| 5 | Efficacy and Economic Evaluation of Nonbiological Artificial Liver Therapy in Acute-on-chronic Hepatitis B Liver Failure显示文摘Background and Aims:Nonbiological artificial liver(NBAL)is frequently used as a first-line treatment for hepatitis B virus-associated acute-on-chronic liver failure(HBV-ACLF).This study aimed to compare the therapeutic efficacy and cost-effectiveness ratio(CER)of comprehensive medical treatment,plasma exchange(PE),and double plasma molecular adsorption system(DPMAS)plus half-dose PE(DPMAS+PE)in patients with HBV-ACLF.Methods:A total of 186 patients with HBV-ACLF randomly received comprehensive medical treatment,PE,or DPMAS+PE and were prospectively evaluated.Patients were divided into four subgroups based on the pretreatment prothrombin activity(PTA):Group I(PTA>40%),group II(PTA 30–40%),group III(PTA 20–30%),and group IV(PTA<20%).The main outcome measures were 28 day effectiveness;90 day liver transplantation-free survival;change of biochemical parameters;and CER.Results:DPMAS+PE treatment was associated with significantly higher 28 day effectiveness and 90 day liver transplantation-free survival compared with PE treatment in patients with group I liver failure.Clearance of serum total bilirubin(TBIL),AST,and creatinine(Cr)were significantly higher in the DPMAS+PE group than in the PE group.For subjects with group I liver failure,DPMAS+PE treatment had advantages of lower CER values and better cost-effectiveness.Conclusions:Compared with comprehensive medical treatment and PE alone,DPMAS with halfdose sequential PE treatment more effectively improved TBIL,AST,and Cr in HBV-ACLF patients,improved 28 day effectiveness and 90 day survival rates in patients with group I liver failure,and was more cost effective.DPMAS+PE is a viable NBAL approach for treatment of HBV-ACLF. | Cichun Wu Wenting Peng Da Cheng Huimin Gu Fei Liu Shifang Peng Lei Fu | 2023 | Journal of Clinical and Translational Hepatology2023,11,2: | 5 |

| 8 | Identifying disease feature genes based on cellular localized gene functional modules and regulation networks显示文摘Identifying disease-relevant genes and functional modules, based on gene expression pro- files and gene functional knowledge, is of high im- portance for studying disease mechanisms and sub- typing disease phenotypes. Using gene categories of biological process and cellular component in Gene Ontology, we propose an approach to selecting func- tional modules enriched with differentially expressed genes, and identifying the feature functional modules of high disease discriminating abilities. Using the differentially expressed genes in each feature module as the feature genes, we reveal the relevance of the modules to the studied diseases. Using three data- sets for prostate cancer, gastric cancer, and leukemia, we have demonstrated that the proposed modular approach is of high power in identifying functionally integrated feature gene subsets that are highly rele- vant to the disease mechanisms. Our analysis has also shown that the critical disease-relevant genes might be better recognized from the gene regulation network, which is constructed using the characterized functional modules, giving important clues to the concerted mechanisms of the modules responding to complex disease states. | 9 | Hepatic encephalopathy complicated with hyponatremia and acid-base disturbance and its prognosis显示文摘Objective:To improve the diagnosis and therapeutic effect of occurrence and development of hyponatremia and disorder of acid-base balance among patients with hepatic encephalopathy(HE) by elucidating the regularity and mechanism,as well as its influence on prognosis.Methods:327 HE patients admitted to our hospital from January 1990 to June 2010 were enrolled.Meanwhile 316 patients hospitalized in the medical department of the same hospital were chosen as the control group.Patients in both groups were given the same methods to measure arterial blood gas parameters(pH value,PaCO2,[HCO3-],TCO2,BE and SaO2),blood biochemistry([Na+],[K+],[Cl-]),liver function,kidney function and blood glucose,serum sodium,and thereupon tocalculate the anion gap(AG) and the potential [HCO3-],and acid-base balance disorder.Results:Among the 327 HE patients,hyponatremia was found in 188 cases(57.4%),of whom 132 patients died(70.2%).While among the 316 patients in control group,68 presented with hyponatremia(21.5%),and 19 died(27.9%).The incidence and mortality were significantly different between the two groups(P<0.001).All the 327 patients presented with different degrees of acid-base balance disorder and 178 died(54.4%),in whom 164(50.2%) belonged to simple acid-base balance disorder and 74(45.1%) died,136(41.6%) were dual acid-base balance disorder and 80(58.8%) died,27(8.2%) were triple acid-base disturbance and 24(88.9%) died.Whereas in the control group only 83 patients(26.2%) were recognized as simple and dual acid-base balance disorder,and 18(21.7%) died.There was higher incidence of acid-base balance disorder and mortality rate in HE group than control one(P<0.001).Conclusion:Hyponatremia is valuable to judge HE patients' prognosis.The key parameters in the judgment and evaluation on acid-base balance disorder among HE patients are the change of pH values and serum electrolyte values.When pH value ≤ 7.30 or > 7.55,it generally suggests a poor prognosis. | Ren Chengshan Wang Lei Zhao Xiaoyan Yang Shiming Zhang Yanqi Wang Xiwen Bo Jianying Zhang Pengbin Guo Hong Ling Xianlong Li Yihui Da Siping Xie Xia Ren Wei Li Chunhua Qian Guisheng | 2012 | Journal of Medical Colleges of PLA(China)2012,27,3: | 3 |

