1创伤性脑损伤后蛛网膜下腔出血与长期卒中风险的关系(英文)显示文摘BACKGROUND Recent studies suggest that traumatic brain injury(TBI)is a risk factor for subsequent ischemic stroke,even years after the initial insult.The mechanisms of the association remain unclear.The presence of traumatic subarachnoid hemorrhage(t SAH)may mediate the effect of TBI on long-term stroke risk,as it has previously been linked to short-term vasospasm and delayed cerebral ischemia.METHODS Using administrative claims data,we conducted a retrospective cohort study of acute care hospitalizations.Patients discharged with a first-recorded diagnosis of t SAH were followed for a primary diagnosis of stroke.They were matched to patients with TBI but not t SAH.Cox proportional hazards modeling was used to assess the association between t SAH and stroke while adjusting for covariates.RESULTS:We identified 40 908 patients with TBI(20 454 patients with t SAH)who were followed for a mean of 4.3+1.8 years.A total of 531 had an ischemic stroke after discharge.There was no significant difference in stroke risk between those with t SAH(1.79%;95%confidence interval[CI]1.54%-2.08%)versus without t SAH(2.12%;95%CI 1.83%-2.44%).The same pattern was found in adjusted analyses even when the group was stratified by age-group or by proxies of TBI severity.CONCLU-SIONS:Our findings do not support a role of t SAH in mediating the association between TBI and protracted stroke risk.Further study is required to elucidate the mechanisms of long-term increased stroke risk after TBI.Morris NA Cool J Merkler AE Kamel H 2017中华神经外科疾病研究杂志2017,16,4:6
2Comparison of Leptosphaeria biglobosa-induced and chemically induced systemic resistance to L.maculans in Brassica napus显示文摘Brassica napus(cv.Madrigal)seedlings pre-treated with ascospores of Leptosphaeria biglobosa or foliar sprays of either acibenzolar-S-methyl(ASM)or menadione sodium bisulphite(MSB)were chal- lenge inoculated with L.maculans ascospores and assessed for phoma leaf spot development and tissue morphology and gene expression responses to infection.Rates of increase in phoma leaf spot area 8―21 d after challenge inoculation were significantly greater on water pre-treated plants than on plants pre-treated with L.biglobosa,ASM or MSB on both pre-treated leaves(local effect)and younger leaves without pre-treatment(systemic effect).Ninety-six h after challenge inoculation,the invasive hyphae of L.maculans were encircled by rings of necrotic mesophyll cells on leaves pre-treated with L. biglobosa,ASM or MSB but not those pre-treated with water.Quantification of transcript levels of genes commonly used as markers of the major defence signalling pathways(PDF1.2,PR-1,NPR1,APX, CHB4)0–96 h after L.maculans challenge inoculation showed expression patterns indicating prefer- ential activation of the jasmonate/ethylene pathway and involved induction of NPR1 locally and sys- temically in leaves of plants pre-treated with L.biglobosa ascospores.LIU ShengYi LIU RenHu Akinwunmi O. 3Lessons from “real life experience” of rifaximin use in the management of recurrent hepatic encephalopathy显示文摘BACKGROUND Hepatic encephalopathy(HE)is a major complication of cirrhosis with independent prognostic significance.The current management of HE is mainly based on lactulose.Rifaximin has been shown to decrease the risk of HE recurrence in patients with episodic forms.HE can also be persistent.However,there is no drug support recommendation for rifaximin use in this setting.AIM To assess the effectiveness of rifaximin in the management of recurrent episodes of HE and recurrent acute exacerbations on persistent HE,in“real life conditions”.METHODS In this retrospective study,using a within-subjects design,we collected data of patients treated with rifaximin for HE in two liver diseases centers,during the six-month period before and during the six-month period after the initiation of rifaximin.The primary effectiveness endpoint was the total number of HE events involving hospitalization.RESULTS Rifaximin was introduced for prevention of recurrent HE episodes in 29 out of 62 patients with normal mental status between episodes and for prevention of recurrent acute exacerbations on persistent HE in 33 out of 62 patients.In the“prevention of recurrent HE episodes”group,fewer HE events(0.79 vs 1.78;P=0.013)were reported during the period of time when rifaximin was used.In the“prevention of recurrent acute exacerbations on persistent HE”group,there was no significant difference in the number of HE-events(1.48 vs 1.77;P=0.582).CONCLUSION In this real-life experience,the effectiveness of rifaximin was confirmed in the prevention of HE episodes recurrence but was not proved in the prevention of acute exacerbations recurrence on persistent HE.Fiona Chautant Maeva Guillaume Marie-Angèle Robic Jean-François Cadranel Jean-Marie Peron Hortensia Lison Charlène Cool Christophe Bureau Véronique Duhalde 2020World Journal of Hepatology2020,12,1:3

5Dietary zinc and metallothionein on small intestinal disaccharidases activity in mice显示文摘AIM: To examine the effect of increasing dietary zinc (Zn) intake and the lack of metallothionein (MT) expression on activity of small intestinal disaccharidases. METHODS: MT-Ⅰ and Ⅱ knockout (MT-/-) and wild-type (MT+/+) female mice at 3.5 wk of age were randomly fed with a diet containing 2 (2 Zn), 15 (15 Zn) or 50 (50 Zn) mg Zn/kg (n = 8/group/genotype) for 5 wk. Small intestinal segments (duodenum, jejunum and ileum) were collected and either fixed in 10% formalin for histological analysis or snap frozen in liquid nitrogen for sucrase, lactase and maltase activity analyses.RESULTS: Plasma Zn was significantly (P < 0.05) lower (33%) in MT-/- compared with MT+/+ mice fed the 2 Zn diet. Villus height and crypt depth were increased by approximately 15% in MT+/+ mice compared with MT-/- mice. Duodenal disaccharidase activities were significantly higher in MT+/+ compared with MT-/- mice particularly in those fed the 2 Zn diet. For the 50 Zn diet, jejunal sucrase and lactase activities were significantly higher in MT-/- (13 313 ± 2314; 4107 ± 364 μmol glucose/well/min/g tissue, respectively) compared with MT+/+ mice (7054 ± 608; 1818 ± 174). Similarly, ileal lactase activities were higher in MT-/- (1480 ± 192) compared with MT+/+ (629 ± 353) mice particularly those fed the 2 Zn diet. CONCLUSION: Increasing dietary Zn has little effect on disaccharidases activity in MT wild-type mice. 7On the road to new treatments for multiple sclerosis: targeting dendritic cell migration into the central nervous system显示文摘Distinct migratory pathways and trafficking of dendritic cells to the central nervous system (CNS): The immune system is a host defense mechanism protecting against invaders, such as bacteria and viruses, while maintaining tolerance to self. Nonetheless, a few sites throughout the body are believed to be immunologically inert, such as the testes, the eye and the brain. Indeed, experiments in the mid-20th century gave rise to the concept of the brain as a site of immune privilege. Originally, the immune privilege of the brain was thought to be absolute, attributed by a physical blood-brain barrier (BBB) protecting the CNS from the entry of pathogens and circulating immune cells.
