3Heterotopic pancreas in the stomach:A case report and literature review显示文摘Ectopic pancreas is defined as pancreatic tissue found outside the usual anatomic location of the pancreas. It is often an incidental finding and can be found at different sites in the gastrointestinal tract. It may become clinically evident when complicated by pathologic changes such as inflammation, bleeding, obstruction, and malignant transformation. In this report, a 40 years old woman with epigastric pain due to ectopic pancreatic tissue in the stomach is described. The difficulty of making an ac- curate diagnosis is highlighted. The patient has remained free of symptoms since she underwent wedge resection of the lesion three years ago. Frozen sections may help in deciding the extent of resection intraoperatively. 6Multiplex RT-PCR-based detections of CEA, CK20 and EGFR in colorectal cancer patients显示文摘AIM: To develop a multiplex reverse transcription polymerase chain reaction (RT-PCR) method detecting cir-culating tumor cells in the peripheral blood of colorectal cancer (CRC) patients. METHODS: Peripheral blood samples were collected from 88 CRC patients and 40 healthy individuals from the blood donors' clinic and subsequently analyzed by multiplex RT-RCR for the expression of carcinoembryonic antigen (CEA), cytokeratin 20 (CK20) and epidermal growth factor receptor (EGFR) mRNA. The analysis involved determining the detection rates of CEA, CK20 and EGFR transcripts vs disease stage and overall survival. Median follow-up period was 19 mo (range 8-28 mo). RESULTS: Rates of CEA, CK20 and EGFR detection in CRC patients were 95.5%, 78.4% and 19.3%, respectively. CEA transcripts were detected in 3 healthy volunteer samples (7.5%), whereas all control samples were tested negative for CK20 and EGFR transcripts. The increasing number of positive detections for CEA, CK20 and EGFR transcripts in each blood sample was positively correlated with Astler-Coller disease stage (P< 0.001) and preoperative serum levels of CEA (P=0.029) in CRC patients. Data analysis using Kaplan-Meier estimator documented signif icant differences in the overall survival of the different CRC patient groups as formed according to the increasing number of positivity for CEA, CK20 and EGFR transcripts. 7Resolution of non-alcoholic steatohepatitis by rosuvastatin monotherapy in patients with metabolic syndrome显示文摘AIM: To investigate the effect of rosuvastatin monotherapy on non-alcoholic steatohepatitis(NASH). At present there is no effective treatment for non-alcoholic fatty liver disease or its advanced form NASH.METHODS: This prospective study included 20 biopsy proven patients with NASH, metabolic syndrome(Met S) and dyslipidaemia. Biochemical parameters of the blood of the patients and an ultrasonography of the liver were performed at baseline. Then patients receivedlifestyle advice and were treated for a 12 mo period with rosuvastatin(10 mg/d) monotherapy. Patients were re-evaluated during the study at 3 mo intervals, during which biochemical parameters of the blood were measured including liver enzymes. A repeat biopsy and ultrasonography of the liver were performed at the end of the study in all 20 patients. Changes in liver enzymes, fasting plasma glucose, serum creatinine, serum uric acid(SUA), high sensitivity C reactive protein(hs CRP) and lipid profile were assessed every 3 mo. The primary endpoint was the resolution of NASH and the secondary endpoints were the changes in liver enzyme and lipid values.RESULTS: The repeat liver biopsy and ultrasonography showed complete resolution of NASH in 19 patients, while the 20 th, which had no improvement but no deterioration either, developed arterial hypertension and substantial rise in triglyceride levels during the study, probably due to changes in lifestyle including alcohol abuse. Serum alanine transaminase, aspartate transaminase, and γ-glutamyl transpeptidase were normalised by the 3rd treatment month(ANOVA P < 0.001), while alkaline phosphatase activities by the 6th treatment month(ANOVA, P = 0.01). Fasting plasma glucose and glycated haemoglobin were significantly reduced(P < 0.001). Lipid values were normalised by the 3rd treatment month. No patient had Met S by the 9th treatment month. Body mass index and waist circumference remained unchanged during the study. Thus, changes in liver pathology and function should be attributed solely to rosuvastatin treatment. A limitation of the study is the absence of a control group.CONCLUSION: These findings suggest that rosuvastatin monotherapy could ameliorate biopsy proven NASH and resolve Met S within 12 mo. These effects and the reduction of fasting plasma glucose and SUA levels may reduce the risk of vascular and liver morbidity and mortality in NASH patients. 