1无保护左主干病变支架置入术预后及影响因素的分析——中国无保护左主干病变支架置入术注册(CHANCE)研究显示文摘目的评价无保护左主干(LMCA)病变支架置入术后的即刻及长期预后,并探讨影响预后的因素.方法本研究为多中心注册研究,对国内23家医院在1997年5月至2003年3月间完成的连续择期无保护LMCA病变裸金属支架置入术的患者进行注册登记,全部患者均填写固定表格.结果共224例患者进行了注册,年龄22~88岁,平均(60.1±12.0)岁.有既往心肌梗死史患者53例(23.7%),合并糖尿病45例(20.1%).孤立性无保护LMCA126例(56.2%),LMCA合并其他血管病变98例(43.8%).平均左室射血分数(63.9±12.3)%. 223例(99.6%)LMCA病变支架置入术成功,91例(92.9%)非LMCA病变支架置入术成功.住院期间死亡1例(0.45%),非Q波心肌梗死(MI)1例(0.4%).平均随访(15.6±12.3)个月,死亡12例(5.4%),其中心原性死亡10例(4.5%),非心原性死亡2例(0.9%),急性MI 4例(1.8%), LMCA 靶病变血管重建术26例(11.7%),累积主要心脏不良事件(MACE)36例(16.1%).左室射血分数<40%、女性和LMCA合并多支血管病变增加死亡危险,多支病变非完全血管重建增加MACE发生率;多因素回归分析显示:左室射血分数<40%和女性为预测死亡和MACE的独立危险因素.102例(45.7%)患者进行了造影复查,LMCA 再狭窄32例(31.4%).术中有回旋支开口部受累者再狭窄率增加.结论经过选择的无保护LMCA病变支架置入术是可行和安全的,并可取得较好近、远期疗效.目前,无保护LMCA支架置入术应选择孤立性LMCA病变或合并多支病变但能达到完全性血管重建、左室射血分数≥40%的患者或不能行冠状动脉旁路移植术的患者.CHANCE研究组 2005中华心血管病杂志2005,33,3:18

3Dual antiplatelet therapy may increase the risk of non-intracranial haemorrhage in patients with minor strokes: a subgroup analysis of the CHANCE trial显示文摘Aim:The aim of this study was to explore the difference between haemorrhagic events among those patients on either aspirin or aspirin plus clopidogrel who were enrolled in the Clopidogrel in High-Risk Patients with Acute Non-disabling Ischemic Cerebrovascular Events(CHANCE)trial.Methods:This was an ad hoc analysis of the CHANCE trial;data on all patients with any haemorrhagic event were reviewed and analysed.Cox proportional hazards regression was used to determine factors association with any bleeding.Results:In the CHANCE trial,there were a total of 101(2%)haemorrhagic events reported from 50 different hospitals.The clopidogrel–aspirin group had 60(2.3%)cases and the aspirin group had 41(1.6%,p=0.09).Moderate or severe haemorrhagic events occurred in 7 patients(0.3%)in the clopidogrel–aspirin group and in 8(0.3%)in the aspirin group(p=0.73).Of 36(0.7%)cases of intracranial haemorrhages,20(0.4%)were in the clopidogrel–aspirin group and 16(0.3%)in the aspirin group.Each group had 8(0.3%)cases of symptomatic haemorrhagic strokes.Other common haemorrhagic events included 24(0.5%)cases of skin bruises,13(0.3%)gastrointestinal haemorrhages,9(0.2%)gum haemorrhages and 8(0.2%)intraocular haemorrhages.Conclusions:There was no overall significant difference in haemorrhagic events(p=0.29),especially in the rate of intracranial haemorrhages between the 2 treatment groups.However,patients enrolled with minor strokes had an increased risk of haemorrhagic events regardless of treatment group,not seen in patients with high-risk transient ischaemic attacks.Being elderly,of male gender and with a history of aspirin or proton pump inhibitor usage were associated with increased risk of haemorrhage.Patients with higher body mass index had lower risk of haemorrhagic events.David Wang Li Gui Yi Dong Hao Li Shujuan Li Huaguang Zheng Anxin Wang Xia Meng Li-Ping Liu Yi-Long Wang Guangyao Wang Jing Jing Zixiao Li Xing-Quan Zhao Yong-Jun Wang on behalf of the investigators for the CHANCE trial 2016Stroke & Vascular Neurology2016,1,2:8

6Dynamical and Chemical Features of a Cutoff Low over Northeast China in July 2007:Results from Satellite Measurements and Reanalysis显示文摘The European Centre for Medium-Range Weather Forecasts Re-Analysis Interim(ERA-Interim) meteorology and measurements from the Microwave Limb Sounder,High Resolution Dynamics Limb Sounder,and Ozone Monitoring Instrument onboard the Earth Observing System Aura satellite were applied to analyze the dynamical and chemical features of a cutoff low(COL) event over northeast China in early July 2007.The results showed the polar stratospheric origin of an upper-level warm-core cyclone at 100-300 hPa,associated with a funnel-shaped tropopause intruding into the mid-troposphere just above the COL center.The impacts of the stratospheric intrusion on both column ozone and ozone profiles were investigated using satellite measurements.When the intensity of the COL peaked on 10 July 2007,the total column ozone(TCO) increase reached a maximum(40-70 DU).This could be dynamically attributed to both the descent of the tropopause(~75%) and the downward transport of stratospheric ozone across the tropopause(~25%).Analysis of the tropospheric ozone profiles provided evidence for irreversible transport/mixing of ozone-rich stratospheric air across the tropopause near the upper-level front region ahead of the COL center.This ozone intrusion underwent downstream transport by the upper tropospheric winds,leading to further increase in TCO by 12-16 DU over broad regions extending from east China toward the northern Japan Sea via South Korea.Meteorological analysis also showed the precedence of the stratospheric intrusion ahead of the development of cyclones in the middle and lower troposphere.刘传熙 刘毅 Xiong LIU Kelly CHANCE 2013Advances in Atmospheric Sciences2013,30,2:5

