A handy review of carpal tunnel syndrome:From anatomy to diagnosis and treatment显示文摘Carpal tunnel syndrome(CTS) is the most commonly diagnosed disabling condition of the upper extremities. It is the most commonly known and prevalent type of peripheral entrapment neuropathy that accounts for about 90% of all entrapment neuropathies. This review aims to provide an outline of CTS by considering anat-omy, pathophysiology, clinical manifestation, diagnostic modalities and management of this common condition, with an emphasis on the diagnostic imaging evaluation.Mohammad Ghasemi-rad Emad Nosair Andrea Vegh Afshin Mohammadi Adam Akkad Emal Lesha Mohammad Hossein Mohammadi Doaa Sayed Ali Davarian Tooraj Maleki-Miyoab Anwarul Hasan 2014World Journal of Radiology2014,6,6:24
Validation of a serum microRNA panel as biomarkers for early diagnosis of hepatocellular carcinoma post-hepatitis C infection in Egyptian patients显示文摘AIM To investigate the prospective importance of serum micro(mi)RNAs(mi R-125 b, mi R-138 b, mi R-1269, mi R-214-5p, mi R-494, mi R375 and mi R-145) as early biomarkers for the diagnosis of hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC).METHODS Two-hundred and fifty HCV4 a patients, 224 HCV4 aHCC patients, and 84 healthy controls were enrolled in the study. Expression levels of mi R214-5p, mi R-125 b, mi R-1269 and mi R-375 were quantified using quantitative real-time PCR.RESULTS Expression of the selected mi RNAs in serum wassignificantly lower in HCC patients than in the healthy controls, except for mi R-1269 and mi R-494. There was a significant difference between HCC and HCV patients, in particular for HCC and late stage fibrosis, rather than HCV patients and early fibrosis. It is obvious that mi R-1269 was significantly upregulated in HCC cases compared to hepatic fibrosis cases. Each mi RNA can show HCC progression. Multivariate logistic regression analysis indicated that the tested panel of mi RNAs(mi R214-5p, mi R-125 b, mi R-1269 and mi R-375) represent accurate and specific indictors of HCC development.CONCLUSION This study presents a panel of mi RNAs with strong power as putative diagnostic and prognostic biomarkers for HCV-induced HCC. Moreover, mi R-214-5p and mi R-1269 could be considered as early biomarkers for tracking the progress of liver fibrosis to HCC.Moustafa Nouh Elemeery Ahmed Noah Badr Marwa Anwar Mohamed Doaa Ahmed Ghareeb 2017World Journal of Gastroenterology2017,23,21:20

Diffusion-weighted magnetic resonance imaging and micro-RNA in the diagnosis of hepatic fibrosis in chronic hepatitis C virus显示文摘BACKGROUND Diffusion-weighted magnetic resonance imaging has shown promise in the detection and quantification of hepatic fibrosis. In addition, the liver has numerous endogenous micro-RNAs(miRs) that play important roles in the regulation of biological processes such as cell proliferation and hepatic fibrosis.AIM To assess diffusion-weighted magnetic resonance imaging and miRs in diagnosing and staging hepatic fibrosis in patients with chronic hepatitis C.METHODS This prospective study included 208 patients and 82 age-and sex-matched controls who underwent diffusion-weighted magnetic resonance imaging of the abdomen, miR profiling, and liver biopsy. Pathological scoring was classified according to the METAVIR scoring system. The apparent diffusion coefficient (ADC) and miR were calculated and correlated with pathological scoring.RESULTS The ADC value decreased significantly with the progression of fibrosis, from controls(F0) to patients with early fibrosis(F1 and F2) to those with late fibrosis(F3 and F4)(median 1.92, 1.53, and 1.25 × 10^(-3) mm^2/s, respectively)(P = 0.001).The cut-off ADC value used to differentiate patients from controls was 1.83 × 10^(-3) mm^2/s with an area under the curve(AUC) of 0.992. Combining ADC and miR-200 b revealed the highest AUC(0.995) for differentiating patients from controls with an accuracy of 96.9%. The cut-off ADC used to differentiate early fibrosis from late fibrosis was 1.54 × 10^(-3) mm^2/s with an AUC of 0.866. The combination of ADC and miR-200 b revealed the best AUC(0.925) for differentiating early fibrosis from late fibrosis with an accuracy of 80.2%. The ADC correlated with miR-200 b(r =-0.61, P = 0.001), miR-21(r =-0.62, P = 0.001), and miR-29(r = 0.52,P = 0.001).CONCLUSION Combining ADC and miRs offers an alternative surrogate non-invasive diagnostic tool for diagnosing and staging hepatic fibrosis in patients with chronic hepatitis C.Tarek Besheer Hatem Elalfy Mohamed Abd El-Maksoud Ahmed Abd El-Razek Saher Taman Khaled Zalata Wagdy Elkashef Hossam Zaghloul Heba Elshahawy Doaa Raafat Wafaa Elemshaty Eman Elsayed Abdel-Hady El-Gilany Mahmoud El-Bendary 2019World Journal of Gastroenterology2019,25,11:9
