1家庭血压监测中国专家共识显示文摘自动的电子血压计正在进入家庭。家庭血压监测已成为血压管理的重要组成部分。近年来,美国、欧洲与日本相继制定了家庭血压监测指南。我国家庭血压监测的研究工作正处起步阶段,尚不能完全根据我国相关研究的结果制定指南。随着高血压患者家庭血压管理意识的迅速提高,许多家庭拥有了自动的电子血压计,同时也在进行家庭血压测量。为了促进家庭血压监测的健康发展,需要对家庭血压监测进行适当的规范。因此,参考国外有关指南并经广泛、深入的讨论,制定本共识文件。就血压计的选择与校准、血压测量的频率(次数)与时间(天数)以及家庭血压的正常值等重要问题提出了建议。Chinese Hypertension Committee of the Chinese Medical Doctors Association, Chinese Hypertension League, Chinese Society of Cardiology of the Chinese Medical Association 2012中华高血压杂志2012,20,6:83

3Inhibiting effect of antisense oligonucleotides phosphorthioate on gene expression of TIMP-1 in rat liver fibrosis显示文摘AIM To observe the inhibition of antisenseoligonucleotides (asON) phosphorthioate to thetissue inhibitors metalloproteinase-1 (TIMP-1)gene and protein expression in the liver tissue ofimmunologically induced hepatic fibrosis rats.The possibility of reversing hepatic fibrosisthrough gene therapy was observed.METHODS Human serum albumin (HSA) wasused to attack rats, as hepatic fibrosis model, inwhich asONs were used to block the gene andprotein expressing TIMP-1. According to theanalysis of modulator, structure protein, codingseries of TIMP-1 genome, we designed fourdifferent asONs. These asONs were injected intothe hepatic fibrosis models through coccygealvein. The results was observed by RT-PCR formeasuring TIMP-1 mRNA expression,immunohistochemistry and in situ hybridizationfor collagen Ⅰ, Ⅲ, special staining of collagenfiber, and electron microscopic examination.RESULTS Hepatic fibrosis could last within 363days in our modified model. The expressinglevel of TIMP-1 was high during hepatic fibrosisprocess. It has been proved by theimmunohistochemical and the electronmicroscopic examination that the asONphosphorthioate of TIMP-1 could exactly expressin vivo. The effect of colchicine wasdemonstrated to inhibit the expressing level ofmRNA and the content of collagen Ⅰ, Ⅲ in theliver of experimental hepatic fibrosis rats.However, the electron microscopy research andthe pathologic grading of hepatic fibrosisshowed that there was no significant differencebetween the treatment group and the modelgroup (P>0.05).CONCLUSION The experimental rat model ofhepatic fibrosis is one of the preferable modelsto estimate the curative effect of anti-hepaticfibrosis drugs. The asON phosphorthioate ofTIMP-1 could block the gene and proteinexpression of TIMP-1 in the liver of experimentalhepatic fibrosis rats at the mRNA level. It ispossible to reverse hepatic fibrosis, and it isexpected to study a new drug of anti-hepaticfibrosis on the genetic level. 5The New Form of Time-Dependent Mild Slope Equation for Random Waves显示文摘The mild-slope equation derived by Berkhoff (1972), has widely been used in the numerical calculation of refraction and diffraction of regular waves. However, it is well known that the random sea waves has a significant effect in the refraction and diffraction problems. In this paper, a new form of time-dependent mild slope equation for irregular waves was derived with Fade approximation and Kubo’s time series concept. The equation was simplified using WKB method, and simple and practical irregular mild slope equation was obtained. 6Analysis of the delayed approach to the management of infected pancreatic necrosis显示文摘AIM: To analyze outcomes of delayed single-stage necrosectomy after early conservative management of patients with infected pancreatic necrosis (IPN) associated with severe acute pancreatitis (SAP). METHODS: Between January 1998 and December 2009, data from patients with SAP who developed IPN and were managed by pancreatic necrosectomy were analyzed. RESULTS: Fifty-nine of 61 pancreatic necrosectomies were performed by open surgery and 2 laparoscopically. In 55 patients, single-stage necrosectomy could be performed (90.2%). Patients underwent surgery at a median of 29 d (range 13-46 d) after diagnosis of acute pancreatitis. Sepsis and multiple organ failure accounted for the 9.8% mortality rate. Pancreatic fistulae (50.8%) predominantly accounted for the morbidity. The median hospital stay was 23 d, and the median interval for return to regular activities was 110 d.CONCLUSION: This series supports the concept of delayed single-stage open pancreatic necrosectomy for IPN. 8NUMERICAL ANALYSIS OF THE MECHANICS OF SOIL REINFORCEMENT显示文摘Roots affect the soil stress and strain and contribute to soil reinforcement. An analysis of stress and strain in the root-soil composite is presented which combines roots, soils, and an interface element by using the Duncan-Chang E- model and a nonlinear-interface-element model. In this research, a finite element numerical analysis method was applied to simulate the stress field resulting in a composite of Pinus tabulaeformis roots and soils. Results show that roots can transmit the stress from surface soils to deep soils, and can reduce soil stress within the surface soil layer; results also show that the effect of soil reinforcement by roots is limited to surface soils. 12LOCAL SCOUR ESTIMATION AT BRIDGE SITE: MODIFICATION AND APPLICATION OF LACEY FORMULA显示文摘In the recent years, it has been established that b/h or h/b (b = length of the structure in the lateral direction and h = approach flow depth) is the dominant factor for the estimation of local scour depth around abutments and piers. One of the major limitations of the Lacey formula is that it predicts only one value of the scour depth for a specific river because of the absence of important parameters b and h. Therefore, its application is quite uncertain for variable b and h. Considering its limitations and experimental facts of flow concentration and shear stress amplification close to above structures, the original Lacey formula is modified introducing the parameters b and h. 13ANALYSIS ON BUCKLING AND POSTBUCKLING OF DELAMINATION IN 3D COMPOSITES显示文摘In this paper, the problem of axisymmetric buckling and postbuckling of a circular thin-film delamination bridged by through-thickness fiber tows in 3D composites is presented. An iterativeprocedure based on Taylor’s series expansion is used to generate a family of nondimensionalized post-buckling solutions of the above problem by yon Karman’s nonlinear plate theory. Attention is fo-cused, herein, on the effects of the bridge force of through-thickness fibers on the buckling and post-buckling behavior of the delamination. It is found that fiber bridge not only increases the ability of re-sisting delamination buckling and postbuckling, but also brings on the jump of the delamination deflec-tion mode during the postbuckling phase. 17DEVELOPMENT OF SOIL EROSION INDEX MODEL IN TAIWAN WATERSHEDS显示文摘With steep terrain and excessive rainfall, Taiwan is affected by severe soil erosion caused by summer typhoons and storms that bring intensive rainfall and rapid flows,. However, the actual erosion is much less than the value predicted by the USLE because the soil erosion types in Taiwan are different from those in America which has mild slope and dry weather. Developing a soil erosion index model applicable to Taiwan is the important goal of this research. Five factors, namely, soil texture, rainfall type, slope steepness, ground cover and land use, are included in this model. Soil index factor is measured by Km from the USLE model, and other index factors are calculated from local field data. The soil erosion index model (SEIM) is as follows: SE=6x10-7AIfor AI≤50 SE = 0.233AI1.8 for AI > 50 where AI is the total index value, and SE is the soil erosion quantity (ton/ha/yr).
