13雷米普利、替米沙坦或二者联用对心血管病高危人群肾脏结局的影响(ONTARGET研究):一项多中心、随机、双盲、对照试验显示文摘背景血管紧张素受体阻断剂(ARB)和血管紧张素转换酶抑制剂(ACEI)都可以减少蛋白尿,二者联用可能比单一用药更有效。目前还没有长期临床试验比较两者对肾功能的影响。本研究比较了雷米普利(一种ACEI)、替米沙坦(一种ARB)或二者联用对患有动脉粥样硬化性血管病或糖尿病合并终末器官损害的人群(年龄≥55岁)肾脏结局的影响。方法临床试验从2001年开始,到2007年结束。经过3周的导入期,25620例受试者被随机分配至10mg/d雷米普利组(n=8576)、80mg/d替米沙坦组(n=8542)或10mg/d雷米普利+80mg/d替米沙坦组(n=8502;中位随访期为56个月),同时检测肾功能和尿蛋白。主要肾脏结局为透析、血清肌酐加倍和死亡的复合终点。采用意向性治疗分析。本研究已在ClinicalTrials.gov注册,编号为NCT00153101。结果试验期间,784例患者因低血压症状提前终止随机治疗(联合用药组406例,雷米普利组149例,替米沙坦组229例)。替米沙坦组和雷米普利组发生复合终点事件的例数相近[替米沙坦组1147例(13.4%),雷米普利组1150例(13.5%);HR1.00(95%C10.92~1.09)],但在联合用药组有所增加[1233例(14.5%);HR1.09(95%CI1.01-1.18),P=0.037]。次要肾脏结局(透析或血清肌酐加倍)的发生例数在替米沙坦组(189例,2.21%)和雷米普利组(174例,2.03%)相近(HR1.09,95%C10.89~1.34),但在联合用药组显著增加(212例,2.49%:HR1.24,95%CI1.01~1.51.P=0.038)。肾小球滤过率估计值在雷米普利组下降最少,为-2.82ml/(min·1.73m^2)(s=17.2),而在替米沙坦组为-4.12ml/(min·1.73m^2)(S=17.4,P〈0.0001),在联合治疗组为-6.11ml/(min·1.73m^2)(S=17.9,P〈0.0001)。与雷米普利组相比,替米沙坦组(P=0.004)和联合用药组(P=0.001)较少发生尿蛋白排泄增加。结论在心血管病高危人群中,替米沙坦和雷米普利对肾脏结局的影响相同。尽管联合用药比单一治疗能更大程度减少蛋白尿,但总体上其增加了肾脏结局事件。基金 勃林格殷格翰公司。Johannes F E Mann, Roland E Schmieder Matthew McQueen Leanne Dyal Helmut Schumacher Janice Pogue Xingyu Wang Aldo Maggioni Andrzej Budaj Suphachai Chaithiraphan Kenneth Dickstein Matyas Keltai Kaj Metsarinne Ali Oto Alexander Parkhomenko Leopoldo S Piegas Tage L Svendsen Koon KTeo Salim Yusuf 刘欣(译) 王兴宇(审) 2009世界临床医学2009,,4:1