Changing dietary n-6:n-3 ratio using different oil sources affects performance, behavior, cytokines mRNA expression and meat fatty acid profile of broiler chickens显示文摘Typical formulated broiler diets are deficient in n-3 poly-unsaturated fatty acids(PUFA) due to widening n-6:n-3 PUFA ratio which could greatly affect performance, immune system of birds and, more importantly, meat quality. This study was conducted to evaluate the effect of modifying dietary n-6:n-3 PUFA ratio from plant and animal oil sources on performance, behavior, cytokine m RNA expression, antioxidative status and meat fatty acid profile of broiler chickens.Birds(n=420) were fed 7 diets enriched with different dietary oil sources and ratios as follows: sunflower oil in control diet(C); fish oil(FO); 1:1 ratio of sunflower oil to FO(C1 FO1); 3:1 ratio of sunflower oil to fish oil(C3 FO1); linseed oil(LO); 1:1 ratio of sunflower oil to linseed oil(C1 LO1); 3:1 ratio of sunflower oil to linseed oil(C3 LO1),resulting in dietary n-6:n-3 ratios of approximately 40:1,1.5:1,4:1,8:1,1:1,2.5:1 and 5:1,respectively.The best final body weight, feed conversion ratio as well as protein efficiency ratio of broilers were recorded in the C1 FO1 and C1 LO1 groups.Compared with the control group, the dressing percentage and breast and thigh yield were highest in the C1 FO1 and C1 LO1 groups.Narrowing the dietary n-6:n-3 ratio increased(P < 0.05) n-3 PUFA content of breast meat. Moreover, the breast meat contents of eicosapentaenoic acid and docosahexaenoic acid increased(P < 0.05) with increasing dietary FO whereas a-linolenic acid content was higher with LO supplementation. Also, enriching the diets with n-3 PUFA from FO and LO clearly decreased(P < 0.05) serum total cholesterol, triglycerides and very low-density lipoproteins and enhanced antioxidative status. The feeding frequency was decreased(P < 0.05) in the C1 FO1 and C1 LO1 groups.Likewise,n-3 PUFA-enriched diets enhanced the frequency of preening, wing flapping and flightiness.Animal oil source addition, compared to plant oil,to broiler diets enhanced the relative m RNA expression of interferon gamma,interleukin-1 beta,interleukin-2 and interleukin-6 genes, especially at low n-6:n-3 ratios. This study has clearly shown that narrowing n-6:n-3 ratio through the addition of FO or LO improved performance and immune response of broilers and resulted in healthy chicken meat, enriched with long chain n-3 PUFA.Doaa Ibrahim Rania El-Sayed Safaa I.Khater Enas N.Said Shefaa A.M.El-Mandrawy 2018Animal Nutrition2018,,1:5
Phytochemical studies and anti-ulcerative colitis effect of Moringa oleifera seeds and Egyptian propolis methanol extracts in a rat model显示文摘Objective: To analyze the phytochemical constituents, and to explore potential protective effect of the methanol extract of Moringa oleifera(M. oleifera) seeds and Egyptian propolis, each alone or concurrently administered on acetic acid-induced ulcerative colitis in rats. Methods: Eight groups of 5 rats each were used: normal control group with distilled water, model group, two groups with M. oleifera seeds(100 and 200 mg/kg), two groups with propolis(50 and 100 mg/kg), one group with concurrent administration of both, and one group with prednisolone(reference drug). Macro-and microscopic picture, ulcer index and lesion scores, oxidative markers, inflammatory mediators, in vitro activity of the inflammatory enzymes and 1, 1-diphenyl-2-picrylhydrazyl free radicals scavenging activity were evaluated. The phytochemical constituents of both extracts were explored by GC-MS analysis. Results: Both treatments modulated the macro-and microscopic picture, decreased the ulcerative index, lesion score, oxidative markers and inflammatory mediators, and inhibited the COX-1 and COX-2 enzymes. Propolis appeared to be powerful free radicals scavenger. A powerful synergistic effect of both treatments in modulating the course of the disease was reported. GCMS analysis of methanol extract of M. oleifera seeds and propolis revealed the presence of 50 and 34 compounds, respectively. Conclusions: M. oleifera seeds and propolis methanol extracts have modulated the course of acetic acid-induced ulcerative colitis. Moreover, both treatments induce a good synergistic effect against the disease. 7Preparation of PVC/PVP composite polymer membranes via phase inversion process for water treatment purposes显示文摘In this work, new composite membranes were successfully prepared via phase inversion technique using polyvinyl chloride(PVC) and polyvinylpyrrolidone(PVP) as polymers and tetrahydrofuran(THF) and N-methyl-2-pyrrolidone(NMP) as solvents. The prepared membranes have been characterized by scanning electron microscope(SEM), and fourier transforms infrared spectroscopy(FTIR). The scanning electron microscope results prove that the prepared membranes are smooth and their pores are distributed throughout the whole surface and bulk body of the membrane without any visible cracks. The stress–strain mechanical test showed an excellent mechanical behavior enhanced by the presence of PVP in the prepared membranes. The membranes performance results showed that the salt rejection reached 98% with a high flux. This, in turn, makes the prepared membranes can be applied for sea and brackish water treatment through membrane distillation technology.Ahmed Bhran Abeer Shoaib Doaa Elsadeq Ayman El-gendi Heba Abdallah 2018Chinese Journal of Chemical Engineering2018,26,4:4
