| 1 | Obstructive jaundice due to hepatobiliary cystadenoma or cystadenocarcinoma显示文摘Hepatobiliary cystadenomas (HBC) and cystadenocarci- nomas are rare cystic lesions. Most patients with these lesions are asymptomatic, but presentation with ob- structive jaundice may occur. The first patient presented with intermittent colicky pain and recurrent obstructive jaundice. Imaging studies revealed a polypoid lesion in the left hepatic duct. The second patient had recurrent jaundice and cholangitis. Endoscopic retrograde cholan- giopancreatography (ERCP) showed a cystic lesion at the confluence of the hepatic duct. In the third patient with intermittent jaundice and cholangitis, cholangioscopy re- vealed a papillomatous structure protruding into the left bile duct system. In the fourth patient with obstructive jaundice, CT-scan showed slight dilatation of the intrahe- patic bile ducts and dilatation of the common bile duct of 3 cm. ERCP showed filling of a cystic lesion. All patients underwent partial liver resection, revealing HBC in the specimen. In the fifth patient presenting with obstructive jaundice, ultrasound examination showed a hyperecho- genic cystic lesion centrally in the liver. The resection specimen revealed a hepatobiliary cystadenocarcinoma. HBC and cystadenocarcinoma may give rise to obstruc- tive jaundice. Evaluation with cross-sectional imaging techniques is useful. | 2 | Results of percutaneous sclerotherapy and surgical treatment in patients with symptomatic simple liver cysts and polycystic liver disease显示文摘AIM: To evaluate the results of the treatment of simple liver cysts (solitary and multiple) and polycystic liver disease (PLD) using percutaneous sclerotherapy and/or surgical procedures in a single tertiary referral centre. METHODS: Retrospective analysis of 54 patients referred for evaluation and possible treatment of simple liver cysts (solitary and multiple) and PLD, from January 1997 to July 2006. RESULTS: Simple liver cysts were treated in 41 pts (76/) with a mean size of 12.6 cm. The most common reason for referral was abdominal pain or discomfort (85/). Percutaneous sclerotherapy was performed as initial treatment in 30 pts, showing cyst recurrence in 6 pts (20/). Surgical treatment was initially performed in 11 pts with cyst recurrence in 3 pts (27/). PLD was treated in 13 pts (24/) with a mean size of the dominant cyst of 13 cm. Percutaneous sclerotherapy for PLD was performed in 9 pts with recurrence in 7 pts (77.8/). Surgical treatment for PLD was undertaken in 4 pts (30.8/) with recurrence in all. Eventually, 2 pts with PLD in the presence of polycystic kidney disease underwent liver-and kidney transplantation because of deterioration of liver and kidney function. CONCLUSION: The majority of patients with simple liver cysts and PLD are referred for progressive abdominal pain. As initial treatment, percutaneous sclerotherapy is appropriate. Surgical deroofing is indicated in caseof cyst recurrence after percutaneous sclerotherapy. However, the results of percutaneous sclerotherapy and surgical treatment for PLD are disappointing. | 3 | Cast iron-a predictable material显示文摘High strength compacted graphite iron (CGI) or alloyed cast iron components are substituting previously used non-ferrous castings in automotive power train applications.The mechanical engineering industry has recognized the value in substituting forged or welded structures with stiff and light-weight cast iron castings.New products such as wind turbines have opened new markets for an entire suite of highly reliable ductile iron cast components.During the last 20 years,casting process simulation has developed from predicting hot spots and solidification to an integral assessment tool for foundries for the entire manufacturing route of castings.The support of the feeding related layout of the casting is still one of the most important duties for casting process simulation.Depending on the alloy poured,different feeding behaviors and self-feeding capabilities need to be considered to provide a defect free casting.Therefore,it is not enough to base the prediction of shrinkage defects solely on hot spots derived from temperature fields.To be able to quantitatively predict these defects,solidification simulation had to be combined with density and mass transport calculations,in order to evaluate the impact of the solidification morphology on the feeding behavior as well as to consider alloy dependent feeding ranges.For cast iron foundries,the use of casting process simulation has become an important instrument to predict the robustness and reliability of their processes,especially since the influence of alloying elements,melting practice and metallurgy need to be considered to quantify the special shrinkage and solidification behavior of cast iron.This allows the prediction of local structures,phases and ultimately the local mechanical properties of cast irons,to asses casting quality in the foundry but also to make use of this quantitative information during design of the casting.Casting quality issues related to thermally driven stresses in castings are also gaining increasing attention.State-of-the-art tools allow the prediction of residual stresses and iron casting distortion quantitatively.Cracks in castings can be assessed,as well as the reduction of casting stresses during heat treatment.As the property requirements for cast iron as a material in design strongly increase,new alloys and materials such as ADI might become more attractive,where latest software developments allow the modeling of the required heat treatment.Phases can be predicted and parametric studies can be performed to optimize the alloy dependent heat treatment conditions during austenitization,quenching and ausferritization.All this quantitative information about the material's performance is most valuable if it can be used during casting design.The transfer of local properties into the designer's world,to predict fatigue and durability as a function of the entire manufacturing route,will increase the trust in this old but highly innovative material and will open new opportunities for cast iron in the future.The paper will give an overview on current capabilities to quantitatively predict cast iron specific defects and casting performance and will highlight latest developments in modeling the manufacture of cast iron and ADI as well as the prediction of iron casting stresses. | Jrg C.Sturm Guido Busch | 2011 | China Foundry2011,8,1: | 4 |
| 4 | Emerging role of caldesmon in cancer:A potential biomarker for colorectal cancer and other cancers显示文摘Colorectal cancer(CRC) is a devastating disease, mainly because of metastasis. As a result, there is a need to better understand the molecular basis of invasion and metastasis and to identify new biomarkers and therapeutic targets to aid in managing these tumors. The actin cytoskeleton and actin-binding proteins are known to play an important role in the process of cancer metastasis because they control and execute essential steps in cell motility and contractility as well as cell division. Caldesmon(CaD) is an actin-binding protein encoded by the CALD1 gene as multiple transcripts that mainly encode two protein isoforms: High-molecular-weight CaD, expressed in smooth muscle, and low-molecular weight CaD(l-CaD), expressed in nonsmooth muscle cells. According to our comprehensive review of the literature, CaD, particularly l-CaD, plays a key role in the development, metastasis, and resistance to chemoradiotherapy in colorectal, breast, and urinary bladder cancers and gliomas, among other malignancies. CaD is involved in many aspects of the carcinogenic hallmarks, including epithelial mesenchymal transition via transforming growth factor-beta signaling, angiogenesis, resistance to hormonal therapy, and immune evasion. Recent data show that CaD is expressed in tumor cells as well as in stromal cells, such as cancerassociated fibroblasts, where it modulates the tumor microenvironment to favor the tumor. Interestingly, CaD undergoes selective tumor-specific splicing, and the resulting isoforms are generally not expressed in normal tissues, making these transcripts ideal targets for drug design.