9Delayed gastric emptying is associated with pylorus-preserving but not classical Whipple pancreaticoduodenectomy:A review of the literature and critical reappraisal of the implicated pathomechanism显示文摘Pylorus-preserving pancreaticoduodenectomy (PPPD) is nowadays considered the treatment of choice for periampullary tumors, namely carcinoma of the head, neck, or uncinate process of the pancreas, the ampulla of Vater, distal common bile duct or carcinoma of the peri-Vaterian duodenum. Delayed gastric emptying (DGE) comprises one of the most troublesome complications of this procedure. A search of the literature using Pubmed/Medline was performed to identify clinical trials examining the incidence rate of DGE following standard Whipple pancreaticoduodenectomy (PD) vs PPPD. Additionally we performed a thorough in-depth analysis of the implicated pathomechanism underlying the occurrence of DGE after PPPD. In contrast to early studies, the majority of recently performed clinical trials demonstrated no significant association between the occurrence of DGE with either PD or PPPD. PD and PPPD procedures are equally effective operations regarding the postoperative occurrence of DGE. 14Active gastrointestinal bleeding:Use of hemostatic forceps beyond endoscopic submucosal dissection显示文摘To the best of our knowledge,this is the f irst report of the application of hemostatic forceps in active gastrointestinal(GI) bleeding that is not related to endoscopic submucosal dissection.An 86-year-old woman with chronic intake of low-dose aspirin had a Dieulafoy's lesion of the third duodenal portion.Bleeding control with epinephrine injection was unsuccessful.A 60-year-old man presented with a bleeding ulcer in the duodenal bulb.Ten days after combined endotherapy,he had recurrent bleeding from two minimal lesions in the same location.A 66-year-old woman under combined antithrombotic treatment was referred to us for chronic GI bleeding of unexplained origin.Endoscopy revealed active diverticular bleeding in the second duodenal portion.A 61-year-old woman underwent endoscopic mucosal resection of superficial gastric adenocarcinoma,which was complicated with immediate bleeding.In all cases,the blood was washed out using a water-jet-equipped,single-channel gastroscope with a large working channel.The bleeding points were pinched and retracted with hemostatic forceps.Monopolar electrocoagulation was performed using an electrosurgical current generator.Hemostasis was achieved.No complications occurred.In conclusion,hemostatic forceps may be an effective as well as safe alternative approach for active GI bleeding of various origins.Dimitri Coumaros Niki Tsesmeli 2010World Journal of Gastroenterology2010,16,16:13
15Low rectal cancer:Sphincter preserving techniques-selection of patients,techniques and outcomes显示文摘Low rectal cancer is traditionally treated by abdominoperineal resection. In recent years, several new techniques for the treatment of very low rectal cancer patients aiming to preserve the gastrointestinal continuity and to improve both the oncological as well as the functional outcomes, have been emerged. Literature suggest that when the intersphincteric resection is applied in T1-3 tumors located within 30-35 mm from the anal verge, is technically feasible, safe, with equal oncological outcomes compared to conventional surgery and acceptable quality of life. The Anterior Perineal Plan E for Ultra-low Anterior Resection technique, is not disrupting the sphincters, but carries a high complication rate, while the reports on the oncological and functional outcomes are limited. Transanal Endoscopic Micro Surgery(TEM) and Trans Anal Minimally Invasive Surgery(TAMIS) should represent the treatment of choice for T1 rectal tumors, with specific criteria according to the NCCN guidelines and favorable pathologic features. Alternatively to the standard conventional surgery, neoadjuvant chemo-radiotherapy followed by TEM or TAMIS seems promising for tumors of a local stage T1sm2-3 or T2. 16Brain stem cells as the cell of origin in glioma显示文摘Glioma incidence rates in the United States are near 20000 new cases per year, with a median survival time of 14.6 mo for high-grade gliomas due to limited therapeutic options. The origins of these tumors and their many subtypes remain a matter of investigation. Evidence from mouse models of glioma and human clinical data have provided clues about the cell types and initiating oncogenic mutations that drive gliomagenesis, a topic we review here. There has been mixed evidence as to whether or not the cells of origin are neural stem cells, progenitor cells or differentiated progeny. Many of the existing murine models target cell populations defined by lineage-specific promoters or employ lineagetracing methods to track the potential cells of origin. Our ability to target specific cell populations will likely increase concurrently with the knowledge gleaned from an understanding of neurogenesis in the adult brain. The cell of origin is one variable in tumorigenesis, as oncogenes or tumor suppressor genes may differentially transform the neuroglial cell types. 