| 1 | 术后活动性疼痛护理评估对疼痛管理质量的影响显示文摘目的探讨术后活动性疼痛护理评估对疼痛管理质量的影响。方法将140例胸腹部术后1d的患者分为实验组和对照组。实验组在接受病区护士的术后疼痛护理措施的基础上,由疼痛专科护士采用功能活动评分法(Functional Activity Score,FAS)和数字评定量表(Numerical Rating Scale,NRS)评估活动性疼痛,并根据评估结果实施干预措施。对照组仅接受病区护士实施的术后疼痛护理措施。结果实验组和对照组术后24h内镇痛治疗方案调整及功能活动前按压镇痛泵比例的差异具有统计学意义(χ2=7.937,χ2=37.303,均P<0.05)。实验组在术后24h时间点的FAS评级结果、患者自评静息性和活动性疼痛强度,以及术后24h内疼痛最剧烈程度、中重度疼痛发生频度及有效咳嗽受疼痛影响程度均低于对照组(P<0.05),首次下床活动时间也早于对照组(P<0.05)。结论开展活动性疼痛护理评估实践有助于提高术后疼痛管理质量。 | 成燕 童莺歌 刘敏君 田素明 Alex H.Konstantatos Dana Boyd | 2015 | 中华护理杂志2015,50,8: | 161 |

| 3 | The International Agency for Research on Cancer(IARC) evaluation of the carcinogenicity of outdoor air pollution:focus on China显示文摘The International Agency for Research on Cancer(IARC)has classified outdoor air pollution and the particulate matter(PM)in outdoor air pollution as carcinogenic to humans,as based on sufficient evidence of carcinogenicity in humans and experimental animals and strong support by mechanistic studies.The data with important contributions to the evaluation are reviewed,highlighting the data with particular relevance to China,and implications of the evaluation with respect to China are discussed.The air pollution levels in Chinese cities are among the highest observed in the world today and frequently exceed healthbased national and international guidelines.Data from high-quality epidemiologic studies in Asia,Europe,and North America consistently show positive associations between lung cancer and PM exposure and other indicators of air pollution,which persist after adjustment for important lung cancer risk factors,such as tobacco smoking.Epidemiologic data from China are limited but nevertheless indicate an increased risk of lung cancer associated with several air pollutants.Excess cancer risk is also observed in experimental animals exposed to polluted outdoor air or extracted PM.The exposure of several species to outdoor air pollution is associated with markers of genetic damage that have been linked to increased cancer risk in humans.Numerous studies from China,especially genetic biomarker studies in exposed populations,support that the polluted air in China is genotoxic and carcinogenic to humans.The evaluation by IARC indicates both the need for further research into the cancer risks associated with exposure to air pollution in China and the urgent need to act to reduce exposure to the population. | Dana Loomis Wei Huang Guosheng Che | 2014 | Chinese Journal of Cancer2014,33,4: | 37 |

| 6 | Exercise-induced irisin in bone and systemic irisin administration reveal new regulatory mechanisms of bone metabolism显示文摘Irisin is a polypeptide hormone derived from the proteolytic cleavage of fibronectin-type III domaincontaining 5(FNDC5) protein. Once released to circulation upon exercise or cold exposure, irisin stimulates browning of white adipose tissue(WAT) and uncoupling protein 1(UCP1) expression, leading to an increase in total body energy expenditure by augmented UCP1-mediated thermogenesis. It is currently unknown whether irisin is secreted by bone upon exercise or whether it regulates bone metabolism in vivo. In this study, we found that 2 weeks of voluntary wheel-running exercise induced high levels of FNDC5 messenger RNA as well as FNDC5/irisin protein expression in murine bone tissues. Increased immunoreactivity due to exercise-induced FNDC5/irisin expression was detected in different regions of exercised femoral bones,including growth plate, trabecular bone, cortical bone, articular cartilage, and bone–tendon interface. Exercise also increased expression of osteogenic markers in bone and that of UCP1 in WAT, and led to bodyweight loss. Irisin intraperitoneal(IP) administration resulted in increased trabecular and cortical bone thickness and osteoblasts numbers, and concurrently induced UCP1 expression in subcutaneous WAT. Lentiviral FNDC5 IP administration increased cortical bone thickness. In vitro studies in bone cells revealed irisin increases osteoblastogenesis and mineralization, and inhibits receptor activator of nuclear factor-k B ligand(RANKL)-induced osteoclastogenesis. | 9 | Endoscopy and polyps-diagnostic and therapeutic advances in management显示文摘Despite multiple efforts aimed at early detection through screening, colon cancer remains the third leading cause of cancer-related deaths in the United States, with an estimated 51000 deaths during 2013 alone. The goal remains to identify and remove benign neoplastic polyps prior to becoming invasive cancers. Polypoid lesions of the colon vary widely from hyperplastic, hamartomatous and inflammatory to neoplastic adenomatous growths. Although these lesions are all benign, they are common, with up to one-quarter of patients over 60 years old will develop pre-malignant adenomatous polyps. Colonoscopy is the most effective screening tool to detect polyps and colon cancer, although several studies have demonstrated missed polyp rates from 6%-29%, largely due to variations in polyp size. This number can be as high as 40%, even with advanced (> 1 cm) adenomas. Other factors including sub-optimal bowel preparation, experience of the endoscopist, and patient anatomical variations all affect the detection rate. Additional challenges in decision-making exist when dealing with more advanced, and typically larger, polyps that have traditionally required formal resection. However,many immune-related adverse events have also been described which mainly occurs as the immune system becomes less suppressed, affecting various organs including the gastrointestinal tract