| 1 | Surveys in Areas of High Risk of Iodine Deficiency and Iodine Excess in China, 2012-2014: Current Status and Examination of the Relationship between Urinary Iodine Concentration and Goiter Prevalence in Children Aged 8-10 Years显示文摘Objective We aimed to evaluate goiter prevalence and iodine nutritional status in areas with high levels of water iodine;to monitor the prevalence of iodine deficiency disorders(IDD)in areas at high risk of IDD;and to compare the prevalence of goiter and urine iodine(UI)concentrations between children living in the two areas.Methods Based on surveillance from 2012-2014,we analyzed the concentration of UI and prevalence of goiter in 8-10-year-old children from 12 high-risk IDD provinces,and from 8 provinces and municipalities with excessive water iodine.We calculated goiter prevalence for each UI level according to World Health Organization(WHO)standards and constructed predictive prevalence curves.Results The goiter prevalence and median UI of children from areas with high water iodine were not optimal,being above the WHO standards(5%and 100-199μg/L,respectively),whereas those in high-risk areas fell within the standard.UI and goiter prevalence exhibited a U-shaped relationship in high-risk endemic areas and a parabolic relationship in areas of iodine excess.Conclusion Iodine surplus in high-iodine areas leads to high goiter prevalence and UI.However,in high-risk areas,UI was optimal and goiter prevalence met the national criteria for IDD elimination. | CUI Si Lu LIU Peng SU Xiao Hui LIU Shou Jun | 2017 | Biomedical and Environmental Sciences2017,30,2: | 6 |
| 2 | A Phase Ⅱ Study of Fotemustine Plus Dacarbazine with Dendritic Cell Vaccines as First-Line Therapy for Chinese Patients with Advanced Acral Lentiginous Melanoma显示文摘Objective:To investigate fotemustine plus dacarbazine(DTIC) with dendritic cell(DC) vaccines on patients with advanced acral lentiginous melanoma(ALM). Fotemustine is a cytotoxic alkylating agent with a remarkable antitumor activity as single agent but also in association with(DTIC). DC is the strongest antigen presenting cell which could induce durable clinical responses. Methods:This was a single-center study. Between July 2003 and June 2006,twenty-eight chemotherapy-naive patients of advanced ALM received fotemustine 100 mg/m2,d1,12,DTIC 400 mg/d d2-6,DC vaccines subcutaneously d7,9,13 repeated every 28 days. Ten HLA-A02+24+ patients received vaccines pulsed with melanoma antigen derived peptides,melanoma antigen recognized by T-cells 1(Mart-1) and S-100. Eighteen patients received DC loaded with allogeneic melanoma lysate. The primary end-point was progression free survival(PFS). Secondary end-points were overall survival(OS),overall response rate(ORR) and toxicity. Tumor assessment was performed every 8 weeks and evaluated according to response evaluation criteria in solid tumors(RECIST). Results:The 15 men and 13 women had a median age of 51 years. 16 patients had stage M1c disease and 11/16 had liver metastasis. Patients received an average of 3.82±1.25 cycles. Follow-up for the 18 surviving patients ranged from 7-41 months with a median of 12 months. Median PFS was 8.5 months(95% CI:7.86-15.21) with 12 patients remaining progression free. Only 10 patients died. Median OS was 12 months(95% CI:10.33-18.24). ORR(CR+PR) was 35.7% including 3 complete response(CR) and 7 partial response(PR). Six patients had disease stable. A total of 19 Grade III/IV toxicities were observed including thrombocytopenia(n=8),neutropenia(n=5),fatigue(n=6) and hypersensitivity reaction(n=1). One patient died of Grade IV thrombocytopenia. Conclusion:Fotemustine and dacarbazine plus DC vaccines are safe and tolerable to Chinese ALM patients. The regimen brings clinical benefit with a higher ORR and may provide a survival advantage. | 6 | A summary of the evidence and evaluation of the effectiveness of nonpharmacological interventions for mild cognitive impairment显示文摘Objective: To summarize and evaluate the evidence of guidelines and systematic reviews(SRs) of nonpharmacological interventions for mild cognitive impairment(MCI) to support the development of future guidelines and clinical decisions for MCI patients.Methods: Scottish Intercollegiate Guideline Network(SIGN), National Institute for Health and Clinical Excellence(NICE), American Academy of Neurology(AAN), Registered Nurses Association of Ontario(RNAO), Web of Science, PubMed, Cochrane Library, CNAHL, VIP, China National Knowledge Infrastructure(CNKI), and Wanfang Database were searched for relevant publications, including guidelines and SRs, from January 2014 to March 2019. Two authors independently screened articles, extracted data, and assessed the publications for adherence to the inclusion criteria. Appraisal of Guidelines for Research and Evaluation(AGREE II) was used to assess the quality of the guidelines, and Assessment of Multiple Systematic Reviews(AMSTAR 2) was used to assess the quality of SRs. In addition, the Grading of Recommendations Assessment, Development and Evaluation(GRADE) approach was used to evaluate the quality of outcomes.Results: Thirty-two articles were retrieved, including 1 guideline and 31 SRs. Fourteen SRs of physical exercise for MCI, six articles describing cognitive interventions, four articles describing acupuncture, and seven articles assessing dietary interventions(including four articles employing a Mediterranean diet, one article using vitamin B supplementation, and two articles assessing the effects of tea, coffee, and caffeine) were included. The quality of the articles was very low for 4(13%), low for 10(32%), and moderate for 17(55%).Conclusions: Based on the evidence available to date, nonpharmacological interventions may improve the current cognitive function of persons with MCI. In particular, physical exercise, cognitive interventions, and acupuncture exerted promising effects.
