| 1 | 新生儿肺炎死亡病例中B族链球菌的检测显示文摘目的对北京儿童医院新生儿肺炎死亡病例进行B族链球菌(GBS)回顾性检测,旨在初步揭示GBS感染在新生儿重症肺炎中的地位。方法收集200例1953年到2004年间新生儿肺炎死亡病例(研究组)和34例非感染性疾病新生儿死亡病例(对照组)的尸检病理标本,提取石蜡组织DNA,通过PCR和Southernblot技术检测GBS特异性基因片断cfb基因。同时对所选病例的临床资料进行复习。结果(1)研究组200例中,PCR对GBS检出率为26%,Southernblot检出率为65%,检出率均明显高于对照组(PCR3%,χ2=8.82,P<0.01;Southernblot,18%,χ2=26.77,P<0.01)。(2)研究组中,年龄小于7d的病例(早发型)103例,年龄大于7d的病例(晚发型)97例,小于7d的患儿GBS检出率明显高于大于7d的患儿,其中PCR检出率在前者为37%,后者为13%(χ2=15.537,P<0.01),Southernblot检出率在前者为72%,后者为52%(χ2=4.37,P<0.05)。早发型阳性病例中,39%为早产儿(29/74)。(3)研究组中,75例有完整临床资料,其中35例可查到一项或多项与GBS感染有关的危险因素,如早产,低体重,胎膜早破,羊水异常等,GBS检测结果均为阳性。阳性病例中,早发型最常见的表现有青紫、窒息,呼吸困难,晚发型最常见的表现有咳喘,呼吸困难。对照组中,1例PCR检测阳性者患恶性畸胎瘤,其他5例仅Southernblot检测阳性者分别为核黄疸、肝癌、先天性无肛合并膀胱尿道瘘、新生儿自然出血、先天性低位无肛合并直肠会阴瘘患者。结论GBS是新生儿肺炎死亡病例中的重要病原,尤其在早发型肺炎病例中GBS感染占很大比重。Southernblot是在石蜡标本中检测GBS的较为敏感方法。 | 邓江红 姚开虎 胡惠丽 俞桑洁 高薇 伏利兵 何乐健 Alexander Dmitriev 杨永弘 | 2006 | 中华儿科杂志2006，44，11： | 53 |
| 2 | Chronic hepatitis B in pregnant women: Current trends and approaches显示文摘Chronic hepatitis B(CHB)is a significant public health problem worldwide.The aim of the present review is to summarize the actual trends in the management of CHB in pregnant women.The prevalence of hepatitis B virus(HBV)infection in pregnant women is usually comparable to that in the general population in the corresponding geographic area.All women have to be screened for hepatitis B surface antigen(HBsAg)during pregnancy.Additional examinations of pregnant women with CHB may include maternal hepatitis B e antigen,HBV viral load,alanine aminotransferase level,and HBsAg level.The management of pregnancy depends on the phase of the HBV infection,which has to be determined before pregnancy.In women of childbearing age with CHB,antiviral therapy can pursue two main goals:Treatment of active CHB,and vertical transmission prevention.During pregnancy,tenofovir is the drug of choice in both cases.A combination of hepatitis B immunoglobulin and vaccine against hepatitis B should be administered within the first 12 h to all infants born to mothers with CHB.In such cases,there are no contraindications to breastfeeding. | Maria Belopolskaya Viktor Avrutin Olga Kalinina Alexander Dmitriev Denis Gusev | 2021 | World Journal of Gastroenterology2021，27，23： | 15 |
| 3 | B族链球菌的红霉素耐药基因研究显示文摘目的 比较不同地区B族链球菌 (GBS)对红霉素耐药性的差异及耐药基因谱特点。方法 用琼脂稀释法 ,测定红霉素、克林霉素对不同地区 (北京 ,广州 ,俄罗斯圣彼得堡 )的 113株GBS的最低抑菌浓度 (MIC) ,同时用PCR和核酸杂交的方法 ,检测红霉素耐药基因mreA、mefA、ermA、ermB、ermC在该批菌株中的出现规律及其与红霉素耐药性的关系。结果 (1)北京、广州两地GBS菌株总耐药率为 4 6 % ,与克林霉素耐药性的一致率为 93.8% ;所有ermA基因均来自北京菌株 ,而广州菌株不含该基因。 (2 )含ermA和 (或 )ermB基因的GBS占所有菌株的 30 .0 9% ,其耐药率高达 97.0 6 % ,在已检测的GBS菌株中未发现ermC基因。 (3)分别有 5 3株和 18株含mreA和mefA的GBS以及 1株含ermA的菌株仍对红霉素敏感。 2株GBS不含上述任何一种基因 ,但仍对红霉素耐药。结论 (1)北京、广州两地GBS对红霉素普遍具有较高的耐药率 ,且与大环内酯 林可霉素 链阳性霉素B类抗生素之间存在较严重的交叉耐药 ,临床使用抗生素时应尽量避免重复选择此类抗生素 ;(2 )ermA和ermB是决定我国北京、广州GBS对红霉素耐药的重要基因 ;(3)GBS的耐药性还有其他分子生物学机制参与。 | 杨明 杨永弘 Dmitriev Alexander 申阿东 佟月娟 沈叙庄 樊寻梅 | 2002 | 中华儿科杂志2002，40，8： | 11 |
| 4 | 新生儿B组链球菌感染传播途径的研究显示文摘目的 确定新生儿B组链球菌 (GBS)感染的传播途径和随机引物扩增多态性 (RAPD)分型方法的应用价值。方法 采用血清学、核酸杂交和PARD方法对从 2例诊断为早发型GBS肺炎和晚发型GBS脑膜炎患儿体内分离的 5株GBS和其母阴道分离的 2株GBS菌株作母婴配对分析 ,对 88株孕妇生殖道携带GBS进行RAPD分型研究。结果 1例早发型GBS肺炎母婴配对GBS血清型均为III/R型致病 ,毒力因子基因片段大小 [α和β基因均为阴性 ,凝聚因子基因 (ScaA) 2kb ,谷氨酰胺基因(glnA) 6 .5kb ,ScpB 2 .6kb]和DNA指纹图完全一致 ;1例晚发型GBS脑膜炎母婴配对GBS菌血清型均为III/R型致病、毒力因子基因片段大小 (α和 β基因均为阴性 ,ScaA 2kb ,glnA 6 .5kb ,ScpB 2 .6kb)均相同 ,而RAPD的DNA指纹图不同 ,孕妇携带GBS菌株为RAPD2 7型 ,而患儿血和脑脊液分离的GBS菌株均为RAPD1型。RAPD分型方法的分型率分辨力高、稳定性强。结论 1例早发型GBS感染为母婴垂直传播 ,1例晚发型GBS感染可能与生后水平传播有关 ;RAPD分型方法在GBS感染流行病学研究中具有潜在使用价值。 | 申阿东 斯悦 钟雁 Dmitriev A 黄醒华 杨永弘 | 2000 | 中华儿科杂志2000，38，8： | 3 |
