| 6 | Management and outcome of recurrent gallstone ileus:A systematic review显示文摘AIM: To help the surgeon in decision making whentreating a patient with recurrent gallstone ileus(RGSI). METHODS: A systematic review related to RGSI was performed using the databases CINAHL, EMBASE, MEDLINE via Pub Med from May 1912 to April 2015. All languages were included and the grey literature was also searched. The abstracts were explored for relevance to the topic and full texts obtained as appropriate. A manual search was carried out by scrutinising the reference lists of all the full text articles and further articles were identified and obtained. Total of 903 articles were identified, 656 were excluded after abstract review, 247 full text articles were reviewed and 91 articles selected for final analysis. There were 113 cases of RGSI. RESULTS: There were 113 cases of RGSI reported in 91 articles. The majority of the recurrences, 62.6%, occurred within 6 wk of the index event. The male to female ratio was 1:7. The mean age was 69.6 years(SD 11.2) with a range of 38-95 years. The small bowel was the commonest site of impaction(92.2%). Treatment data was available for 104 patients. The two main operations performed were:(1) Enterolithotomy without repair of biliary fistula in 70.1% of all patients with a procedural mortality rate of 16.4%(12/73) and(2) a single stage surgery approach involving enterolithotomy with cholecystectomy and repair of the biliary enteric fistula in 16.3% with a procedural mortality of 11.7%(2/17). A subset analysis over last 25 years showed mortality from eneterolithotomy was 4.8% while single stage mortality was 22.2%. Enterolithotomy alone was the commonest operation performed for RGSI with four patients(5.4%) having a further recurrence of gallstone ileus. | 8 | Hemodynamic monitoring in heart failure and pulmonary hypertension: From analog tracings to the digital age显示文摘Hemodynamic monitoring has long formed the cornerstone of heart failure(HF) and pulmonary hypertension diagnosis and management. We review the long history of invasive hemodynamic monitors initially using pulmonary artery(PA) pressure catheters in the hospital setting, to evaluating the utility of a number of implantable devices that can allow for ambulatory determination of intracardiac pressures. Although the use of indwelling PA catheters has fallen out of favor in a number of settings, implantable devices have afforded clinicians an opportunity for objective determination of a patient's volume status and pulmonary pressures. Some devices, such as the CardioM EMS and thoracic impedance monitors present as part of implantable cardiac defibrillators, are supported by a body of evidence which show the potential to reduce HF related morbidity and have received regulatory approval, whereas other devices have failed to show benefit and, in some cases, harm. | 9 | Involvement of 90-kuD ribosomal S6 kinase in collagen type Ⅰ expression in rat hepatic fibrosis显示文摘AIM: To investigate the relationship between 90-kuD ribosomal S6 kinase (p90RSK) and collagen type □expression during the development of hepatic fibrosis in vivo and in vitro.METHODS: Rat hepatic fibrosis was induced by intraperitoneal injection of dimethylnitrosamine. The protein expression and cell location of p90RSK and their relationship with collagen type □were determined by co-immunofluoresence and confocal microscopy. Subsequently, RNAi strategy was employed to silence p90RSK mRNA expression in HSC-T6, an activated hepatic stellate cell (HSC) line. The expression of collagen type □in HSC-T6 cells was assessed by Western blotting and real-time polymerase chain reaction. Furthermore, HSCs were transfected with expression vectors or RNAi constructs of p90RSK to increase or decrease the p90RSK expression, then collagen type □promoter activity in the transfected HSCs was examined by reporter assay. Lastly HSC-T6 cells transfected with p90RSK siRNA was treated with or without platelet-derived growth factor (PDGF)-BB at a final concentration of 20 μg/L and the cell growth was determined by MTS conversion.RESULTS: In fibrotic liver tissues, p90RSK was overexpressed in activated HSCs and had a significant positive correlation with collagen type □levels.In HSC-T6 cells transfected with RNAi targeted to p90RSK, the expression of collagen type □was down-regulated (61.8% in mRNA, P < 0.01, 89.1% in protein, P < 0.01). However, collagen type □promoter activity was not increased with over-expression of p90RSK and not decreased with low expression either, compared with controls in the same cell line (P = 0.076). | 10 | Establishment of Withania somnifera Hairy Root Cultures for the Production of Withanolide A显示文摘Withania sominifera (Indian ginseng) was transformed by Agrobacterium rhizogenes. Explants from seedling roots, stems, hypocotyls, cotyledonary nodal segments, cotyledons and young leaves were inoculated with A. rhizogenes strain R1601. Hairy (transformed) roots were induced from cotyledons and leaf explants. The transgenic status of hairy roots was confirmed by polymerase chain reaction using nptII and rolB specific primers and, subsequently, by Southern analysis for the presence of nptII and rolB genes in the genomes of transformed roots. Four clones of hairy roots were established; these differed in their morphology. The doubling time of faster growing cultures was 8-14 d with a fivefold increase in biomass after 28 d compared with cultured, non-transformed seedling roots. MS-based liquid medium was superior for the growth of transformed roots compared with other culture media evaluated (SH, LS and N6), with MS-based medium supplemented with 40 g/L sucrose being optimal for biomass production. Cultured hairy roots synthesized withanolide A, a steroidal lactone of medicinal and therapeutic value. The concentration of withanolide A in transformed roots (157.4 μg/g dry weight) was 2.7-fold more than in non-transformed cultured roots (57.9 μg/g dry weight). | Hosakatte N. | 11 | Organic mulching promotes soil organic carbon accumulation to deep soil layer in an urban plantation forest显示文摘Background:Soil organic carbon(SOC)is important for soil quality and fertility in forest ecosystems.Labile SOC fractions are sensitive to environmental changes,which reflect the impact of short-term internal and external management measures on the soil carbon pool.Organic mulching(OM)alters the soil environment and promotes plant growth.However,little is known about the responses of SOC fractions in rhizosphere or bulk soil to OM in urban forests and its correlation with carbon composition in plants.Methods:A one-year field experiment with four treatments(OM at 0,5,10,and 20 cm thicknesses)was conducted in a 15-year-old Ligustrum lucidum plantation.Changes in the SOC fractions in the rhizosphere and bulk soil;the carbon content in the plant fine roots,leaves,and organic mulch;and several soil physicochemical properties were measured.The relationships between SOC fractions and the measured variables were analysed.Results:The OM treatments had no significant effect on the SOC fractions,except for the dissolved organic carbon(DOC).OM promoted the movement of SOC to deeper soil because of the increased carbon content in fine roots of subsoil.There were significant correlations between DOC and microbial biomass carbon and SOC and easily oxidised organic carbon.The OM had a greater effect on organic carbon fractions in the bulk soil than in the rhizosphere.The thinnest(5 cm)mulching layers showed the most rapid carbon decomposition over time.The time after OM had the greatest effect on the SOC fractions,followed by soil layer.Conclusions:The frequent addition of small amounts of organic mulch increased SOC accumulation in the present study.OM is a potential management model to enhance soil organic matter storage for maintaining urban forest productivity. | Xiaodan Sun Gang Wang Qingxu Ma Jiahui Liao Dong Wang Qingwei Guan Davey L.Jones | 2021 | Forest Ecosystems2021,8,1: | 2 |