17Overview of coronary artery variants, aberrations and anomalies显示文摘Coronary artery anomalies and variants are relatively uncommon congenital disorders of the coronary artery anatomy and constitute the second most common cause of sudden cardiac death in young competitive athletes. The rapid advancement of imaging techniques, including computed tomography, magnetic resonance imaging, intravascular ultrasound and optical coherence tomography, have provided us with a wealth of new information on the subject. Anomalous origin of a coronary artery from the contralateral sinus is the anomaly most frequently associated with sudden cardiac death, in particular if the anomalous coronary artery has a course between the aorta and the pulmonary artery. However, other coronary anomalies, like anomalous origin of the left coronary artery from the pulmonary artery, atresia of the left main stem and coronary fistulae, have also been implicated in cases of sudden cardiac death. Patients are usually asymptomatic, and in most of the cases, coronary anomalies are discovered incidentally during coronary angiography or on autopsy following sudden cardiac death. However, in some cases, symptoms like angina, syncope, heart failure and myocardial infarction may occur. 18Feature-Based Aggregation and Deep Reinforcement Learning:A Survey and Some New Implementations显示文摘In this paper we discuss policy iteration methods for approximate solution of a finite-state discounted Markov decision problem, with a focus on feature-based aggregation methods and their connection with deep reinforcement learning schemes. We introduce features of the states of the original problem, and we formulate a smaller 'aggregate' Markov decision problem, whose states relate to the features. We discuss properties and possible implementations of this type of aggregation, including a new approach to approximate policy iteration. In this approach the policy improvement operation combines feature-based aggregation with feature construction using deep neural networks or other calculations. 19Quantifying land degradation in the Zoige Basin,NE Tibetan Plateau using satellite remote sensing data显示文摘Considerable efforts have been dedicated to desertification research in the arid and semi-arid drylands of central Asia. However,there are few quantitative studies in conjunction with proper qualitative evaluation concerning land degradation and aeolian activity in the alpine realm. In this study,spectral information from two Landsat-5 TM scenes(04.08.1994 and 28.07.2009,respectively) was combined with reference information obtained in the field to run supervised classifications of eight landscape types for both time steps. Subsequently,the temporal and spatial patterns of the alpine wetlands/grasslands evolutions in the Zoige Basin were quantified and assessed based on these two classification maps. The most conspicuous change is the sharp increase of ~627 km^2 degraded meadow. Concerning other land-covers,shallow wetland increases ~107 km^2 and aeolian sediments(mobile dunes and sand sheets) have an increase of ~30 km^2. Considering the deterioration,an obvious decrease of ~440 km^2 degraded wetland can be observed. Likewise,decrease of deep wetland(~78 km^2),humid meadow(~80 km^2) and undisturbed meadow(~88 km^2) were determined. These entire evolution matrixes undoubtedly hint a deteriorating tendency of the Zoige Basin ecosystem,which is characterized by significantly declined proportion of intact wetlands,meadow,rangeland and a considerable increase ofdegraded meadow and larger areas of mobile dunes. In particular,not only temporal alteration of the landcover categories,the spatial and topographical characteristics of the land degradation also deserves more attention. In the alpine rangelands,the higher terraces of the river channels along with their slopes are more liable to the degradation and desertification. This tendency has significantly impeded the nomadic and agriculture activities. The set of anthropozoogenic factors encompassing enclosures,overgrazing and trampling,rodent damaging and exceedingly ditching in the wetlands are assumed to be the main controlling mechanisms for the landscape degradation. A suite of strict protection policies is urgent and indispensable for self-regulation and restoration of the alpine meadow ecosystem. 20FAIR Data and Services in Biodiversity Science and Geoscience显示文摘We examine the intersection of the FAIR principles(Findable,Accessible,Interoperable and Reusable),the challenges and opportunities presented by the aggregation of widely distributed and heterogeneous data about biological and geological specimens,and the use of the Digital Object Architecture(DOA)data model and components as an approach to solving those challenges that offers adherence to the FAIR principles as an integral characteristic.This approach will be prototyped in the Distributed System of Scientific Collections(DiSSCo)project,the pan-European Research Infrastructure which aims to unify over 110 natural science collections across 21 countries.We take each of the FAIR principles,discuss them as requirements in the creation of a seamless virtual collection of bio/geo specimen data,and map those requirements to Digital Object components and facilities such as persistent identification,extended data typing,and the use of an additional level of abstraction to normalize existing heterogeneous data structures.The FAIR principles inform and motivate the work and the DO Architecture provides the technical vision to create the seamless virtual collection vitally needed to address scientific questions of societal importance.Larry Lannom Dimitris Koureas Alex R.Hardisty 2020Data Intelligence2020,2,1:12
