| 1 | Molecular basis and management of gastrointestinal stromal tumors显示文摘Molecularly targeted agents have dramatically impacted the management of several cancers. Targeting KIT has led to a new treatment paradigm in gastrointestinal stromal tumors (GISTs). KIT is a cell surface receptor with tyrosine kinases that, upon binding of its ligand, stem cell factor, activates various signaling pathways. Imatinib and sunitinib, both tyrosine kinase inhibitors directed to KIT, were approved for firstand secondline treatment of metastatic and unresectable GISTs. In this article, we will review the molecular pathogenesis of GISTs followed by a discussion of imatinib and sunitinib’s role in the treatment of GISTs. | 8 | Recurrent ischemic strokes in a young celiac woman with MTHFR gene mutation显示文摘Celiac disease(CD) is frequently associated with neurological disorders,but very few reports concern the association with ischemic stroke.A 26-year-old woman affected by CD with secondary amenorrhea,carrier of a homozygous 5,10-methylenetetrahydrofolate reductase mutation with hyperhomocysteinemia,was affected by two occipital ischemic strokes within a period of 5 mo.At the time of the second stroke,while she was being treated with folic acid,acetylsalicylic acid and a gluten-free diet,she had left hemianopsia,left hemiparesthesias,and gait imbalance.Brain magnetic resonance imaging showed a subacute right occipital ischemic lesion,which was extended to the dorsal region of the right thalamus and the ipsilateral thalamocapsular junction.Antitransglutaminase and deamidated gliadin peptide antibodies were no longer present,while antinuclear antibodies,antineuronal antibodies and immune circulating complexes were only slightly elevated.Since the patient was taking folic acid,her homocysteine levels were almost normal and apparently not sufficient alone to explain the clinical event.A conventional cerebral angiography showed no signs of vasculitis.Finally,rare causes of occipital stroke in young patients,such as Fabry's disease and mitochondrial myopathy,encephalomyopathy,lactic acidosis and stroke-like symptoms,were also excluded by appropriate tests.Thus,the most probable cause for the recurrent strokes in this young woman remained CD,although the mechanisms involved are still unknown.The two main hypotheses concern malabsorption(with consequent deficiency of vitamins known to exert neurotrophic and neuroprotective effects) and immunemediated mechanisms.CD should be kept in mind in the differential diagnosis of ischemic stroke in young patients. | Elisa Fabbri Lisa Rustignoli Antonio Muscari Giovanni M Puddu Maria Guarino Rita Rinaldi Elena Minguzzi Giacomo Caio Marco Zoli Umberto Volta | 2012 | World Journal of Gastroenterology2012，18，26： | 1 |

| 13 | Severe colitis associated with docetaxel use:A report of four cases显示文摘Diarrhea is a common side effect of chemotherapy.Pseudomembranous colitis is a well known complication of antibiotic treatment that can also be observed,albeit rarely,with certain chemotherapeutic agents.We present four cases of severe colitis in patients undergoing treatment with taxane-based chemotherapy for pancreatic,lung and breast cancer.None of them had recently received antibiotics.One patient presented with a bowel perforation and three had endoscopic findings of pseudomembranous colitis.Two of these three patients had negative stool toxin assays for Clostridium difficile.In the patient presenting with perforation,an emergency left hemicolectomy was performed and the pathological findings in the colon were acute inflammation and ischemic necrosis;the other three patients were treated with oral vancomycin and/or oral or intravenous metronidazole leading to complete resolution of the symptoms.Apart from pseudomembranous colitis,we describe patients presenting with neutropenic enterocolitis as well as ischemic colitis after docetaxel use.These cases provide some insight into the spectrum and varied clinical presentations of severe colitis associated with taxane-based chemotherapy. | Andres F Carrion Peter J Hosein Eugene M Cooper Gilberto Lopes Liset Pelaez Caio M Rocha-Lima | 2010 | World Journal of Gastrointestinal Oncology2010，2，10： | 0 |