MicroRNA signature in patients with hepatocellular carcinoma associated with type 2 diabetes显示文摘BACKGROUND Nonalcoholic steatohepatitis-related cirrhosis is one of the liver complications in type 2 diabetes mellitus(T2DM)and reported to be a risk factor for developing hepatocellular carcinoma(HCC).A reliable screening biomarker of liver cirrhosis(LC)and HCC among T2DM patients is important to reduce the morbidity and mortality of this disease.MicroRNA(miRNA)is considered a key player in HCC and T2DM,and it might be a hidden culprit in diabetes-associated HCC,making it a promising reliable prognostic tool.AIM To investigate the signature of serum miRNAs as early biomarkers for the screening of HCC among diabetic patients.METHODS Expression profiles of miRNAs in serum samples of diabetic LC and diabetic HCC patients were assessed using Illumina sequencing;then,RT-qPCR was used to validate significantly altered miRNAs between the two groups.Candidate miRNAs were tested in serum samples of 200 T2DM patients,270 LC patients,200 HCC patients, and 225 healthy control subjects.Additionally, receiver operating characteristic(ROC)analysis, with area under the curve(AUC),was performed to assess the diagnostic performance of the screened miRNAs for discriminating HCC from LC and nonmalignant patients(LC+T2DM).RESULTS Expression of the sequenced miRNAs in serum was different in HCC vs LCpositive T2DM patients. Two miRNAs(miR-34a,miR-221)were significantly upregulated and five miRNAs(miR-16,miR-23-3p,miR-122-5p,miR-198,miR-199a-3p)were significantly down-regulated in HCC compared to LC patients.Analysis of ROC curve demonstrated that the combination of these seven miRNAs can be used as a reliable biomarker for detection of HCC in diabetic patients,as it could identify HCC with high diagnostic accuracy in diabetic LC patients(AUC=0.993)and in diabetic nonmalignant patients(AUC=0.961).CONCLUSION This study validates a panel of serum miRNAs that can be used as a reliable noninvasive screening biomarker of HCC among T2DM cirrhotic and noncirrhotic patients. The study recommends further research to shed light on a possible role of c-Met in T2DM-associated HCC via the miRNA regulatory pathway.Moustafa Nouh Elemeery Marwa Anwar Mohamed Marwa Ahmed Madkour Mohammed Mohammed Shamseya Noha Mahmoud Issa Ahmed Noah Badr Doaa Ahmed Ghareeb Cheol-Ho Pan 2019World Journal of Gastroenterology2019,25,42:3

Cardiac changes in infants of diabetic mothers显示文摘Diabetes mellitus(DM)is a systemic chronic metabolic disorder characterized by increased insulin resistance and/orβ-cell defects. It affects all ages from the foetal life, neonates, childhood to late adulthood.Gestational diabetes is a critical risk factor for congenital heart diseases(CHDs).Moreover, the risk increases with low maternal education, high body mass index at conception, undiagnosed pregestational diabetes, inadequate antenatal care, improper diabetes control, and maternal smoking during pregnancy.Maternal DM significantly affects the foetal heart and foetal-placental circulation in both structure and function.Cardiac defects, myocardial hypertrophy are three times more prevalent in infants of diabetic mothers(IDMs).Antenatal evaluation of the cardiac function and structures can be performed with foetal electrocardiography and echocardiography.Postnatal cardiac evaluation can be performed with natal and postnatal electrocardiography and echocardiography, detection of early atherosclerotic changes by measuring aortic intima-media thickness, and retinal vascular changes by retinal photography.Ameliorating the effects of diabetes during pregnancy on the offspring depends mainly on pregestational and gestational diabetes prevention. However, other measures to reduce the risk, such as using medications, nutritional supplements,or probiotics, still need more research. This review discusses the mechanism of foetal sequels and the risk factors that increase the prevalence of CHDs in gestational DM,the various cardiac outcomes of gestational DM on the foetus and offspring, cardiac evaluation of foetuses and IDMs,and how to alleviate the consequences of gestational DM on the offspring.Mohammed Al-Biltagi Osama El razaky Doaa El Amrousy 2021World Journal of Diabetes2021,12,8:2
