a method which may be broadly referred to as“inventions”)are clinically implemented.Most of the researchers’efforts are centered on the advance of technical and scientific aspects of an invention.The development and implementation of an invention can be arduous and very costly.Historically,it has proven to be crucial to protect intellectual property rights(IPR)to an invention(i.e.,a patent)to ensure that companies can obtain a fair return on their investment that is needed to develop an academic invention into a product for the benefit of patients.However,the importance of IPR is not generally acknowledged among researchers at academic institutions active in biomedical research.Therefore this paper aims to(1)raise IP awareness amongst clinical and translational researchers;(2)provide a concise overview of what the patenting trajectory entails;and(3)highlight the importance of patenting for research and the researcher.Importance for patients:Adequate patent protection of inventions generated through biomedical research at academic institutions increases the probability that patients will benefit from these inventions,and indirectly enables the financing of clinical studies,mainly by opening up funding opportunities(e.g.specific grants aimed at start-ups,pre-seed and seed capital)that otherwise would not be accessible.As a consequence,patented inventions are more likely to become clinically tested and reach the market,providing patients with more treatment options. | Joris J.Heus Elmar S.de Pauw Mirjam Leloux Margherita Morpurgo Michael R Hamblin Michal Heger | 2017 | Journal of Clinical & Translational Research2017,3,2: | 3 |

| 8 | Effect of Vachellia tortilis on understory vegetation,herbaceous biomass and soil nutrients along a grazing gradient in a semi-arid African savanna显示文摘The spatial pattern and abundance of herbaceous vegetation in semi-arid savannas are dictated by a complex and dynamic interaction between trees and grasses. Scattered trees alter the composition and spatial distribution of herbaceous vegetation under their canopies. Therefore, we studied the effect of Vachellia tortilis on herbaceous vegetation composition, biomass and basal area, and soil nutrients on sites with varying grazing intensities in the central rift valley of Ethiopia. Data were collected on species composition, cover and biomass of herbs and grasses, and soil moisture and nutrient contents under light,medium, and heavy grazing pressures, both under the inside and outside of V. tortilis canopies. Species richness was similar in both locations but decreased with increased grazing. Only the overall biomass and herb cover were significantly greater under the canopy than outside, and overall biomass showed significant unchanging decline with increased grazing. However, vegetation cover was significantly greater on moderately grazed sites compared to low and heavily grazed sites. All soil variables were significantly higher under V. tortilis canopies than outside.Our findings suggest that V. tortilis has more effect on composition and diversity of herbaceous vegetation than on species richness, and that V. tortilis promotes the herbaceous layer biomass by reducing soil moisture loss and increasing soil fertility under the inside than outside the canopies. Therefore, we suggest that management practices should be directed on reducing pressure on V. tortilis by regulating grazing. | 10 | Damage visualization and deformation measurement in glass laminates during projectile penetration显示文摘Transparent armor consists of glass-polymer laminates in most cases.The formation and propagation of damage in the different glass layers has a strong influence on the ballistic resistance of such laminates.In order to clarify(he course of events during projectile penetration,an experimental technique was developed,which allows visualizing the onset and propagation of damage in each single layer of the laminate.A telecentric objective lens was used together with a microsecond video camera that allows recording 100 frames at a maximum rate of 1 MHz in a backlit photography set-up.With this technique,the damage evolution could be visualized in glass laminates consisting of four glass layers with lateral dimensions 500 mm x 500 mm.Damage evolution was recorded during penetration of 7.62 mm AP projectiles with tungsten carbide core and a total mass of 1 1.1 g in the impact velocity range from 800 to 880 m/s.In order to measure the deformation of single glass plates within the laminates,a piece of reflecting tape was attached to the corresponding glass plate,and photonic Doppler velocimetry(PDV) was applied.With the photonic Doppler velocimeter.an infrared laser is used to illuminate an object to be measured and the Doppler-shifted light is superimposed to a reference light beam at the detector.The simultaneous visualization and PDV measurement of the glass deformation allow determining the deformation at the time of the onset of fracture.The analysis of the experimental data was supported by numerical simulations,using the AUTODYN commercial hydro-code. | Elmar STRASSBURGER Steffen BAUER Gregor POPKO | 2014 | Defence Technology(防务技术)2014,10,2: | 2 |

Medical decision making in clinical practice显示文摘AIM:To analyzes the decision whether patients with chronic hepatitis C virus (HCV) infection are treated or not.METHODS:This prospective cohort study included 7658 untreated patients and 6341 patients receiving pegylated interferon α2 a/ribavirin,involving 434 physicians/institutions throughout Germany (377 in private practice and 57 in hospital settings).A structured questionnaire had to be answered prior to the treatment decision,which included demographic data,information about the personal life situation of the patients,anamnesis and symptomatology of hepatitis C,virological data,laboratory data and data on concomitant diseases.A second part of the study analyzes patients treated with pegylated interferon α2a.All questionnaires included reasons against treatment mentioned by the physician.RESULTS:Overall treatment uptake was 45%.By multivariate analysis,genotype 1/4/5/6,HCV-RNA ≤ 520 000 IU/mL,normal alanine aminotransferase (ALT),platelets ≤ 142 500/μL,age > 56 years,female gender,infection length > 12.5 years,concomitant diseases,human immunodeficiency virus co-infection,liver biopsy not performed,care in private practice,asymptomatic disease,and unemployment were factors associated with reduced treatment rate.Treatment and sustained viral response rates in migrants (1/3 of cohort) were higher than in German natives although 1/3 of migrants had language problems.Treatment rate and liver biopsy were higher in clinical settings when compared to private practice and were low when ALT and HCV-RNA were low.CONCLUSION:Some reasons against treatment were medically based whereas others were related to fears,socio-economical problems,and information deficits both on the side of physicians and patients. | Claus Niederau Dietrich Hüppe Elmar Zehnter Bernd Mller Renate Heyne Stefan Christensen Rainer Pfaff Arno Theilmeier Ulrich Alshuth Stefan Mauss | 2012 | World Journal of Gastroenterology2012,18,12: | 2 |