and causing diarrhea and colitis. The incidence of immune-mediated colitis(IMC) ranges from 1%-25% depending on the type of ICI and if used in combination. Endoscopically and histologically there is a significant overlap between IMC and inflammatory bowel disease,however more neutrophilic inflammation without chronic inflammation is usually present in IMC. Corticosteroids are recommended for grade 2 or more severe colitis while holding the immunotherapy. About one third to two thirds of patients are steroid refractory and benefit from infliximab. Recently vedolizumab has been found to be efficacious in steroid and infliximab refractory cases. | 11 | Impact of on-range choice feeding with black soldier fly larvae(Hermetia illucens)on flock performance,egg quality,and range use of free-range laying hens显示文摘Semi-intensive free-range farm systems are common in Australia, and these systems frequently practise on-range feeding. The objective of this study was to investigate the benefit of on-range choice feeding on flock performance, egg quality, and range use of free-range laying hens using black soldier fly larvae(Hermetia illucens, BSF). A total of 160 mature ISA brown laying hens, previously determined to range daily, were allocated to a control group(control) or a treatment group(BSF) with various replicates depending on the parameter investigated. All hens were fed ad libitum indoors with a wheat-soy based diet formulated according to breed requirements. Black soldier fly hens were offered dried BSF larvae ad libitum on the range. Body weight, feed intake, BSF intake, egg production, feed conversion ratio, internal and external egg quality parameters, and individual range use using radio-frequency identification(RFID)technology was evaluated. Black soldier fly hens consumed on average 15 ± 1.7 g BSF larvae/hen per day.There were no differences between BSF and control hens for any of the performance parameters obtained(P > 0.05). Egg weight, shell weight, and shell thickness of eggs from BSF hens were significantly lower(P = 0.003, P = 0.001, and P = 0.004, respectively) than those of eggs from control hens. Egg yolk colour was significantly paler in eggs from BSF hens(P < 0.001). No significant ranging differences between the BSF and control hens were observed(P > 0.05) except for BSF hens showing longer total maximum time for a single visit to the range(P = 0.011). In conclusion, the average intake of BSF larvae indicated a good level of acceptance. Feed formulation should be adjusted for the intake of the choice fed source. | 12 | RANKL deletion in periodontal ligament and bone lining cells blocks orthodontic tooth movement显示文摘The bone remodeling process in response to orthodontic forces requires the activity of osteoclasts to allow teeth to move in the direction of the force applied. Receptor activator of nuclear factor-κB ligand(RANKL) is essential for this process although its cellular source in response to orthodontic forces has not been determined. Orthodontic tooth movement is considered to be an aseptic inflammatory process that is stimulated by leukocytes including T and B lymphocytes which are presumed to stimulate bone resorption. We determined whether periodontal ligament and bone lining cells were an essential source of RANKL by tamoxifen induced deletion of RANKL in which Cre recombinase was driven by a 3.2 kb reporter element of the Col1α1 gene in experimental mice(Col1α1.CreER^(TM+).RANKL^(f/f)) and compared results with littermate controls(Col1α1.CreER^(TM-).RANKL^(f/f)). By examination of Col1α1.CreER^(TM+).ROSA26 reporter mice we showed tissue specificity of tamoxifen induced Cre recombinase predominantly in the periodontal ligament and bone lining cells. Surprisingly we found that most of the orthodontic tooth movement and formation of osteoclasts was blocked in the experimental mice, which also had a reduced periodontal ligament space. | 18 | Association between sarcopenic obesity and higher risk of type 2 diabetes in adults: A systematic review and meta-analysis显示文摘BACKGROUND The coexistence of sarcopenia and obesity is referred to as sarcopenic obesity(SO)and it has been hypothesized that the two components of SO may synergistically increase their negative effects. However, many uncertainties still surround this condition especially with regard to its potential negative effects on health outcomes.AIM To conduct a systematic review to determine the prevalence of sarcopenia among adults with overweight and obesity and to investigate whether SO was associated with a higher risk of type 2 diabetes(T2 D).METHODS This study was conducted in adherence with the Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines. Literature searches, study selection, methodology development and quality appraisal were performed independently by two authors and the data were collated by means of metaanalysis and narrative synthesis.RESULTS Of the 606 articles retrieved, 11 studies that comprised a total of 60118 adults with overweight and obesity of both genders met the inclusion criteria and were reviewed, revealing two main findings. First, the overall prevalence of sarcopenia is 43% in females and 42% in males who are with overweight and obesity.Secondly, the presence of SO increases the risk of T2D by 38% with respect to those without SO(OR = 1.38, 95%CI: 1.27-1.50).CONCLUSION A high prevalence of sarcopenia has been found among adults with overweight and obesity regardless of their gender and this condition seems to be associatedwith a higher risk of T2D.