Towards hepatitis C virus elimination:Egyptian experience, achievements and limitations显示文摘Worldwide, more than one million people die each year from hepatitis C virus(HCV) related diseases, and over 300 million people are chronically infected with hepatitis B or C. Egypt used to be on the top of the countries with heavy HCV burden. Some countries are making advances in elimination of HCV, yet multiple factors preventing progress; remain for the majority. These factors include lack of global funding sources for treatment, late diagnosis, poor data, and inadequate screening. Treatment of HCV in Egypt has become one of the top national priorities since 2007. Egypt started a national treatment program intending to provide cure for Egyptian HCV-infected patients. Mass HCV treatment program had started using Pegylated interferon and ribavirin between 2007 and 2014. Yet, with the development of highly-effective direct acting antivirals(DAAs) for HCV, elimination of viral hepatitis has become a real possibility. The Egyptian National Committee for the Control of Viral Hepatitis did its best to provide Egyptian HCV patients with DAAs. Egypt adopted a strategy that represents a model of care that could help other countries with high HCV prevalence rate in their battle against HCV. This review covers the effects of HCV management in Egyptian real life settings and the outcome of different treatment protocols. Also, it deals with the current and future strategies for HCV prevention and screening as well as the challenges facing HCV elimination and the prospect of future eradication of HCV.Dalia Omran Mohamed Alboraie Rania A Zayed Mohamed-Naguib Wifi Mervat Naguib Mohamed Eltabbakh Mohamed Abdellah Ahmed Fouad Sherief Sahar Maklad Heba Hamdy Eldemellawy Omar Khalid Saad Doaa Mohamed Khamiss Mohamed El Kassas 2018World Journal of Gastroenterology2018,24,38:2

Phytochemical, antioxidant and hepatoprotective effects of different fractions of Moringa oleifera leaves methanol extract against liver injury in animal model显示文摘Objective: To evaluate the potential antioxidant and hepatoprotective effects of n-hexane, dichloromethane(DCM), ethyl acetate(EtOAc), n-butanol and aqueous fractions of Moringa oleifera(M. oleifera) leaves methanol extract against carbon tetrachloride(CCl_4)-induced liver injury in rats. Methods: These fractions were prepared from the M. oleifera leaves methanol extract by solubilization in water and partitioning in n-hexane, EtOAc, DCM and n-butanol. Their phyto-components were identified by GC-MS analysis. The in vitro antioxidant effect of these fractions was carried out by assessment of 1,1-diphenyl-2-picrylhydrazyl scavenging activity. A total of 40 Sprague Dawley rats were allocated into 8 equal groups: group 1 given olive oil(1 m L/kg b.wt.), group 2 injected with CCl_4, group 3 to 7 administered with n-hexane, DCM, EtOAc, n-butanol and aqueous fractions, respectively after CCl_4, group 8 administered with silymarin after CCl_4. The activities of aspartate aminotransferase, alanine aminotransferase, and the levels of total cholesterol, triglycerides, glucose, total proteins and albumin in serum were determined spectrophotometrically. Glutathione reduced, lipid peroxide by-products levels, glutathione-s-transferase and catalase enzyme activities in the liver homogenate were determined by spectrophotometer. Liver specimens were also examined for histopathological alterations under light microscope. Results: The GCMS analysis of different fractions of the M. oleifera leaves methanol extract revealed that n-hexane, DCM, EtOAc, n-butanol, and aqueous fractions contained 17, 22, 23, 19 and 32 compounds, respectively. The percent and the molecular structure of each component in each fraction were identified. The n-butanol and EtOAc fractions exhibited the strongest in vitro antioxidant activity against 1,1-diphenyl-2-picrylhydrazyl. CCl_4 significantly decreased glutathione reduced and total proteins concentration and glutathione-s-transferase and catalase activities but increased lipid peroxide by-products and total cholesterol levels. The n-hexane followed by aqueous and DCM fractions were the most potent to regulate serum enzyme activities and lipid peroxide by-products levels in the liver homogenate. Conclusions: n-hexane, DCM, and aqueous fractions have the highest effectiveness against CCl_4-induced hepatotoxicity. Isolation and purification of the active constituents require further experiments.Attia H.Atta Soad M.Nasr Abduljalil H.Almaweri Doaa Sedky Amany M.Mohamed Hassan M.Desouky Mostafa A.Shalaby 2018Asian Pacific Journal of Tropical Medicine2018,11,7:1