8MITOCHONDRIAL REDOX IMAGING FOR CANCER DIAGNOSTIC AND THERAPEUTIC STUDIES显示文摘Mitochondrial redox states provide important information about energy-linked biological processes and signaling events in tissues for various disease phenotypes including cancer.The redox scanning method developed at the Chance laboratory about 30 years ago has allowed 3D highresolution(∼50×50×10µm^(3))imaging of mitochondrial redox state in tissue on the basis of the fluorescence of NADH(reduced nicotinamide adenine dinucleotide)and Fp(oxidized flavoproteins including flavin adenine dinucleotide,i.e.,FAD).In this review,we illustrate its basic principles,recent technical developments,and biomedical applications to cancer diagnostic and therapeutic studies in small animal models.Recently developed calibration procedures for the redox imaging using reference standards allow quantification of nominal NADH and Fp concentrations,and the concentration-based redox ratios,e.g.,Fp/(Fp+NADH)and NADH/(Fp+NADH)in tissues.This calibration facilitates the comparison of redox imaging results acquired for different metabolic states at different times and/or with different instrumental settings.A redox imager using a CCD detector has been developed to acquire 3D images faster and with a higher in-plane resolution down to 10µm.Ex vivo imaging and in vivo imaging of tissue mitochondrial redox status have been demonstrated with the CCD imager.Applications of tissue redox imaging in small animal cancer models include metabolic imaging of glioma and myc-induced mouse mammary tumors,predicting the metastatic potentials of human melanoma and breast cancer mouse xenografts,differentiating precancerous and normal tissues,and monitoring the tumor treatment response to photodynamic therapy.Possible future directions for the development of redox imaging are also discussed.LIN Z.LI HE N.XU MAHSA RANJI SHOKO NIOKA BRITTON CHANCE 2009Journal of Innovative Optical Health Sciences2009,2,4:3
9Antidepressant foods:An evidence-based nutrient profiling system for depression显示文摘AIM To investigate which foods are the most nutrient dense sources of nutrients demonstrated by the scientific literature to play a role in the prevention and promotion of recovery from depressive disorders. METHODS A systematic literature review was conducted to derive a list of Antidepressant Nutrients from the 34 nutrients known to be essential for humans using level of evidence criteria. Nutritional data was extracted for a subset of foods with a high content of at least 1 Antidepressant Nutrient using a USDA database. These foods were analyzed for Antidepressant Nutrient density resulting in an Antidepressant Food Score(AFS). Plant and animal foods were analyzed separately. RESULTS Twelve Antidepressant Nutrients relate to the prevention and treatment of depressive disorders: Folate, iron, longchain omega-3 fatty acids(EPA and DHA), magnesium, potassium, selenium, thiamine, vitamin A, vitamin B6, vitamin B12, vitamin C, and zinc. The highest scoring foods were bivalves such as oysters and mussels, various seafoods, and organ meats for animal foods. The highest scoring plant foods were leafy greens, lettuces, peppers, and cruciferous vegetables. CONCLUSION The AFS is based on a nutrient profiling system devised to identify foods with the highest nutrient density of nutrients with clinical evidence to support theirrole in depressive disorders. This list of foods and food categories with the highest density of the 12 Antidepressant Nutrients, the Antidepressant Foods, should be considered by researchers in the design of future intervention studies and clinicians as dietary options to support prevention and recovery from depression disorders.Laura R La Chance Drew Ramsey 2018World Journal of Psychiatry2018,8,3:2

12QUANTITATIVE REDOX SCANNING OF TISSUE SAMPLES USING A CALIBRATION PROCEDURE显示文摘The fluorescence properties of reduced nicotinamide adenine dinucleotide(NADH)and oxidizedflavoproteins(Fp)including flavin adenine dinucleotide(FAD)in the respiratory chain are sensitive indicators of intracellular metabolic states and have been applied to the studies of mitochondrial function with energy-linked processes.The redox scanner,a three-dimensional(3D)low temperature imager previously developed by Chance et al.,measures the in vivo metabolicproperties of tissue samples by acquiring fluorescence images of NADH and Fp.The redox ratios,i.e.Fp/(Fp+NADH)and NADH/(Fp+NADH),provided a sensitive index of the mitochondrialredox state and were determined based on relative signal intensity ratios.Here we report thefurther development of the redox scanning technique by using a calibration method to quantifythe nominal concentration of the fluorophores in tissues.The redox scanner exhibited very goodlinear response in the range of NADH concentration between 165–1318µM and Fp between90–720µM using snap-frozen solution standards.Tissue samples such as human tumor mousexenografts and various mouse organs were redox-scanned together with adjacent NADH and Fpstandards of known concentration at liquid nitrogen temperature.The nominal NADH and Fpconcentrations as well as the redox ratios in the tissue samples were quantified by normalizing the tissue NADH and Fp fluorescence signal to that of the snap-frozen solution standards.This calibration procedure allows comparing redox images obtained at different time,independent of instrument settings.The quantitative multi-slice redox images revealed heterogeneity inmitochondrial redox state in the tissues.HE N.XU BAOHUA WU SHOKO NIOKA BRITTON CHANCE LIN Z.LI 2009Journal of Innovative Optical Health Sciences2009,2,4:2