| 14 | Enhancing hydrothermal stability of framework Al in ZSM-5:From the view on the transformation between P and Al species by solid-state NMR spectroscopy显示文摘HZSM-5,with good surface acidity and shape selectivity,was reported as hydrocarbon cracking catalyst for multiple decades,however the hydrothermal stability,especially dealumination of tetrahedrally coordinated framework aluminum(TFAl),has been proved extensively as one of the major challenges during reactionregeneration process.Phosphorus was proposed to stabilize TFAl and indeed it enhanced the hydrothermal stability.Unfortunately,most of the phosphorus species would remain outside of the zeolite pore,mainly as polyphosphate species,and block the micropore severely,with only a limited portion introduced into the channel being able to interact with TFAl.Enlarging the pore size by alkali treatment(desilication)is one of the most convinced methods,but the details about specific P species during alkali treatment and its transformation upon hydrothermal activation is not acquired,thus the mechanism has not been fully understood.Herein,the P-containing species and its transformation during direct P modification and acid/alkali treatment followed by Pmodification have been studied,and the mechanism on the interaction between P and Al species has been investigated,using several analytical methods,especially Solid-state nuclear magnetic resonance(SSNMR)spectroscopy.It was found that the combination of desilication and subsequent phosphorus modification can enhance the activity of the ZSM-5 for the cracking of ethylcyclohexane,due to the better hydrothermal stabilization of acid sites by the enhanced interaction between phosphorus and TFAl,resulting from the improved accessibility of TFAl because of the successful generation of mesoporosity.Whereas the acid treatment followed by phosphorus modification,with declined retention of crystallinity and P/Al ratio,monoclinic/orthorhombic transition during steam activation,and the failed generation of mesopores,would cause obvious aggregation of the phosphorus species and could not improve the hydrothermal stability of the ZSM-5 effectively,and the direct phosphatation turned out much worse.Finally,a specific index that the intensity of the signal at 39 in 27Al MAS NMR spectra before steam activation was proposed as the indicator for determining the efficiency of phosphorus modification.And the proposed mechanism on the interaction between phosphorus and TFAl during the phosphorus modification could also be applicable in other zeolites. | Lei Han Ying Ouyang Enhui Xing Yibin Luo Zhijian Da | 2020 | Chinese Journal of Chemical Engineering2020,28,12: | 3 |

| 19 | Effect of six-stitch pancreaticojejunostomy on pancreatic fistula:A propensity score-matched comparative cohort study显示文摘Background:Clinically relevant postoperative pancreatic fistula(CR-POPF)is the most common and severe complication after pancreaticoduodenectomy(PD).Despite the development of numerous anastomotic surgical techniques to minimize CR-POPF,more than 30%of patients who undergo PD develop CR-POPF.Herein,we propose a novel pancreaticojejunostomy(PJ)technique and evaluate its efficacy and safety compared to traditional PJ.Methods:This retrospective study enrolled 164 consecutive patients who underwent PJ after PD between January 2012 and June 2017.Of them,78(47.6%)underwent traditional PJ and 86(52.4%)underwent sixstitch PJ.The primary outcome was CR-POPF at 1-month follow-up defined according to the revised 2016 International Study Group on Pancreatic Fistula definition.To adjust for baseline differences and selection bias,patients were matched by propensity scores,which left 63 patients with traditional PJ and 63 with six-stitch PJ.Results:Compared to patients who underwent traditional PJ(mean age 56.2±9.4 years),patients who underwent six-stitch PJ(mean age 57.4±11.4 years)had a lower CR-POPF rate.The risk of CR-POPF among patients who underwent six-stitch PJ was decreased by 81.7%after adjustment for age,sex,body mass index,and disease severity compared to patients who underwent traditional PJ.Additionally,the surgery time was reduced from 29 min for traditional PJ to 15 min for six-stitch PJ(P<0.001).Adverse effects such as abdominal fluid collection,abdominal bleeding,and wound infection were similar between two groups.Conclusion:Six-stitch PJ may be an effective and efficient PJ technique for patients who undergo PD surgery. | Zhao-Lin Zeng Yan Sun Da Xue Pi-Li Liu Wang-Ming Chen Lei Zhang | 2020 | Hepatobiliary & Pancreatic Diseases International2020,19,3: | 2 |
